The Surgical Treatment of Chronic Avulsion Fracture of the Anterior Cruciate Ligament (진구성 전방 십자 인대 견열 골절의 수술적 치료)

  • Objective : The purpose of this study was to evaluate the clinical and radiological results after treatment of old ACL(anterior cruciate ligament) avulsion fracture of tibia. Materials and Method : 11 cases who were followed up at least 2 years after treatment in chronic ACL avulsion fracture of tibia were included in this study. The average age of patients was 26(9-66) years and the mean period of follow up was 53(24-131) months. After evaluation of the lesion and treatment of associated lesion under arthroscope, open reduction and internal fixation or fragment removal was performed. Pull-out suture was performed in 8 cases, screw fixation in 2 cases. Results : The Lysholm knee score was 64.3 in average preoperatively and improved to 96.2 average at follow up. On the Lachman test, there were mild(+) instabilities in 7 cases, moderate(++) in 4 cases. On the postoperative Lachman test, there were no instabilities in 9 cases, mild(+) instabilities in only 2 cases. According to Meyers and Mckeever's evaluation protocol, 9 cases $(81.8\%)$ were excellent, 2 case $(18.2\%)$, good. On preoperative instrumented anterior laxity test with $Telos^{\circledR}$, side to side difference in 201b was 7.8(10-4) min in average and 2.1(6-0) mm in average at follow-up. Conclusion : In chronic ACL avulsion fracture, it is thought that combination of arthroscopic evaluation of associated injury and open reduction and fixation and fragment removal can bring about satisfactory results.

Arthroscopic Treatment of Displaced Tibial Spine Fracture in Children (소아에서의 전위된 경골극 골절의 관절경적 치료)

  • Purpose: To analyze the clinical result of the arthroscopic reduction and fixation for the displaced tibial spine fractures in children and report the utility of the arthroscopic treatment with the review of the literature. Methods: Between December 2000 and July 2004, five patients (average age 9.1 years) received an arthroscopic reduction and fixation of displaced tibial spine fracture. A male-to-female ratio was 3:2 and mean follow-up was 38.2$(13{\sim}56)$ months. All five patients were classified type III by Movers and McKeever classification. The average period from injury to operation was 4.8 days $(3{\sim}8days)$, the avulsed fragment was reduced by operative arthroscopy and fixated by pull-out suture in 3 patients and by cannulated screw in 2 patients. Postoperatively long leg cast was applied for 2 weeks, and then gradual range-of-motion exercise was permitted. Full weight-bearing ambulation was permitted after 6 weeks. The clinical evaluation was performed by range of motion, Lachman and pivot shift test, KT-1000 arthrometer, Lysholm knee score and the modified Feagin score. Results: All five patients had no symptom and recovered full range of motion of the affected knees. Lachman test was positive finding of 1+ laxity in one patient, the others were negative, and all patients were negative findings for pivot shift test. The result of KT-1000 arthrometric assessment is mean maximum side-to-side differences 1.9 mm. Average Lysholm knee score was 99.4. All patients had excellent results in modified Feagin score. Conclusion: Arthroscopic reduction and fixation of displaced tibial spine fracture in children showed excellent result without complication. Both pull-out suture fixation and cannulated fixation provide an effective treatment option for fixation of the displaced anterior tibial spine fracture.

