A Study on the Relationships between the Oral Health Activities and Oral Health Conditions of the Elderly (노인의 구강보건행위와 구강건강상태와의 관련성 조사 연구)

  • This study set out to analyze the relationships between the senior citizens' dental health activities and dental health conditions, to provide basic data to develop a program for their better dental health, and finally to search for the ways to improve their life quality through dental health care. The subjects were 142 senior citizens(58 were male and 84 were female) who were 65 years old or older and visited a dental hospital or clinic in the Seoul metropolitan area from April 17 to April 28, 2006. With the cooperation from the dentists, the investigators examined their dental conditions and conducted one-on-one interviews to collect the information about their demographic characteristics, dental health activities, and subjective dental health conditions. The collected data were analyzed with T-test, ANOVA and Pearson's correlation coefficient using the SPSS WIN 11.5 program. The analysis results were summarized as follows: First, 58 men(40.8%) and 84 women(59.2%) consisted of the subject group, where percentage of the female participants was bigger Those who were aged from 65 to 69 made the biggest age group in the subjects, accounting for 58.5% with 83. Second, those who had three dental caries or less and then didn't treat them or treated them(F) accounted for the biggest percentage with 117(82.4%) and 72(50.7%) respectively. As for the remaining teeth, the biggest number of them(40 subjects, 28.2%) had three or less remaining. Third, it turned out the female subjects had a higher level of dental health activities than their male counterparts(P = 0.00). As for living expenses, those who were paid salary or earned income themselves carried out their dental health activities in a higher level than those who lived on the benefit from the government(p = 0.02). Fourth, the subjects' subjective dental health conditions had negative correlations with their dental caries not treated. And there were positive correlations between their dental caries not treated and dental caries lost and between their dental caries treated and remaining teeth. Fifth, those subjects who earned their living expenses themselves had the most dental caries treated at 5.4(p = 0.02), and there was statistically significant difference with the numbers. Sixth, those subjects who brushed their teeth in their own manner had 11.8 teeth lost(p = 0.05), which was more than the number of those who brushed their teeth in other methods. And there was statistically significant difference among them. The remaining teeth were found most at 17.3(p = 0.00) among those who brushed their upper and lower teeth separately. In addition, those who visited the public health center often had significantly more teeth treated(4.3) than others(p = 0.00).

Middle School Home Economics Teachers' Recognition of the Importance and the Level of Teaching Performance regarding the Contents of Morality Education in Home Economics Instruction (중학교 가정과교사의 가정교과에서의 도덕성 교육에 대한 중요도와 지도수행 정도)

  • The purpose of this study is to provide the basic data for morality of education in home economics education. Through research on the state of morality of education, the following criteria can be achieved ; recognition of the importance, and the level of teaching performance regarding the contents of morality education in home economics instruction. The basic data were obtained by looking at the differences between the recognized importance and the level of teaching performance. Data were collected from the survey mailed to the teachers responsible for home economics area in $\lceil$Technology${\cdot}$Home Economics$\rfloor$ in middle schools in Korea. The 335 questionnaires collected were used for final analysis. SPSS/WIN 10.0program was used for calculating and analysis the frequency, percentage, average. standard deviation. Firstly, most of the home economics teachers show that they continually try to implement morality education through each home economics lesson plan. They focus their attention on the main theme of the lesson of a particular nit, that deals with family life. For example, an excerpt from a particular unit may be the 'Understanding of myself and family members'. Most home economics teachers believe that a family setting and/or schools, whether private or public institutions, are appropriate places for productive education. As well, each of these educational settings, should take complete responsibility for teaching morality of education. As a way to activate the morality education, teachers believe that working with the students and understanding the needs of their students, along with the cooperation of parents, are key factors in the teaming process. Another way is the change of teacher's perceptions and educational conditions or environment. Secondly. home economics teachers in middle school. are showing a higher level of understanding on the importance regarding the contents of morality education, which can be noticed through the high level of their teaching performances. Regarding the importance of morality education, the first priority goes to showing respect for parents and the elderly, as well as sexual ethics and respect for life must always be taken into consideration. In regards to the level of teaching performance, the first priority goes to responsibility and co-operation. Secondly. showing respect for parents and the elderly and sexual ethics will be the third. Through data achieved through this study, the majority of home economics education teachers believe that morality education in home economics education is very important and a key factor in their teaching practices.

