Difference in Quadriceps and Hamstring torque during exercise with concentric and eccentric contractions according to hip joint anglet (등속성 근력측정 시 Hip Joint Angle에 따른 대퇴사두근과 햄스트링의 단축성과 신장성 근력 차이)

  • 본 연구의 목적은 등속성 운동수행 및 측정시 hip joint angle에 따라 대퇴근육의 concentric 과 eccentric 근력차이를 분석하기 위하여 등속성 기기 의자 등받이 경사각도를 $100^{\circ}$, $120^{\circ}$, $140^{\circ}$에서 peak torque를 검사하였다. 대상자는 20대 초반의 건강한 남자 대학생 12명을 대상으로, 등속성 기기(Isomed 2000, Germany)를 이용하여 seated position에서 $60^{\circ}/s$ 의 각속도를 사용하여 검사를 수행하였다. 측정순서는 등속성기기 의자 등받이 경사각도 $100^{\circ}$, $120^{\circ}$, $140^{\circ}$ 순으로 2-3일 간격으로 진행하였다. 통계처리 방법은 SPSS 12.0 프로그램을 이용하여 평균과 표준편차를 산출하였으며, 각각의 hip angle에 따른 단축성과 신장성 peak torque 값의 차이는 one-way ANOVA(3RM)을 이용하여 처리하였고, 사후검증은 Tukey를 사용하였으며, 유의수준은 p<.05로 하였다. 연구결과 대퇴사두근의 단축성 근력은 등속성 기기 의자 등받이 경사각 $100^{\circ}$, $120^{\circ}$, $140^{\circ}$사이에 유의한 차이(p=.000)가 나타나 사후 분석결과 $100^{\circ}$$120^{\circ}$ 사이(p<.01) 그리고 $100^{\circ}$$140^{\circ}$ 사이(p<.001)에 유의한 차이를 나타내었으며, 신장성 근력에서는 유의한 차이가 나타나지 않았다. 햄스트링의 단축성 근력은 3종류의 hip angle사이에 유의한 차이(p=.001)가 나타나 사후분석결과 $100^{\circ}$$140^{\circ}$사이(p<.05) 그리고 $120^{\circ}$$140^{\circ}$ 사이(p<.01)에 유의한 차이를 나타내었으며, 그러나 신장성 근력에서는 유의한 차이가 나타지 않았다. 이러한 결과로 미루어 볼 때 등속성 운동수행 및 근력측정 시 hip joint angle 에 따라 대퇴사두근과 햄스트링의 단축성 근력에서 유의한 차이가 나타나 hip joint angle을 고려하여 대퇴근육의 운동및 측정이 이루어져야 할 것으로 사료된다.

  • These studies show that I applied to functional insole (a specific S company) for minimizing shocks and sprain people's ankle arising from running. How to an effect on human body which studied a kinematics and kinetics from 10 college students during experiments. This study imposes several conditions by barefoot, normal running shoes and put functional insole shoes ran under average $2.0{\pm}0.24$ meter per second by motion analysis, ground reaction force and electromyography that used to specific A company. First of all, Motion analysis was caused by Achilles tendon angle, Angle of the lower leg, Angle of the knee, Initial sole angle and Barefoot angle. Second, Contact time, Vertical impact force peak timing, Vertical active force and Active force timing, and Maximum loading rate under impulse of first 20 percent and Value of total impulse caused Ground reaction force. Third. The tendon fo Quadriceps femoris, Biceps femoris, Tibialis anterior and gastronemius medials caused. electromyography. 1. Ground reaction force also showed that statically approximates other results from impact peak timing (p.001), Maximum loading rate(p<.001), Maximum loading rate timing (p<.001) and impulse of first 20 percent (p<.001). 2 Electromyography showed that averagely was distinguished from other factors, and did not show about that. Above experiment values known that there was statically difference between Motion analysis and Ground reaction force under absorbing of the functional insole shoes which was not have an effect on our body for kinetics and kinematics.

