A Study on the Nurses' Thinking Skills and Problem Solving Abilities for Quality Management in Healthcare Services (질 관리에 대한 간호사의 사고유형과 문제해결력)

  • Background : There have been many studies and efforts about quality management in health services, but these were not focused on the thinking ability of the personnel. In this study, We planned to define the relationship between Quality Management(QM) and thinking skill, and analyzed critical thinking skill and problem solving ability of the nurses for QM in healthcare. Methods : We surveyed 260 nurses in 3 hospitals by using questionnaires about thinking skills and QM from November 18 to 28, 1998. The response rate was 76.9%. We described and analyzed the data by descriptive statistics, t-test, ANOVA, Pearson correlation, and non-parametric Friedman test using SPSS Win 7.5. Results : This study revealed that 1) understanding level of QM concepts was very low and insufficient, 2) willingness to practice QM in healthcare was relatively high and affirmative, but the positive attitude taking part in the program of QM is very low(4.5%), 3) even though the necessity of QM was recognized, actual application of critical thinking skills and problem solving abilities is lacking, 4) the production of idea for QM was the most insufficient, and so the next of the problem solving process(QA cycle) was the same. Conclusion : Based on the above results, we can conclude that understanding the concepts of QM is very important to solve problems about QM in healthcare. For development of thinking skill to promote critical problem solving ability for QM in healthcare, it is necessary to improve curriculum of nursing education and teaching-learning methods of QM strategy and technique.

Field measurements of wind pressure on an open roof during Typhoons HaiKui and SuLi

  • Full-scale measurements of wind action on the open roof structure of the WuXi grand theater, which is composed of eight large-span free-form leaf-shaped space trusses with the largest span of 76.79 m, were conducted during the passage of Typhoons HaiKui and SuLi. The wind pressure field data were continuously and simultaneously monitored using a wind pressure monitoring system installed on the roof structure during the typhoons. A detailed analysis of the field data was performed to investigate the characteristics of the fluctuating wind pressure on the open roof, such as the wind pressure spectrum, spatial correlation coefficients, peak wind pressures and non-Gaussian wind pressure characteristics, under typhoon conditions. Three classical methods were used to calculate the peak factors of the wind pressure on the open roof, and the suggested design method and peak factors were given. The non-Gaussianity of the wind pressure was discussed in terms of the third and fourth statistical moments of the measured wind pressure, and the corresponding indication of the non-Gaussianity on the open roof was proposed. The result shows that there were large pulses in the time-histories of the measured wind pressure on Roof A2 in the field. The spatial correlation of the wind pressures on roof A2 between the upper surface and lower surface is very weak. When the skewness is larger than 0.3 and the kurtosis is larger than 3.7, the wind pressure time series on roof A2 can be taken as a non-Gaussian distribution, and the other series can be taken as a Gaussian distribution.

Saving Effects Cost and Time in Nursing through Improving Sterilization Method of Inner Cannula (기관내관 소독 방법 개선에 따른 간호업무 시간단축 및 비용 절감 효과)

  • Background : The purpose of inner cannula is to protect the upper air way and permit air to pass freely, in addition, to provide endotracheal suction, artificial respiration and to maintain adequate oxygen saturation. The tube needs to be sterilized for maintenance and cleanness of air way and for prevention of bronchospasm. However, it has been reported that there is no guideline for sterilization and many hospitals conduct their own sterilization methods, for example, once a day(13's general hospital), three times a day(The Catholic University of Korea ST Mary's hospital) or even no cleansing. Consequently, the QI team of our hospital suggested the SOP(standard operating procedure) of sterilization and evaluate cost and time effect in nursing. Method : 1) Benchmarking of 13's neurosurgery department of general hospital in Seoul 2) Investigation of test records of sputum culture from patients with intubation for tracheotomy 3) Check of results of O2 Sat. monitoring to confirm of maintaining opened air way Result : 1) Improvement of process: decrease of excess sterilization of inner cannula (from 3 times a day to once a day) 2) Cost effects: saving over 10 million won per one year 3) Providing better nursing: time effects (30 min a day) permit to conduct more nursing activities Conclusion : It can get Cost and time effects in nursing with improved sterilization method of inner cannula. It needs to do research on improvement of the monthly exchange protocol of outer cannula and provide supporting data for the proper exchange schedule. The result of additional microorganism detection from patients with new process needs to be evaluated further more.