A Study on the Applicability of Social Security Platform to Smart City (사회보장플랫폼과 스마트시티에의 적용가능성에 관한 연구)

  • Given that with the development of the 4th industry, interest and desire for smart cities are gradually increasing and related technologies are developed as a way to strengthen urban competitiveness by utilizing big data, information and communication technology, IoT, M2M, and AI, the purpose of this study is to find out how to achieve this goal on the premise of the idea of smart well fair city. In other words, the purpose is to devise a smart well-fair city in the care area, such as health care, medical care, and welfare, and see if it is feasible. With this recognition, the paper aimed to review the concept and scope of smart city, the discussions that have been made so far and the issues or limitations on its connection to social security and social welfare, and based on it, come up with the concept of welfare city. As a method of realizing the smart welfare city, the paper reviewed characteristics and features of a social security platform as well as the applicability of smart city, especially care services. Furthermore, the paper developed discussions on the standardization of the city in terms of political and institutional improvements, utilization of personal information and public data as well as ways of institutional improvement centering on social security information system. This paper highlights the importance of implementing the digitally based community care and smart welfare city that our society is seeking to achieve. With regard to the social security platform based on behavioral design and the 7 principles(6W1H method), the present paper has the limitation of dealing only with smart cities in the fields of healthcare, medicine, and welfare. Therefore, further studies are needed to investigate the effects of smart cities in other fields and to consider the application and utilization of technologies in various aspects and the corresponding impact on our society. It is expected that this paper will suggest the future course and vision not only for smart cities but also for the social security and welfare system and thereby make some contribution to improving the quality of people's lives through the requisite adjustments made in each relevant field.

Cost Analysis of Home Nursing Care Patients in Rural Hospital (농촌 지역 중소병원의 가정간호사업소 등록환자의 방문비용분석)

  • The home nursing care system is an integral part of the health care delivery system in order to meet the various needs of consummer, in particular, early discharge patient from the hospital, patient with long term care needed and the elderly. To find out the cost of home nursing care services, the home nursing care records of patients registered by home nursing care units established in public hospital with 150beds during the period of 1996 - 1997 were analyzed. The subjects were 102patients, 45 of male patients and 57 of female patients, those who live in a rural area in Kymiggi - Do The results obtained are as follows : 1. The male patients accounted for 44.1% of the total, with 45cases : group aged 60 years and more was the largest group, accounting for 79.5%. 2. The most frequent disease revealed was the osteoporosis which constitute 35.3% of the total registered patients, followed, in order, by malignant tumor, cerebrovascular disease. 3. It revealed that the cost per visit for the male was 47,764won ; the female, 46,078 won per visit. Noteworthy the cost per visit was high in the older patient. It was clearly that the gender, years of age and the cost per visit were statistically significant at 0.01 level and 0.05 level. 4. The cost per visit for the non complicated disease was slightly higher than the complicated disease, but it is not statistically significant, the cost per visit by type of disease varied, the cost per visit for COPD was the highest, followed, in order, by in malignant tumor, cancer, diabetes, osteoporosis etc. 5. It revealed that home nursing care cost for a eligible disease for home nursing care was less than the cost for hospitalization of the same disease, therefore, we expect that the home nursing care is cost efficiency. In conclusion, the home nursing care costs are needed to analyze further in comparison with the hospitalization costs for a certain disease.