  • "When it comes to art, nationalism is a goodticket to ride with", says the title of a report in the Indian Express (Mumbai, 29 Oct 2000). The newspaper report goes on to say that since Indian art was kept "ethnic" by colonialism, national liberation meant opening up to the world on India's own terms. Advocacy, at the tail end of the 20th century, would contrast dramatically with the call by Rabindranath Tagore, the founder of the academy at Santiniketan in 1901, to guard against the fetish of nationalism. "The colourless vagueness of cosmopolitanism," Tagore pronounced, "nor thefierce self-idolatry of nation-worship, is the goal of human history" (Nationalism, 1917). This contrast is significant on two counts. First is the positive aspect of "nation" as a frame in art production or circulation, at the current point of globalization when massive expansion of cultural consumers may be realized through prevailing communication networks and technology. The organization of the information market, most vividly demonstrated through the recent FIFA World Cup when one out of every five living human beings on earth watched the finals, is predicated on nations as categories. An extension of the Indian Express argument would be that tagging of artworks along the category of nation would help ensure greatest reception, and would in turn open up the reified category of "art," so as to consider new impetus from aesthetic traditions from all parts of the world many of which hereto fore regarded as "ethnic," so as to liberate art from any hegemony of "international standards." Secondly, the critique of nationalism points to a transnational civic sphere, be it Tagore's notion of people-not-nation, or the much mo re recent "transnational constellation" of Jurgen Habermas (2001), a vision for the European Union w here civil sphere beyond confines of nation opens up new possibilities, and may serve as a model for a liberated sphere on global scale. There are other levels of collectivity which art may address, for instance the Indonesian example of local communities headed by Ketua Rukun Tetangga, the neighbourhood headmen, in which community matters of culture and the arts are organically woven into the communal fabric. Art and collectivity at the national-transnational level yield a contrasting situation of, on the idealized end, the dual inputs of local culture and tradition through "nation" as necessary frame, and the concurrent development of a transnational, culturally and aesthetically vibrant civic sphere that will ensure a cosmopolitanism that is not a "colourless vagueness." In art historical studies, this is seen, for instance, in the recent discussion on "cosmopolitan modernisms." Conversely, we may see a dual tyranny of a nationalism that is a closure (sometimes stated as "ethno-nationalism" which is disputable), and an internationalism that is evolved through restrictive understanding of historical development within privileged expressions. In art historical terms, where there is a lack of investigation into the reality of multiple modernisms, the possibility of a democratic cosmopolitanism in art is severely curtailed. The advocacy of a liberal cosmopolitanism without a democratic foundation returns art to dominance of historical privileged category. A local community with lack of transnational inputs may sometimes place emphasis on neo-traditionalism which is also a double edged sword, as re kindling with traditions is both liberating and restrictive, which in turn interplays with the push and pull of the collective matrix.

The Results of Posterior Cruciate Ligament Reconstruction. -Transtibial Two Tunnel Technique vs. Modified Tibial Inlay Technique- (관절경적 후방십자인대 재건술에 있어서 Transtibial Two Tunnel방법과 Modified Tibial Inlay방법의 비교)

  • From 1989 to 1994, authors have reconstructed the posterior cruciate ligament(PCL) in 51 knees with an autogenous central one-third of the patellar tendon by transtibial two tunnel technique, but there were not a few cases of unfavorable results. So from January 1995, we have reconstructed the PCL deficient knees by "modified tibial inlay technique" to avoid the grafted tendon abrasion at the posterior opening of the tibial tunnel(killer turn). Purpose of this study was to compare the results of two surgical techniques and what its advantages and disadvantages are. We could follow up 39 cases of transtibial two tunnel techique group(group A) more than one year, average being 23.7 months and 21 cases of modified tibial inlay technique group(group B) more than 12 months, average being 14.7 months. The clinical results were evaluated by the OAK knee scoring system ($M{\ddot{u}}ller$'s criteria) and the posteror stress roentgenography (push view) with Telos stress device compared with the uninjured knees. The arthroscopic second-look findings were also evaluated. In group A : The $M{\ddot{u}}ller$'s knee score was average 80.1 points, the posteror displacement in push view was average 4.4mm at the last follow up. There were 17 cases(44%) of unfavorable results which showed unstable posterior displacement more than 4mm compared with the uninjured knee in push view. Among the 19 cases of arthroscopic second look examinations, nearly normal PCL appearances of the grafted tendons were noted only in 9 cases(47%). In group B : The $M{\ddot{u}}ller$'s knee score was average 86.7 points, the posterior displacemnet in push view was average 3.6mm at the last follow up. There were 5 cases(23.8%) of unfavorable results which showed unstable posterior displacement more than 4mm compared with the uninjured knee in push view but 4 out of 5 cases showed 6mm posterior displacement in push views. Among the 7 cases of arthroscopic second-look examinations, 6 cases(86%) showed nearly normal PCL appearances of the grafted tendons. In modified tibial inlay technique of PCL reconstruction, it was easier to pull out the BPTB and in cases of remained laxed meniscofemoral ligament it was easier to preserve the remained structures than transtibial two tunnel technique. We expect the "modified tibial inlay technique" may solve the problem of grafted patellar tendon abrasion at the posterior orifice of tibial tunnel and may contribute to the successful PCL reconstruction.

