Effect of Self-determination Motivation on Job Engagement and the Moderating Effect of Compensation Satisfaction in Dental Hygienists (치과위생사의 자기결정성동기가 직무열의에 미치는 영향 및 보상 만족도의 조절효과)

  • Purpose : The purpose of this study was two-fold: to identify the effect of dental hygienist's self-determination motivation on their job engagement and to analyze the moderating effect of compensation satisfaction that affect the relevance. Methods : Data were collected using a structured self-report questionnaire administered to 260 dental hygienists working in dental hospitals and clinics in Busan, South Korea. These were analyzed using IBM SPSS Statistics version 26.0 and SPSS Process Macro 3.5. A frequency analysis, including the respondents' general characteristics, frequency, percentages, and standard deviations, was performed. A regression analysis was also performed using SPSS Process Macro to verify the moderating effect of compensation satisfaction in the effect of self-determination motivation on job engagement. Results : The self-determination motivation of the dental hygienists had a statistically significant positive effect on their job engagement, which was the dependent variable. Higher intangible compensation satisfaction levels led to a stronger effect of intrinsic motivation but a weaker effect of extrinsic motivation on job engagement. Moreover, higher tangible compensation satisfaction levels strengthened the effect of intrinsic motivation on job engagement. Conclusion : Recently, the demand for oral health care has been increasing, Competition in the dental medical service market warrants, high-quality dental services based on accurate diagnosis and treatment. In this context, dental hygienists' job engagement must be improved. For this purpose, increasing the satisfaction of dental hygienists with self-determination motivation and appropriate compensation is effective. In addition, attention must be paid to the moderating effect of compensation satisfaction on the relationship between intrinsic motivation and job engagement. On the basis of the implications of this study, the results can be used as basic data for improving dental hygienists' welfare system and manpower management.

Oral health status and care needs of elderly patients in long-term care hospital (요양병원 노인환자의 구강건강상태와 구강건강관리 요구도)

  • Objectives: The purpose of this study is to identify oral health status and oral health care needs of elderly patients in long-term care hospital. Methods: Oral health examination was carried out by a dentist and questionnaire was completed by direct individual interview by a dental hygienist for 245 elderly patients in seven long-term care hospitals in Daegu from March 12 to April 16, 2011. Results: Those who need dental care were 188(76.7%) and was done by oral health examination by a dentist. 93 patients(38.0%) wanted dental care and 63 patients(25.3%) need dental care. The most important dental service in the elderly patients was denture and prosthetic service. As demand for denture and prosthetic service exceeds the supply, but only 35.9 percent of the elderly received the dental care service by the estimation of the caregivers. Conclusions: It will be necessary to establish the customized dental care service for the elderly patients in long term care hospitals. The oral health education for the caregivers is very important and the continuing education program must be implemented in the future.

Analysis of trade newspapers related to dental hygienists as healthcare professionals using language analysis technique: using R program (언어분석기법을 활용한 치과위생사의 의료인화 관련 신문기사 분석: R 프로그램 이용)

  • Objectives: The purposes of this study were to analyze the trade newspapers related to 'recognition of the dental hygienist as the healthcare professional' using R program and to identify opinions of groups concerned with dental hygienists. Methods: This study was designed with contents analysis and cross-sectional. The subjects of the study were the articles for the last three years in medical and dental newspapers about the recognition of the dental hygienist as the healthcare professional. The collected articles were categorized and classified for each group's opinions about the issue. The key words were extracted according to the priorities of the opinions of agreement and disagreement. They were visualized after frequency analysis using R, a big data analysis program. Results: A total of 237 newspaper articles were extracted among 270 ones containing opinions. 245 were positive opinions and 25 were negatives. The main key words of the agreement were 'Amendment of Medical Law', 'Medical Practice', and 'Legal Guarantee of the Practice'. Advocates addressed that the issues should be resolved with the amendment of the law, as dental hygienists are not guaranteed to work based on the current law although they are actually doing the medical practices. Main key words of disagreement were 'Legal Guarantee of the Practice', 'Revision of Medical Technician Law', and 'Review of Job Type'. They described that the problem can be resolved by revising medical technicians act, and it needs to consider as job types of all healthcare professional. Conclusions: In the group who showed the positive opinions, it is possible to utilize measures such as promoting the cooperation of dental hygienists and developing public consensus through publicity.

Changes in oral health status of the elderly with visiting oral hygiene care intervention based on community health care: Case reports (커뮤니티케어에 기반한 방문 구강 관리 중재 노인의 구강 건강 상태 변화에 관한 증례 보고)

  • Objectives: This case - study was conducted to assess the oral health status change of the elderly through visiting oral care interventions based on a community health care project. Methods: Professional dental hygiene treatment and oral health education, including brushing using interdental toothbrushes and sponge brushes, were performed on three senior citizens who received home visiting oral health care benefit. Results: The subject's periodontal conditions improved including gingival inflammation and bleeding. The gingival color tured pink by controlling the dental plaque. Conclusions: Visiting oral health care contributed to the improvement of oral health of the elderly. Therefore, based on the characteristics of the elderly with various systemic diseases, it is necessary to discover various cases that can perform professional and customized visiting oral health care programs.