A Study on the Effects of User Participation on Stickiness and Continued Use on Internet Community (인터넷 커뮤니티에서 사용자 참여가 밀착도와 지속적 이용의도에 미치는 영향)

  • The purpose of this study is the investigation of the effects of user participation, network effect, social influence, and usefulness on stickiness and continued use on Internet communities. In this research, stickiness refers to repeat visit and visit duration to an Internet community. Continued use means the willingness to continue to use an Internet community in the future. Internet community-based companies can earn money through selling the digital contents such as game, music, and avatar, advertizing on internet site, or offering an affiliate marketing. For such money making, stickiness and continued use of Internet users is much more important than the number of Internet users. We tried to answer following three questions. Fist, what is the effects of user participation on stickiness and continued use on Internet communities? Second, by what is user participation formed? Third, are network effect, social influence, and usefulness that was significant at prior research about technology acceptance model(TAM) still significant on internet communities? In this study, user participation, network effect, social influence, and usefulness are independent variables, stickiness is mediating variable, and continued use is dependent variable. Among independent variables, we are focused on user participation. User participation means that Internet user participates in the development of Internet community site (called mini-hompy or blog in Korea). User participation was studied from 1970 to 1997 at the research area of information system. But since 1997 when Internet started to spread to the public, user participation has hardly been studied. Given the importance of user participation at the success of Internet-based companies, it is very meaningful to study the research topic of user participation. To test the proposed model, we used a data set generated from the survey. The survey instrument was designed on the basis of a comprehensive literature review and interviews of experts, and was refined through several rounds of pretests, revisions, and pilot tests. The respondents of survey were the undergraduates and the graduate students who mainly used Internet communities. Data analysis was conducted using 217 respondents(response rate, 97.7 percent). We used structural equation modeling(SEM) implemented in partial least square(PLS). We chose PLS for two reason. First, our model has formative constructs. PLS uses components-based algorithm and can estimated formative constructs. Second, PLS is more appropriate when the research model is in an early stage of development. A review of the literature suggests that empirical tests of user participation is still sparse. The test of model was executed in the order of three research questions. First user participation had the direct effects on stickiness(${\beta}$=0.150, p<0.01) and continued use (${\beta}$=0.119, p<0.05). And user participation, as a partial mediation model, had a indirect effect on continued use mediated through stickiness (${\beta}$=0.007, p<0.05). Second, optional participation and prosuming participation significantly formed user participation. Optional participation, with a path magnitude as high as 0.986 (p<0.001), is a key determinant for the strength of user participation. Third, Network effect (${\beta}$=0.236, p<0.001). social influence (${\beta}$=0.135, p<0.05), and usefulness (${\beta}$=0.343, p<0.001) had directly significant impacts on stickiness. But network effect and social influence, as a full mediation model, had both indirectly significant impacts on continued use mediated through stickiness (${\beta}$=0.11, p<0.001, and ${\beta}$=0.063, p<0.05, respectively). Compared with this result, usefulness, as a partial mediation model, had a direct impact on continued use and a indirect impact on continued use mediated through stickiness. This study has three contributions. First this is the first empirical study showing that user participation is the significant driver of continued use. The researchers of information system have hardly studies user participation since late 1990s. And the researchers of marketing have studied a few lately. Second, this study enhanced the understanding of user participation. Up to recently, user participation has been studied from the bipolar viewpoint of participation v.s non-participation. Also, even the study on participation has been studied from the point of limited optional participation. But, this study proved the existence of prosuming participation to design and produce products or services, besides optional participation. And this study empirically proved that optional participation and prosuming participation were the key determinant for user participation. Third, our study compliments traditional studies of TAM. According prior literature about of TAM, the constructs of network effect, social influence, and usefulness had effects on the technology adoption. This study proved that these constructs still are significant on Internet communities.