Clinical and Radiographical Follow-up for Residual Displacement of Fracture Fragments after Interlocking Intramedullary Nailing in Humeral Shaft Fractures (상완골 간부 골절에서 교합성 골수강 내 금속정 고정 후 잔존한 골절편 전위에 대한 임상적, 방사선학적 추시)

  • Purpose: This study is designed to evaluate the clinical and radiographical results for the displacement of fracture fragments after interlocking intramedullary nailing in humeral shaft fractures. Materials and Methods: We retrospectively reviewed the results of 8 cases of humeral shaft fractures that have displacements of over 10 mm and under 20 mm after interlocking intramedullary nailing between July 2004 and August 2011. The mean age was 54.1 years (range, 43 to 70 years) and there were 3 male and 5 female patients. Radiographically, the time to bony union, change of displacement and angulation of the fracture site, and degree of improvement of these two factors were measured. Clinically, the range of motion of shoulder and elbow joints, postoperative pain and complications were evaluated. Results: All cases showed complete bony union in last follow-up. The mean time to bony union was 16.1 weeks. At the last follow-up, almost all cases had normal range of motion of shoulder and elbow joints. But, one case had stiffness of shoulder joint. Therefore, arthroscopic capsular release and manipulation was performed. One case had transient shoulder pain and the other case had transient elbow pain. In the two cases, pull-out of proximal interlocking screw were noted, but they finally had bony union. Conclusion: Although considerable displacement of fracture fragments after interlocking intramedullary nailing in humeral shaft fractures was present, we had excellent radiographical and clinical outcomes. Therefore, an additional procedure, such as open reduction or another fixation for the fracture site, was not necessary.

  • In this paper, By the time that the tilting technology is adopted in railway rolling stock, we analyzed quantitatively energy saving by reducing the power consumption with the reduction of the operation time through the speed improvement and suggested the necessity to introduce tilting technology in the domestic rail vehicles. To achieve this purpose, the effect of energy saving by comparing and analyzing the power consumption during the operation by TTX HANVIT 200 and 8200 electric locomotives to pull the trains on the same line was suggested and the efficiency of the main devices was compared and analyzed by measuring the power consumption by a single unit. As the energy saving is the world topic, the studies on reducing energy usage goes on constantly in many areas. In addition, as of the time to improve the conventional tracks to speed up and change the signals, Tilting technology will be contributed to the management environment by enlarging the passengers' demand through the reduction of the operation time and saving energy using the existing infrastructure.

  • This study researched into the left-right inclination of the rear foot at the lower limb joints, knee joint angle, angular velocity of the knee joint, angular acceleration and the max. Based on the analysis of kinematics according to the changes in the height of step box (6, 8, 10 inches) during step aerobics of female college students majoring in physical education. The findings of this study are as follows: Then angle of the knee joint decreased as the height of the step box increased the min. Angle was measured right before the right foot was on the step box, and the angle tended to decrease as the step box get heightened. The left-right inclination of the rearfoot angle according to the height of step box increased as the height increased. In the 'pull-up' stage during which the weight was loaded on the right foot the angle increased, while in the right foot stepping stage during which the right foot was on the ground, the left-right inclination of the rearfoot angle increased as the height of the step box increased. The angular velocity of the knee joint according to the height of step box started increasing when the right foot initially stepped on the step box and during the initial stepping section, the angular velocity decreased as the height of step box increased. The changes in angular acceleration of the knee joint according to the height of step box increased as the height of step box increased.

Highly Efficient and Stable Organic Photo-Sensitizers based on Triphenylamine with Multi-anchoring Chromophore for Dye-sensitized Solar Cells (트리페닐아민을 이용한 염료감응형 태양전지 고효율 염료합성)