A study on the performance and necessity of dental hygienists for oral health promotion activities for the elderly according to their learning experiences and performance experiences (노인 구강건강증진 활동에 대한 치과위생사의 학습경험과 수행경험에 따른 수행가능성 및 필요성 조사)

  • Objectives: The purpose of this study was to investigate the awareness of the performance and necessity of oral health promotion activities and seek ways to revitalize the professional role of dental hygienists in oral healthcare for the efficient oral care of the elderly. Methods: Eighty-five dental hygienists in charge of oral health promotion projects at public health centers and 38 dental hygienists in network dental clinical trials were investigated for their learning experience, performance experience, feasibility, and necessity of dental hygienists for general and oral health service items for the elderly. The collected data were analyzed using frequency analysis, chi-square test, and Mann-Whitney U test. Results: The degree of performance possibility according to the learning experience and performance experience of the dental hygienist for the whole body and oral health promotion activity items for the elderly showed that the degree of performance possibility was higher among those with experience compared to the non-experienced person, and it was statistically significantly higher (p<0.05). Conclusions: The dental hygienist's professional oral health service is a necessary system to improve practical knowledge and skills and to provide a wide range of professional oral health services for the elderly.

Dental hygienists' perspective and coping measures towards medical market opening (의료시장 개방에 대한 치과위생사의 인식과 대응방안)

  • Objectives : This research seeks to present the data needed for the development of coping strategy, following medical market opening by identifying dental hygienists' perspective and coping measures towards the opening. Methods : One hundred eighty-eight dental hygienists were targeted to identify their level of perception towards medical market opening, attitude towards medical market opening, question of whether they agree or not with the opening and reasons, and coping measures and benefits of the medical market opening. t-test, chi-square test and cross-tabulation analysis were used for the analysis Results : First, team leaders are more aware of the medical market opening and hold greater sense of crisis towards opening compared to the rank and file. Second, the reasons cited for agreeing with the medical market opening included improvement of medical services' quality and diversification of services. As for the reasons for disagreeing, they cited the increase medical expenses paid by public. Third, limitation of the hospital management technique was cited the most when it comes to the scope of Korean hospitals' management crisis, followed by the limitations of the diagnosis procedure, limitations of the medical services, limitations of the medical techniques and increase in the number of large hospitals, in the order cited. Fourth, team leaders perceive greater need to seek coping measures from the aspect of realizing medical insurance fee from the policy development, service and system level aspects when it comes to the coping measures depending on their ranks. Conclusions : Therefore, Dental Hygienist has a comparatively low awareness of medical market opening, coping measures need to be explored to cope with the medical market opening by ensuring the dissemination of accurate knowledge through the education on the fees for dental hygienist and seminars in relation to the medical market opening.

  • The purpose of this study was to determine the status of dental hygienists in Japan. The study explicated the history, core curriculum, National Examination for Dental Hygienists, work roles of dental hygienists, and workplaces of dental hygienists. This study was based on the government policy report on dental hygienists and the information published by each public institution. The latest statistics presented by the institutions were collected through official websites. The employment information of graduates from Hiroshima University was analyzed based on actual field study. The results of the study revealed that social demand for dental hygienists has steadily increased and policies and education have been revised accordingly. The work roles of dental hygienists have expanded to meet the needs of the treatment and those of public health fields. In line with major policy changes, the educational period has been extended from 1 year to 3 or 4 years, while the mandatory credits for graduation have been established. Licensing examinations were being performed by the local governments since 1948 due to the different situations of dental hygienists in different areas. In 1992, they were converted into a single national examination. The work roles have expanded from assisting dental treatments to health guidance, home care, and perioperative care. Consequently, the number of dental hygienists has increased, especially in healthcare facilities for the elderly. Dental hygienists perform various roles. However, the most essential role is to provide the best oral care services to the patient. The expected role of dental hygienists has expanded in alignment with public healthcare needs and support for the elderly. The government and universities are expected to bring about improvements such as diversifying the channels of education and establishing policies to respond to growing patient needs by cultivating excellent dental hygiene professionals.

