The study examined what effects 100kHz PWM LED light irradiation causes to bone marrow cells of SD-Rat when LED characterized cheap and safe is used onto the light therapy by replacing the low 1evel laser. We developed the equipment palpating cell proliferation using a high brightness LED. This equipment was fabricated using a micro-controller and a high brightness LED, and designed to enable us to control light irradiation time, intensity, frequency and so on. Especially, to control the light irradiation frequency, FPGA was used, and to control the change of output value, TLC5941 was used. Consequent1y, the current value could be controlled by the change of 1eve1 in Continue Wave(CW) and Pulse Width Modulation(PWM), and the output of a high brightness LED could be controlled stage by stage. MTT assay method was chosen to verify the cell increase of two groups and the effect of irradiation on cell proliferation was examined by measuring 590nm transmittance of ELISA reader. As a result, the cell increase of Rat bone marrow cells was verified in 100kHz PWM LED light irradiation group as compared to non-irradiation group.

The intracavity transformation of the Nd :YAP main radiation 1079 nm was Raman converted in barium nitrate crystal and the Stokes frequency (1216 nm) was doubled using KTP or RTA crystals. The LiF or Cr:YAG crystals are used for the Q-switch. The radiation Parameters were obtained at 100 Hz pump repetition frequency. The average power at 608 nm radiation with LiF and KTP was 700 mW at multi-mode generation. The 3-6 single 10-15 ns pulses were generated during one cycle of pumping. The doubling efficiency with RTA was two times more than with KTP. The cells of Ehrlich adenocarcinoma (0.1 ml) were i.m. implanted in hind thighs of ICR white non-imbred mice. The cells were preliminarily diluted in medium 199 in the ratio of 1 to 5. HpD was intravenous administered in a dose of 10 mg/kg. The left clean-shaven hind leg was irradiated with laser light 21-27 hours after the administration of the preparation. The right non-Irradiated leg of each animal served as a control. The animals with the transplanted tumor that were not injected with HpD sewed as a control to estimate the complex effect (HpD+ irradiation). Before the administration of HpD and on 3 and 4 days after irradiation the tumor size was measured and the percent of the tumor growth inhibition was calculated. The results of animal treatments has shown high efficiency of PDT method for cancer treatment by means 0.608 m high power pulse solid state laser.
A real-time digital time-stamp sorting algorithm used in the In-Beam positron emission tomography (In-Beam PET) is presented. The algorithm is operated in the field programmable gate array (FPGA) and a small amount of registers, MUX and memory cells are used. It is developed for sorting the data of annihilation event from front-end circuits, so as to identify the coincidence events efficiently in a large amount of data. In the In-Beam PET, each annihilation event is detected by the detector array and digitized by the analog to digital converter (ADC) in Data Acquisition Unit (DAQU), with a resolution of 14 bits and sampling rate of 50 MS/s. Test and preliminary operation have been implemented, it can perform a sorting operation under the event count rate up to 1 MHz per channel, and support four channels in total, count rate up to 4 MHz. The performance of this algorithm has been verified by pulse generator and 22Na radiation source, which can sort the events with chaotic order into chronological order completely. The application of this algorithm provides not only an efficient solution for selection of coincidence events, but also a design of electronic circuit with a small-scale structure.
The International Society for the Study of Vascular Anomalies classifies vascular anomalies into vascular tumors and vascular malformations. Vascular tumors are neoplasms of endothelial cells, among which infantile hemangiomas (IHs) are the most common, occurring in 5%-10% of infants. Glucose transporter-1 protein expression in IHs differs from that of other vascular tumors or vascular malformations. IHs are not present at birth but are usually diagnosed at 1 week to 1 month of age, rapidly proliferate between 1 and 3 months of age, mostly complete proliferation by 5 months of age, and then slowly involute to the adipose or fibrous tissue. Approximately 10% of IH cases require early treatment. The 2019 American Academy of Pediatrics clinical practice guideline for the management of IHs recommends that primary care clinicians frequently monitor infants with IHs, educate the parents about the clinical course, and refer infants with high-risk IH to IH specialists ideally at 1 month of age. High-risk IHs include those with life-threatening complications, functional impairment, ulceration, associated structural anomalies, or disfigurement. In Korea, IHs are usually treated by pediatric hematology-oncologists with the cooperation of pediatric cardiologists, radiologists, dermatologists, and plastic surgeons. Oral propranolol, a nonselective beta-adrenergic antagonist, is the first-line treatment for IHs at a dosage of 2-3 mg/kg/day divided into 2 daily doses maintained for at least 6 months and often continuing until 12 months of age. Topical timolol maleate solution, a topical nonselective beta-blocker, may be used for small superficial type IHs at a dosage of 1-2 drops of 0.5% gel-forming ophthalmic solution applied twice daily. Pulse-dye laser therapy or surgery is useful for the treatment of residual skin changes after IH involution.