  • Organic dyes, because of their many advantages, such as high molar extinction coefficients, convenience of customized molecular design for desired photophysical and photochemical properties, inexpensiveness with no transition metals contained, and environment-friendliness, are suitable as photosensitizers for the Dye-sensitized Solar Cell (DSSC). The efficiency of DSSC based on metal-free organic dyes is known to be much lower than that of Ru dyes generally, but a high solar energy-to-electricity conversion efficiency of up to 8% in full sunlight has been achieved by Ito et al. using an indoline dye. This result suggests that smartly designed and synthesized metal-free organic dyes are also highly competitive candidates for photosensitizers of DSSCs with their advantages mentioned above. Recently, the performance of DSSC based on metal-free organic dyes has been remarkably improved by several groups. We had reported the novel organic dye with double electron acceptor chromophore, which was a new strategy to design an efficient photosensitizer for DSSC. To verify the strategy, we synthesized organic dyes whose geometries, electronic structures and optical properties were derived from preceding density functional theory (DFT) and time-dependent density functional theory (TD-DFT) calculations. In this paper, we successfully synthesized the chromophore containing multi-acceptor push-pull system from triphenylamine with thiophene moieties as a bridge unit. Organic dyes with a single electron acceptor and double acceptor system were also synthesized for comparison purposes. The photovoltaic performances of these dyes were compared, and the recombination dark current curves and the incident photon-to-current (IPCE) efficiencies were also measured in order to characterize the effects of the multi-anchoring groups on the open-circuit voltage and the short-circuit current. In order to match specifications required for practical applications to be implemented outdoors, light soaking and thermal stability tests of these DSSCs, performed under $100mWcm^{-2}$ and $60^{\circ}C$ for 1000h.

  • Objective & Methods: In order to ascertain the names. locations. the way of getting acu-point and indications for moxibustion about Ko-Hwang(膏盲) and Go-Hwang-Su(膏盲兪). I have translated the text for Ko-Hwang(膏盲) and Go-Hwang-Su(膏盲兪) collected in several relative classical books. and performed literal investigation about it. Through the study, the following results are obtained. Results & Conclusions: 1. The two words Go(膏) and Hwang(盲) have different concepts each other originally. and Go-Hwang(膏盲) is the abbreviation of the words "Upon the Hwang(盲), Under the Go(膏)", which means "A gap of Heart and diaphragm" that is under the heart and on the diaphragm 2. Go-Hwang-Su(膏盲兪) is the acu-point which can treat the diseases located in the gap of Go(膏) and Hwang(盲), so they must not be named as Go-Hwang(膏盲) or Go-Hwang-Hyui(膏盲穴). 3. The origination of Go-Hwang-Su(膏盲兪) is speculated as the interval of Hwang-Bo-Mil(皇甫謐) and Son-Sa-Mak(孫思邈). And I think it had been created by Son-Sa-Mak(孫思邈) up to now. 4. The words "Four ribs and Three spaces(四肋三間)" those are founded in the text of The Entrance of Medicine(醫學入門) and The Great Success on Acupuncture and Moxibustion(針灸大成) are either the words those had been quoted from The Bible of Helping Life by Acupucture and Moxibustion(針灸資生經) in wrong way. or the misunderstanding of the original mind of Son-Sa-Mak(孫思邈) as being quoted in the text. So you should know it exactly as Son-Sa-Mak(孫思邈) had said. 5. When you search the acu-point Go-Hwang-Su(膏盲兪), you must pull the arms of patients forwardly to reveal the acu -point. because it is covered by the scapula in normal posture. 6. The indications for the use of Go-Hwang-Su(膏盲兪) are same as mentioned in this study. And it can grow the Yang-Gi(陽氣) more prosperous.

Design of Truncated Mooring Line Model in KRISO's Deepwater Ocean Engineering Basin

  • The present work was an attempt to investigate the applicability of truncated mooring systems to KRISO's deep ocean engineering basin (DOEB) with ratios of 1:100, 1:60, and 1:50. The depth of the DOEB is 15 m. Therefore, the corresponding truncated depths for this study were equal to 1500 m, 900 m, and 750 m. The investigation focused on both the static and dynamic characteristics of the mooring system. It was shown, in a static pull-out test, that the restoring force of a FPSO vessel could be modified to a good level of agreement for all three truncation cases. However, when the radius of the mooring site was reduced according to the truncation factor, the surge motion response during a free-decay test showed a significant difference from the full-depth model. However, the reduction of this discrepancy was achieved by increasing the radius up to its maximum possible value while considering the size of the DOEB. Especially, in terms of the time period, the difference was reduced from 24.0 to 5.3 s for a truncation ratio of 1:100, 54.1 to 8.6 s for a truncation ratio of 1:60, and 31.7 to 3.9 s for a truncation ratio of 1:50. As a result, the study verified the applicability of the truncated mooring system to the DOEB, and therefore it could represent the full-depth mooring system relatively well in terms of the static and dynamic conditions.
