스테로이드 저항성 신증후군 환아에서의 Methylprednisolone 충격 요법의 치료 효과 (The Effect of Methylprednisolone Pulse Therapy against Steroid Resistant Nephrotic Syndrome in Children)

  • 목적 : 소아의 스테로이드 저항성 신증후군은 예후가 불량함에도 불구하고 지금까지 효과적인 치료방법이 없는 병이었으나 1990년 Mendoza등이 Methylprednisolone 충격 요법이 스테로이드 저항성 신증후군에서 효과적이라는 것을 보고하였다. 하지만 2년 뒤 Waldo 등은 Methylprednisolone 충격 요법이 Mendoza 등의 보고와는 다르게 스테로이드 저항성 신증후군에 효과가 적었다고 보고하여 이에 저자는 한국 소아에서 스테로이드 저항성 신증후군이 발생한 경우 Methylprednisolone 충격 요법의 효과를 알아보고 위의 보고의 서로 다른 결과를 확인하고자 본 연구를 시행하였다. 방법 : 1990년부터 1995년까지 만 5년간 서울대병원 소아과에 스테로이드 저항성 신증후군으로 입원한 소아를 대상으로 Methylprednisolone 충격 요법을 시행하여 $30{\pm}11$개월동안 추적 관찰하였다. 결과 : 1) Methylprednisolone 충격 요법 치료로 신증후군이 완해가 유도된 환아는 20명중 9명으로 45%이었다. 2) Methylprednisolone 충격 요법 치료후 신증후군의 완해가 유지된 환아는 20명중 9명으로 45%이었다. 3) Methylprednisolone 충격 요법에 반응이 없었던 환아중 만성 신부전으로 이행된 경우는 5례로서 25%이었다. 결론 : Methylprednisolone 충격 요법은 스테로이드 저항성 신증후군 소아의 45%에서 신증후군의 완해를 유도하고 유지시키는 효과적인 치료 방법이었다.

A Comparison of the Immediate Effects of Alternate Methods to Medication in Lowering the Blood Pressure in Prehypertensive Patients

  • Purpose: The purpose of this study was to compare the immediate effects of alternative treatment methods in lowering the blood pressure and pulse of prehypertensive patients. Methods: A total of 48 prehypertensive patients were randomly divided into 4 groups of 12 patients and each group received a different intervention. The control group was made to rest, the heat pack (HP) group received heat pack treatment, the ultrasound (US) group received an ultrasound massage, and the myofascial release (MFR) group received myofascial release. The blood pressure and pulse rate were measured before and after intervention. Results: Following intervention, the patients of the MFR group showed a decrease in systolic pressure, diastolic pressure, and pulse rate. Although blood pressure was similar in all groups, a significant decrease in pulse rate was seen in the MFR group compared to others. Conclusion: The application of MFR was shown to be an effective method for lowering the blood pressure and pulse rate in prehypertensive patients.

아로마 향기요법이 자궁근종 여성의 수술전 불안, 혈압 및 맥박에 미치는 효과 (Effect of Aroma Therapy on the Anxiety, Blood Pressure and Pulse of Uterine Leiomyoma Patients before Surgery)

  • Purpose: The study was performed to evaluate effects of aroma therapy on the anxiety, blood pressure and pulse of uterine leiomyoma patients before surgery and to apply it as the actual nursing intervention. Method: From Aug1st to Nov. 30th, 2007, the experimental group (n=30) and the control group (n=30) were selected in 1 hospital, Busan. Lavender and Bergamot oil mixed together at a 3:2 ratio was applied to the experimental group two times at 8-9 pm the day before surgery and twice 30 minutes before the surgery. Anxiety, VAS, blood pressure, and pulse were measured to evaluate the effects of aroma therapy. The data analysis was done using the SPSS/PC 12.0 program using chi-square test and t-test. Results: Hypothesis 1 was supported. The VAS of the experimental group was lower than control group after aroma therapy. Hypothesis 2 was partly supported. The systolic blood pressure and pulse of experimental group was significantly decreased, but diastolic blood pressure wasn't difference. The VAS, systolic blood pressure and pulse of control group were increased. Conclusion: The study verified that the aroma therapy is a very effective nursing intervention that can reduce anxiety before surgery. It is recommended to apply aroma therapy actively to patients experiencing anxiety before surgery procedure.