Renal cortical necrosis (RCN) is patchy or diffuse ischemic destruction of the renal cortex caused by significantly reduced renal arterial perfusion. It is a rare cause of acute kidney injury (AKI) and is associated with high mortality. Here, we review the case of RCN in a 15-year-old boy who developed AKI. A 15-year-old boy was referred to our hospital from a local hospital due to a sharp decrease in his renal function. He presented with acute flank pain, nausea with vomiting, and oliguria for the past two days. He had taken a single dose of antihistamine for nasal congestion. At our hospital, his peak blood pressure was 148/83 mmHg and he had a high body mass index of $32.9kg/m^2$. The laboratory data showed a blood urea nitrogen (BUN) of 28.4 mg/dL, a creatinine of 4.26 mg/dL, and a glomerular filtration rate estimated from the serum cystatin C of $20.2mL/min/1.73m^2$. Proteinuria (spot urine protein to creatinine ratio 1.66) with pyuria was observed. Kidney sonography showed parenchymal swelling and increased renal echogenicity. Due to rapidly progressing nephritis, steroid pulse therapy (750 mg/IV) was done on the second day of his admission and the patient showed complete recovery with normal renal function. However, the kidney biopsy findings revealed renal cortical hemorrhagic necrosis. Multifocal, relatively well-circumscribed, hemorrhagic necrotic areas (about 25%) were detected in the tubulointerstitium. Although RCN is an unusual cause of AKI, especially in children, pediatricians should consider the possibility of RCN when evaluating patients with rapidly decreasing renal function.

Purpose: This study investigated periodontal ligament (PDL) restoration in osseointegrated implants using stem cells. Methods: Commercial pure titanium and zirconium oxide (zirconia) were coated with beta-tricalcium phosphate (β-TCP) using a long-pulse Nd:YAG laser (1,064 nm). Isolated bone marrow mesenchymal cells (BMMSCs) from rabbit tibia and femur, isolated PDL stem cells (PDLSCs) from the lower right incisor, and co-cultured BMMSCs and PDLSCs were tested for periostin markers using an immunofluorescent assay. Implants with 3D-engineered tissue were implanted into the lower right central incisors after extraction from rabbits. Forty implants (Ti or zirconia) were subdivided according to the duration of implantation (healing period: 45 or 90 days). Each subgroup (20 implants) was subdivided into 4 groups (without cells, PDLSC sheets, BMMSC sheets, and co-culture cell sheets). All groups underwent histological testing involving haematoxylin and eosin staining and immunohistochemistry, stereoscopic analysis to measure the PDL width, and field emission scanning electron microscopy (FESEM). The natural lower central incisors were used as controls. Results: The BMMSCs co-cultured with PDLSCs generated a well-formed PDL tissue that exhibited positive periostin expression. Histological analysis showed that the implantation of coated (Ti and zirconia) dental implants without a cell sheet resulted in a well-osseointegrated implant at both healing intervals, which was confirmed with FESEM analysis and negative periostin expression. The mesenchymal tissue structured from PDLSCs only or co-cultured (BMMSCs and PDLSCs) could form a natural periodontal tissue with no significant difference between Ti and zirconia implants, consequently forming a biohybrid dental implant. Green fluorescence for periostin was clearly detected around the biohybrid implants after 45 and 90 days. FESEM showed the invasion of PDL-like fibres perpendicular to the cementum of the bio-hybrid implants. Conclusions: β-TCP-coated (Ti and zirconia) implants generated periodontal tissue and formed biohybrid implants when mesenchymal-tissue-layered cell sheets were isolated from PDLSCs alone or co-cultured BMMSCs and PDLSCs.
고혈압 이외에 급성 폐쇄동맥경화증의 내과적 위험요소가 없던 75세 남자 환자가 경한 발열을 동반한 기침, 호흡 곤란으로 내원하여 코로나바이러스감염증-19 진단을 받았다. 격리입원 치료를 시작한 지 1주일째에, 우측 족부의 동통 및 저린감을 호소하기 시작하였으며, 그 다음 1주일 동안 서서히 한랭감, 전족지의 색깔 변화와 족배동맥의 소실이 나타나게 되었다. 하지 혈관조영 3차원 컴퓨터 단층촬영 검사에서 좌, 우측 모두에서 슬와동맥 이하로 전, 후경골 동맥 및 비골동맥의 혈류가 관찰되지 않았으며, 우측 총장 골동맥 내부에 혈전이 생성되어 있는 모습을 확인할 수 있었다. 우측 하지에 대하여 경피적 풍선 혈관 성형술 및 스텐트 삽입술, 항응고제 투여를 시행하였으며, 시술 직후부터 우측 족배동맥의 맥박이 온전히 촉지되고, 호소하던 한랭감이 개선되었음을 확인하였다. 혈류가 개선되었음에도 불구하고 우측 족무지의 괴사는 회복되지 않아 결국 족무지 절단술을 시행하였다.
