Purpose: The purpose of this study was to describe quality of life in school age children and the relationship between quality of life and parenting attitude as perceived by children and parent-child communication pattern. Methods: Data were collected between May 26 and June 3, 2009 using self-report questionnaires from 148 pairs of children and their parents. The children were enrolled in 4th to 6th grades. The questionnaires included the Parental Bonding Instrument (PBI), Parent-Child Communication (PCC) Scale, and Pediatric Quality of Life Inventory (PedsQL). The data were analyzed using the SPSS WIN 17.0 program. Results: The Quality of Life (QOL) score for the children was significantly higher among children reporting higher parental bonding attitude (mother r=.38, father r=.34) and parent-child communication (mother r=.43, father r=.36). Results of multiple regression showed that factors significantly associated with QOL were academic achievement, caregiver available after school, parent-child communication, school satisfaction, relationship with peers, and birth order among siblings. Conclusion: Positive relationships and good communication with parents and satisfaction with school life were important factors associated with quality of life in school age children.
본 연구는 연하장애 환자의 삶의 질 향상을 목적으로 연하장애 환자의 우울, 사회적 고립감, 생의 의미와 삶의 질수준을 파악하고, 삶의 질의 영향요인을 확인하고자 시도되었다. 연구대상은 서울, 인천, 경기 지역에 소재한 종합병원과 재활병원 등 8개 기관에서 뇌졸중, 퇴행성 신경계 질환으로 인하여 연하장애가 발생한 환자 총87명을 대상으로 하였다. 자료수집기간은 2015년 2월부터 4월까지였으며, 연구도구는 CES-D, RULS, PIL, SWAL-QOL을 사용하였다. 수집된 자료는 SPSS 22.0 통계프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson's correlation, Stepwise multiple regression으로 분석하였다. 연구 결과 우울이 있는 군은 대상자의 20.7%였고, 보통 이상의 사회적 고립감을 느끼는 군 92.0%, 실존적 공허상태에 있는 군이 64.4% 였다. 즉, 대다수의 대상자들이 사회적 고립감을 경험하고 실존적 공허상태에 있었으며, 삶의 질의 평균 점수는 158.89(35.97)점이었다. 연하장애 환자의 삶의 질 영향요인을 확인한 결과, 경관급식의 경우가 정상식이보다 삶의 질이 유의하게 낮았고(${\beta}=-0.57$, p<.001), 생의 의미가 낮아 실존적 공허상태에 있는 경우(${\beta}=0.19$ p=.014)가 삶의 질이 낮았다. 30-49세의 연령은 80세 이상보다 삶의 질이 높았으며(${\beta}=0.26$, p=.001), 대학교 수준 이상의 교육수준이 무학에 비해 삶의 질이 높았다(${\beta}=0.17$, p=.032). 이 중 연하장애 환자의 삶의 질에 가장 많은 영향을 주는 변수는 경관급식이었고 (${\beta}=-0.57$, p<.001), 이러한 4개의 변수는 연하장애 환자의 삶의 질에 대해 50.7%(F=28.84 p=.031)의 설명력을 나타냈다. 그러므로 연하장애 환자의 삶의 질을 높이기 위해 생의 의미를 높일 수 있는 중재 프로그램의 개발과 함께, 경관급식 환자의 삶의 질이나 정서 상태를 확인할 수 있는 후속연구가 필요하다.