파라쿼트로 발생한 폐 섬유화증에서 pulse therapy로 호전된 환자 1례 (A Case of Moderate Paraquat Intoxication with Pulse Therapy in the Subacute Stage of Pulmonary Fibrosis)

  • In South Korea, attempted suicide by paraquat (PQ) intoxication is fairly common, and is lethal by pulmonary fibrosis and hypoxemia. However, the treatment of PQ poisoning is primarily supportive management. To increase the survival rate associated with PQ intoxication, many treatments have been developed. Here, we treated a case of PQ intoxication with steroid pulse therapy. A 23-year-old man was admitted to the hospital because of PQ intoxication. He drank two mouthfuls of Gramoxon (24% commercial paraquat). His vital signs were stable, but he had a throat infection, and navy blue urine in the sodium dithionite test. Standard treatment, including gastric lavage with activated charcoal was performed, and emergent hemoperfusion with a charcoal filter was initiated 11 h after PQ ingestion. Pharmacotherapy was initiated 18 h after PQ ingestion with the administration of 5 mg dexamethasone. On day 10, chest PA showed pulmonary fibrosis. Therefore, we initiated steroid pulse therapy, with 1g methylprednisolone in 100 mL of D5W administered over 1 h repeated daily for 3 days, and 1 g cyclophosphamide in 100 mL of D5W administered over 1 h daily for 2 days. On day 15, dexamethasone therapy was initiated. On day 30, pulmonary fibrosis was improved. Thus, if pulmonary fibrosis becomes exacerbated after dexamethasone therapy during the subacute stage, pulse therapy with methylprednisolone and cyclophosphamide could be helpful.

초음파 재활치료 시 PTT와 생리변수의 상관관계 분석 (Analysis of Relations Between Physiologic Parameters and Pulse Transit Time on the Ultrasound Therapy)

  • Currently, the signal of the human body is measured with various methods, and a noninvasive investigation of various methods is useful diagnosis method. PTT(Pulse Transit Time) which is noninvasive investigation make use of to estimate the physiological phenomena. PTT has a latent information of cardiovascular system. So we have the experiments for analysis of the relations between PTT and physiological parameters. We examine to correlate to the physiological parameters, an age and degree of paralysis on the ultrasound therapy. The 40 patients who has a such paralysis join our experiment, and we obtain the PTT data that normal condition and states after ultrasound therapy. We study that PTT after the ultrasound therapy for patients who have a paralysis was related to an age and degree of paralysis.

경피적 관상동맥 중재술 후 온요법이 요통, 혈압 및 맥박에 미치는 효과 (The Effects of Heat Therapy on Low Back Pain, Blood Pressure and Pulse Rate after Percutaneous Coronary Intervention)

  • Purpose: This study was done to examine the effects of heat therapy on low back pain, blood pressure and pulse rate after percutaneous coronary intervention. Method: The participants in this study were 40 patients who were admitted after having percutaneous coronary intervention. The experimental group, 20 patients, had heat therapy and the control group, 20 patients, maintained a supine position for 12 hours after the intervention. Back pain (VAS), blood pressure and pulse rate were measured just after removal of the sheath, and at 2-hour intervals up to 6 hours. Data were analyzed using SPSS 15.0. Results: The experimental group had significantly lower VAS for low back pain (F=23.44, p=.001). However no significant differences were found between two groups for blood pressure and pulse rate. Conclusion: The findings indicate that heat therapy is effective in reducing low back pain in patients who have had percutaneous coronary intervention. Therefore, heat therapy could be used as nursing intervention percutaneous coronary intervention.

중풍(中風)의 치료(治療)에 있어 청열법(淸熱法)에 대(對)한 문헌적(文獻的) 고찰(考察) (The literatual study on the therapy for clearing away heat with apoplexy therapy)

  • The literatual study on the therapy for clearing away heat with apoplexy therapy, the result were obstained as follows. 1. In apoplexy therapy, therapy for clearing away heat is used excessive heart - fire by overacting of the five emotions, liver fire, deficiency of kidney - fluid, wind - heat. 2. The fire of aetiology of apoplexy is used therapy for clearing away eat, in aspect of viscera and bowels, divied into heart - fire, liver - fire, deficiency fie of kidney yin, wetness - phlegm of spleen heat. The treatment is clear away heart - fire, clear away liver - fire, clear away spleen - heat and sthenic water. 3. Symptom of excessiveness symptom - complex is used therapy for clearing away heat that are fever, flushed face, halitosis, heart burn, easy anger, apoplestic stroke, unconsciouness, trismus, paralysis, constipation, red tongue with yellow coat, taut - smooth pulse or full - rapid pulse and symptom of insufficiency symptom - complex that are dizziness, tinitus, blurring of vision, deficiency sleeping, dreaminess, lassitude of the loins and legs, hemiplegia, red tongue with white coat or thin - yellow coat taut - thready - rapid pulse. 4. Therapy for norish vital essence - clearing away heat is availed in excessive fire caused by deficiency of yin of the liver and kidney, therapy for break through phlegm - clearing away heat in stagnant heat therapy for waking up a patient from unconsciousness - clearing away heat in yang type sthenia - syndrom of coma of apoplexy involving viscera and bowels. 5. Commonly used recipes of therapy for clearing away heat are Yang gyolksan(凉膈散), Bang pongtongseongsan(防風通聖散), Sotongseongsan(小通聖散), Jibodan(至寶丹), Supungsungisan(搜風順氣散), Woowhangchengshimwhan(牛黃淸心丸), Chengungsekgong(川芎石膏湯), Samwhatang(三化湯) etc in excessiveness symptom- complex, and are Yukmijiwhangweon(六味地黃元), Jiwhangtang(地黃湯), Palmiji whangtang(八味地黃湯), Samultanggagam(四物湯加減) etc in insufficiency symptom - complex.

