Malassezia Dermatitis in a South American Sea Lion (Otaria byronia) (오타리아 물개(South American Sea Lion, Otaria byronia)에서 발생한 Malassezia 피부염)

  • Malassezia spp. was isolated from the skin lesion of a South American Sea Lion (Otaria byronia) in South Korea. Colonies were cream to yellowish with smooth margin on Sabouraud dextrose agar without lipid supplements. Yeast cells were ovoid to cylindrical in shape and budding daughter cells from broad base. We conducted 26s rRNA sequencing to specify the strain of the yeast and found out this isolate highly matched with Malassezia pachydermatis isolated from canine otitis. The pulse therapy of oral itraconazole was very effective in this case. M. pachydermatis has a wide range of host animals but this is the first report in a sea lion in South Korea.

Analysing of the recent trends on the constitution diagnosis and treatment device patents (체질진단 및 치료기기 최신 특허동향 분석)

  • In this paper, We tried to analyze the patent trend on constitution diagnosis and treatment related technologies. For this, constitution diagnosis and treatment related technologies divided into 16 sub-technology by the advisory committee. And we analyzed patents applied in Korea, Japan, U.S.A., China, and Europe. The 16 sub-technology consist of pulse analyzer, skin diagnosis, tongue analyzer, face and body detector, face analyzer, voice analyzer, intelligence ontology, meridian diagnosis, infrared thermography, electric stimulation, laser, high and low frequency, physical, magnetic, and ultrasound therapy. As a result we found that patents of constitution diagnosis and treatment in Korea has been growing steadily in both quality and quantity since 1980s. The number of patent the pulse analyzer and low frequency therapy are larger than others. But applied relevant international patents, marketability of the patent and Cites per Patent (CPP) index are shown to be very low. In conclusion, to occupy key original technologies of the Oriental medical device, we need to apply more patent of the whole related technology and international patents.

  • Functional Electrical Stimulation (FES) is used for muscle reeducation, reduction of spasticity, delay of atrophy, and muscle strengthening. FES stronger stimulation than other forms of electrical stimulation. The efficacy of FES in improving function has been substantiated in the literature. Treatment programs employing FES - activation of muscular tissue through the intact peripheral nervous system - can be broken into five major categories, according to the goal of treatment. These broad areas would include the use of FES to: (1) a direct excitation onto the alpha motoneuron, through peripheral stimulation of the Ia myotatic sensory system and ascending afferent information, which will be integrated at conscious and subconscious level of the CNS. (2) The quality of a stimulated muscle contraction is determined by combination of many parameters, including stimulus amplitude, pulse duration, stimulus frequency, and duty cycle. (3) A unit that has a pulse duration between 200 and $400{\mu}sec$ will be more than adequate for FES applications. (4) The neuromuscular plasticity is critically important to return of function using muscle re-education and facilitation applications. (5) Prior to using FES as an electrical orthosis, the patient should build up endurance in the muscles to be stimu1ated during the gait cycle.

Study on Effect of Aromatherapy on Physiological and Psychological Aspect of Patients with Pain (향기요법이 통증환자의 생리적 및 심리적 측면에 미치는 효과)

  • The purpose of this study was to investigate the effect of aromatherapy on pain level, blood pressure, pulse rate, sleep, anxiety and depression of patients with pain. This study used a non equivalent quasi-experimental design. Aromatherapy was the inhalation of blending oils with lavender, eucalyptus, peppermint for 3 weeks. As the estimation for the physiological aspect, the visual analogy scale (VAS) for the pain level, blood pressure and pulse rate were measured. Also, as the estimation for the psychological aspect, Sleep of Snyder-Halpern & Verra, anxiety of Spielbeger and depression of Beck were used. The data were analyzed by SAS program using $x^2$ test, Fisher's exact test, Cronbach ${\alpha}$ and t-test. The results of this study were as follows. Pain level(p=0.001), state anxiety(p=0.018), trait anxiety(p=0.002), and depression(p<0.001) improved significantly in the experimental group. Also, there was significant difference between the experimental group and the control group on the depression(p=0.012). Aromatherapy was effective in physiological and psychological therapy. Also it can be applied in combination with existing Oriental medical therapy.

  • In this study, the application was developed for the simulation of functional electrical stimulation. It functions to calculate the electrical energy which is transmitted to the patient, to visualize the electrical stimulation waveform, Therapy, Burst and Pulse section as setup of time period requested. The application was verified by comparing the oscilloscope and the graph of the application. XML schema was developed to utilize the contents of simulation which consist of the standard codes that are identified by OID. Using the application, medical experts will be able to research and share the contents of simulation.

Pulsed Ultrasound and Pulsed Electromagnetic Field in the Treatment of Muscle Contusion in Rats

  • Muscle contusion usually results from a direct blunt impact and is frequently associated with contact sports. Muscle contusion results from microscopic muscle fiber and capillary disruption causing a microhemorrhage dissecting torn fibers and remaining viable muscle fibers. Recent studies concluded that some physical methods, including pulsed ultrasound (PU) and pulsed electromagnetic field (PEMF) treatment, accelerate and facilitate wound healing, improve scar quality and have beneficial effects on muscle and tendon healing. However, there are few studies on the effects of the early use of physical methods, such as PU and PEMF, on the expression of neurotrophic factors. The objective of this study was to investigate the effects of the early application of PU and PEMF, measured through the expression of BDNF in the muscles (gastrocnemius) and spinal cords of rats after skeletal muscle contusion. In the spinal cords and muscles, there was a significant increase of BDNF expression in the PEMF and PU groups, a greater increase was found in the PEMF group than in the PU group. In conclusion, PEMF is a useful therapeutic method that improves muscle healing after muscle contusion.

