판막 치환 수술 후 생긴 판막 주위 누출과 관련된 대동맥동류의 심실 중격 박리-1례 보고- (Aneurysm of the Sinus of Valsalva Oissecting into the Ventricular Septurn Associated with Paravalvular Leakage After Double Valve Replacement)

  • 저자들은 대동맥 판막치환술 후 생긴 대동맥 판막 주위 누출과 관련된 대동맥동류의 심실 중격 박리를 경험하였다. 37세 남자 환자가 호흡 곤란의 증가를 주소로 응급실을 통하여 입원하였다. 그는 6년전 대동맥판 폐쇄 부전과 승모판 폐쇄 부전으로 인공 기계 판막(Carbomedic 23mm와 31mm)으로 치환 수술을 받고 정기적으로 추적 검진 중이었다. 진단은 경흉부및 경식도 심초음파로 이루어졌고, 대동맥 조영술로 확인되었다. 폐동맥판 동종이식편(Pulmonary homoyaft)를 사용하여 대동맥 판막 치환술을 다시 시행하였다. 심실중격내의 낭 입구에서 3-0 prolene을 사용하여 심근 내막, 외막과 homograft muscle shoulder를 한꺼번에 연속 봉합하였다. 술후 경과는 양호하여 환자는 수술 후 11일째 퇴원하였다.

Takayasu 동맥염에 동반된 심판막질환에서의 삼중판막수술 치험 1례 (Double valve replacement in Takayasu's disease -Report of one case-)

  • Takayasu`s disease is an arteritis of unknown etiology involving larger elastic arteries such as aorta and its branches, pulmonary arteries and rarely coronary arteries. Especially, aortic root involvement with the valvular leaflets has been reported in several cases of Takayasu`s arteritis. Recently we have experienced one case of Takayasu`s arteritis involving left subclavian artery, descending aorta, left renal artery and multiple valvular leaflets. The patient was 33 year-old female and admitted with complaints of cough, dyspnea and general weakness. Aortogram revealed extensive type of arteritis showing dilatation of ascending aorta, segmental narrowing of thoracic aorta and Riolan`s anastomosis. Double valve replacement [mitral and aortic valve] and tricuspid valve annuloplasty were performed. The patient made an excellent postoperative recovery and has shown striking improvement in cardiac status, NYHA functional class II eight months after operation.

The change of QRS duration after pulmonary valve replacement in patients with repaired tetralogy of Fallot and pulmonary regurgitation

  • Purpose: This study aimed to analyze changes in QRS duration and cardiothoracic ratio (CTR) following pulmonary valve replacement (PVR) in patients with tetralogy of Fallot (TOF). Methods: Children and adolescents who had previously undergone total repair for TOF (n=67; median age, 16 years) who required elective PVR for pulmonary regurgitation and/or right ventricular out tract obstruction were included in this study. The QRS duration and CTR were measured pre- and postoperatively and postoperative changes were evaluated. Results: Following PVR, the CTR significantly decreased (pre-PVR $57.2%{\pm}6.2%$, post-PVR $53.8%{\pm}5.5%$, P=0.002). The postoperative QRS duration showed a tendency to decrease (pre-PVR $162.7{\pm}26.4$ msec, post-PVR $156.4{\pm}24.4$ msec, P=0.124). QRS duration was greater than 180 msec in 6 patients prior to PVR. Of these, 5 patients showed a decrease in QRS duration following PVR; QRS duration was less than 180 msec in 2 patients, and QRS duration remained greater than 180 msec in 3 patients, including 2 patients with diffuse postoperative right ventricular outflow tract hypokinesis. Six patients had coexisting arrhythmias before PVR; 2 patients, atrial tachycardia; 3 patients, premature ventricular contraction; and 1 patient, premature atrial contraction. None of the patients presented with arrhythmia following PVR. Conclusion: The CTR and QRS duration reduced following PVR. However, QRS duration may not decrease below 180 msec after PVR, particularly in patients with right ventricular outflow tract hypokinesis. The CTR and ECG may provide additional clinical information on changes in right ventricular volume and/or pressure in these patients.

