Profitability and Publicity of the Regional Public Hospitals in Korea - With focus on administration assessment of regional public hospitals - (공공의료기관의 공공성과 수익성 상호관계에 대한 연구 - 지방공사의료원의 경영평가를 중심으로-)

  • The administration of a regional public hospitals are expending from profit preference to publicity preference. The weight rate for a profitability and publicity of performance assessment has changed from 84:16 which was resulted by the assessment executed firstly in 1989 to 39:61 as resulted in 2004, the final assessment execution in 2005. Regional public hospitals are exerting and promoting a magnification in public sector to raise up the public-score. With comparison between publicity scores and profitability scores in original scores basis excluding weight rate, the publicity scores ranked higher than profitability scores although the latter was higher by 2002. However, for the administration performance of the regional public hospitals, the deficits increased 11 times from \92.6billion deficits with \460.3billion cost increased by 457% although income as \367.7billion increased by 394% comparing the last 2004 year to the first 1989 year for profit & loss statement of a regional public hospitals. There was analysis for the relation in yearly basis partitioning publicity and profitability for the assessment scores of the to regional public hospitals confirm the accumulated deficits of the hospitals like this attribute to the extension of public sector. The result showed that there was distinct plus relationship from 1999 although a minus relationship in general until 1997 except 1992 and there is a more plus relationship as approaching 2004. That is, it is hard to tell that the accumulated deficits increase of regional government medical center attributed to extension of public sector. On the contrary, the analysis showed the extension of public sector has a mutual relationship with uplift of profitability Meanwhile, it showed that operation cost rate and labor cost are the factors which influence a revenue & expenditure rate among the profitability index according to the results of relation analysis for the representative index of profitability and that of publicity.

  • This research analyzed acceptability of local public enterprise management assessment targeting Chungcheongnam-do direct management enterprises. To do this, this research analyzed influence with acceptability of local public enterprise management assessment by composing the area with 5 aspects related to local public enterprise management assessment. More differentiated strategy is required to operate management assessment efficiently, since direct management enterprises have different shape from other public enterprises in respect of organizational structure. In case of water and sewage, it tends to be difficult to return the result of management assessment to the local government organization, so that managers' interest and effort is necessarily required. Also, responsible person's professionalism is required to raise achievement of direct management enterprise through management assessment, politic management plan for this is necessary. Above all, the structure of incentive for management assessment result is difficult in making intended effect as other public enterprises. Therefore, differentiation of compensation system for assessment result is required.

The Analysis on Causes of Areas with Public Appeals to the Ecosystem and Nature Map (생태·자연도 이의신청지역의 원인 분석)

  • The objective of this study is to seek for improvement measures to make the Ecosystem and Nature Map by analyzing causes for grade changes and distributional characteristics of areas with public objections to the Ecosystem and Nature Map notified by e-official gazette from 2014 to June 2016. The receipt of public appeals has been recently increased from average 23 cases a year(2007-2013) to average 33 cases a year(2014-June 2016) while there were 42 areas with public appeals for less than the minimum area($62,500m^2$) for the evaluation of grade of the Ecosystem and Nature Map. Most of the public appeals focused on the 1st grade zone of the Ecosystem and Nature Map. Before grade changes by public appeals, the 1st grade zone of the Ecosystem and Nature Map were 76.0% of the whole areas with public appeals. However, after grade changes by public appeals, it was rapidly decreased to 25.2%, which means that a lot of the 1st grade zone with public appeals were lowered. In the results of analyzing the distributional characteristics of areas with public appeals, they were mostly distributed in lowland(less than 250m altitude), section with $10{\sim}20^{\circ}$ slope, and areas close to or within 100m from built-up area. Regarding areas with public appeals, the biggest time difference between the period of the existing research by National Ecosystem Survey and the period of notice after completing the treatment of public appeals was 18 years while areas showing 6-15 years of time difference were about 70%. Thus, there were huge differences in time of research and notice. Also, the biggest causes for grade changes were boundary errors caused by small-scale survey, and then followed by changes in evaluation of endangered species and occurrence of built-up area and damaged land. Analyzing areas with public appeals in each evaluation item of the Ecosystem and Nature Map, vegetation part was 73.0%, and endangered species area was 23.1% while topography and wetland was less.

A Study on Rational Alternative Comparison of Prior Review on Environmental Conditions for Basic Plan of Public Waters Reclamation (공유수면매립 기본계획 사전환경성검토의 합리적인 대안비교에 관한 연구)

  • In carrying out the prior review on environmental conditions for basic plan of public waters reclamation, the important item is review on conditions of location on prearranged reclaimed land. In this study, environmental assessment which needs careful consideration in the first stage of selecting location is presented by analyzing environmental impact assessment on public waters reclamation projects for about 10 years. The assessment index for alternative comparison is divided into two parts according to importance level. What is most important in alternative comparison is the comparison on direct influence due to reclamation. In other words, it figures out damaged extent of natural shoreline as well as intertidal and infralittoral zones. Comparing influence extent due to reclamation through the 1st comparison on this is regarded as to be important. In case there is no significant difference by alternatives through the 1st assessment, the 2nd assessment should be carried out. In the 2nd assessment indexes are damaged quantity of benthic organism, change in seawater current, erosion, accumulation and exchangeable rate of seawater. The plan for selecting optimal location which minimizes environmental influence in establishing public waters reclamation plan is demonstrated through model cases. For this, 3 districts which have similar environment and scale are selected, and then comparison on location alternative is made through environmental assessment index. In addition, the comparison on 3 arrangement alternatives are conducted, targeting certain areas. It is thought that carrying out comparison review on respective areas in the same cities and counties, not merely comparing arrangement plan by selecting 1 targeting area in advance, is the most appropriate. Besides, selection of arrangement plan considering environment, which minimizes damage of natural shoreline and influence due to change of seawater current is required in alternative comparison on arrangement plan as well. In case of the prior review on environmental conditions though alternative comparison method presented in this study is carried out, it is safely said that the influence on ocean environment due to public waters reclamation can be minimized.