The Scanning Electron Microscopic study on the effect during subgingival calculus removal using Nd:YAG laser (치은연하치석제거술시 Nd : YAG 레이저를 이용한 효과에 관한 주사전자현미경적 연구)

  • The purpose of this study was to evaluate in vitro the effects during subgingival calculus removal using Nd:YAG laser. The study group was consisted of 30 teeth with advanced periodontal disease extracted before the start of periodontal therapy. The specimens were divided into 8 different groups : 1) untreated control 2) scaling and root planing only 3) laser treated using 150mJ/pulse, 1sec, 5sec, contact mode 4) laser treated using 200mJ/pulse, 5sec, contact mode 5) laser treated using 150mJ/pulse, 1sec, non-contact mode 6) laser treated using 200mJ/pulse, 5sec, non-contact mode 7) laser treated using l5OmJ/pulse, 1sec, contact mode with water irrigation 8) laser treated using 200mJ/pulse, 5sec, contact mode with water irrigation. All specimens were prepared for evaluation by scanning electron microscopy(SEM). Specimens from Group 2 exhibited a smear layer of scale like texture with parallel instrument tracks resulting from curet use. Specimens treated by contact mode, Group 3 and 4 featured surface changes not observed· in controls such as charring, randomly distributed pitting and crater formation, and melting down of the tooth material and calculus. Specimens treated by noncontact mode, Group 5 and 6 featured similar surface changes observed in contact mode. However, the differences between contact and non-contact groups not significant. Specimens treated by contact mode with water irrigation, Group 7 and 8 featured slight surface change compared to other groups. The results suggested that Nd: YAG laser did not completely remove the subgingival calculus but was possible the application as adjunctive method.

Comparison of the Therapeutic Efficacy of Methylprednisolone Pulse Therapy and Oral Steroid Therapy in Children with IgA Nephropathy and HSP Nephritis Combined with Proteinuria (단백뇨를 동반한 IgA 신병증 및 HSP 신염을 가진 소아에서 메틸프레드니솔론 충격요법과 경구 스테로이드 요법의 비교)

  • Purpose: The purpose of this study was to assess the therapeutic efficacy of methylprednisolone pulse therapy in children with IgA nephropathy and Henoch- Sch$\ddot{o}$nlein Purpura (HSP) nephritis combined with proteinuria. Methods: We retrospectively reviewed the clinical records of 21 patients who were diagnosed with IgA nephropathy and HSP nephritis based on percutaneous renal biopsy. Of the 21 patients, 15 were diagnosed with IgA nephropathy and 6 were diagnosed with HSP nephritis. They had mild to severe proteinuria at the time of diagnosis or during follow-up. Group 1 (n=7) received methylprednisolone pulse therapy three times every couple of months, and Group 2 (n=14) received oral steroid therapy. The follow-up periods for Group 1 and 2 were 14.0 (9-54) months and 26.5 (14-34) months, respectively. There was no significant difference in the follow-up duration between the two groups. Results: The average age at diagnosis and biopsy was lower in Group 1 compared to Group 2, but it was not significantly different. At admission, all patients in both groups had hematuria and 5 patients (71.4%) of Group 1 and 14 patients (100 %) of Group 2 had proteinuria. Before treatment, there was no significant difference of spot urine protein/creatinine ratio between the two groups. During followup, 7 patients of Group 1 (100%) and 10 patients of Group 2 (71.4%) showed complete improvement of proteinuria and the spot urine protein/creatinine ratio in Group 1 was significantly lower than Group 2. Conclusion: In patients with IgA nephropathy and HSP nephritis with proteinuria, methylprednisolone pulse therapy was more effective than oral steroid therapy in the reduction of proteinuria. To investigate the effects on long-term prognosis, large-scale prospective studies are needed.

A Study on the Analasys of the Eight-Constitution Questionnaire (팔체질 설문문항 분석-I)

  • Background: Eight-Constitution Medicine (ECM) devides the human body into eight constitutions. Up until now, checking the pulse types is only way to classify the eight constitution. Recently we try to make a questionnaire that is based on the Eight-Constitution characters and analyze the answer from patients who were classified into one of the eight constitutions according to their pulse types and their responses to constitution-acupuncture therapy Objective: This study intends to analyze each answer of the Eight-Constitution Questionnaire with independent sample t-test. Methods: Participants of this study were outpatients in ECM clinics located in Korea. The resources were collected from 925 patients who were classified into one of the eight constitutions according to their pulse types and their responses to constitution-acupuncture therapy. SPSS 13.0 for Windows was used for independent sample t-test. Results and Conclusions: 1. The proportion of participants' constitutions is in the order of Pancreotonia (28.0%), Colonotonia (17.3%), Pulmotonia (18.1%), Hepatonia (18.1%), Vesicotonia (8.5%), Cholecystotonia (7.0%), Renotonia (2.6%) and Gastrotonia (0.4%). 2. Eight-Constitution Questionnaire consists of 5 parts; Appearance, Character, Body reaction, Physiology & Pathology and Food reaction. 3. 10 items were chosen among 42 question about Appearance. 4. Due to the deficiency of Gastrotonia examples, useful items have to be given up. If we can gather Gastrotonia examples, we can use meaningful items more than 10.