삼첨판막 치환술의 장기성적 (Long-Term Result of Tricuspid Valve Replacement)

  • 배경: 삼첨판막친환술은 매우 드물게 시행되는 수술이며 그 장기 성적은 만족치 못한 수준이다. 또한 어떤 종류의 인공판막을 사용하느냐에 대하여도 논란이 많은 상황이다. 서울대학교병원 흉부외과에서는 1989년 1월부터 1998년 12월까지 10년동안 71명의 환자에서 72례의 삼첨판막 치환술을 시행하였으며 이 결과를 토대로 장단기 성적과 위험요인들을 분석하였다. 대상 및 방법: 평균나이는 42$\pm$13세(16~65세)였으며 남여비는 32/39였다. 술전진단은 50례의 후천성판막질환과 18례의 선천성심장질환이 있었고 삼첨판폐쇄부전만 단독으로 있었던 경우도 4례 있었다. 사용된 인공판막은 기계판막이 69개, 조직판막이 3개였다. 승모판막치환술 또는 대동맥판막치환술과 같이 시행된 경우는 50례였고 1례에서는 폐동맥판막 치환술이 같이 시행되었다. 결과: 조기사망은 7례(9.7%), 만기사망은 7례(13.0%)였고 10년 생존율은 59.2$\pm$7.2%였다. 삼천판막혈전증은 5례에서 11번에 결쳐 발생하였으며 그중 1례는 재수술을 시행받았다. 생존자들의 대부분은 심장기능분류 I-II의 상태로 현재까지 외래 추적관찰중이다. 결론: 삼첨판막치환술은 비록 혈전증등의 위험이 상존하기는 하지만 비교적 낮은 사망률과 이환율을 보이고 있으며 기계판막의 경우에도 조직판막과 비교하여 큰 차이 없이 좋은 장기성적을 얻을 수 있었다.

대동맥판협착 및 폐쇄부전증을 동반한 관상동정맥루 -1례 치험- (Coronary Arteriovenous Fistula Associated with Aortic Stenosis and Regurgitation -Report of a Case)

  • Congenital coronary arteriovenous fistula is relatively uncommon and often associated with additional congenital and acquired heart disease. If coronary arteriovenous fistula is suspected, the diagnosis can be made readily by cardiac catheterization and selective coronary arteriography. Surgical treatment is very satisfactory, with a low mortality and apparent good long term result. Recently, we experienced one case of congenital coronary arteriovenous fistula which was associated with aortic stenosis and regurgitation. The tortuous fistula tract was noted between the left anterior descending coronary artery and the main pulmonary artery. Under the cardiopulmonary bypass, aortic valve replacement[Carbomedics 23mm] and suture closure of the draining orifice of coronary arteriovenous fistula in the main pulmonary artery just above the pulmonary valve were performed, Postoperative hospital course was uneventful and the patient was discharged postoperative 9th day without any problems.

자가폐동맥판막을 이용한 대동맥심실성형술 -1례 보고- (Aortoventriculoplasty with The Pulmonary Autograft The "Ross-Konno"Procedure -1 case report)

  • 복합적인 좌심실유출로협착이 있는 환자에서는 여러종류의 대동맥심실성형술이 치료의 옵션이다 소아에서 대동맥심실성 형술시 폐동맥자가판막은 판막치환의 재료로써 매우 희망적 이라고 하겠다. 세종병원 흉부외과는 선천성대동맥헙 착, 대동맥 폐쇄부전, 그리고 심한 좌심실기능부전이 있는 6개월된 여아 1례에서 폐동맥자가판막을 이용한 대동맥심실성형술을 시행하여 좋은 결과를 얻었기에 보고하는 바이다.

