A 76-year-old woman with a history of chest pain and palpitation, was diagnosed with bilateral coronary to pulmonary artery fistulae with a concomitant saccular aneurysm, which is quite rare. Suture closure of the fistular vessels around the pulmonary artery root, the removal of a saccular aneurysm, and the transpulmonary closure of coronary to pulmonary artery fistulae were performed. The patient was well at 4 months after surgery.
Bronchopulmonary sequestration (BPS) is a rare congenital abnormality of the lower airway, generally characterized by blood supply received from the systemic circulation. We present a rare case of a 19-year-old man with incidentally detected BPS supplied by a branch of a pulmonary artery, rather than a systemic artery. Computed tomography showed a sequestered segment supplied by a branch of the left pulmonary artery and containing an ectopic bronchus. As chest computed tomography revealed necrosis in the sequestered tissue, infection was presumed, and the tissue was surgically removed. This may represent a very unusual occurrence, as such cases have yet to be reported in the literature.
A 32-year-old woman presented with cough and hemoptysis. The radiologic findings showed increased interstitial markings in the right lung, a slightly decreased lung volume in the RLL and a hypoplastic right pulmonary artery with collaterals in the mediastinum and subpleural area. The pulmonary angiography showed an abrupt occlusion of the right lower pulmonary artery. The echocardiographic findings indicated pulmonary hypertension. A doppler leg ultrasonograph disclosed that the left popliteal vein was occluded with collateral veins, not filling the defect in the venous lumen. The D-dimer increased 1.0 ug/ml. This condition was initially misdiagnosed as a congenital pulmonary artery agenesis. Finally, a chronic pulmonary thromboembolism with a deep vein thrombosis was confirmed.
Ten patients with tetralogy of Fallot were studied angiocardiographically before a modified Blalock-Taussig shunt and again 25*3.2 months after the previous shunt. All of ten patients had patent previous shunt at the time of follow up examination. Pre-and postoperative diameters of left and right pulmonary artery and descending aorta were measured and pulmonary artery index [the sum of the crossectional areas of the right and left pulmonary arteries standardized by the body surface area] was calculated. The ratio of mean diameter of left and right pulmonary arteries to the diameter of the descending aorta [LPA+RPA/2xDA] was increased postoperatively by 0.20*0.068 [p=0.020]. Mean PAI [pulmonary artery index] increased from 283.8*178.4 mm/m2 BSA to 345.8~144.5 mm/m2 BSA after shunt operation [p=0.019]. This results suggested that the modified Blalock-Taussig shunt was effective to help growth of the pulmonary arteries in most cases of the study populations but the ones with the PAI>233mm*/m* BSA appeared less benefited by Blalock Taussig shunt. Calculation of PAI could be an aid to making a decision whether to perform a one stage corrective surgical procedure or a palliative shunt procedure in the patient with small pulmonary arteries.
Congenital coronary artery fistula is a rare condition, and with widespread use of cardiac catheterization, angiography and selective coronary arteriography are being recognized with increasing frequency. Fistula originating from the right coronary artery are more common then those from the left coronary artery. The fistula empties into the right side of the heart in 90% of the cases with the right ventricle being the most common recipient chamber followed by the right atrium and the pulmonary artery. Recently we experienced two cases of congenital coronary artery fistula which originated from the left coronary artery each other. The first case was 17 moth-old-male, who have had the symptoms of frequent URI, dyspnea and continuous murmur in physical examination. The fistulous communication was noted between the left circumflex coronary artery and the right ventricle with aneurysmal dilation of RV wall. The proximal opening of the fistulous tract was directly close with partial aneurysmorrhaphy of RV wall. Also the termination site of fistulous tract in RV chamber was closed. The second case was 35-year-old female, who have had the symptom of exertional dyspnea and continuous murmur in physical examination. The tortuous and dilated fistulous tract was noted between the left anterior descending coronary artery and the pulmonary artery. The proximal opening of the fistula was ligated near the left anterior descending coronary artery with preservation of normal continuity of coronary artery. And the dilated tortuous vessel was excised. Also the terminal site in pulmonary artery was directly closed just above the pulmonic valve. Postoperative hospital courses of two patients were uneventful without any specific complications and discharged without problems.

