의료 질 관리 활동에 대한 병원종사자들의 태도와 인식 (Quality Improvement Activity in Hospitals and Its Acceptance among Hospital Personnels)

  • Background : It is a primary goal for hospital personnel to improve the quality of patient care. In Korea the concept of quality assurance has been spread over the last five years. Many hospitals have implemented quality assurance programs that fit their own philosophy and needs. As a result, they are supposed to have different experiences and attitudes toward quality assurance. To investigate their diversity will be helpful to have a direction to the future and to enhance the quality assurance activities in Korean hospitals. Objectives : The aim of this study is to obtain information about hospital personnel's attitude and opinion toward quality assurance in hospitals. Methods : A questionnaire was developed which consisted of five parts; the general characteristics of respondent, the concepts of quality assurance, need for quality assurance program, current status of quality improvement activities, and participation in programs. Using the registry of Korean hospitals, 102 hospitals with more than 400 beds were selected. Questionnaires were mailed to hospital staffs of each hospital; top managers, clinical department heads, registered nurses, medical recorders, and administrators. Results : Of 2038 questionnaires sent, 877 were returned, giving response rate of 44%. Most respondents(70%) regarded quality assurance as efforts to provide patient with care in highest quality and to improve effectiveness or resource utilization. Ninety-nine percent of respondents agreed to need for quality assurance in their hospitals. There were current quality improvement programs implemented in the department of 553 respondents(62%), and most of the(85%) have participated in at least one program. Lack of motivation was pointed out as a barrier to implement the programs. Conclusion : Although most respondents have known of the concept and need for quality assurance, this study suggests that education and motivation of hospital personnel be needed to activate the quality assurance programs in hospitals.

Kano 모델과 PCSI 지수 이용한 국방품질보증 서비스품질에 관한 연구 (A study on Service Quality of Defence Quality Assurance Activites using Kano Model & PCSI Index)

  • Purpose: Classify quality assurance work for munitions as service quality attributes. And to derive service elements that need improvement to improve customer satisfaction through calculation of customer satisfaction coefficient. Methods: The quality assurance service for munitions is classified as service quality based on the Kano model through a questionnaire survey of workers in military service companies located in Busan and Kyung-nam areas. Investigate current customer satisfaction with service quality of defense quality assurance activites, and calculate customer satisfaction coefficient and potential customer satisfaction improvement index. Results: Identify current satisfaction levels for service quality of defense quality assurance activites and suggest service quality factors that should be improved to improve customer satisfaction. Conclusion: The service quality factor for the defense quality assurance service was classified into 13, and the customer satisfaction coefficient was lower than the satisfaction coefficient. When looking at the current level of customer satisfaction through the Potential Customer Satisfaction Index, attractive quality factors with PCSI indices were found to be necessary for service improvement.

  • 이러닝 품질관리는 정보화 시대의 교육에 대한 책무성의 하나로 양적 팽창 보다는 질적 강화를 위한 기본 방침으로 이해되어야 한다. 이에 본 연구는 해외에서 이루어지고 있는 이러닝 품질관리 동향 조사를 통해 우리나라의 이러닝 품질관리가 어떤 형태로 진행되어야 하는지에 시사점을 제공하고자 하였다. 따라서 5개 국가 9개 기관의 이러닝 품질관리 현황, 영역 및 대상, 지표의 쓰임 등에 대해 알아보고, 5개국의 이러닝 품질관리 특징을 분석하였다. 분석결과, 이러닝을 위한 다양한 자원 확보, 국가 수준의 이러닝 품질관리 통합, 전문가 양성 그리고 품질관리 시스템에 대한 집중과 선택이라는 시사점을 찾을 수 있었다. 즉, 이러닝 품질관리는 업무의 측면이 아닌 교육의 질적 향상이라는 범 국가적 차원에서 접근할 필요가 있음을 알 수 있었다.

  • The Comprehensive quality assurance for radiation oncology provides an overall organizational structures, responsibilities, procedures, processes and resources for assuring the quality of patient management by radiation treatment. Superior performance of modern radiotherapy equipment will be essential part of quality assurance in radiation oncology, which high degree of accuracy and consistency should be maintained under the optimal quality assurance program. Besides quality control of all radiation equipment, this review also emphasizes quality assurance of clinical aspect such as adequacy of the medical decision-making which eventually leads to the treatment prescription, accuracy of treatment procedure from treatment preparation to radiation delivery, and the significance of assessment of treatment outcomes with structure and process.