Objectives: This study was performed to determine the levels of quality of life (QOL) according to the grade of long-term care service for the elderly people who were admitted from long-term care insurance, and to reveal its association with the physical and mental functioning such as the Activity of Daily Living (ADL), the Instrumental Activity of Daily Living (IADL), the Center for Epidemiologic Studies Depression Scale (CES-D), and the Mini-Mental State Examination-Korean version (MMSE-K). Methods: The interviews were performed during the period from March 1 to May 31, 2009, for 958 elderly people in urban and rural areas. The questionnaire items included various indices such as the ADL, IADL, CES-D, and MMSE-K, as independent variables and the index of QOL, as the dependent ones. For statistical analysis, t-tests were used for the mean scores of QOL according to gender and the grade of long-term care services, and Spearman's correlation was used for each variable. The effects of physical and mental functioning for QOL were assessed by covariance structure analysis. The statistical significance was set at p<0.05. Results: The mean scores of QOL among all the subjects was $55.4{\pm}15.62$ (Grade I: $49.7{\pm}14.17$, Grade II: $56.8{\pm}14.62$, Grade III: $59.4{\pm}16.36$), and it was lower according to the higher grade of long-term care insurance. In terms of the correlation matrix of the QOL and the physical and mental function factors, the QOL showed positive correlation with the ADL, IADL and MMSE-K, while it had negative correlation with depression. On the analysis of covariance, mental functioning (depression and the MMSE-K) had a greater influence on the level of QOL than the physical functioning (ADL and IADL). Conclusions: The level of the QOL in the elderly people who were admitted from long-term care insurance was lower according to higher the grade of long-term care insurance. Also, the mental functioning (depression and MMSE-K) was more influential on the level of the QOL than the physical functioning (ADL and IADL).

비만이 청소년에 미칠 수 있는 사회 심리적 영향을 평가하기 위하여 471명의 중학생에서의 체 질량 지수와 삶의 질의 관계를 알아보았다. 연구 참여 학생 중 우울 위해 CDI검사를 성향을 보이는 학생들을 배제하기 실시하였고 삶의 질을 평가하기 위한 도구로 $PedsQL^{TM}$ 4.0을 사용하였다. 체 질량 지수를 기준으로 저 체중 군, 정상 체중 군, 비만위험 군으로 나누어 삶의 질을 비교하였다. 중학생에서의 체 질량 지수는 삶의 질과 관계가 있으며 비만 중학생들의 경우 정상 체중 중학생들에 비해 자신의 삶의 질을 낮게 평가한다.
Objective : Atopic dermatitis (AD) may profoundly affect patient's quality of life (QOL), and also cause economic impact. The aim of our study was to evaluate the quality of life and the economic impact of adult atopic dermatitis patient in Seoul. We also evaluate the relationship of QOL and economic impact with severity of AD. Methods : 30 adult atopic dermatitis patient were included and evaluated by using the SCORAD Index and EASI. Patients were asked to fill in the questionnaires about their quality of life and financial costs during the past year by AD. Data about sleep disturbance and pruritus were also obtained. Pearson's correlation were used for statistical analysis. Results : 1. Among 30 patients, women were 19(63.3%), men were 11(36.7%). The mean age of the patients were 27.3 years old, patients between the ages of 17 and 30 years were 23, over 30 were seven. 2. The mean score of Objective SCORAD was $32.89{\pm}7.30$, Subjective SCORAD was $8.13{\pm}3.53$ and EASI was $9.15{\pm}6.90$이었다. 3. The mean score of Skindex-29 was $28.26{\pm}7.58$, DLQI was $10.17{\pm}5.55$. 4. By analyzing the questionnaire, a monthly average cost of 583,200 won for each patient was determined. Direct cost was 236,800 won and indirect cost was 346,300 won. 5. By analyzing the correlation between the severity of AD and QOL, Objective SCORAD and EASI were positively correlated with QOL(Skindex-29, DLQI) but not significant, meanwhile subjective SCORAD were significantly and positively correlated with QOL(Skindex-29, DLQI). 6. By analyzing the correlation between the severity of AD and economic impact, Objective SCORAD were significantly and positively correlated with direct cost and indirect cost. Also, EASI were significantly and positively correlated with direct cost and oriental medical hospital visits. Conclusions : The above results show that the QOL of the patients with atopic dermatitis is significantly related to their disease severity. Atopic dermatitis patients pay 583,200 won a month, and the economic impact of the patients is significantly related to their disease severity.
