In order to find out health problems among inhabitants in slum areas in Kwanak-Ku, Seoul, a series of health survey was conducted upon 510 households by interview from March to December, 1976. The results obtained were as follows: 1. Employments of householders were unstable; Out of 508 householders, 164(32.3%) were unemployed and 184 (36.2%) were daily or temporary employees. 2. Average number of households per house was 2.0 and average area of residential room per person was $4.0m^2$. 3. 476(93.3%) out of 510 households were supplied with tap water and rest of them made use of ground water as a source of drinking water. 4. Only 279(18.3%) out of 1527 live births were delivered at medical facilities, 496(32.7%) were at home attended by doctors or midwives and 358(25.1%) took prenatal care. The above findings were worse in urban slum area than in other urban area of relatively high economic level, but were better than in rural area of less medical facilities. 5. Initiation of treatment were delayed until their illnesses were advanced in most of the households, 472(92.5%) out 510. In the early stage of the illness, 131(25.6%) of the house-holds sought physicians in their clinics or general hospitals and 250 (40.9%) visited chemists, to toy drugs at first hand. Frequency of visits to physician increased to 52.8% as the disease aggravated in later stages. 6. Cost of medical expenditure per household amounted to 815 won, and was paid to, in the order of chemists, physicians, chinese herb stores, chinese herb doctors. 7. Concerning the health knowledge of the inhabitants, 273(53.9%) out of 506 respondents were aware of the infectivity of pulmonary tuberculosis, and 68(13.4%) of them checked regularly their chest findings by X-ray at least once every two years. 8. As for the family planning, although 448(87.3%) out of 510 respondents were in favor of it, 215 (41.8%) of them were actually practicing contraception. 9. About 40.6% (125 respondents) of them obtained information and knowledge concerning contraception through personal contact with family planning workers. 10. Nutritional status of housewives was generally poor: 49(38.3%) out of 128 housewives were found to be anemic and average serum protein level was $7.5{\pm}0.82g/dl$.
The survey was carried out on 1,031 primary school children for about 1 month, from June 1, to June 30, 1974, for the purose of understanding indirectly scio-medical status of a remote rural area, reflecting health status of primary school children. The diseases of respiratory system were the highest 124.74 in girls and next, the diseases of digestive system (52.3 in girls). According to school year, healthy children were increased to higher grades. 3) Average sick day per total surveyed children and patient during last month and last year. a) Per surveyed 1,031 children was 0.28 days during last month and 0.98 days during last year. And also per patient was 1.78 days during last month and 3.22 days during last year. b) Average sick day was higher in girls than in boys during last month, but inverted during last year. 4) Average absented day per total surveyed children and patient during last month and last year. a) Per surveyed 1,031 children were 0.43 days during last month and 3.81 days during last year (omitted 1st school year). b) Per absented children were 2.0 days during last month and 7.10 days during last year. c) Per absented children were 8.16 days in boys and 3.17 days in girls. 5) Utilization of medical facilities: The drug-store was consulted 91.2% of the total utilized boys and girls. The strengthening of medical facilities and medical care activities in health subcenter is urgently required 6) Medical Expenses by period and sex: a) Average medical expenses per surveyed 1,031 children were 35.95 won, per patient were 298.04 won during last month. b) Average medical expenses per survey children (omitted 1st school year) were 80.56 won, per patient were 243.98 won and per treated patient were 318.87 won during last year. c) Medical expenses were higher in boys than in girls. 7) Rohrer index by sex, present illness and school year. a) Average Rohrer index was 129.8 in boys and 126.2 in girls. b) Average Rohrer index was increased for 1st school year to 2nd school year and thereafter falling down step by step by school year. c) Under 151-160 Rohrer index number of code III was the highest and above 151-160 Rohrer index of code XII was peaked.
