본 실험은 유성견에서 인위적인 정출 시 조직학적, 임상학적인 반응과 시간경과에 따른 치아 및 치아 주위조직의 회복을 연구하기 위해 생후 평균 10 개월 웅성 잡종 유성견 하악 소구치를 대상으로 Energy chain을 이용한 정출 실험으로, 실험 1 군은 힘을 작용 시킨 후 일 주일만에 희생시키고 실험 2 군은 정출력을 가한 후 일 주일 후, 정출력이 제거되고 3 주간의 회복 기간 경과 후에 희생시켰다. HE 및 Masson's Trichrome 염색법을 이용하여 치아와 주위 조직의 반응을 치경부, 치근 중앙부, 치근단부, 치수부로 분류하여 광학 현미경하에서 관찰하여 다음과 같은 결과를 얻었다. 1. 힘을 가한 후 일 주일 후, 모든 부위에서 치주인대의 주행방향이 치아를 향하여 사선 방향을 나타내 주었으며 치주인대의 간격이 전반적으로 증가되었다. 또 부분적인 치주 인대의 파열이 치근단부에서 뚜렷하게 관찰되었다. MT 소견에서는 대조군에 비해 치조능 부위에서 미성숙 골의 형성을 보여주었다. 주위 치조골에서는 편측으로 활발한 골조직의 형성이 관찰되었다. 치수에서는 치근단 부위에서 많은 혈관의 파열을 보여 주었고 공포의 형성과 충혈을 나타냈다. 2. 힘을 제거 된 후3주간의 보정을 한 군의 모든 치주인대의 주행 방향과 간격이 대조군과 같이 회복되었으며 밀도는 치밀하게 나타났다. MT소견에서 1군에서 형성된 치조능의 미성숙 골이 석회화가 이루어짐을 보여주었고 치수의 공포와 충혈의 감소를 보여주었다. 수복과정에서의 치수의 섬유화가 더욱 진행되었다. 이상의 결과를 종합해 볼 때, 3주간의 회복 기간 중에도 치주 인대 부위에서는 대조군과 유사한 조직 소견을 보여 상당한 회복능력을 보였지만 치수에서는 섬유화가 진행됨을 보여 더욱 긴 회복 기간이 필요함을 보여주었다.
본 연구에서는 DSP (dentin sialoprotein)에서 유래된 합성 펩타이드를 동물실험 모델에 적용하여 치수노출 부위에서 상아질 재생을 확인하고, 기존 치 수복조제와의 성능 비교를 통해 새로운 치수복조제로서의 가능성을 확인하고자 하였다. 6마리 비글견의 73개의 치아를 이용하여, 실험적으로 치수를 노출하고 직접 치수 복조술을 시행하였다. 사용한 치수복조제는 (1) $Ca(OH)_2$ (CH군) (2) DSP유도 합성 펩타이드 (PEP군) (3) 합성 펩타이드와 $Ca(OH)_2$ 혼합제 (PEP+CH군) (4) White MTA (WMTA군) 이다. 노출된 치수에 치수복조제를 적용한 후 와동은 강화형 글라스 아이오노머로 충전하였다. 시술 후 2주, 1 개월 및 3개월에 각각 2마리씩 비글견을 희생시키고 조직시편을 제작하였다. 시편은 H&E 염색 후 광학 현미경으로 치수 염증 반응과 경조직 형성 정도를 관찰하였다. PEP군에서는 17개의 시편 중 3 개의 시편에서만 경조직 형성을 관찰할 수 있었으며, 대부분의 시편에서 적절한 치수 회복을 관찰할 수 없었다. PEP군은 CH군에 비해 심한 염증반응을 보이고, 경조직 형성은 불량하였다. CH군과 WMTA군은 기계적으로 노출된 치수에서의 염증반응과 경조직 형성에 있어서 유사한 결과를 보였다.
Dental amalgam in the clinic practice of dentistry is one of the most important materials. Mercury, one of the component of this alloy, is emitted in the form of vapor after filling as long as 5 days Silver particles penetrate into the dentine deeply along the tubles underneath cavity floor. To determine the permeability of mercury in the teeth following experiments were performed. Class 5 cavities total 40 from 10 dogs were prepared on upper and lower canines and amalgam alloys which contain about 10uCi of radioactive mercury were inserted. The animals were sacrificed 7 days after the experiments and the teeth were decalcified, sectioned and autoradiographed by means of emulsion and stained by H & E. Following are the results obtained from this experiments.
1. Blackened silver grains were found along the dentinal tubules underneath the cavity floor. Beyond the border of dentine and pulp chamber grains were seen in odontoblastic layer (Fig. 1, 2, 3, 4)
2. Underneath the odontoblastic layer, the pulp tissue showed almost normal appearance except slight dilatation of blood vessel.
