Purpose: This research was done to compare the pain relief effects of various nursing interventions, such as cold therapy, attention diversion and 10% Lidocaine spray during arteriovenous fistula needling for patients on hemodialysis, and also to identify and develop more effective nursing interventions for pain relief in these patients. Methods: This research was conducted from October 1, 2010 to January 31, 2011 with 8 hemodialysis patients, who were on regular dialysis (3 times a week) at K University Hospital in Seoul and had an arteriovenous fistula within the past 3 months. Each patient received the three nursing interventions (cold therapy, attention diversion and 10% Lidocaine spray therapy) prior to the arteriovenous fistula needling and applied in turn with the series being repeated 4 times. After each intervention, physiologic indexes, subjective and objective pain were measured at the time of needling. ANOVA was used with SPSS/WIN 12.0 to analyze pain scores and comparison of physiologic indexes (BP, pulse). Results: No significant differences were found for subjective pain (p=.574), objective pain (p=.562) and total pain (p=.506) among the 3 interventions. Systolic blood pressure (p=.689), diastolic blood pressure (p=.969) and pulse (p=.980) also showed no significant difference among the 3 interventions. Conclusion: These 3 interventions are all possible for pain relief during arteriovenous fistula needling for these patients. However, the only interventions that nurses can do independently are cold therapy and attention diversion so we recommend that these nursing interventions be used.
Medical magnetic stimulator generates strong magnetic field pulses. Clinical applications of the magnetic pulse are the stimulation of nervous system and the contraction of muscle. The unique source of the strong magnetic pulse is a capacitor-inductor resonator and this inductor generates a strong sinusoidal magnetic pulse by discharging the capacitor with high initial voltage. Continuous muscle contraction needs sequential generation of the magnetic pulses. However, to keep the magnitude of sequential pulses identical, an expensive high-voltage power supply have to support voltage drop of the capacitor between the pulses. A protection circuit between the supply and the resonator is necessary to protect the supply from reverse current caused by capacitor voltage reversal. In this paper, a new circuit structure of the magnetic stimulator adopting a low-frequency fly-back switching is proposed. The new circuit supports sequential pulse generation and allows the reverse current without damage. Performance of the new circuit is examined and a low-cost magnetic stimulator for urinary incontinence therapy is being developed using the presented method.
A power supply for magnetic-stimulation devices was designed via a control algorithm that involved a start current application based on a resonant converter. In this study, a new power supply for magnetic-stimulation devices was designed by controlling the pulse repetition frequency and pulse width. The power density could be controlled using the start-current-compensation and ZCS (zero-current switching) resonant converter. The results revealed a high-repetition-frequency, high-power magnetic-stimulation device. It was found that the stimulation coil current pulse width and that pulse repetition frequency could be controlled within the range of 200-450 ${\mu}S$ and 200-900 pps, respectively. The magnetic-stimulation device in this study consisted of a stimulation coil device and a power supply system. The maximum power of the stimulation coil from one discharge was 130 W, which was increased to 260 W using an additional reciprocating discharge. The output voltage was kept stable in a sinusoidal waveform regardless of the load fluctuations by forming voltage and current control using a deadbeat controller without increasing the current rating at the starting time. This paper describes this magnetic-stimulation device to which the start current was applied.
Purpose: This study is aimed to confirm the effects of Yakson therapy on the growth and physical response of preform infants, and maternal attachment to them compared with GHT therapy. Method: The design of this study is nonequivalent control group with repeated measuring by quasi experimental study. The subjects are preterm infants in 26 - 34 gestational age hospitalized in the NICU of 4 university hospitals with an experimental group of 15 and a control group of 14. Yakson therapy consists of three phases: laying a hand, caressing by hand, and laying a hand again taking 5 minutes for each phase. Result: As a result of administering Yakson therapy to preform infants; the average weight gain of the Yakson group was higher than that of the GHT group, but there is no significant difference between groups. The oxygen saturation and maternal attachment difference between the Yakson and the GHT group were not significant. Significant differences in the average daily increase of oral intake and apical pulse rate were observed between the Yakson group and GHT group. Conclusion: These data suggested that Yakson therapy may be an effective nursing intervention which can facilitate growth and physical response of preform infants.
Purpose: This study was performed to identify the effects of intraoperative music therapy on the anxiety level of adult patients undergoing local anesthesia in the operating room. Method: The research design was a nonequivalent control group pretest-posttest. Data collection was done with 48 patients undergoing local anesthesia from June 2003 to September 2()03. The 24 patients in the experimental group were given music therapy through headphones from the beginning of the operation to the end of the operation with local anesthesia. The data were analyzed using SPSS/PC 10.0 for Windows. Results: Postoperative state anxiety scores were significantly different between the experimental group and the control group. Intraoperative pulse rates were significantly different between the two groups. But systolic and diastolic blood pressures and respiratory rates were not significantly different between the two groups. Conclusion: Considering these results, music therapy can be regarded as an effective nursing intervention that relieves anxiety for adult patients undergoing local anesthesia. However, to determine the best use of this therapy, further study is required for different types of anesthesia and operations such as ambulatory surgery.