The Literature Study on the Efficacy and Manufacturing Process of Gyeongoggo (경옥고 효능 및 제법에 대한 문헌고찰)

  • Gyeongoggo is first described in the Collected Prescription by Hong Family in the Song Dynasty in China. It is composed of Radix Rehmnniae, Panax ginseng, Poria cocos, and Mel. Its main efficacy is to treat weakness of primordial essence of body and dry cough, and to invigorate qi and replenish yin principle. It is one of the most important prescriptions that people have been using for a long time. We studied the documents recorded in the medical classics and comprehended the following results. Gyeongoggo has efficacy to keep a person healthy and live long age, to treat amnesia and dizziness from brain weakness, to strengthen muscle and bone by improving function of stomach and colon, to improve a person's memory and judgement, to invigorate brain weakness, and, to treat tuberculosis and lung cancer. The longer a person take it, the better it is for one's health and meditation. When it is made, it is important to mix four components up, to boil it with an oak tree for three days and nights, and then to add water from a well to reduce heat for a full day, and to boil up again for a full day to mature fully. As gyeongoggo is acquired not only by the full heart of a manufacturer but also the sympathy of nature, it is important to choose a clean place to make and keep. When it is taken, it is proper to take it with warm water or liquors. And when it is made, we came to know that it is possible to make gyeongoggo with special efficacy by adding one to three more components.

DNA Repair Gene Polymorphisms Do Not Predict Response to Radiotherapy-Based Multimodality Treatment of Patients with Rectal Cancer: a Meta-analysis

  • Background: A number of association studies have been carried out to investigate the relationship between genetic polymorphisms in DNA repair genes and response to radiotherapy-based multimodality treatment of patients with rectal cancer. However, their conclusions were inconsistent. The objective of the present study was to assess the role of DNA repair gene genetic polymorphisms in predicting genetic biomarkers of the response in rectal cancer patients treated with neoadjuvant chemoradiation. Materials and Methods: Studies were retrieved by searching the PubMed database, Cochrane Library, Embase, and ISI Web of Knowledge. We conducted a meta-analysis to evaluate the association between genetic polymorphisms and the response in rectal cancer treated with neoadjuvant chemoradiation by checking odds ratios (ORs) and 95% confidence intervals (CIs). Results: Data were extracted from 5 clinical studies for this meta-analysis. The results showed that XRCC1 RS25487, XRCC1 RS179978, XRCC3 RS861539, ERCC1 RS11615 and ERCC2 RS13181 were not associated with the response in the radiotherapy-based multimodality treatment of patients with rectal cancer (p>0.05). Conclusions: This study shows that DNA repair gene common genetic polymorphisms are not significantly correlated with the radiotherapy-based multimodality treatment in rectal cancer patients.

Predictive and Prognostic Biomarkers for Patients Treated with Anti-EGFR Agents in Lung Cancer: A Systemic Review and Meta-Analysis

  • Background: Several studies have investigated predictive and prognostic biomarkers for patients treated with anti-epidermal growth factor receptor (EGFR) agents in lung cancer. However, the conclusion is controversial. Materials and Methods: A meta-analysis was conducted to evaluate the associations of mutant K-ras, PIK3CA and PTEN deficiency with the efficacy of anti-EGFR agents in lung cancer. The primary endpoint was objective response rate (ORR). The secondary endpoints were overall survival (OS) and progression-free survival (PFS). Results: A total of 61 studies were included in the final meta-analysis. The result showed that K-ras mutation was a good predictor for ORR (RR=0.42, 95%CI, 0.33-0.55, p=0.000) and an effective prognostic marker for OS (HR=1.37, 95%CI, 1.15-1.65, p=0.001) and PFS (HR=1.33, 95%CI, 1.05-1.69, p=0.019). However, PTEN deficiency or PIK3CA mutation did not show any significance predictive value for ORR (PTEN, RR=0.82, 95%CI, 0.56-1.19, p=0.286; PIK3CA, RR=1.08, 95%CI, 0.17-6.66, P=0.938). And PTEN deficiency or expression of PIK3CA did not show significance prognostic value for OS (PTEN, HR=0.88, 95%CI, 0.31-2.46,P=0.805; PIK3CA, HR=0.79, 95%CI: 0.23-2.68, P=0.706). Conclusions: Our meta-analysis showed that K-ras mutation may be an effective predictor in lung cancer patients treated with anti-EGFR agents. Whereas, the predictive and prognostic value of PTEN deficiency and PIK3CA mutation need to be further investigated.

The Study on the Characteristic of Physical Touch in Caring Situation (돌봄상황에서 신체적 접촉의 특성에 관한 연구)

  • This study was performed to investigate the characteristic of physical touch in caring situation. The subjects of this study were adults in caring situation, therefore they were composed of 7 patients, 6 nurses, 3 doctors, 3 pharmacologists, 3 men in paramedicine area, 3 nursing educators and 13 normal general adults. The datas were gathered through nonstructured questionaire from June, 20th to September, 15th in 1995. The datas were subjects' descriptions about the intention, perception, and form of physical touch in caring situation and analysed by content analysis. The results as follow : The datas were divided into four areas. There were the situation being necessiated the physical touch, meaning of physical touch, form of physical touch and perception about physical touch in caring situation. The situation being necessiated physical touch were the situation that required physical treatment, to deliver the active expression of concern about the patients, to determine the treatment due to the identification of physical condition of patients and to induce the psychological eqilibrium into patient's mind. The meanings of physical touch in caring situation were - Good meanings that intention is to encourage, to be have hope about health and to deliver the affection willing to help patient. - therapeutic methods that were to facillitate the circulation of blood, to reduce the pain perception and to facillitate the circulation of qi. - interpersonal affectionate relation that the intentions were to deliver the understanding of patient's pain, were to delivery the meaning to the patient not be alone. The forms of physical touch in caring situation were none invasive forms just like laying hands on hand, head, shoulder, gentle knocking on the shoulder or back, massage of legs and back and finger pressure on acupuncture points. The perception of physical touch in caring situation divided into two parts. In family, the perceptions of physical touch in caring situation were to promote health status because physical touch induce the psychological peace, and to evoke the importance of relationship among family members. In relation with care giver, perception of physical touch in caring situation were inevitable process in treatment, and to deliver the trust and concern about patients.