  • Background: Emerging infectious diseases, such as Middle East respiratory syndrome or coronavirus disease 2019, pose a continuous threat to public health, making a risk assessment necessary for infectious disease control and prevention. Therefore, we aimed to investigate the risk assessment methods for infectious diseases used by major foreign countries and organizations. Methods: We conducted an investigation and comparative analysis of risk assessment and risk determination methods for infectious diseases. The risk assessment tools included the strategic toolkit for assessing risks, influenza risk assessment tool, pandemic severity assessment framework, and rapid risk assessment methodology. Results: The most frequently reported risk elements were disease severity, antiviral treatment, attack rate, population immunity, and basic productive ratio. The risk evaluation method was evaluated quantitatively and qualitatively by the stakeholders at each institution. Additionally, the final risk level was visualized in a matrix, framework, and x and y-axis. Conclusion: Considering the risk assessment tools, the risk element was classified based on the duplicate of each indicator, and risk evaluation and level of risk assessment were analyzed.

  • This paper diagnosed the assessment problems and development types of the simple statement of the Sea Area Utilization Consultation System. and suggested the key assessment items for system improvement. The major types of Public Water occupation and use, and distribution characteristic of regional and coastal were analyzed by evaluate 529 review items during 2010-2012. The artificial structure installation including harbor and fishing port facilities in the South and West coast, and seawater supply and drainage for land-based aquaculture and power plant were dominated. The checklists considering each types of occupation and use in Public Water were suggested. In addition, policy proposals for system improvement were suggested.

  • Public participation in Environmental Impact Assessment (EIA) process has been implemented since the introduction of EIA project on August 1 1991, followed with Environmental Policy Act promulgation on August 1 1990. Though public participation has room for improvement. This study compared before and after the introduction of public participation on EIS preparation cost, project cost, EIS preparation term and project term through 293 EISs analysis. Also public opinions in EIS after introduction of public participation were analyzed. The results are expected to be contributed to the system improvement of public participation.

Restructuring the Public Assessment of Housing Management Toward Standardization

  • This research is to restructure the public assessment of housing management in a standardized fashion, and a self-administered questionnaire survey was employed for data collection and analysis. In doing so, all the assessment tools nationwide were identified and assembled to extract the items included in the survey form. The survey form was mailed out to local officers in charge of housing management and professional housing managers in the nation, and the responses were statistically analyzed in order to structure the standard assessment tool of housing management. This research classified 3 main areas (General Maintenance, Operation and Community Living) in evaluating the housing management performance, and identified 35 essential items and 52 optional items. The 35 essential items were drawn from the 16 categories of the 3 areas (11 items of 5 categories for General Maintenance, 7 items of 4 categories for Operation, and 17 items of 7 categories for Community Living), and the rest 52 optional items were from the 16 categories of the 3 areas (24 items of 7 categories for General Maintenance, 26 items of 7 categories for Operation, and 2 items of 2 categories for Community Living). While all the items in the research are quantifiable, it's suggested that qualitative measurements be complementarily utilized in the assessment not only to explore the best practices of housing management but also to refine and modify the standardized tool in response to various environmental changes. As the public assessment of housing management is context-sensitive and critically related to local housing profile, the continuous implementation of the assessment will facilitate shifting the housing paradigm from production and consumption to management and reproduction.

The Effect of Public Report on Antibiotics Prescribing Rate (급성상기도감염 항생제 처방률 공개 효과 분석)

  • Controlling inappropriate antibiotics prescribing for acute upper respiratory infections(URI) is a very important for prudent use of antibiotics and resistance control. Health Insurance Review and Assessment Service (HIRA) introduced Prescribing Evaluation Program and publicly reported antibiotics prescribing rate for URI of each health institution. We performed segmented regression analysis of interrupted time series to estimate the effect of public report on antibiotics prescribing rate using national health insurance claims data. The results indicate that just before the public report period, clinics' monthly antibiotics prescribing rate for URI was 66.7%. Right after the public report, the estimated antibiotics prescribing rate dropped abruptly by 12.3%p. There was no significant changes in month-to-month trend in the prescribing rate before and after the intervention.

  • According to the Public Records Act, the actual conditions of Government Agencies will be assessed by the concept of 'Assessment' not by 'Instruction and Checking'. In this Study, before adopting a new National Assessment of complying with Records Management, on the basis of the analysis of current 'Instruction and Checking' and other Countries' system which will be able to the model of our new system, we suggest that a Assessment system which will play a role to fulfill and spread the Public Records Act and Records management Standard in this nation's public sector.