This retrospective, echocardiographic study using 144 dogs with clear systolic tricuspid regurgitation on Doppler echocardiography was performed to determine the diagnostic value of the systolic tricuspid regurgitation velocity/pulmonary artery flow velocity time integral to predict the Doppler estimates of dogs with tricuspid regurgitation pressure gradient compared with other cardiac indices of pulmonary hypertension, and to investigate a cutoff value to select patients with a potentially poor outcome. The systolic tricuspid regurgitation velocity/pulmonary artery flow velocity time integral increased significantly as the severity of pulmonary hypertension increased and had a correlation coefficient that was analogous to those of other conventional cardiac indices. A cutoff value greater 1.65 provided the best-balanced sensitivity (84%) and specificity (80%) in determining patients with a poor prognosis. In conclusion, the systolic tricuspid regurgitation velocity/pulmonary artery flow velocity time integral is readily obtained using routine echocardiography and could provide a non-invasive, novel, and supplementary index for evaluating dogs with pulmonary hypertension as useful prognostic criteria, particularly in those with advanced pulmonary hypertension.
A 43-year-old woman who had had an invasive mole 5 years previously required emergent pulmonary embolectomy under cardiopulmonary bypass. Curative resection was impossible because the tumor invaded the right main pulmonary artery and left lower pulmonary artery. The pathologic diagnosis made by the tumor emboli specimens was choriocarcinoma. The patient received post-operative chemotherapy over a 6-month period and had complete remission. Although rare, choriocarcinoma should be considered in the differential diagnosis of fertile women presented with pulmonary embolism.
Not only morphology of pulmonary artery or pulmonary artery resistance, but major aortopulmonary collateral arteries are risk factors of Fontan procedure. We report a successful Fontan procedure after rehabilitation of pulmonary arteries by unifocalization and systemic to pulmonary shunt in a high risk Fontan candidate with functional single ventricle combined with hypoplastic pulmonary arteries and major aortopulmonary collateral arteries supplying most of the bilateral lung field.
Intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derives its arterial blood supply through aberrant vessel directly of systemic circulation. Intralobar pulmonary sequestration Is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We experienced a case of pulmonary sequestration preoperatively confirmed. The patient was 17 year old female whose complaints were mild fever and profuse purulent sputum. Chest film showed a large thin walled cystic lesion with air-fluid level at the left lower posterior basal lung field. Aortogram revealed an aberrant artery originated from thoracic aorta just above the diaphragm and that drained via pulmonary vein into the left atrium. At time of operation, a large abscess cavity measuring 9x8x3 cm in dimension at the left lower lobe was noted. And the aberrant artery, measuring 0.5 cm in diameter and 2 cm in length, arising from thoracic aorta just above the diaphragm was noted. After division and ligation of the aberrant artery, a left lower lobectomy was performed and the patient`s postoperative course was uneventful.
Children with congenital cardiac defects associated with high pulmonary artery pressure may die despite accurate surgery. Postoperative mortality and morbidity have been attributed to acute rises in pulmonary artery pressure and resistance. Acute pulmonary hypertensive crisis is defined as a paroxysmal event in which pulmonary arterial systolic pressure rises to or above systemic levels followed by a rapid fall in systemic pressure and a minor pulmonary hypertensive event is defined as an acute rise in pulmonary arterial pressure to more than 80 % of systemic levels but without a fall in systemic pressure. From Oct. 1988 to Jul. 1989, we experienced 23 patients who showed many pulmonary hypertensive crises after operation in the Department of Thoracic and Cardiovascular Surgery, Seoul National University Children\ulcorner Hospital. Their preoperative PAP/SAPs were 53 to 123 %[mean 93.3%] and diagnoses were VSD[7], TAPVR[5], TGA[4], AVSD[3], MS[1], DORV[1], Truncus arteriosus[1], and AP window[l]. There were 9 deaths among 23 patients and they showed many pulmonary hypertensive crisis episodes during postoperative intensive care, which was managed by sedation, hyperventilation, oxygen, and acidosis correction and which decreased after using tolazoline. In view of our experience, we recommend that pulmonary artery pressure should be monitored in congenital heart defected patient with preoperative pulmonary hypertension to confirm and to manage the pulmonary hypertensive crisis accurately and using tolazoline is helpful in the treatment of pulmonary hypertensive crisis.