Objective: The purpose of this case study was to report the symptom improvement effect of integrative Korean medicine treatment based on Pyongsim Solution: Cheonwangbosim-dan in a patient with variant angina and asthma symptoms. Methods: This 68-year-old male patient was treated with Pyongsim Solution, Gamihyangsayukgunja-tang, Gwakhyangjunggi-san, acupuncture, pharmacopuncture, and cupping therapy for 24 days. The European Quality of Life-5 Dimensions-5 Levels profile, Numerical Rating Scale score, and pulse rate per minute were measured on admission, at two weeks of hospitalization, and upon discharge to assess the clinical outcomes. Results: After treatment, the "mobility" dimension profile of the European Quality of Life-5 Dimensions-5 Levels changed from 1 to 1, "self-care" changed from 1 to 1, "usual activity" changed from 3 to 2, "pain/discomfort" changed from 4 to 3, and "anxiety/depression" changed from 3 to 2. The Numerical Rating Scale score for chest pain decreased from 7 to 5, chest tightness decreased from 6 to 4, sleep disturbance decreased from 6 to 3, polydipsia decreased from 5 to 3, and stomach discomfort decreased from 5 to 3. All the profiles of these five dimensions were unchanged for 11 days in the period between the two weeks of hospitalization and discharge. There were also no changes in the profiles of either "mobility" or "self-care" throughout the entire 24-day hospitalization period. Conclusion: This case report shows that integrative Korean medicine treatment based on Pyongsim Solution can improve the symptoms of patients with variant angina and mild intermittent asthma, suggesting that Pyongsim Solution could be an important herbal medication for treatment.
목적: 급속 진행성 사구체신염은 병리조직학적으로 사구체가 광범위한 반월상을 형성하며 임상적으로는 수개월내에 급성 신부전에 빠지게 되는 신질환이다. 저자들은 이 질환의 임상 경과의 이해와 치료 방침의 결정에 도움을 주고자 소아급속 진행성 사구체 신염 환자들의 임상양상과 병리 소견을 후향적으로 고찰하였다. 방법: 1991년부터 2003년까지 소아과에 내원하여 임상 양상과 신생검 소견을 종합하여 급속 진행성 사구체 신염으로 진단 받고 추적 관찰이 가능하였던 12명을 대상으로 임상양상 및 병리소견에 대해 후향적으로 조사하였다. 신부전으로 진행한 군과 정상 신기능이 유지된 군으로 나누어 임상-병리학적 지표들을 비교하였다. 결과: 12명의 환자 중 남자는 4명, 여자는 8명이었다. 발병 당시 연령은 평균 11세 5개월이었고 경과 관찰 기간은 평균 25개월이었다. 신조직 검사 결과에 따라 분류하면 면역복합체 매개성 사구체 신염인 경우가 10예(83%), 무면역 침착 사구체 신염이 2예(17%)였고 항사구체 기저막 항체 신염인 경우는 없었다. 모든 환자가 경구 스테로이드 투여를 받았고 10명(83.3%)에서 methylprednisolone pulse therapy를 시행하였고 이중 8명(65.7%)은 cyclophosphamide 병합 투여도 시행하였고 4명(33.3%)은 혈장교환을 병행하였다. 경과 관찰하는 동안 1예에서 정상 신기능으로 회복되었고 7예는 정상 신기능은 유지되나 신증후군 범위 이하의 단백뇨가 지속되는 부분 회복을 보였다. 2예에서는 지속되는 단백뇨와 혈청 크레아티닌 상승을 보이는 만성 신부전 상태를 보였고 2예에서는 말기 신질환으로 진행하였다. 진단시 높은 혈청 크레아티닌과 낮은 헤모글로빈 수치를 보인 경우와 나이가 어린 경우 예후가 불량하였다. 결론: 소아에서 급속 진행성 사구체신염은 면역 복합체 신염이 대부분을 차지하고 조기 진단과 적극적인 치료를 시행하였을 때 대부분의 환자에서 신기능의 호전이 관찰되었다. 이 질환의 임상 경과와 치료 방침의 확립을 위하여 다기관의 전향적 연구가 필요하다.