A Study of Correlation among the Knowledge of the Disease, Health Promoting Behaviors and the Quality of Life in the Female Patients with Osteoporosis (여성 골다공증환자의 골다공증에 대한 지식, 건강증진행위 및 삶의 질과의 관계 연구)

  • Osteoporosis is the disease that is the most controversial issue with the aging tendency in 1990. It is an important issue in dealing with the health management for women in middle life. This study is designed to determine the relationships among the level of the knowledge of osteoporosis, the health promoting behaviors and the quality of life. The test has conducted a questionnaire survey of women with osteoporosis. This is a study that focused an interrelation of above issues and its purpose is to gain a foundation of a nursing intervention that can help to improve the qualify of life of an osteoporosis patients. The method of collecting data is collecting a questionnaire survey, individual interviews and records and the collected 123 questionnaire surveys are utilized for an data analysis. Questionnaire surveys are conducted in K and S Universities infirmaries and K general hospital in Seoul and C and J general hospital in near Seoul area from Sep. 27th to Nov. 4th, 2000. The instruments of this study were as follows: Yoon's used the revised and supplemented instrument to test the level of the knowledge of osteoporosis and she referred to the substance of Cassells & Knotz. Lee's revised and supplemented the instrument called Health Promoting Life style Profile that were invented by Walker, Sechrist & Pender and she used it to test the health promoting behaviors with the questionnaires added by this researcher. They are not related to the quality of life but relevant to the patients. Ro's tested the quality of life with the instrument that was invented by herself. The results of this study were as follows. 1. The general characteristics of this study subjects showed that the age of the patients was concentrated from 50 to 59. The periods after menopause was mainly from 6 to 10 years. Subjective symptoms that patients usually complain were musculoskelectal symptoms. 2. The results of measuring the level of the knowledge of osteoporosis, the health promoting behaviors and the quality of life were following. The level of the knowledge was the score 19.83 and it was above middle score out of the score 27 as full marks. In the result of the health promoting behaviors test, the score average was 2.58 with 4 point rating scale and it was about middle score. The quality of life test showed that average score was 2.92 with 5 point rating scale and it indicated that the quality was also around middle score. 3. The statistical test in the activities for promoting health according to the general characteristics of the patients showed that only monthly income has statistically significant difference(F-4.761, p=.004). The same test for the qualify of life indicated that the quality of life was higher from 1 to 5 years after menopause but it was not meaningful statistically. 4. In the correlation among the level of knowledge of osteoporosis, the health promoting behaviors and the qualify of life of the patients, showed a relationship between the health promoting behaviors and the qualify of life. The higher the health promoting behaviors, it was also higher the quality of life. But it was statistically significant with significant positive correlation.

A Survey on the Dietary Intake of the Elderly in an Urban Area and on Analysis of the Related Factors (일부 도시지역 노인의 영양섭취 실태와 그 관련요인에 관한 조사연구)