Yoon Gil-Young's "A Study on the Methodology of Traditional Korean Medical Physiology" : Review from an Insider's Viewpoint (현곡(玄谷) 윤길영(尹吉榮)의 "한방생리학(漢方生理學)의 방법론(方法論) 연구(硏究)" 재 조명)

  • Yoon Gil-Young(1911-1987) was a frontier of the field of Traditional Korean Medical Physiology(TKMP) who was academically active since mid 1950s until early 1970s. He wrote several important papers and books. Among his writings, "A Study on the Methodology of Traditional Korean Medical Physiology", which was published serially in the "Korean Oriental Medical Journal" from June 1966 until March 1967, played an important role in establishing the contemporary TKMP and showed his academic stance on Traditional Korean Medicine(TKM). This review will evaluate this paper's contribution to the establishment of TKMP in historical context, summarize the unique characteristics of his understanding of TKMP by analyzing the text, and then based on those information, examine his view on TKMP and TKM. Historically, this paper was written in transition period from the traditional TKM knowledge system to the modernized one. Aim of this paper was to provide a methodology for establishing TKMP as a scientific knowledge system like that of the western medicine. Based on his study of Yin-Yang theory and theory of five circuits and six qi in Huangdi's Internal Classics, he suggested those theories as methods to observing life phenomena and systematizing the observations. And he regarded these methods as of great value in determining the unique characteristics of TKM compared to those of western medicine. Through re-reading this paper, it was found that he had pride and confidence in the methodology of TKM, and also that he thought in scientizing and modernizing TKM it was very important to understand and efficiently put in use the methodology of TKM. It was also found that his view on TKMP and TKM was pro modernization.

  • Astragali Radix, known as Huangqi, is the most important tonic in the traditional oriental medicine. It reinforces 'qi' (vital energy), strengthens the superficial resistance and promotes the discharge of pus and the growth of new tissue. It has long been used as an anti-perspirant, anti-diuretic or a tonic. Eleven compounds were isolated from the hexane and EtOAc fractions from the roots of Astragalus membranaceus (Leguminosae) and their structures were identified as four triterpenoids [lupenone (1), friedelin (2), lupeol (3), soyasapogenol E (9)] and seven sterols [${\beta}-sitosterol$ (4), stigmastane-3,6-dione (5), $7{\alpha}-hydroxysitosterol$ (6), $5{\alpha},6{\beta}-dihydroxysitosterol$ (7), $7-oxo-{\beta}-sitosterol$ (8), ${\beta}-sitosterol$ glucoside 6'-O-palmitate (10), ${\beta}-sitosterol$ glucoside (11)]. The chemical structures of these compounds were identified on the basis of spectroscopic methods and comparison with literature values. Among these compounds, lupenone (1), friedelin (2), lupeol (3), stigmastane-3,6-dione (5), $7{\alpha}-hydroxysitosterol$ (6), $5{\alpha},6{\beta}-dihydroxysitosterol$ (7), $7-oxo-{\beta}-sitosterol$ (8), soyasapogenol E (9), and ${\beta}-sitosterol$ glucoside 6'-O-palmitate (10) were isolated from this plant for the first time.

Differential microRNA Expression by Solexa Sequencing in the Sera of Ovarian Cancer Patients

  • MicroRNAs are a class of small noncoding RNA which play important regulatory roles in a variety of cancers. MiRNA-specific expression profiles have been reported for several pathological conditions. In this study, we combined large scale parallel Solexa sequencing to identify 11 up-regulated miRNAs and 19 down-regulated miRNAs with computational techniques in the sera of ovarian cancer patients while using healthy serum as the control. Among the above, four miRNAs (miR-22, miR-93, miR-106b, miR-451) were validated by quantitative RT-PCR and found to be significantly aberrantly expressed in the serum of ovarian cancer patients (P<0.05). There were no significant differences between samples from cancer stage I/II and III/IV. However, the levels of miR-106b (p=0.003) and miR-451 (p=0.007) were significantly different in those patients under and over 51 yearsof age. MiR-451 and miR-93 were also specific when analyzed with reference to different levels of CA125. This study shows that Solexa sequencing provides a promising method for cancer-related miRNA profiling, and selectively expressed miRNAs could be used as potential serum-based biomarkers for ovarian cancer diagnosis.