The Correlation Between Foot and Knee Posture Characteristics in Knee Osteoarthritis People (무릎관절 뼈관절염 환자의 발과 무릎관절 자세 특성간의 상관관계)

  • PURPOSE: This study investigated to correlation between foot and knee posture characteristics in knee osteoarthritis patients. METHODS: The subjects of this study were 103 patients with clinically and radiographically-confirmed knee osteoarthritis was investigated using the quadriceps angle (QA), distance of both knee (DBK), ankle dorsiflexion angle (ADA), navicular drop (ND). One-way ANOVA and Pearson's correlation were used to investigate the correlation between foot and knee posture measurements. All of whom agreed to participate in the study. In order to assure the statistical significance of the results, we used for SPSS ver. 18.0 for windows. RESULTS: The results of this study were as follows : 1) There were statistically significant difference in the correlation between QA and DBK. 2) There were statistically significant difference in the correlation between DBK and ADA. CONCLUSION: According the results of this study, patients with osteoarthritis exhibit more genu varus knee posture. Moreover patients with osteoarthritis were not correlation between foot and knee Posture characteristics.

The Effect of Spor ts Taping on Lower Extremity Muscles in Ver tical Jump (수직점프 시 스포츠 테이핑이 하지의 운동학적 변인에 미치는 영향)

  • The purpose of this study was to evaluate the effect of taping-tape with or without using spiral taping on vertical jump. The subjects for this study were about 20 years old healthy male college students without muscloskeletal diseases. Data for EMG activity and Ground Reaction Force(GRF) were estimated at three knee angles(i.e., 45, 90 & full degree). As a result, there was no statistical significance in max GRF at 90 and full degree regardless of spiral taping-tape. On the other hand, statistical significance was found when vertically jumping at 45 degree knee angle(p<.05). All the data for EMG activity at the three knee angles were not statistically significant, but there was a trend for a decrease in average EMG activity in elector spinae & Medial gastrocnemius at 90 degree knee angle. Based on these data, initial flexor action of knee was stabilized with spiral taping-tape when vertically jumping, resulting in improved muscular activity in Medial gastrocnemius. In conclusion, taping technique for jumping ability associated muscles like quadriceps is also required to develop.

Analysis on Differences in Dynamic Stability of Lower Extremity Caused by Unbalance of Hamstring/Quadriceps Ratio During Drop-landing (드롭랜딩 시 Hamstring/Quadriceps ratio 불균형에 따른 하지의 동적 안정성 차이 분석)

  • Objectives : The purpose of this study was to present quantitative data and basic references to decrease the accident risk of soccer instructors. Methods : To obtain data, we conducted an investigation on how H/Q ratio affects the dynamic stability of the lower extremity at the time of drop landing. The study targeted 13 soccer players from C University who have not had any injuries or wounds in the lower extremity joints and in any other parts of their bodies over the last 6 months. By using CMIS (USA), the players were divided into two groups according to H/Q ratios higher and lower than 69%, respectively. The subjects in each group were instructed to perform a drop landing. Results : The H/Q ratio did not affect the maximal flexion angle of the knee joints at the time of drop landing. In addition the dominant group with a relatively high H/Q ratio was observed to have increased time to reduce shock and to efficiently absorb the ground reaction force during drop landing. Also, the dominant group with a relatively high H/Q ratio utilized the strong performances of the antagonistic muscles around the hamstrings and the controlled rotatory powers of the thighs that were applied to the tibias supported by the ground. Finally, H/Q ratio, load factors, and mean and maximum EMG were significantly negatively related, whereas GRFx showed a positive relationship. In fact, these factors all affected the impact of the load from the H/Q ratio to the knee joints. Conclusion : From these findings it can be concluded that unbalanced H/Q ratio can be considered as a predictor of knee joint injury at the time of drop landing.

