뇌혈류 초음파 검사에서 기저동맥 박동지수에 영향을 미치는 인자 (Factors Affecting Basilar Artery Pulsatility Index on Transcranial Doppler)

  • 뇌혈류 초음파 검사는 비침습적인 방법으로 대뇌 혈관 혈류 속도와 방향을 도플러 원리로 측정하는 검사이다. 뇌혈류 초음파 검사의 박동지수는 원위부 혈관 저항을 반영하는 측정 지표이며 대뇌 소혈관 질환 발생과 확산지표로 사용되고 있다. 본연구의 목적은 허혈성 뇌졸중 환자에서 기저 동맥의 박동지수에 영향을 미치는 위험 인자를 평가하는데 있다. 2014년 1월부터 2015년 5월까지 내원 환자를 대상으로 뇌혈류 초음파 검사 박동지수를 측정하여 기저동맥 박동지수 대상자 422명을 선정하였다. 기저동맥 박동지수를 종속변수로 하여 단변량 분석을 실시하였고, 박동지수에 영향을 미치는 인자를 변수에 특징을 고려하여 다중회귀 분석을 실시하였다. 단변량 분석 결과 박동지수에 영향을 미치는 인자는 연령, 고혈압 유무, 당뇨 유무, 고지혈증 유무, 적혈구용적으로 나타났다(P<0.1) 다중회귀분석을 실시한 결과 연령(P<0.001), 당뇨 유무(P=0.004), 고지혈증 유무(P=0.041)가 통계적으로 유의한 결과를 보였다. 뇌혈류 초음파 검사의 박동지수에 영향을 미치는 위험인자는 고령, 당뇨, 고지혈증이다. 이후 추가적인 연구를 통하여 두 개 내 박동지수 증가는 고령, 당뇨, 고지혈증을 반영하는 지표로 활용 될 수 있을 것으로 생각된다.

  • Objective : This study was performed to evaluate the effect acupuncture on cerebral blood flow by Transcranial Doppler Ultrasonography(TCD). Methods : Monitoring of TCD was examined in stroke patients before and after acupuncture on seven points of CVA. Mean Velocity, pulsatility index, systolic velocity, diastolic velocity values are analyzed from TCD at middle cerebral artery. Results : The results showed a insignificant(p>0.05: Paired T-test) decrease in mean velocity, pulsatility index, systolic velocity, diastolic velocity, which was measured at normal middle cerebral artery. The results showed a significant($p{\leq}0.05$: Paired T-test) decrease in mean velocity, pulsatility index, systolic velocity, which was measured at injuried middle cerebral artery, except diastolic velocity. Conclusion : These results suggest that acupuncture could have a specific effect on injuried cerebral artery.

Pulsatility of middle cerebral arteries is better correlated with white matter hyperintensities than aortic stiffening

  • Background: Pulsatility of cerebral arteries and aortic stiffness have been associated with white matter hyperintensities (WMH). We explored which is better correlated with the severity of WMH in a population with acute lacunar infarct. Methods: We included patients with acute small subcortical infarcts who underwent transcranial Doppler (TCD) and brachial ankle pulse wave velocity (baPWV). Exclusion criteria were any stenosis or occlusion on major cerebral arteries on magnetic resonance angiography; poor temporal insonation windows; ankle brachial index < 0.9; and atrial fibrillation. We assessed the performance of the pulsatility index of bilateral middle cerebral arteries (PI-MCA) and baPWV for predicting moderate-to-severe WMH, defined as an Age Related White Matter Changes score > 5, and then sought to find independent predictors using binary logistic regression analysis. Results: Eighty-three patients (56 males, mean age $61.5{\pm}11.4$) participated in the study. Uni-variate analysis showed old age and high PI-MCA were significantly correlated with moderate-to-severe WMH. However, baPWV was not associated with the severity of WMH. Multivariate analysis revealed old age (odds ratio per 1-year increase, 1.068; p = 0.044) and upper tertile of PI-MCA (odds ratio, 5.138; p = 0.049) were independently associated with moderate-to-severe WMH. Receiver-operating characteristics showed PI-MCA differentiated those with and without moderate-to-severe WMH with an area under the curve of 0.719. Conclusions: PI-MCA derived from TCD was better correlated with the severity of WMH than baPWV in a population with lacunar infarction. Pulsatility of cerebral arteries may better predict cerebral small vessel disease than the aortic stiffness index.

이중 초음파 검사를 이용한 미숙아의 전 뇌동맥 혈류 측정 (The Measurement of Blood Flow of Anterior Cerebral Artery in Premature Newborns Using Duplex Doppler Ultrasonography)

  • 55명의 미숙아를 대상으로 이중 도플러 초음파 검사를 실시하여 대천문을 통한 전 뇌동맥의 혈류속도 및 혈류형태를 조사하였다. 미숙아에서 뇌 혈류속도 및 pulsatility index는 재태기간, 출생 체중, 및 미숙아의 나이 등의 증가에 따라 비례하여 증가하였으나 resistive index는 감소하였다. 뇌실 출혈이 있는 환아 군에서는 특징적인 resistive index의 증가가 보였지만 혈류속도 및 plusatility index는 불규칙하였다. 정상 미숙아의 뇌혈류 관찰 및 환아 군에서의 뇌혈류 검사에서 이중 도플러 초음파 검사는 비침습적이고 재현성도 뛰어나 선별검사로 유용한 방법이라고 생각한다.