음악요법이 제왕절개 임부의 수술불안에 미치는 영향 (The Effect of Music Therapy on Anxiety of Cesarean Section Wemen)

  • The purpose of this study was to identify the effects of music therapy on anxiety of cesarean section wemen. The research design was a nonequivalent control group non-synchronized design. The subjects of this study were 65 cesarean section wemen scheduled for surgery. The study was conducted from October 15, 1999 to January 5, 2000. Two groups consisted of subjects assigned : one the experimental group(33 wemen), and the other the control group(32 wemen). The music therapy was performed 3 times to experimental group just before surgery day, on surgery day. The level of anxiety was measured by State Anxiety Inventory, blood pressure, pulse rate, respiratory rate. The data were analyzed using descriptive statistics, $x^2-test$, t-test, Repeated measures of ANCOVA. The results of study were as fellows : 1) State anxiety scores were significantly different between the experimental group and the control group after music therapy. 2) Systolic blood pressures and diastolic blood pressures were significantly different between the experimental group and the control group after music therapy. 3) Pulse rates were significantly different between the experimental group and the control group after music therapy. 4) Respiratory rates were significantly different between the experimental group and the control group after music therapy. According to these results, music therapy can be regarded as an effective nursing intervention that relieves anxiety of cesarean section wemen.

미약 전자기장을 이용한 말초장애 치료시스템 개발 (Development of stimulator for peripheral disturbance therapy using A variable Micro-electromagnetic)

  • 당뇨 합병증 등으로 인한 말초혈류 및 신경장애, 수족냉증 및 손가락 관절염, 통증 질환이 증가하고 있으며, 이러한 문제를 개선할 수 있는 손가락 질환 전용 치료기기가 전무한 상태로 이와 관련된 치료기의 개발이 필요하다. 본 연구에서는 시변 미약자기장을 이용하여 비침습적으로 말초혈류/신경개선과 손가락 류마티스 치료를 위한 솔레노이드 원통형 코일을 제작하고, 다양한 치료목적에 사용하기 위해서 3가지 자극모드(N, S, N/S)와 자극주파수(0.25, 0.5, 1hz)를 선택할 수 있는 시스템을 설계하였다. 자속밀도 측정 프로브를 솔레노이드의 중심에서부터 거리를 0 ~ 3cm 단계로 자극방식과 주파수별로 자속밀도를 측정하였다. 자속밀도를 측정한 결과, 모든 자극방식과 주파수에서 솔레노이드 내부의 중심(0cm)에 근접할수록 자속밀도가 급격히 증가하였다. 솔레노이드 중심 자속밀도는 N자극 (294.3mT)과 S자극(293.8mT)에서는 1Hz, N/S자극(275.4mT)에서는 0.25Hz에서 가장 높게 측정되었다. 본 논문에서는 다양한 패턴과 강도의 자기장을 이용하여 다양한 손가락 질환을 치료할 수 있는 솔레노이드형 치료기기를 개발하였다. 솔레노이드 내부 자속밀도를 측정함으로써 손가락 말초질환 개선을 위한 미약자기장 시스템의 임상 적용 가능성을 확인하였다.

음악요법 유형이 척추마취 수술환자의 수술 중 불안 및 활력징후에 미치는 영향 (Effects of Types of Music in Music Therapy on Anxiety and Vital signs of Surgical Patients Undergoing Operation Using Spinal Anesthesia)

  • Purpose: The purpose of this research is to examine types of music (relax music or preferred music that patients have chosen) can effects on anxiety, blood pressure and pulse, and whether there are differences depending on the kinds of music in order to reduce anxiety of surgical patients using spinal anesthesia. Methods: This research's design is quasi-experimental design and non-equivalent control group pretest-posttest experimental design conducted on 60 surgical patients(experimental group 1=relax music therapy group, experimental group 2=preferred music therapy group, and group 3=control group) using spinal anesthesia. The Variables were trait anxiety, state anxiety, blood pressure, and pulse. Results: 1) In the state anxiety, there was a significant difference among the experimental groups 1, group 2 and the control group. 2) There was a significant difference in systolic blood pressure among the experimental group 1, group 2, and the control group. 3) In pulse, no significant difference among the experimental group 1, group 2 and the control group was detected. Conclusion: Regardless of the types of music, music therapy is thought to be effective nursing mediation to mitigate the state anxiety of surgical patients undergoing spinal anesthesia.