Effects of Muscle Activity and Muscle Strength According to Verbal Command Volume in Isokinetic and Isometric Exercise of Quadriceps (넙다리네갈래근의 등속성 및 등척성 운동에서 구두명령 크기에 따른 근 활성도와 근력에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of muscle activity and muscle strength according to verbal command volume during isokinetic and isometric quadricep exercises. Methods: To measure muscle activity and muscle strength, surface electrodes were attached to the participants, as they sat on a Biodex chair. The isometric exercise was performed three times, with maximum exercise at $30^{\circ}$ bending angle, based on a maximum extension state of the knee at $0^{\circ}$. The average holding time was unified to three seconds. In addition, the isokinetic exercise was performed three times, at $60^{\circ}/sec$. The verbal command ranged between 0∾60 dB and 0∾75 dB. Muscle activity was measured using surface electromyography (4D-MT, Relive, Gimhae, Korea). The Biodex System 4 was used to measure the isometric and isokinetic strength of the nodal line, and 4D-MT was used to measure muscle activity. Results: There were significant improvements in the maximal and relative muscle strengths, when the 0∾ 60 dB and 0∾75 dB verbal commands were applied with isokinetic extension/flexion (p<.05). The isokinetic exercise (0∾75 dB) group showed a significant difference in the vastus medialis oblique muscle activity change (p<.05), while the isometric exercise (0∾75 dB) group showed a significant difference in the rectus femoris muscle activity change (p<.05). Conclusions: Our results reveal that verbal commands effectively improve muscle activity and muscle strength during isokinetic and isometric quadricep exercises.

Clinical Study on Safety, Clinical Indicators of Polydioxanone Sutures Inserted into Vastus Medialis Muscle in Degenerative Knee Osteoarthritis (무릎 관절염 환자에서 안쪽넓은근에 폴리디옥사논 봉합사 시술 연구)

  • Objective: Physiologically, the vastus medialis muscle is the first muscle to undergo muscle atrophy, and it was thought that pain in patients with knee osteoarthritis could be reduced if this muscle could be strengthened and stabilized. The purpose of this study was to prove the effectiveness in knee osteoarthritis using polydioxanone sutures that have been tried in other musculoskeletal areas. Method: Forty knee osteoarthritis patients voluntarily participated in the study, and divided into 30 polydioxanone suture needle (MEST-B2375 produced by Ovmedi Co.) and 10 sham needle (without suture). And the needles were inserted into the vastus medialis muscle. In all patients, safety evaluation including blood tests and ultrasonography as well as efficacy evaluation including isometric maximal contractile strength of quadriceps muscle, weight bearing pain, impression of change, quadriceps angle, rescue drug intake were evaluated up to 30 weeks after the procedure. Results: Isometric maximal contractile strength showed a significant improvement at 4 weeks after the procedure in the polydioxanone suture group, and the weight-bearing pain showed a significant improvement at every visit in the polydioxanone suture group compared with baseline values. Patient global impression of change score showed significant improvement at 20 and 30 weeks, and clinical score showed improvement at every visit. Conclusion: Insertion of polydioxanone sutures showed improvement in muscle strength and knee pain by supporting and fixation of the vastus medialis muscle in patients with degenerative knee osteoarthritis. Insertion of polydioxanone sutures is considered to have a therapeutic effect in knee osteoarthritis patients.

  • Background: Investigation in gender differences of kinetics and kinematics for individuals with patellar femoral pain syndrome (PFPS) was not sufficiently performed. Objects: The purpose of this study is that whether there is a difference depending on gender from muscle activity and strength and knee valgus angle during controlled single-leg squat which is widely used as clinical movement test for the patient with PFPS. Methods: 20 young adults (10 men, $20.0\pm}2.1$years; 10 women, $20.4{\pm}2.1$years) with PFPS were voluntarily recruited in this study. Muscle activity and strength and knee valgus angle were collected during single-leg squat. Independent t-test and Mann-Whitney test were used to compare the differences between groups of male and female. Results: Rectus femoris (t=-2.204, p=.041) and vastus medialis oblique (t=-2.151, p=.045) muscle activity of women were significantly higher than male group. Normalized muscle strength of hip and knee muscles showed a significant difference between men and women (p<.05). Valgus angle of the knee in women (t=-2.450, p=.025) were increased significantly than men. Conclusion: The therapist would consider the characteristics of these gender differences during performing movement test, exercise, and education for the individuals with PFPS.

