The influence that the medical service quality of the dental clinic affects to the patient satisfaction and use intention (치과 병·의원의 의료서비스 품질이 환자만족도와 재이용 의도에 미치는 영향)

  • Purpose: This aim of this paper is to research the influence of customer satisfaction and reutilization on Dental hospitals and clinics of medical service quality. Methods: The study examines 140 Patients who received treatment in Daegu from 1 July 2012 to 30 July 2012. The methods of research were used to factor analysis, reliability test using Cronbach's ${\alpha}$ factor, correlation analysis, multiple regression analysis and spss/win18.0. Results: The factor of quality of care has the highest impact on customer satisfaction. the influence of reutilization depends on the highest reliability and the physical environment was significant. Conclusion: To improve customer satisfaction and reutilization is required to maintenance of hospital facilities, medical services mind and quality of care. The medical service providers are important to develop patient education programs, self-development and customer satisfaction training techniques.

The Effects of ICT Enhanced Home-visit Nursing in Long-Term Care Insurance on Health-related Quality of Life among Community-Dwelling Older Adults (ICT 활용 방문간호서비스가 재가노인의 건강관련 삶의 질에 미치는 효과)

  • Purpose: This secondary data analysis study evaluated the effects of ICT enhanced home-visit nursing in long-term care insurance on health-related quality of life among community-dwelling older adults. Methods: This study included data of 131 older adults who had experienced a pilot service for ICT enhanced home-visit nursing. ICT enhanced home-visit nursing refers to a method of sharing health records and teleconference between a visiting nurse and a doctor during the home-visit nursing services to community-dwelling older adults. Health-related quality of life and influencing factors were analyzed by t-tests, logistic regression analysis using the Stata 17/SE program. Results: After a pilot service for ICT enhanced home-visit nursing, their health-related quality of life increased. The teleconferencing method had a significant effect on the increase in health-related quality of life. Conclusion: The findings indicate a pilot service for ICT enhanced home-visit nursing can be applied to the domestic community-based healthcare service model in terms of health management. In the future, the advanced service model of a pilot service for ICT enhanced home-visit nursing in which subjects conduct detailed for each health problem, and a well-designed evaluation system should be developed.

Clinical Nutrition Service in Korean Tertiary Hospitals and General Hospitals: Result of Nationwide Cross-Sectional Survey (국내 상급종합병원과 전국 분포 종합병원의 임상영양서비스 실태 조사 연구: 전국 규모 설문조사를 통하여)

  • The purpose of this study was to investigate the status of clinical nutrition services provided at tertiary hospitals and general hospitals in Korea. In total, 157 questionnaires were distributed to the departments of nutrition at hospitals on September 2013. The results of this study are as follows. The median number of beds was 607 and average length of stay was 8 days. 63.1% of dietitians had over 5 years of career experience. Nutritional screening rate was 97% in tertiary hospitals but only 67.2% in general hospitals (P<0.001). The rate of equipment with computerized nutritional screening system was 100% in tertiary hospitals but 71.9% in general hospitals (P<0.001). Hospitals with the best regarding nutritional care were hospitals accredited by JCI (Joint Commission International). On the other hand, hospitals not accredited by the JCI but KOIHA (Korea Institute for Healthcare Accreditation) showed the lowest performance rate of nutritional care. Nutrition support teams (NSTs) were established in all tertiary hospitals but in only 73% of general hospitals (P<0.001). The rate of actively operating NSTs was 89% in tertiary hospitals but only 62% in general hospitals (P<0.001). There is a need to provide proper standardized clinical nutrition services as a primary treatment and we observed large variations in the quality of nutritional service between hospitals. Therefore, local solutions are needed to implement nutritional programs and policies for improved service and care.

Early-stage Diagnosis of Bladder Carcinoma by a Clinical Pharmacist's Effective Anti-coagulation Service in a Patient with Anticoagulation Therapy: a Clinical Case Report (항응고제 투여 환자에서 약사의 효과적인 ACS를 통한 방광암 조기발견 사례)

  • Recently, the prevalence of bladder cancer is increasing in the Korean society. As the risk factors of bladder carcinoma are variable, the early-stage diagnosis is regarded the best preventive practice. Hematuria is a specific sign of the malignancy as well as a kind of various medication-related adverse reactions. Some anti-coagulation therapy can cause bleedings including hematuria to the patients with cardiovascular diseases such as paroxysmal atrial fibrillation (PAF). Therefore, to the clinical pharmacists working in the anti-coagulation services (ACS), a closer monitoring of patients can give an opportunity to find certain ailments unexpectedly. In this case, a patient with PAF had episodes of sporadic hematuria in the course of warfarin therapy even though with its low levels of INR. An ACS pharmacist found a discrepancy between the bleeding symptoms and INR values, and recommended properly the patient to refer urologist. Fortunately, an early-stage of bladder carcinoma was found then followed by an excision performed to the lesion. Therefore, alert-minded and precise monitoring done by ACS pharmacist could optimize the therapeutic outcomes as well as increase the quality of life of the patient.

Research of Awareness for Medical Radiation Safety in Radiography (방사선검사 시 의료방사선 안전성에 대한 인식도 조사)

  • Most patients and parents and guardians display frequent anxiety due to radiation exposure during outpatient, ward, and pediatric general radiographic examinations. This is a behavior that perceives only the harmfulness of radiation. For the recognition of medical radiation, we conduct surveys on outpatients, inpatients, and pediatric parents and guardians to identify their awareness, and then use the radiation dose promotional materials After providing accurate information on the use of radiation, the outpatient, inpatient, and pediatric parents and guardians were asked to explain the change in awareness. The questionnaire items were classified into five categories: repetitive radiation awareness for diagnosis, awareness of exposure dose, availability of exposure information, awareness of radiation risk, and awareness of health problems caused by radiation. There was a statistically significant difference in the items of recognition result of medical radiation, although there was a slight difference in the individual items in the pre and post-recognition results of providing information about the radiologists of the protector and the outpatient(p<0.05). Therefore, through the installation of these promotional materials, we will improve our awareness of medical radiation safety during general radiography surveillance in the Department of Radiology to provide better quality medical information and medical services, thereby contributing to strengthening the competitiveness of the hospital.