The purpose of this study was to investigate whether or not the physical therapist's explanation of the treatment prior to electrical stimulation affects the subjects level of anxiety. Eighty healthy subjects participated in this study and were assigned either to the experimental group receiving information on the treatment ($n_1$=40) or to the control group not receiving information on the treatment ($n_2$=40). Transcutaneus Electrical Nerve Stimulation (TENS), a sphygomamometer and the Spielberger State Anxiety Scale Questionnaire were used to investigate the threshold of anxiety. Analysis of data revealed significant differences in pulse and diastolic blood pressure compared to the control group when the subjects were stimulated electrically after receiving information on the treatment. We found significant differences between the two groups. The results of the study indicate that the patient's anxiety can be reduced by the physical therapist's explanation on the treatment.
Purpose: This study was to examine the effects of music therapy on the levels of anxiety and discomfort in patients undergoing gastrofibroscopy. Method: This study was equivalent control group pre-post test design. The subjects were 61 patients who visited H hospital internal medicine department for gastrofibroscopy ; 31 were randomly selected for the control group and the remaining 30, for the experimental group. Result: 1. The experimental group who used music therapy showed a significantly lower level of anxiety during gastrofibroscopy than the control group. 2. The experimental group revealed a lesser change in pulse rate between pre and post gastrofibroscopy than the control group. 3. There was a significant difference in the levels of subjective and objective discomfort during gastrofibroscopy between the two groups. Conclusion: The results suggested that music therapy may help relieve anxiety and discomfort for patients undergoing gastrofibroscopy. These results showed that the use of music aided in the reduction of anxiety and discomfort during the unpleasant diagnostic procedure and, testing the effectiveness of music therapy deserves further study in other hospital settings.
In this study, a horticultural therapy program was performed by employing the anger experienced by people during daily life activities in the emotion regulation strategy-based program to identify the influence of the horticultural program on anger control in the caregivers for patients with dementia. To measure this influence, two tools were used: a self-administered questionnaire survey, which is a subjective test, and an instrumental test, which can measure the physical index that detects the physical changes through anger. For the preliminary test, depression, stress, self-esteem, anxiety, and anger state-characteristic were tested using the self-administered questionnaire. For the follow-up test, the self-administered survey and the test using the physical index were performed in a manner similar to that of the preliminary test. The self-administered questionnaire comprised questions suitable to this study and considered the age, education level, and economic aspect of the subjects. The test results indicated that the experimental group subjected to the emotion regulation strategy-based horticultural therapy showed very significant reduction in depression and anger and significant reduction in stress, self-esteem, and anxiety. In the physical index test, the systolic and diastolic blood pressures were reduced, and the pulse rate and respiration rate reduced before and after the therapy, thus, proving the effectiveness of the therapy in anger control.

Purpose: This research was intended to evaluate the sedative effect of the music by measuring the anxiety-level, the pain-level, the blood pressure, and the pulse rate in patients who were going through extraction of the mandibular impacted 3rd molar. Materials and methods: Patients were selected among those who visited St. Vincent's Hospital in Suwon for the surgical extraction of 3rd molar from April to May, 2007 and were divided into two groups : the first, experimental group consisting of 32 people was going through extraction with the music and the second, control group also consisting of 32 people was going through extraction without the music. The anxiety-level was measured by filling out Spielberger's State-Trait Anxiety questionnaire. The pain-level was measured by VAS (Visual analog Scale). The blood-pressure and the pulse rate were recorded at waiting period, preparation and anesthesia period, 5 minutes after anesthesia, beginning of extraction and every five minutes after extraction to the end of surgery. Results: The anxiety-level was significantly lower in the experimental group than in the control group during the extraction procedure (p<0.01). The pain-level was significantly lower in the experimental group than in the control group (p<0.05). The mean systolic blood pressure was significantly lower in the experimental group than in the control group at the beginnig of extraction and 10 minutes after extraction (p<0.01, p<0.05 each). The mean diastolic blood pressure was significantly lower in the experimental group than the control group at 10 minutes after extraction (p<0.01). The mean of pulse rate was significantly lower at the beginning of extraction (p<0.05). Conclusion: The music therapy could offer not only the psychological stability but also the physiological stability during the surgical extraction of impacted 3rd molar.

OHAT establishes objectiveness and accuracy of diagnosis based upon the traditional method and procedure of pulse taking. In OHAT, a person's state of illness is diagnosed by applying the comparative examination of the palpitation of the pulse. It is the fact that the pulse varies according to the state, and that OHAT treatment has proven the positive results by using the victor-vanquished relationship on The Nan Jin 75. On the basis of this, it is necessary to add the sixty ninth Nan(69難), to research the theory of the generation of the Five Element. Conclusions : Ohaeng-Hwa Acupuncture is very effective in treating the wide range of illness, and thus it has gained an increasing attention of many scholars and practitioners in the field of traditional Korean oriental medicine. However, it is the first theoretical attempt to the clinical research and scientific methodology of Ohaeng(Five) Ohaeng-Hwa Acupuncture, and more active Ohaeng-Hwa Acupuncture R&D is being conducted nationwide.