Gait Analysis of a Pediatric-Patient with Femoral Nerve Injury : A Case Study (대퇴신경 손상 환아의 보행분석 : 사례연구)

  • The femoral nerve innervates the quadriceps muscles and its dermatome supplies anteromedial thigh and medial foot. Paralysis of the quadriceps muscles due to the injury of the femoral nerve results in disability of the knee joint extension and loss of sensory of the thigh. A child could walk independently even though he had injured his femoral nerve severely due to the penetrating wound in the medial thigh. We measured and analyzed his gait performance in order to find the mechanisms that enabled him to walk independently. The child was eleven-year-old boy and he could not extend his knee voluntarily at all during a month after the injury. His gait analysis was performed five times (GA1~GA5) for sixteen months. His temporal-spatial parameters were not significantly different after the GA2 or GA3 test, and significant asymmetry was not observed except the single support time in GA1 results. The Lower limb joint angles in affected side had large differences in GA1 compared with the normal normative patterns. There were little knee joint flexion and extension motion during the stance phase in GA1 The maximum ankle plantar/dorsi flexion angles and the maximum knee extension angles were different from the normal values in the sound side. Asymmetries of the joint angles were analyzed by using the peak values. Significant asymmetries were found in GA1with seven parameters (ankle: peak planter flexion angle in stance phase, range of motion; ROM, knee: peak flexion angles during both stance and swing phase, ROM, hip: peak extension angle, ROM) while only two parameters (maximum hip extension angle and ROM of hip joint) had significant differences in GA5. The mid-stance valleys were not observed in both right and left sides of vertical ground reaction force (GRF) in the GA1, GA2. The loading response peak was far larger than the terminal stance peak of vertical ground reaction curve in the affected side of the GA3, GA4, GA5. The measured joint moment curves of the GA1, GA2, GA3 had large deviations and all of kinetic results had differences with the normal patterns. EMG signals described an absence of the rectus femoris muscle activity in the GA1 and GA2 (affected side). The EMG signals were detected in the GA3 and GA4 but their patterns were not normal yet, then their normal patterns were detected in the GA5. Through these following gait analysis of a child who had selective injuries on the knee extensor muscles, we could verify the actual functions of the knee extensor muscles during gait, and we also could observe his recovery and asymmetry with quantitative data during his rehabilitation.

The Effect of Sling Exercise on Muscular Strength and Range of Motion in Female Patients who Received Total Knee Replacement (슬링 운동이 무릎관절 전치환술을 시행한 여성 환자의 근력과 관절가동범위에 미치는 영향)

  • The purpose of study was to compare the effectiveness of sling exercise on the muscle strength and range of motion in female patients who received a total knee replacement. The participants were allocated randomly into 2 groups: sling exercise group (n=15) and control group (n=15). The subjects were evaluated using the Biodex system for the muscle strength test and a goniometer for the range of motion test. The data was analyzed using a paired t-test and independent t-test to determine the statistical significance. As a result, the sling exercise group before and after intervention showed a statistical significance difference in the flexion angle, quadriceps femoris, and hamstring muscle strength. The control group before and after the intervention revealed a statistically significant increase in the flexion & extension angle, quadriceps femoris, and hamstring muscle strength. Muscle strength test and flexion range of motion test in the sling exercise group showed statistical significance differences compared to the control group (p<.05). Therefore, the sling exercise group has a positive influence on the muscle strength and ROM in patients with a total knee replacement.