Finger reconstruction involves paramount significance of both functional and aesthetic aspects, due to its great impact on quality of life. The options range from primary closure, skin grafts, local flaps, pedicled flaps, and free flaps. The optimal method should consider various circumstances of the patient and surgeon. We would like to report a case of a young woman who initially presented with cellulitis and necrosis of the left second finger-tip who underwent reconstruction with bilateral toe pulp free flap. The patient could successfully return to her job that involves keyboard typing and playing the piano, with acceptable donor site morbidity.

Introduction: The first aim of this study was to isolate the dental tissue-derived stem cells from the dental follicle (DF), dental pulp (DP), and root apical papilla (RAP) of the extracted wisdom teeth. Second was to evaluate their characterization with the expressions of transcription factors and cell surface markers. Finally, their ability of the in vitro multi-lineage differentiations into osteogenic and adipogenic cells were compared, respectively. Materials and Methods: Dental tissues, including dental follicle, dental pulp, and root apical papilla, were separated in the extracted wisdom teeth. These three dental tissues were cultured in Dulbecco’s modified Eagle’s medium (DMEM) with supplements, respectively. After passage 3, the homogeneous shaped dental tissue-derived cells were analyzed the expression of transcription factors (Oct-4, Nanog and Sox-2) and cell surface markers (CD44, CD90 and CD105) with reverse transcription polymerase chain reaction (RT-PCR) and fluorescence-activated cell sorting (FACS) analysis. In order to evaluate in vitro multi-lineage differentiations, the culture media were changed to the osteogenic and adipogenic induction mediums when the dental tissue-derived cells reached to passage 3. The characteristics of these three dental tissue-derived cells were compared with immunohistochemistry. Results: During primary culture, heterogenous and colony formatted dental tissue-derived cells were observed in the culture plates. After passage 2 or 3, homogenous spindle-like cells were observed in all culture plates. Transcription factors and mesenchymal stem cell markers were positively observed in all three types of dental tissue-derived cells. However, the quantity of expressed transcription factors was most large in RAP-derived cells. In all three types of dental tissue-derived cells, osteogenic and adipogenic differentiations were observed after treatment of specific induction media. In vitro adipogenic differentiation was similar among these three types of cells. In vitro osteogenic differentiation was most strongly and frequently observed in the RAP-derived cells, whereas rarely osteogenic differentiation was observed in the DP-derived cells. Conclusion: These findings suggest that three types of human dental tissue-derived cells from extracted wisdom teeth were multipotent mesenchymal stem cells, have the properties of multi-lineage differentiations. Especially, stem cells from root apical papilla (SCAP) have much advantage in osteogenic differentiation, whereas dental follicle cells (DFCs) have a characteristic of easy adipogenic differentiation.
Background Fingertip injuries involving subtotal or total loss of the digital pulp are common types of hand injuries and require reconstruction that is able to provide stable padding and sensory recovery. There are various techniques used for reconstruction of fingertip injuries, but the most effective method is functionally and aesthetically controversial. Despite some disadvantages, cross-finger pulp flap is a relatively simple procedure without significant complications or requiring special techniques. Methods This study included 90 patients with fingertip defects who underwent cross-finger pulp flap between September 1998 and March 2010. In 69 cases, neurorrhaphy was performed between the pulp branch from the proper digital nerve and the recipient's sensory nerve for good sensibility of the injured fingertip. In order to evaluate the outcome of our surgical method, we observed two-point discrimination in the early (3 months) and late (12 to 40 months) postoperative periods. Results Most of the cases had cosmetically and functionally acceptable outcomes. The average defect size was $1.7{\times}1.5$ cm. Sensory return began 3 months after flap application. The two-point discrimination was measured at 4.6 mm (range, 3 to 6 mm) in our method and 7.2 mm (range, 4 to 9 mm) in non-innervated cross-finger pulp flaps. Conclusions The innervated cross-finger pulp flap is a safe and reliable procedure for lateral oblique, volar oblique, and transverse fingertip amputations. Our procedure is simple to perform under local anesthesia, and is able to provide both mechanical stability and sensory recovery. We recommend this method for reconstruction of fingertip injuries.