The study on the Carpal Tunnel Syndrome by dental hygienists at Gwangju·Jeonnam Area (광주·전남지역 치과위생사의 수근관증후근에 대한 연구)

  • It has been suggested in research results thai dental hygienists have high risk of carpal tunnel syndrome, mainly caused by the repeated motion of extensor and flexor or the use of vibration tools, compared to other occupations. To find out the situation of the carpal tunnel syndrome of dental hygienists, who are exposed to work-related musculoskeletal disorders, this study used 132 questionnaires given on May 22, 2004, the period of continuing education of the first half year in the Gwangju Jeonnam area, and obtained the following results. 1. Subjects worked at a dental ciinic(32.6%), a hospital(31.8%), and a public health center(35.6%). Age by work was under 24 in a clinic(17.4%) and a hospital(15.9%), and over 30 in a public health center(35.6%). 2. In practice conducted over one time a day, a scaling accounted for 90.7% in a doctor's office; 595% in a hospital; and 3.0% in a public health center, suggesting significant difference(p<0.01). Pit and fissure sealant accounted for 53.5% in a clinic; 53.2% in a hospital; and 95% in a public health center, also suggesting significant difference(p0.01). 3. Symptoms of carpal tunnel syndrome appeared in a wrist(12.1%), a right hand(14.4%), and a left hand(5.3%). 4. In case of temporary crown practice, symptoms appeared in a wrist(22.0%), a right hand(14.0%), and a left hand(4.0%), suggesting significant difference(p<0.01). The above results showed that 12.1% of dental hygienists was exposed to carpal tunnel syndrome. Thus, it is considered very important that dental hygienists should be given education of the danger of continuous work in certain motions and prevention education of improving repeated position, and make efforts to reinforce self-control ability.

The Relationship between Emotional Labor Status and Depression in Dental Hygienists (치과위생사의 감정노동 실태와 우울에 미치는 영향)

  • This study examines the relationship between emotional labor and depression in dental hygienists. Data were collected from 186 dental hygienists working in dental clinics in Daegu from early March to early April 2016. The survey was composed of questionnaires related to general characteristics, work-related characteristics, emotional labor, and depression. As a result of analyzing the subordinate factors of emotional labor, 57.0% of the factors of 'organization support and protection system' were the highest, and the effect of emotional labor on depression was 'emotionaldisharmony and damage', 'Organizational support and protection system', and especially, the ' emotionaldisharmany and damage' factors were 4.042 times higher than normal. The dental hygienist's degree of emotional labor risk is high, and education, prevention and management measures for emotional labor will be prepared. In addition, emotional labor affects the depression of dental hygienists as well, and comprehensive mental health measures are needed to take into consideration general and occupational characteristics that affect depression.

A Survey on Preliminary Dental Hygienists' Senses of Employment (예비치과위생사의 취업에 관한 의식 조사)

  • The purpose of this study was to conduct a survey on the senses that preliminary dental hygienists have on employment, who applied for the National Dental Hygienist Examination, so that it could provide a basic reference required for fostering good human resources specializing in oral health with a sense of mission and responsibility as professional. To meet these goals, a self-administered questionnaire survey was conducted for participants in a refresher education meeting for preliminary dental hygienists across three major regions(Seoul, Daejeon and Busan), which was held by the Korean Dental Hygienists Association(KDHA) on Jan. 15, 2006. Total 289 sheets of questionnaire were collected, and 262 sheets except invalid or incomplete ones were used for examination and analysis. As a result, this study came to the following conclusions: First, it was found that 40.4% respondents wanted to be employed in dental clinic or hospital. Most respondents(90.1%) answered that faithfulness is most valued as prerequisite qualification for employment in hospital, and highest percentage of respondents(39.3%) wanted to earn 16 to 17 million Korean won annually. For internship opportunities, 61.9% respondents replied that they could willingly accept the internship, provided that basic pay and switchover to regular employee are all assured. Second, as the results of survey on employment circumstances around respondents, it was found that most respondents(58.3%) were employed in dental clinic or hospital, and highest percentage of respondents(35.0%) earned 15 to 16 million Korean won. Third, it was found that interpersonal relationships among employees had most significant effects(4.81) on selection of employment, which was followed by in-house welfare benefits 1(monthly/annual vacation, resting room, etc; 4.56). For possible influential factors on the selection of employment depending on whether employed or not, it was found that there were Significant differences in pay, welfare benefit 2(seminar, orientation, refresher training opportunities, etc), post-wedding continued employment and merit system between employees and non-employees(pE0.05). Fourth, the survey on respondents' occupational senses of dental hygienist showed that major tasks of dental hygienists were represented by oral disease prevention(92.6%), case management(71.5%), oral health education(66.3%), hospital/clinic management and dental management support(10.4%) and public oral health activities(6.7%) respectively. For job satisfaction, it was found that almost half respondents(48.9%) showed satisfaction at their job and 32.6% felt satisfied at and proud of their job. That is, majority of respondents(81.5%) felt satisfied at their job as dental hygienist. For follow-up requirements to qualify for dental hygienists, it was found that 73.3% felt it necessary to give more investments to self-development, 62.2% respondents considered protection of membership's rights as one of future assignments that KDHA should be more committed to. For future social awareness about dental hygienists as occupation, 69.2% respondents expected that they would be better developed and treated as professional. And desired training courses as a part of qualification for professional dental hygienists were represented by case management(services, manners, etc; 33.3%), implant(28.9%), esthetics(correction, prosthesis, whitening; 18.9%) and so on. For an item on working years, it was noted that 75.9% respondents would keep working as dental hygienists as long as they could.