Nurses' Experience of Career Ladder Programs in a General Hospital (간호사들의 경력개발 프로그램 참여 경험)

  • Purpose: The purpose of this study was to identify and describe nurses' experience of a career ladder program (CLP) in a general hospital in Korea. Methods: The data were collected from 15 nurses who participated in the CLP. Each nurse completed individual in-depth interviews, for which the main interview question was "How do you describe your experience of CLP?" All interview data were transcribed and were analyzed using the grounded theory method. Results: The core category of the experience of CLP was identified as "challenging to rebirth as an expert". The participants used five action/interaction strategies such as "taking some time for introspection", "appreciating the essence of nursing", "making use of feedback positively", "throwing away doubts about their own capabilities", "exerting themselves with confidence". The consequence of the process of adjusting to CLP was "positioning a differentiated expert". Conclusion: The results of this study show that participants recognized CLP as a powerful resource which leads them to professional growth and development. Thus, CLP could be utilized as a clinical career-promoting program and ultimately as a program for providing high-quality nursing services.

  • While medical services are recognized to be crucial in a process of disease treatment, the research has been done with the service space and waiting time in a medical institution. This paper's objective is to evaluate the waiting areas of lobby, outpatient clinic, operating room in a general hospital and to draw factors of architecture design for further improvement, identifying strengths and weaknesses to be analyzed in terms of medical service. Research method is to define concepts of waiting time through literature review and advanced researches, analyze characteristics of architectural plan through drawing analysis and site survey. This research will contribute to the improvements of the planning methods which will reduce the waiting time and increase the quality of medical service.

Randomized Clinical Controlled Trials of a Herb Remedies in Korea - Systematic Review (한약을 이용한 한국에서의 무작위 임상시험 개괄적 검토)

  • Objective: The study aimed to review all randomized controlled clinical trials (RCTs) performed in Korea with herbal medicine, to produce further useful information for herbal-drug development. Methods: All papers reporting RCT with herbal drugs were collected via public database of Korea Science and Technology Integration Services, and various Oriental medicine journals. Then, the clinical question, herbal medicine, result, RCT design, and its conductor were analyzed. Results: A total of 20 RCTs were finally selected, and most of them were performed after 2003. The main subjects of RCT were related to heart and blood circulation, dermatitis, respiration disorder, Sasang constitution, or psychiatric disorders. Eight RCTs were done with a single herb while 12 RCTs were with multiple herbal formulae. In particular, three RCTs for atopic dermatitis with three formulae showed positive results. The quality of RCT was considered as better than average. Conclusions: This study revealed the current status of RCT using herbal drugs. This result would provide helpful information for research and drug development using traditional herbal medicine.

  • Background: Based on that the key function of health technology is improving the quality of healthcare services, our study purports to explore the process of medical device development in detail and to discuss its policy implications. Methods: A total of 12 in-depth interviews were conducted with four groups of industry, hospital, academia, and civil society. All of the interviewees except those from civil society were involved in the new medical device development between 2009 and 2018. We performed a text network analysis and content analysis of the interview data. Results: The frequency and the degree centrality rankings suggested a close association between the utilization issue and the technology development. Similarly, the results of the content analysis showed that the appropriate intervention in the utilization of technology has a direct impact on the progress of development. Under the continuous industrial effort to boost profits by developing new technology, service providers and citizens should be knowledgeable of and make good use of the new technology for the provision of better services. Conclusion: As the development itself would not guarantee the improvement of service quality and better health outcomes, health technology policies should take a more comprehensive view to serve the unmet needs and even to facilitate the technology development.

Prehospital Care of 119 Emergency Medical Technician to Trauma Patients (119구급대원의 외상환자에 대한 병원 전 응급처치와 업무수행현황)

  • Purpose: This study intended to improve quality of prehospital emergency care for trauma patients by figuring out its current situations and problems based on run-sheets and questionnaires of 119 emergency medical technicians (EMTs). Methods: This study conducted a research of 425 trauma patients transferred to the 3rd hospital in G-city by 119 ambulances from July 1, 2008 to June 30, 2009. We aslo utilized 114 copies with questionnaires of 119 EMTs working in J-province. The data were analyzed with SPSS 18.0. Results: There were 425 trauma patients including 272 men and 137 patients with traffic accident. When it comes to types of 119 EMTs who delivered cares to patients, there were 206 (48.5%) advanced EMTs, 101 (23.8%) basic EMTs, 50 (11.8%) nurses and 43 (10.2%) rescue education receivers. The most frequent measured vital sign was pulse rate (54.1%). Regarding assessment of systolic blood pressure, pulse rate and respiration rate, there were some significant differences in accordance with type of 119 EMTs. Among the 317 patients evaluated 'emergency' in field, 137 patients returned to their home. Prehospital emergency cares accounted for 861, around 2.0 treatments per a patient. In view of questionnaire, the 74.6% of 119 EMTs hoped supplement of man power for proper prehospital care to trauma patients. Conclusion: This study suggested that it is necessary to develop detailed guidelines for trauma patients so as to improve quality of trauma patient evaluation and prehospital care. Furthermore, improvement of emergency care systems will reduce mortality of trauma patients and lead to their good outcome.
