Arthroscopic Double Bundle ACL Reconstruction using Autogenous Quadriceps Tendon -Technical Note- (대퇴 사두건을 이용한 관절경적 이중 다발 전방 십자 인대 재건술)

  • Recently, the interest on anatomical ACL reconstruction with double bundle technique is increased to reproduce the original load distribution, and kinematics of the knee. We developed an arthroscopic double bundle ACL reconstruction technique using autogenous quadriceps tendon with 2 splitted graft and patellar bone plug. The anteromedial bundle and posteolateral bundle of the ACL is replicated with each splitted graft of quadriceps tendon and fixed with biodegradable interference screw on the 2 femoral tunnels. The patellar bone plug of quadriceps tendon is fixed with biodegradable interference screw within the 1 tibial tunnel. We suggest that our technique using quadriceps tendon may be an alternative in arthroscopic double bundle ACL reconstruction.

Comparative Study of Arthroscopic ACL Reconstruction Using Bone-Patellar Tendon-Bone Versus Four-Stranded Semitendinosus-Gracilis Tendon (관절경적 전방십자인대 재건술시에 골-슬개건-골과 네가닥 반건양건-박건을 이용한 방법의 비교연구)

  • This study is retrospective analysis of 31 patients treated by arthroscopically assisted ACL reconstruction, from September 1995 to September 1996. ACL reconstructions using autogenous bone patellar tendon bone (B-PT-B) were done in 18 patients, and using hamstring tendon were done in 13 patients. We used four-stranded hamstring tendon grafts and fixed the grafts using Endobutton and screw. The mean postoperative Lysholm knee score was 87.2 points in B-PT-B. group, and 89.0 points in hamstring tendon group. There were no clinically significant results between two groups in Telos test, quadriceps atrophy, Lachman test, anterior drawer test and pivot shift test, but the incidences of anterior knee pain were lower in hamstring group. Four-stranded hamstring tendon graft showed enough stability and good functional outcome similar to that of patellar tendon graft, and had an advantage of quicker return of quadriceps function and less donor site morbidity.

  • Simultaneous bilateral spontaneous rupture of the quadriceps tendon is a very rare condition and only a few cases have been reported in the literature. The etiology is not clear yet. But it occurs infrequently in patients with chronic metabolic disorders, such as secondary hyperparathyroidism due to chronic renal failure. We describe a case of simultaneous spontaneous bilateral quadriceps tendon tupture in a 36-year-old male patient with secondary hyperaprathyroidism due to chronic renal failure.

Bilateral Comparison of Effective Moment Arms of the Quadriceps force on Unilateral ACL-Reconstructed Individuals (전방십자인대 수술이 시행되어진 슬관절과 정상 슬관절의 유효 모멘트암 (effective moment arm) 비교 분석)

  • The purpose of this study is (a) to estimate effective moment arms of quadriceps forces and (b) to compare the $d_e$ between the ACL-reconstructed and uninjured knees from the same individual. One female (20 yrs old, 2 yrs post-op, hamstring tendon autograft) and two males (22 yrs old, 2 yrs post-op; 28 yrs old, 4 yrs post-op; Patellar tendon autografts for both). Sagittal view radiographs were obtained for 6-7 different angles $(range\;5^{\circ}-110^{\circ})$ from each knee. The do was determined by the method of Chow et al. (1999a). The results showed that the maximum de values ranged from 4.61 to 5.59cm and 4.59 to 4.89cm for the ACL-reconstructed and uninjured knees, respectively. The maximum $d_e$ occurred between $35^{\circ}\;and\;50^{\circ}\;and\;20^{\circ}\;and\;50^{\circ}$ for the ACL-reconstructed and uninjured knees, respectively. The minimum do values ranged from 4.12 to 4.35 cm and 3.12 to 3.63cm for the ACL-reconstructed and uninjured knees, respectively. The effective moment arm of the knee extensor affects the loads on knee ligaments during knee-extension exercises. Because apparent differences in the moment arm of the quadriceps in different participants, it is very important to use personalized knee joint geometry for the computation of knee joint force. In the present study, no noticeable bilateral difference was found in the male subjects. However, apparent bilateral differences in de were observed in the female subject. This suggests that the effects of ACL reconstruction surgery on patellar mechanism deserve further investigation.