This study was done in order to analyze the factors which influenced the performance level of community health practitioners' task. Interview survey was done during the period from August to October, 1986. Interviewee were 166 CHPs among total of 217 CHPs in Kyonggi province area. Multiple stepwise regression and canonical correlation analysis were used to identify major factors influenced to perform community health practitioners' task. The results of this study were summarized as follows: 1. General characteristics of CHPs 1) Personal characteristics The average age of CHPs was 37.8 years and their marital status was $77.6\%$ of married, educational back-ground was $65.3\%$ of junior college graduation. Their job career was $38.6\%$ of between 1-3 years, $33.3\%$ of between 3-5 years, $22.2\%$ of less than 1 years. Most of CHPs$(62.8\%)$ were fully satisfied with their job, $33.3\%$ were moderately, and $3.8\%$ were not satisfied. 2) Working environmental condition Only $31.7%$ of CHPs were satisfied with their working condition of primary health post, $26.6\%$ were not satisfied. Half of CHPs$(52.5\%)$ replied having good cooperation with health center, $10.1\%$ replied bad. Cooperation with health subcenter was good in $32.9\%$, and bad in $21.9%$. Cooperation with private health institutions was good in $34.2\%$, bad in $21.6%$. 2. Performance level of community health practitioners' task Among a total of 52 contents of their functions medical history taking. physical examination, referral of diagnostic laboratory work-up($(86.4\%)$, health assessment of pregnant women$(82.1\%)$, development of health information system$(79.4\%)$, supervision of health workers $(78.4\%)$, follow-up of family planning acceptors$(77.3\%)$, and follow-up of family planning acceptors' side effects$(77.3\%)$ were actively performed. Diagnosis of pregnancy$(62.1\%)$, sampling of drinking water for quality test$(52.5\%)$, making list of equipment' & supplies $(51.5\%)$, evaluation of primary health post activities $(37.6\%)$, organization of village health workers$(32.4\%)$ and management of village health workers $(30.1\%)$ were poorly performed. 3. Stepwise multiple regression analysis of job function The factors which influenced the performance level of community health practitioners' function were age, marital status, educational level, job career, job satisfaction, satisfaction of working environment of primary health post, cooperation of health center, cooperation of health center, cooperation of private health instiutions in orders. These 9 variables were able to explain job function from $25.7\%$ of program planning to $6.7\%$ of management of common disease. 4. Canonical correlation analysis between the performance of function and general characteristics of CHPs. Cooperation of private health institutions was found to be the factor influencing task performance of community organization, management of primary health post, technical supervision of health personnels. Job satisfaction of CHPs was also found to be the factor influencing task performance of family planning, management of common disease and maintenance of health information system.
Giardia lamblia is a pathogenic flagellate causing intestinal disorders such as diarrhea, abdominal pain and malabsorption of nutrients. Giardia is mainly infected by the ingestion of contaminated foods per os. Craun (1979) has recently reported that mass infection of this flagellate through the contaminated water supply systems is one of public health hazards. Also, so-called traveller's diarrhea is sometimes caused by Giardia infection (CDC, U.S.A., 1971). However, a few epidemiological studies figuring out the mode of infection or control measures of Giardia infection has been done so far in Korea. The present study was aimed to know the prevalence of Giardia infection in several Korean populations, detectability of this flagellate in water systems and the viability of the cysts against sewages and disinfectants applying to drinking water. In the present study, 388 stool specimens from orphanage children in Chun-joo, Chung-joo, On-yang and Chun-an areas and 538 stool specimens from inhabitants in Woo-do, In-chon, and Chun-joo were examined by formalin-ether concentration technique to detect out Giardia cysts. On the other hand, water samples from 14 sites of Han River and its tributaries were collected in May through July, 1984. Fifty liter of water sample in each sampling site was then filtered through water filtering system deviced by U.S. Environmental Proutection Agency and the sediments rinsed out from the thread rolls, a part of water filtering system, were examined to detect out the Giardia cysts. In order to observe the viability of Giardia cysts in the sewage samples, the cysts were treated in it at $4^{\circ}C$ or $25^{\circ}C$ for 7 through 28 days. For this purpose, the cysts were also exposed to various concentrations of disinfectants such as chlorine, iodine and ozone gas for proper time intervals. After treatment, the viability test of the Giardia cysts were carried out by method of Rice and Schaefer (1981) with minor modification. The results obtained in this study were as follows : 1) The detection rates of G lamblia cysts in the stool specimens were 18.3% in orphans and 4.3% in general examinees. 2) The prevalences of Giardia Infection were higher in the young age groups than in-adults. The highest positive rate was 18.4% in the age group less than 10. 3) Of 14 water specimens sampled from Han River system and its tributaries around the Seoul area, the Giardia cysts were detected from 4 samples, and no cyst was found in the water supply systems. 4) The cysts treated in the sewage survived for 28 days at $4^{\circ}C$ and for 13 days at $25^{\circ}C$. 5) The cysts were completely destroyed within 60 minutes by exposure to 8 mg/l of residual chlorine at 4g and within 30 minutes by exposure to the same concentration of chlorine at $25^{\circ}C$. 6) The cysts were all dead when exposed to 1 mg/1 of iodine for 60 minutes at $4^{\circ}C$ or $25^{\circ}C$. 7) The cysts were destroyed after 10 minute exposure in 0.15 mg to 0.25mg of residual ozone gas per liter. Summarizing the above results, it is considered that Giardia infection is regarded as water-borne disease and the cysts are able to be controlled by the application with the disinfectants in the water supply systems.
