  • Purpose: The study investigated pain and falls as psychological factors in Total Knee Replacement patients and examined the relationship between these factors. Methods: Ninety-six total knee replacement patients aged 69.31±7.01 (male: 21, female: 75) participated in the study. Post-surgery pain was assessed on a visual analog scale, and fall risk scores were evaluated on fall fear and fall efficacy scales. Other psychological factors assessed included national anxiety and trail anxiety, fear prevention, Beck depression score, and psychological health measures. The data were analyzed using SPSS version 22.0. Results: A statistically significant correlation was found between pain before surgery and FES fall fear score before surgery (p < 0.05). A statistically significant correlation was found between pain after surgery and PWBS fall fear score after surgery (p < 0.05). A statistically significant correlation was found between state anxiety and PWBS, Trail anxiety (p < 0.05). A statistically significant correlation was also found between trail anxiety and BDI, PWBS (p < 0.05). State anxiety and fear avoidance before surgery were found to affect trail anxiety, and pain before surgery and fall fear score before surgery were found to affect FES. In addition, PWBS was found to affect BDI. Conclusion: The study confirms that increased pain in Total Knee Replacement patients can be a risk factor for falls. In addition, fall fears increase fear avoidance and cause psychological deterioration. This suggests that efforts should be made to reduce pain and to manage psychological factors.

  • Purpose: The purpose of this study was to define the concept for psychological insulin resistance in the Korean population with diabetes. Methods: The Hybrid model was used to perform the concept analysis of psychological insulin resistance. Results from both the theoretical review with 26 studies and a field study including 19 participants with diabetes were included in final process. Results: The preceding factors of psychological insulin resistance were uncontrolled blood glucose and change in daily life. The concept of psychological insulin resistance was found to have three categories with 8 attributes such as emotional factors (negative feeling), cognitive factors (low awareness and knowledge, low confidence for self-injection) and supportive factors (economic burden, dependency life, embarrassing, feeling about supporters, feeling of trust in, vs mistrust of health care providers). The 8 attributes included 30 indicators. Conclusion: The psychological insulin resistance of population with diabetes in Korea was defined as a complex phenomenon associated with insulin therapy that can be affected by emotional factors, cognitive factors, and supportive relational factors. Based on the results, a tool for measuring psychological insulin resistance of Koreans with diabetes and effective programs for enhancing insulin adherence should be developed in future studies.

  • Amputation changes the lives of patients and their families. Consequently, the patient must adapt to altered body function and image. During this adaptation process, psychological problems, such as depression, anxiety, and posttraumatic stress disorder, can occur. The psychological difficulties of patients with amputation are often accepted as normal responses that are often poorly recognized by patients, family members, and their primary physicians. Psychological problems can interfere with rehabilitation and cause additional psychosocial problems. Therefore, their early detection and treatment are important. A multidisciplinary team approach, including mental health professionals, is ideal for comprehensive and biopsychosocial management. Mental health professionals could help patients set realistic goals and use adaptive coping styles. Psychiatric approaches should consider the physical, cognitive, psychological, social, and spiritual functions and social support systems before and after amputation. The abilities and limitations of physical, cognitive, psychological, and social functions should also be considered. To improve the patient's adaptation, psychological interventions such as short-term psychotherapy, cognitive behavioral therapy, mindfulness meditation, biofeedback, and group psychotherapy can be helpful.

To cope with sexual harassment, there should be a department capable of counseling and processing in the workplace, and what occurs should be analyzed. In addition, educational programs are needed to prevent sexual harassment in consideration of the characteristics of hospitals.

In order to have insights on the participants behaviors, this qualitative research was conducted. The participants in this study were six women who have been taking at least one of the alternative therapies to relieve the menopausal symptom. Data had been collected from November 2002 to May 2003. The interview questions were about motivations to seek the alternative therapy, menopausal symptoms, feelings or positive effects after the treatment. The interviews were recorded and transcribed into written documents. The method described by Giorgi was utilized for data analysis. The study results were as follows : 1. The participants sought the alternative therapy based on their own information or by friends' recommendations. The motivations of taking the therapy were to live younger and healthier by reliving menopausal symptoms including somatic symptoms, such as irregular menstruation, flushing sense, cold sweating, insomnia, dull headache, vagina dryness, cystitis after sexual intercourse, joint pain, nervousness, depression, skin dryness, and suppression of activity. 2. The participants expressed that they became more active and felt better with disappearing menopausal symptoms after about 2 to 3 weeks treatment. 3. The participants did not want to accept the fact that they had menopause, which caused avoidance of having medical diagnosis and medical treatment. They also did not want to let their spouses know about their symptoms. 4. The major reason of taking the alternative therapy was to avoid the side effects of estrogen therapy in spite of high costs of the alternative way. 5. The participants suggested that menopausal women needed to be educated about physical and psychological symptoms of menopause. They also needed to be informed about how to manage it. In conclusion, accurate information about foods and drugs utilized for the alternative therapy was needed. Medically proved methods to recognize positive effects and side effects were also necessary. These goals could be achieved by continuous medical observations and further research.

As a result, the index of greenness increased by 0.73 points(t=2.555, p <.01) and restorative environment by 1.01 points (t=2.567, p <.01), showing statistical significance. Negative emotion increased by 0.04 points (t=0.183, p >.05), which was not significant. On the other hand, positive emotion decreased by 0.42 points (t=-1.918, p <.05), facial expression by 0.57 points (t=-1.775, p <.05), and psychological assessment by 0.29 points (t=-0.981, p >.05), showing significance in positive emotion and facial expression.