  • This study was conducted to evaluate nutritional status with the analysis of the factors which might influence on the prevalent condition of nutrients-intake. 373 persons who are above 60 years old in Chung-Gu, Seoul were surveyed from March 15 to March 30 in 1985. Results were as follows ; 1. General Characteristics 1) Male respondents among the entire respondents comprised 54.3% and female respondents comprised 45.7%. As for age, male and female respondents comprised 55.0% and 37.2% respectively. 2) As for the educational level of the respondents, illiteracy was 49.9% which was the highest. and those who graduated elementary school was 28.1% which was the second highest. And as for the comparison of the education of the male and female respondents, the educational level of male respondents was much higher than that of female respondents. 3) As for the final occupation of the respondents, those who were engaged in commerce comprised 30.8% which was the highest rate, and who were engaged in agriculture comprised 20.1% which was the second highest rate. And those who were engaged in labor comprised 12.3%. 2. The State of Nutrients-intake The energy of the male respondents was 61.4% of RDA and the protein was nothing but 66.8% of RDA. Except vitamin A, vitamin $B_1$ and niacin, calcium, iron, vitamin $B_2$, vitamin C were lower than RDA (the range: $67.5%{\sim}97.3%$). The energy of the female respondents was 70.0% of RDA and the protein was 58.8% which were lower than RDA. Except vitamin A and vitamin $B_1$, calcium, iron, vitamin $B_2$ vitamin C, niacin were lower than RDA (the range: $68.2%{\sim}98.9%$). 3. The various factors related to nutrients-intake 1) The composition of family and the food environment those who were living with their spouses comprised 44.8% which was the highest rate, and those who were living with their son's family comprised 33.5% which was the second highest rate and those who were living with their doughter's family comprised only 4.6%. Those who were living alone comprised 16.4%. The correlation with nutrient intake didn't show a clear relation. 2) Socio-economic state Those who said that they lacked in living expense comprised 32.7% and those who said that they lacked in pocket-money comprised 23.9%, The female respondents outnumbered the male respondents in lacking living expense or pocket-money. In the correlation with socio-economic, there was no striking relation with education level. However, it was shown that little relation in energy, fat in male and also fat and iron in female. The living expense and the satisfaction in pocket-money had also little relation with energy fat and vitamin C in male. 3) Nutrition knowledge and food habit (a) The state of nutrition knowledge and food habit In nutrition knowledge, those who scored less than 60 points were 32.9%, and those who scored more than 90 points were 21.0%. In food habit, those who scored less than 60 points were 26.5% and those who scored more than 80 points were 45%. (b) The corelation with nutrition knowledge and food habit In case of female respondents, nutrition knowledge had a little relation with the intake of cabohydrate, iron, and vitamin C. And we couldn't see a distinct phenomenon in food habit.

Fisheries Management of an Abalone Haliotis diversicolor in the Eastern Coastal Waters of Jeju Island using Yield-per-recruit Model (가입당생산량(yield-per-recruit) 모델을 이용한 제주 동부연안 오분자기류(Haliotis diversicolor)의 어업관리)

  • We studied the management policy for Haliotis diversicolor fisheries in the coastal area of Sungsanpo using Yield-per-recruit model from 2004 to 2006. The age at first capture($t_c$) and fishing mortality(F) annually estimated during the study period decreased and increased, respectively. The maximum yield-per-recruit in 2004 was increased by increasing $t_c$ from the 2.012 year of current $t_c$ to 2.7 year or increasing F from the 0.574/year of current F to 0.800/year, and that in 2005 was increased by increasing $t_c$ from the 1.946 year of current $t_c$ to 2.5 year or increasing F from the 0.578/year of current F to 0.880/year. In 2006, the maximum yield-per-recruit was increased by increasing $t_c$ from the 1.926year of current $t_c$ to 3.1 year or decreasing F from the 1.088/year of current F to 0.810/year. Further, although the current F in 2004 and 2005 was lower than the estimated $F_{MAX}$, that in 2006 was higher than the estimated $F_{MAX}$. These results indicate that the likelihood of growth overfishing with increasing catch of smaller H. diversicolor in 2006 was greater than in 2004 and 2005. As action that could prevent growth overfishing in fisheries management of H. diversicolor, increasing for the current $t_c$ could be a more appropriate policy because the artificial decrease of the number of woman divers related F is actually difficult.

Epidemiological Studies on Giardia Infection Associated with environmental Pollution (Giardia에 의한 환경오염(環境汚染)과 감염(感染)에 관한 역학적(疫學的) 연구(硏究))