  • 일반적으로 폐질환과 심장 질환이 동시에 발견되었을 때 동시 수술은 술자가 고려해야 되는 사항이 많다. 59세 여자 환자가 우중엽절제술과 이중판막 재치환술과 삼첨판막륜성형술을 동시에 시행 받았다. 동시 수술은 폐와 심장의 병변을 일거에 해소하면서 2차적으로 시행해야 하는 폐절제술 또는 심장수술에 따르는 위험성을 피할 수 있는 장점이 있으며 안전하게 시행할 수 있는 술식이다.

A Hybrid Intervention for Post-infarction Papillary Muscle Rupture with Severe Mitral Regurgitation: A Case Report

  • Papillary muscle rupture with severe acute mitral regurgitation is a rare complication of acute myocardial infarction (AMI) that causes pulmonary congestion and cardiogenic shock. Moreover, it has a poor prognosis. Surgical intervention, including revascularization, is indicated; however, surgical mortality remains high. We report the case of an 85-year-old woman with cardiogenic shock from severe acute mitral regurgitation, in whom a hybrid intervention, combining percutaneous coronary intervention with mitral valve replacement via minithoracotomy, was performed after post-infarction papillary muscle rupture. She was discharged in a favorable clinical condition. We describe a novel hybrid intervention for treating a rare complication of AMI, which could minimize surgical invasion in elderly patients, prevent disuse syndrome after the intervention, and improve prognosis. However, mitral valve surgery via minithoracotomy for emergency cases requires technical proficiency, as well as collaboration with other healthcare professionals, and the choice to perform this procedure requires careful consideration.

심장혈관질환의 수술치험 (clinical experience of cardiovascular surgery -An Analysis of 116 cases-)

  • From Aug. 1984 to May 1986, 116 cases of cardiovascular surgery including 85 cases of open heart surgery were performed at Korea Veterans Hospital. There were 108 congenital anomalies and 8 acquired valvular heart diseases. Among 108 congenital malformations 92 operations were done for acyanotic group, and 16 operations for cyanotic group. Thirteen cases of open heart surgery for infants or small children under 10kg of body weight were performed, which occupied 15.3 percent of total open heart surgery done in the same period. Common congenital cardiovascular anomalies were ventricular septal defect [40.7%], patent ductus arteriosus [29.6%], tetralogy of Fallot [12.[%], atrial septal defect [11.1%], and pulmonary stenosis [1.9%] in order of frequency. Valve replacement using lonescu-Shiley or Carpentier-Edwards valve was performed for 8 cases of acquired mitral valve disease, and valve replacement using St. Jude valve was done for a case of patent ductus arteriosus with severe mitral insufficiency. There was no mortality in acyanotic congenital malformations and acquired valve lesions, and 5 cases of mortality in cyanotic congenital malformations. Overall mortality was 4.3 percent for total cases and 5.9 percent for 85 cases of open heart surgery.

후천성 삼첨판폐쇄부전증의 외과적 치료에 대한 연구 (Clinical Study of Surgical Treatment of Acquired Tricuspid Regurgitation)

  • Fourty-five cases of operation were performed for the correction of tricuspid regurgitation [TR] in Pusan National University Hospital between 1982 and 1991.The mean age of the patients was 32.6 years and female was dominant[M:F=1:2.2].Isolated tricuspid regurgitation was rare and 43 patients underwent concomitant other valvular operation including mitral valve replacement. Functional cause was in 39 cases and organic lesions were found in 6 cases. Operative methods were Kay annuloplasty[29], De Vega annuloplasty[12], and tricuspid valve replacement[4]. Ring annuloplasty was not performed. Operative mortality rate was 11.1%[5/45] and late mortality rate was 6.7%[2/30]. The tricuspid valve surgery itself was not a serious risk factor for hospital death and no heart block nor thrombosis was complicated. By echocardiogram early[within 30 days] and late [mean:4.9years] changes of postoperative TR were evaluated. De Vega annuloplasty seemed to bring better late result than Kay annuloplasty[p<0.05]. In four patients with late severe TR,previously replaced tissue valve degeneration[2], pulmonary hypertension[1] and rheumatic TR[1] were the possible causes. This experience suggests that De Vega annuloplasty can be a reliable method in the majority of patients with moderate-to-severe functional TR.