  • Tendon reflex responses generated from mechanical stimulus had been studied for quantitative analysis of activity of tendon reflex, especially patellar tendon reflex responses are known to be a criterion for diagnosing the L3 or L4 radiculopathy. In this paper, we developed a digital signal processing system for analysis of the tendon reflex response. The system parameter, i.e., $\textit{sampling frequency, pre-amplification gain, input channel and filter bank}$ are selected by Using software switches. From the view points of flexibility, the system hardware is connected to an IBM PC for analyzing the tendon reflex parameters, amplitude, latency duration We applied the proposed system to the analysis of the patellar tendon reflex reponses. In the experiment, we measured latency, duration, amplitude of the reflex action potentials generated from vastus medialis, vastus lateralis and rectus femoris that compose quadriceps, and the measured data are analyzed througll the ANOVA test which has 5% significant level. As a result, we showed that the mean amplitude of reflex action potential at the vastus lateralis is larger than any other muscle and the mean latency of the reflex action potential at the rectus femoris is shorter than any other muscle.

  • Most analytical models of the human body have focused on conscious responses. A patellar tendon reflex, a representative example of spinal reflexes, occurs without a neural command. Muscle forces and activation of the quadriceps femoris muscles in healthy adults during patellar tendon reflex are identified in this study. The model is assumed to move in the sagittal plane, and the thigh and the trunk are assumed to be fixed in a sitting position so that the shank can move similar to a pendulum. The knee joint is modeled as a revolute joint, and the ankle joint is modeled as a fixed joint so that the shank and the foot can be regarded as one rigid body. Muscle forces are calculated following the inverse dynamic approach. Kinematic data obtained from an experiment (Mamizuka, 2007) are used as input data. Muscle activations are identified using a Hill-type muscle model. The obtained simulation results are compared with experimental results for validating the model and the underlying assumptions.

  • The all inside anterior cruciate ligament reconstruction technique places an anterior ligament substitutes within two bony sockets rather than hone tunnel. This approach is accomplished through arthroscopic three portal which avoids the surgical exposure and morbidity associated with creating traditional bone tunnel. This technique has several distinct advantages when compared with the traditional ACL reconstruction through the bone tunnels. It offers the surgeon a less morbid method for ACL reconstruction that positions an ACL substitute at the anatomic attachment sites of the original ACL with two bone sockets, obviating the need for traditional bone tunnels. Graft fixation at or near the anatomic attachment points of the original ACL minimizes creep with early range of motion and reduces the abrasive 'wind-shield wipe' motion of the graft which occur with bone plugs positioned inside bone tunnels. The sagittal posterior angle to the tibial socket increases fixation strength to pullout with anterior translation force for the tibia on the femur. This technique is not graft specific and can accomodate any graft in which graft length can be customized to the intraarticular native ACL length.

Clinical Results of Anatomical Single Bundle Anterior Cruciate Ligament Reconstruction Using Autogenous Quadriceps Tendon (자가 대퇴사두건을 이용한 해부학적 단일 다발 전방십자인대 재건술의 임상적 결과)

  • Purpose: The purpose of this study was to evaluate the clinical outcomes and stability of anatomical single bundle anterior cruciate ligament reconstruction (SBACLR) with quadriceps tendon comparable to double bundle anterior cruciate ligament reconstruction (DBACLR). Materials and Methods: We retrospectively reviewed 28 consecutive patients (16 male, 12 female) who underwent SBACLR using quadriceps tendon from March 2009 (Group 1) and compared its clinical results to whom DBACLR with semitendinosus tendon for 53 patients (51 male, 2 female) from August. 2006 (Group 2). Mean age were 34.9 (range, 16-52) in Group 1 and 21.6 (range, 17-55) in Group 2. Mean follow up periods were 13.9 months (range, 12-20) in Group 1 and 36.2 months (range, 20-52) in Group 2. Lysholm score, International Knee Documentation Committee (IKDC) evaluation form and Tegner score were performed for evaluating the clinical outcome. Lachman test, pivot shift test and KT-1000 arthrometer (MEDmetric Corp., San Diego, CA, USA) were performed for stability. Results: There was no statistical significant difference between the two groups in terms of Lysholm score (Group 1: $85.9{\pm}2.6$, Group 2: $90.9{\pm}1.0$, P=0.226), IKDC score (P=0.345) and Tegner score (Group 1: $6.9{\pm}1.4$, Group 2: $7.1{\pm}1.3$, P=0.523). Nor was there statistical significance between the two groups in terms of KT-1000 arthrometer (Group 1: $1.5{\pm}1.1\;mm$, Group 2: $1.5{\pm}1.6\;mm$, P=0.457), Lachman test (P=0.547) and pivot shift test (P=0.073). Conclusion: Anatomical SBACLR with quadriceps tendon shows similar clinical outcomes and stability comparable to anatomical DBACLR with hamstring tendon.