In the present study, the concentration levels of polychlorinated biphenyls (or PCBs) in the environments in Korea are estimated based on some measured data in Korea, in comparison with the data from the other countries. Even though PCBs were banned as electrical fluids in 1970s in Korea, PCBs are still detected in the environment. PCBs levels in Korea are greatly lower than those in other countries, which are gradually decreased as well. However, the measured data are not sufficient in both numbers and quality, to estimate the average PCBs levels in Korea. The regulation limit on polychlorinated biphenyls (or PCBs) is 2mg/kg (ppm), which is too low compared to 50 ppm of many other countries including U.S. With this strict regulation, there are many problems expected, for example, in the analysis of PCBs in the transformers using in the field as well as the safe treatment of PCBs. The risk assessment on the PCBs in the environment is surely necessary prior to the change in the limit. Also the PCBs concentration monitoring in the environmental media (i.e. air, water, soil and sediment) and exposure assessment will be essential for the accurate risk assessment. If the PCB-waste guideline maintain as 2 ppm after 10 years, the excess cancer risk of PCBs exposure by ambient air, drinking water and soil was $10^{-8}$. But if the guideline mitigate as 50 ppm after 10 years, the cancer risk was increased by $10^{-7}$. The ecological risk quotient by regulation change was not exceed '1'.
Family has emerged as a key concept for health, and it has been identified as one of the most important conditions. The relationship between health habit and its management is different depending on family. The odd pair family, mostly rural lower income class, worry to have poor health because of no spouse and small family size. One thousand eight hundred and seventy(1870) subjects were collected in 9 provinces through the sampling of Probability Proportional to Size (PPS). Questionnaire method was conducted on health checking, bath states, alcohol consumption, cigarette smoking, and the prevalence of farmer's health related problems. The main results were as follows: 1) The characteristics of odd pair families are that the head of household is female(77% ), the size of family is small(1.76 persons), the education level is low(7.5 years for male, 3.1 years for female) and the age group is old (male: 89.78 year old, female: 73.69 year old). 2) For the odd pair family, the frequency of health checking is quite low with one or two times per year(l0.2%) and the rate of no-health checking is much higher(35.8%) .3) Bathing utility is not available 29.6% of the odd pair family and only cold water is supplied at home for the 11.5 % of them. However, for the paired family, 9.8 % of them has no bathing utility and the rate of the family supplied with only cold water is just 7.9%. 4) The bathing frequency score of odd pair family is l.74points for male and 1.25 points for female. 5) The rate of smoking habits for odd pair family is 68.5 % and specially it is 7.6% for female, which is higher comparing with that of pair family. 6) The smoking frequency score of odd pair family is 1.57 points. 7) Alcohol drinking frequency score of odd pair family is 1.79 points for male, and 3.24points for female. 8) Farmers' syndrome(FS) revealed 38.7% of odd pair family and it is lower than that of pair family(57.3%). Special pain of FS was huckle bone and muscle(28.4%) and articular pain(24.l %). The pain rate of huckle bone and muscle(43.l %) and articular pain(33.5%) were higher in a year in odd pair family were lower than those of pair family: farming machine caused accidents(6.5%) and pesticide poisoning(5.7%). l0) The odd pair family use more frequently medical clinic or public health center for the treatment of FS(74.7%) and pesticide poisoning(62.5%) than the pair family for FS(69.0%) and for pesticide poisoning(.53.6%). The score of FS treatment is 5.70 points for odd pair family and it is not significantly different from 5.62 points of the paired family. The result of pesticide poisoning treatment score is as same as that of FS.