공급자 부품품질 향상을 위한 품질보증시스템 개선방안 (Enhancement of Quality Assurance System for Improving the Quality of Suppliers' Product)

  • A prerequisite for improving the quality of a product is to enhance the quality of its parts supplied from outsourced enterprises. An enterprise certified by ISO 9001 Quality Management System is expected to develop their effective quality assurance system and maintain it through continuous control and improvement, thereby ensuring the good quality of products that they produce. However, it is reported that small and medium-sized enterprises certified by ISO 9001 have difficulty establishing an effective quality assurance system. This study aims to propose a new method of improving a quality assurance system that can be more effective in small and medium-sized enterprises. This study collectively considered requirements specified in several international standards on quality system and the results of expert group brainstorming to identify evaluation areas and items for assessing a quality assurance system. Based on these evaluation areas and items, a set of checklists for diagnosing and improving a quality assurance system was developed. In order to demonstrate the usefulness of the proposed method, this study applied it to ten small and medium-sized enterprises and could identify four approaches for improving their current quality assurance system. The presented improvement method, which consists of establishment of quality goals and quality innovation plan, defective products corrective activities, 4M and design change process, improvement of customer quality information management, is expected to be utilized as a useful tool for the enhancement of the quality assurance system of small and medium-sized enterprises.

병원 간호사의 질 관리 및 의료기관평가제도에 대한 인식과 간호업무성과와의 관계 (The Relationships among the Perception of Quality Assurance and National Hospital Evaluation Program and Nursing Performance of Hospital Nurses)

  • Purpose: The objective of this study was to identify relationships among the perception of quality assurance and national hospital evaluation program and nursing performance of hospital nurses. Method: The participants were 401 nurses who worked at the A medical center. The data were collected from 25 April to 9 May. 2008. The cross-sectional descriptive survey was done using a structured questionnaire. Result: The perception of the quality assurance correlated significantly with nursing performance. Also, the perception of national hospital program correlated significantly with nursing performance. However, the scores of perception of quality assurance & national evaluation program of hospital nurses is relatively low. Conclusions: These results means positive perception of quality assurance of nurses can produce good nursing performance. It suggests that nurses need more education and training for quality assurance. And the incentives will be needed to enhance the perception of quality assurance. Moreover, the detailed nursing performance indicators should be developed to measure the quality of nursing performance properly.

  • Many papers point out that the e-Learning is one of the most important industries, and the effect on other industries can be more powerful than any other business. Therefore, we think about social, cultural, industrial and technological effect of the e-Learning in order to enlarge industry scale as well as educational performances. In many cases of developed countries, various kinds of study have been performed for the e-Learning quality assurance because quality of the e-learning should operate on effective and efficient learning and continuous market development of education industries. The e-Learning quality assurance has import function not only for learning contents reusability like a SCORM and metadata but also for learning system, solution and service operation, so activities for the quality assurance should consider of cultural and tactical approach when it is applied in the e-learning business. In this paper, we present the concept, domain and purpose of the e-Learning quality assurance. Furthermore, this paper proposes the process and methodology in order to make the quality assurance standard model which is consist of 6 phase such as Environment Research, Needs Analysis, Framework, Metrics, Development and Implementation, Evaluation and Feedback through the analysis and comparison of pre-studied worldwide quality control, management and assurance documents.

  • Positron emission tomography-computed tomography (PET/CT) provides both functional and anatomical images of high quality non-invasively with better precision in localization than PET alone. Increase in the use of PET/CT, coupled with increasing concerns about the quality of medical services accrued the demands for accurate evaluation of system performance and quality assurance. Thus, well designed programs for performance evaluation and quality assurance are needed. Widely used protocols for performance evaluation of PET are the methods proposed by National Electrical Manufacturers Association (NEMA) in 1994 and 2001. In addition, in order to maintain high quality of PET/CT images, quality assurance programs including periodic (daily, monthly, and yearly). Therefore, in this article, the methods and present state of performance evaluation and quality assurance of PET/CT are reviewed.

A Pilot Study of the Scanning Beam Quality Assurance Using Machine Log Files in Proton Beam Therapy

  • The machine log files recorded by a scanning control unit in proton beam therapy system have been studied to be used as a quality assurance method of scanning beam deliveries. The accuracy of the data in the log files have been evaluated with a standard calibration beam scan pattern. The proton beam scan pattern has been delivered on a gafchromic film located at the isocenter plane of the proton beam treatment nozzle and found to agree within ${\pm}1.0mm$. The machine data accumulated for the scanning beam proton therapy of five different cases have been analyzed using a statistical method to estimate any systematic error in the data. The high-precision scanning beam log files in line scanning proton therapy system have been validated to be used for off-line scanning beam monitoring and thus as a patient-specific quality assurance method. The use of the machine log files for patient-specific quality assurance would simplify the quality assurance procedure with accurate scanning beam data.