Purpose: The purpose of this study is to identify factors affecting the quality of life among community-dwelling older women with urinary incontinence (UI). Methods: A cross-sectional study was conducted with 475 women aged 60 or over who were recruited from 10 primary health care facilities in rural Korea. Data were collected using a structured questionnaire consisting of socio-demographic, health-related, and UI-related characteristics. The quality of life was assessed using Incontinence Quality of Life (I-QOL). SPSS/WIN 23.0 program was used to analyze descriptive statistics, $x^2$ test, t-test, ANOVA, Pearson's Correlation, and hierarchical regression. Results: Of 475 subjects, 180 (37.9%) had urinary incontinence. The mean scores of I-QOL of women with and without UI were 76.87 and 94.77, respectively. The results of hierarchical regression analysis show that the Questionnaire for Urinary Incontinence Diagnosis total score was the greatest influencing factor, followed by age and the International Consultation on Incontinence Questionnaire-Short Form total score. Conclusion: The study revealed that factors related to UI symptoms are more likely to have impact on the quality of life in women with UI. It suggests that early detection or management of UI is important in improving the quality of life of women with UI.
본 연구는 삼킴장애 환자를 대상으로 한국판 SWAL-QOL의 신뢰도와 타당도를 알아보기 위해 실시되었다. 비디오 투시 영상을 통해 삼킴장애로 진단받은 환자 71명과 일반인 80명을 대상으로 하였다. 한국판 SWAL-QOL의 문항내적일치도는 Cronbach's $\alpha$ 값이 .86~.96이었고, 급간내상관계수는 .80~.93으로 신뢰도가 높았다. 한국판 SWAL-QOL의 각 척도간 상관관계를 알아본 결과 피어슨 상관계수는 .17~.74로 모든 척도간에 유의한 상관성을 보였다(p<.05). 삼킴장애군과 정상군간에 한국판 SWAL-QOL을 비교한 결과 모든 척도에서 정상군의 점수가 높았으며 두 그룹간에 유의미한 차이가 있는 것으로 나타났다(p<.01). 관 식이 그룹은 관 식이를 하지 않은 그룹에 비해 모든 척도에서 점수가 낮았으며 수면척도를 제외하고 모든 척도에서 유의한 차이가 있었다(p<.05). 식이단계에 따른 차이를 살펴보면 식이 4단계 환자의 점수가 가장 높았으며, 식사욕구, 의사소통, 두려움 척도를 제외한 모든 척도에서 식이 단계간에 통계적으로 유의한 차이가 있었다(p<.05). 따라서 한국판 SWAL-QOL는 신뢰도와 임상타당도가 높기 때문에 임상에서 삼킴장애 환자의 삶의 질을 측정하는 도구로 사용되는데 적절한 것으로 판단된다.
Purpose : The aim of this study was to compare shift satisfaction, sleep, fatigue, quality of life (QOL), and patient safety incidents between a newly implemented two-shift system and a traditional three-shift system. Methods : A total of 127 intensive care unit nurses (48 two-shift nurses and 79 three-shift nurses) working in a tertiary hospital in Seoul were recruited from January 1, 2017, to March 31, 2017. They completed a self-reported questionnaire about their work hours, shift satisfaction, sleep patterns, sleep quality, fatigue, QOL, and patient safety incidents in the past 2 weeks. Data were analyzed using SPSS version 23.0. Results : The two-shift group showed higher shift satisfaction scores compared with the three-shift group (6.93 vs. 4.37, p<.001). Sleep latency was shorter and sleep quality was better in the two-shift group compared with the three-shift group. There were no significant differences in other sleep parameters, fatigue, QOL, and patient safety incidents between the two groups. Conclusion : Although a two-shift system did not improve nurses' fatigue or QOL in this study, it may effectively serve as an alternative shift-work system that can increase sleep quality and shift satisfaction without increasing patient safety incidents.