Objectives: This study was performed to identify the epidemiological features of a food poisoning outbreak in a company cafeteria located in Chungcheongnam-do Province, Korea in June of 2020 and to suggest preventive measures for a similar incidence. Methods: A total of 84 patients with acute gastroenteritis were examined. Environmental samples were obtained from 16 food handlers, six food utensils, 135 preserved foods served over three days and nine menus, and six drinking water samples. These are analyzed to detect viruses and bacteria. Results: Ninety-four out of the 402 people who were served meals (23.4%) predominantly showed symptoms of diarrhea, and the number was over 3 times. Among the 84 patients under investigation, 17 cases (20.2%) were positive for Enteropathogenic E. coli (EPEC) and 18 cases were positive for Clostridium (C.) perfringens (21.4%). Based on the investigation, it was concluded that the main pathogens were EPEC and C. perfringens. For EPEC, it was detected in three of the food service employees and in the preserved food and curry rice. The results of pulsed field gel electrophoresis indicate that all EPEC cases are closely related except for one food service employee. Assuming that isolated EPEC originated from the preserved food, the incubation period is about 25 hours. The origin of the C. perfringens was not determined as it was not detected in the food service employees or environmental samples. Conclusions: This case suggests that food provided in group food service centers must be thoroughly managed. In addition, identifying the pathogens in preserved food is very important for tracing the causes of food poisoning, so food must be preserved in an appropriate condition. To prevent similar food poisoning cases, analyzing cases based on epidemiological investigation and sharing the results is needed.
The high radon (Rn222) potentials of soil, groundwater, hotspring and indoor environments in the Taejon city area were delineated by use of an EDA RDA-200 radon detector. The U and Th contents were also analysed using a Multi Channel Analyzer to illustrate the sources of the radon potentials. The average U concentrations in Taejon vary according to the type of granites such as $4.14{\pm}2.36ppm$ in schistose granite (SG), $3.13{\pm}1.70ppm$ in biotite granite (BG) and $3.01{\pm}1.95ppm$ in two mica granite (TG). The U contents in the granites are closely related with the amounts of uraniferous minerals. However, the U contents in the soil are found to be $5.05{\pm}4.75ppm$ in TG, $4.07{\pm}1.69ppm$ in BG and $3.87{\pm}1.91ppm$ in SG which are mainly explained by the different cation exchange capacities (CEC) of the soils from various granites. The levels of soil radon are $552{\pm}656pCi/l$ in SG, in which levels at two locations exceed the level of 1,350 pCi/l established as guideline for follow-up action by the U.S. Environmental Protection Agency (EPA), $443{\pm}284pCi/l$ in TG and $224{\pm}115pCi/l$ in the BG. The soil radon concentrations are found to be proportional to the U content and hardness of the soils. The groundwater radon concentrations in the domestic wells of - 30~-100 m depth show that $6,907{\pm}4,665pCi/l$ in TG, $5,503{\pm}6,551pCi/l$ in SG and $2,104{\pm}1,157pCi/l$ in BG which are positively related with U contents in soils. The radon levels of six groundwater wells in TG and two in SG are greater than guideline for drinking water level, 10,000 pCi/l by EPA (1986). Average radon contents of hotsprings and public bathes in the TG area are $7,071{\pm}1,942pCi/l$ and $1,638{\pm}709pCi/l$, respectively, which are below the EPA standard for remedial action value of the 10,000 pCi/l. The mean indoor radon concentrations of the TG and SG areas are $1.60{\pm}1.20pCi/l$ and $1.60{\pm}0.70pCi/l$, respectively. The elevated indoor radon levels of 5.6 pCi/l and 6.7 pCi/l are found to be particularly in TG area, which exceeds 4 pCi/i guideline, correlating positively with the U contents in the soil and radon concentration in the groundwater.