  • Giardia lamblia is a pathogenic flagellate causing intestinal disorders such as diarrhea, abdominal pain and malabsorption of nutrients. Giardia is mainly infected by the ingestion of contaminated foods per os. Craun (1979) has recently reported that mass infection of this flagellate through the contaminated water supply systems is one of public health hazards. Also, so-called traveller's diarrhea is sometimes caused by Giardia infection (CDC, U.S.A., 1971). However, a few epidemiological studies figuring out the mode of infection or control measures of Giardia infection has been done so far in Korea. The present study was aimed to know the prevalence of Giardia infection in several Korean populations, detectability of this flagellate in water systems and the viability of the cysts against sewages and disinfectants applying to drinking water. In the present study, 388 stool specimens from orphanage children in Chun-joo, Chung-joo, On-yang and Chun-an areas and 538 stool specimens from inhabitants in Woo-do, In-chon, and Chun-joo were examined by formalin-ether concentration technique to detect out Giardia cysts. On the other hand, water samples from 14 sites of Han River and its tributaries were collected in May through July, 1984. Fifty liter of water sample in each sampling site was then filtered through water filtering system deviced by U.S. Environmental Proutection Agency and the sediments rinsed out from the thread rolls, a part of water filtering system, were examined to detect out the Giardia cysts. In order to observe the viability of Giardia cysts in the sewage samples, the cysts were treated in it at $4^{\circ}C$ or $25^{\circ}C$ for 7 through 28 days. For this purpose, the cysts were also exposed to various concentrations of disinfectants such as chlorine, iodine and ozone gas for proper time intervals. After treatment, the viability test of the Giardia cysts were carried out by method of Rice and Schaefer (1981) with minor modification. The results obtained in this study were as follows : 1) The detection rates of G lamblia cysts in the stool specimens were 18.3% in orphans and 4.3% in general examinees. 2) The prevalences of Giardia Infection were higher in the young age groups than in-adults. The highest positive rate was 18.4% in the age group less than 10. 3) Of 14 water specimens sampled from Han River system and its tributaries around the Seoul area, the Giardia cysts were detected from 4 samples, and no cyst was found in the water supply systems. 4) The cysts treated in the sewage survived for 28 days at $4^{\circ}C$ and for 13 days at $25^{\circ}C$. 5) The cysts were completely destroyed within 60 minutes by exposure to 8 mg/l of residual chlorine at 4g and within 30 minutes by exposure to the same concentration of chlorine at $25^{\circ}C$. 6) The cysts were all dead when exposed to 1 mg/1 of iodine for 60 minutes at $4^{\circ}C$ or $25^{\circ}C$. 7) The cysts were destroyed after 10 minute exposure in 0.15 mg to 0.25mg of residual ozone gas per liter. Summarizing the above results, it is considered that Giardia infection is regarded as water-borne disease and the cysts are able to be controlled by the application with the disinfectants in the water supply systems.

A Study on Effective Smoking Cessation Policy of Smoker Patients (흡연환자의 효과적인 금연정책에 대한 연구)

  • The purpose of this study was to examine the subjective awareness of smoker patients about the preparation of smoking cessation plans in an effort to lay the foundation for smoking cessation policy setting. The subjects were the selected patients at a dental clinic. A self-administered survey was conducted to grasp their personal characteristics, and the selected answer sheets from 236 respondents were analyzed. The findings of the study were as follows: 1. As for the extension of existing smoking cessation plans, the largest group (32.6%) preferred smoking cessation education, followed by smoking cessation counseling (28.8%), prescription of an smoking cessation aid (18.6%). 2. Concerning the necessity of a smoking cessation law, the men and the women respectively gave 2.37 and 3.00 to that, and the gender gap was significant. The patients had a significantly different opinion on the necessity of a rise in tobacco price according to their occupation and monthly mean household income, and their took a significantly different view of the necessity of smoking cessation counseling according to their residential area. 3. As a result of analyzing the correlation between their on effective smoking cessation policies and the related variables, all the variables had a statistically significant correlation to each other. 4. As a result of analyzing their opinions on what institution should be in charge of smoking cessation plans, the biggest group answered that dental clinics should be responsible for smoking cessation-aid prescription (50.0%), and the largest group replied that smoking cessation education and smoking cessation counseling should respectively be provided by public dental clinics (37.3%) and dental clinics (44.1%).The above-mentioned findings suggest that the preparation of new smoking cessation measures and the extension of existing smoking cessation plans are urgently required, and that dentists and dental hygienists should make a concerted effort to offer counseling and education to stimulate dental patients to abstain from smoking.