Objective : Only a few studies have investigated the life expectance and health related quality of life (HRQOL) about stroke patients. The purpose of this study is to analyze the life expectancy, preference based quality of life(QOL) and quality adjusted life years(QALYs) of stroke patients. Methods : We used data of 10,533 adults from 4th Korean national health and nutritional examination survey 2009 for evaluating HRQOL of stroke patients. We also analyzed the life expectancy for stroke patients using life table from national public health data. Finally we calculated the QALYs with and without stroke conditions and assumed the difference of QALYs. Results : The mean age of stroke patients was assumed to be 65. Lower income and less educated groups were prone to be exposed to the stroke conditions. Common comorbidities of stroke patients were ischemic heart attack, hypertension, diabetes and hyperlipidemia. The proportions of participants who reported problems in each of the five EQ-5D dimensions increased significantly at chronic stroke group. Participants with chronic stroke conditions had an almost 6-fold higher risk of impaired health utility(the lowest quartile of EQ-5D utility score) compared with non stroke participants, after adjustment of age, gender, income, education, comorbidity variables. The differences of life expectancy and QALYs between non-stroke and stroke group from the age of 65 till death were assumed to be 0.767 year and 3.103 QALYs. Conclusions : Although the authors analyzed the affecting factors of QOL and assumed the differences of life expectance and QALYs about stroke patients using domestic national data and statistic references, well designed cohort studies should be needed to prove the causal effects of affecting factors and to assume more correct QALY differences.
목 적 : 본 연구에서는 OSA환자의 임상적 특징과 CPAP 사용 전·후의 삶의 질 정도를 비교하여 CPAP 사용에 따른 삶의 질의 개선 정도를 알아보고자 하였다. 방 법 : 진료기록부를 통한 나이, 성별, 신장, 체중, 체질량지수, 앱워스 주간졸림척도, 수정된 말람파티척도, 한국판 몬트리올 인지평가, 피츠버그 수면의 질 지수를 조사하여 분석하였다. CPAP 사용에 따른 삶의 질의 개선정도를 알아보기 위해 연구대상자에게 전화 연락을 취하여 CPAP 사용 전·후의 삶의 질에 대해 시각형 아날로그 척도(VAS)를 이용하여 조사하였다. 결 과 : 신장(Z = -4.525, p < 0.001), 체중(Z = -2.844, p < 0.05), 수면의 질(Z = -2.671, p < 0.05)과 각성 지수(Z = -2.105, p < 0.05)에서 통계학적으로 남·녀 간에 차이(p < 0.05)가 있으며 나머지 변수에서는 차이가 없음이 확인되었다. 교차분석에서 χ2 = 7.724, p = 0.024로 p < 0.05보다 작으므로 OSA의 수준별 심각도와 성별 간에 차이가 있음이 확인이 되었다. PreCPAP QOL, PostCPAP QOL, CPAPUse Months, CPAP4Hr/d (%)의 OSA의 수준별 심각도에 따른 크기에는 통계적으로 유의미한 순서가 존재하지 않는 것으로 확인되었다(p > 0.05). CPAP 사용 전 AHI와 사용 후 AHI의 차이가 36.48 ± 21.54 (t = 11.609, p < 0.001)이었고, CPAP 사용 전 QOL과 사용 후 QOL의 차이가 -25.43 ± 22.06 (t = -7.901, p < 0.001)이었으며 통계적으로 의미 있는 차이(p < 0.001)를 보였다. 결 론 : OSA환자 중 남녀간 임상적으로 신장(HT), 체중(BW), 수면의 질(PSQI), 각성지수(AI), OSA의 수준별 심각도에서 남녀 간의 차이가 있었다. 그러나 CPAP전·후에 삶의 질은 남녀간 차이가 없었다. 또한 OSA환자에서 CPAP 사용 후 삶의 질이 개선되었음을 확인하였다.