Polymerization of light-activated restorations results in temperature increase caused by both the exothermic reaction process and the energy absorbed during irradiation. Within composite resin, temperature increases up to 2$0^{\circ}C$ or more during polymerization. But, insulation of hard tissue of tooth lowers this temperature increase in pulp. However, many clinicians are concerned about intrapulpal temperature injury. The purpose of this study was to evaluate temperature changes in the pulp according to various restorative materials and bases during curing procedure. Caries and restoration-free mandibular molars extracted within three months were prepared Class I cavity of 3$\times$6mm with high speed handpiece fissure bur. 1mm depth of dentin was evaluated with micrometer in mesial and distal pulp horns. Pulp chambers were filled with 37.0$\pm$0.1$^{\circ}C$ water to CEJ. Chromium-alumina thermocouple was placed in pulp horn below restorative materials for evaluating of temperature changes. This thermocouple was connected to temperature-recording device(Multiplication analyzer MX, 6.000, JAPAN). Temperature changes was evaluated from initial 37.$0^{\circ}C$ after temperature changes to 37.$0^{\circ}C$. Tip of curing unit was placed in the center of prepared cavity separated 1mm from restorative materials. Curing time was 40s. The restorative materials were used with Z 100, Fuji II LC, Compoglass flow and bases were used with Vitrebond, Dycal. Resrorative materials were placed in 2mm. The depth of bases were formed in 1mm and in this upper portion, resin of 2mm depth was placed. This procedure was performed 10 times. The results were as follows. 1. All the groups showed that the temperature in pulp increased as curing time increased 2. The temperature increase of glass ionomer was significantly higher than that of Resin and Compomer during curing procedure (P<0.05). 3. The temperature increase in glass ionomer base was significantly higher than that of Calcium hydroxide base during Resin curing procedure (P<0.05).
The responses of human pulp fibroblastic cells to Ga-As Semi-Conductor-Dens-Bio Laser (Frequency: 5 Hz~10,000 Hz Model: SD-101A RCA, U.SA)) were examined in vitro using pulp fibroblastic cells obtained from the pulp tissue of human tooth. The mitogenic effect of soft laser was assessed by measuring the MTT assay. The morphologic effect for soft laser showed under the scanning and transmission electron microscopy. The results as follows; 1. The mitogenic response of the soft laser was not observed until 4th time of radiation, while the mitogenic response at 4th time increased mitogenic effect by as much as 1.7 fold compared to the control value. 2. The mitogenic response of the soft laser on pulp fibroblast differ from the mitogenic response on other fibroblasts. 3. In scanning electron microscopic study, The microvilli of cell surface increased gradually with width and length after laser radiation, it demonstrate that development of microvilli have close connection with differentiation of cells. 4. Under the transmission electron microscope, The laser-treated cells maintained their elongated shape and a high degree of cellular polarization. The large cell body containing a well developed Golgi complex, a large number of profiles of rough endoplasmic reticulum, and great numbers of mitochondria. 5. The laser-treated cells maintained the long straight bundles of closely apposed microfilaments or individual filaments forming a cross-linked network. These findings suggest that the laser may have important roles in promotion of pulp healing and consequently may be useful for clinical application in pulp regenerative procedures.
This study was performed to elucidate the distribution and correlation of immunoglobulin G subclasses with the degree of inflammation in the experimentally induced rat pulp and periapical pathoses. The pulp exposures were made in 108 mandibular 1st molars of 54 rats and the teeth were left open to the oral environment The animals were sacrified at 3, 7, 15, 30, 60 and 90 days after pulp exposure, and examined microscopically and radiographically Seventy one specimens were routinely sectioned at the thickness of 4 - $6{\mu}$ and stained with Hematoxylin - eosin for histologic examination, with toluidine blue for mast cells, and with the primary antibodies against rat IgG subclasses by using the Avidin - Biotin complex method. The following results were obtained: 1. As the degree of inflammation of rat pulp and periapeces intensified, the number of IgG subclass containing cells per unit area, especially IgG2a and IgG2c, decresased. 2. The IgG2c cells were most predominantly found in the lesions with slight inflammation, IgG1 cells in mild or severe inflammation, and IgG2a cells in moderate inflammation. 3. IgG subclass containg cells were more predominantly observed in the periapical granuloma than periapical abscess or cyst(p<0.01). 4. IgG2a containing cells were predominant in pulp inflammation, IgG1 containing cells in periapical granuloma, IgG2a cells and IgG1 cells in periapical abscess, and IgG2a cells were significantly predominant in periapical cyst. 5. The number of IgG subclass containing cells and mast cells in periapical tissue decreased with time lapse after pulp exposure. And correlation index between mast cells and IgG1, IgG2a, IgG2b was stastically high.
저자는 흰쥐 27수의 하악제일대구치를 교정용 고무선으로 이동시킨후, 이동의 초기에 나타나는 치수의 반응을 조직학적 방법으로 관찰하는 것을 연구목적으로 하였다. 30 Gm부터 120 Gm까지의 힘을, 12수는 3일간, 15수는 7일간, 각 실험치아에 부여하였다. 관찰결과는 다음과 같았다. 1. 치수조직의 주변화는 치수혈관의 순환장애와 조상아세포의 공포형상으로 나타났다. 2. 적용시킨 힘의 증가에 따라서 상기 조직변화의 범위가 확대되었다. 3. 기외에 치근의 상아질과 백악질의 부분적 흡수도 볼 수 있었다.