  • Objective : We present reference values of flow velocities of intracranial and extracranial cerebral arteries and relation with change factor. Method : We checked transcranial Doppler ultrasonography on mean velocity, systolic velocity, pulsatility index, and resistance index of 252 patients. We also compared differences of change factor. Result : The result showed a difference by sex and age and relevance between vessels besides mean velocity of vessels related with past history and social history. Conclusion : According to the above results, females showed higher velocity of all vessels. With advancing age, subjects showed reduction in velocity and increase in pulsatility index and resistance index. Anterior cerebral, middle cerebral, and posterior cerebral artery increased in proportion to velocity of internal carotid artery and basilar artery. Vessel velocity correlated with diabetes mellitus, hypertension and hyperlipidemia.

합곡(合谷) 및 족삼이(足三里) 혈(穴) 자침이 인영혈(人迎穴)부위의 뇌혈류에 미치는 영향 (Effects of Hap-Kok(LI4) and Jok-Sam-Lee(ST36) Acupuncture on Cerebral Blood Flow in In-Young(ST9))

  • Objective : This study was performed to evaluate the effects of Hap-Kok(LI4) and Jok-Sam-Lee(ST36) acupuncture on blood flow in In-Young(ST9). Method : Monitoring of TCD was examined in each 25's healthy men and wemen in 20s before and after acupucture on both(right and left) Hap-kok(LI4) and was done on both(right and left) Jok-Sam-Lee(ST36) by different group with same condition. Mean velocity and Pulsatility index analyzed from TCD at both In-Young(ST9). Both group was acupuctured for 15-20 minutes laying at bed. Results : The results showed a significant($p{\le}0.05:$ Paired T-test) decrease in mean velocity and increase in pulsatility index at both group. Conclusion : These results suggest that acupuncture could have a specific effect on blood flow called In-Young acupoint.

  • Objectives : This study was performed to change of blood flow in In-Young(ST9) after treatment on chronic daily headache patients. Methods : Monitoring of TCD was examined in 10' s chronic daily headache patients before and after treatment. Mean velocity flow, systolic velocity and pulsatility index were analyzed from TCD at In-Young(ST9). The patients was laid for 20 minutes before treatment and also for treatment. Results : The results showed a significant(p<0.05: Paired T-test) decrease in mean velocity flow and systolic velocity. but there were no significant differences in pulsatility index. Conclusions : These results suggest that there were significant change on blood flow in In-young of chronic daily headache patients.

  • Objective : This study was performed to evaluate the effect of acupuncture on cerebral blood flow by Transcranial Doppler Ultrasonography(TCD) Method : Monitoring of TCD was examined in 11 healthy women volunteer(mean $age:\;24.5{\pm}2.9\;\;years$) before, during and after acupuncture on Paekoe(GV20). Mean velocity, pulsatility index, systolic velocity and diastolic velocity values are analyzed from DWL. MDX4 at different depths of both middle cerebral artery. Results : The results showed a significant(p<0.05; Friedman test & Wilcoxon signed ranks test) increase in mean velocity, systolic velocity and diastolic velocity decrease in pulsatility index, which was measured at different depths of both middle cerebral artery. Conclusion : These results suggest that acupuncture could have a specific effect on cerebral blood flow.

  • Sensors need to be implanted to obtain necessary information for LVAS (Left Ventricular Assist System) operations. Size of the sensors can prevent them from being implanted in a patient and reliabilities of the sensors are questionable for a long term use. In this wort we utilize a developed pump model to estimate flow and pressure difference across the pump without implanted sensors and present a method to obtain the physiological variables as aorta pressure and left ventricle pressure from the pump model and pulsatility of flow estimate or pressure difference estimate. These estimated variables can be used for LVAS control as an index or indices.

Transcranial Doppler and Cerebrospinal Fluid Flow Study in Normal Pressure Hydrocephalus

  • Objective : The authors analyze prospectively the result of transcranial doppler[TCD] in normal pressure hydrocephalus and compared its cerebral blood flow parameters to radionuclide cerebrospinal fluid[CSF] flow study, postoperative brain computed tomography[CT] findings and clinical outcome, and studied the relationship between cerebral hemodynamics and clinical performance. Methods : Twenty five patients with hydrocephalus undertook pre- and post-operative TCD but only preoperative CSF flow study was performed. Mean flow velocity[Vm], pulsatility index[PI] and resistance index[RI] were assessed through TCD and changes in ventricle size and cortical gyral atrophy were checked through brain CT. Results : Postoperative hydrocephalus showed an increase in Vm[ACA P=0.037, MCA P=0.034], decrease in PI[ACA P=0.019, MCA P=0.017] and decrease in RI [ACA P=0.017, MCA P=0.021] compared to preoperative TCD parameters in the postoperative improvement group. In the postoperative improvement group, postoperative TCD parameters correlated with CSF flow study grade [Vm : $R^2=-0.75$, PI : $R^2=0.86$, RI : $R^2=0.78$] and ventriculocranial ratio change correlated with PI change [$R^2=0.73$]. The convexity gyral atrophy and initial TCD parameters showed close relationship to outcome. Conclusion : PI and RI can be used as an indicator of post operative prognosis, and with the addition of CSF flow study values, can also be used as a tool to predict pre-op and post-op patient status and successful shunt surgery.
