Psychiatric Analysis about Transsexuals (성주체성장애 환자의 정신의학적 분석)

  • Purpose: Transsexualism is considered to be the extreme end of the spectrum of gender identity disorders characterized by, among other things, a pursuit of sex reassignment surgery (SRS). We evaluated psychologic status, health-related quality of life in female-to-male (FTM), male-to-female (MTF) transgender individuals. Methods: We used the Minnesota Multiphasic Personality Inventory, Beck Anxiety Inventory, Beck Depression Inventory, Moudsley obsessive-compulsive Inventory, SCL-90-R, Short-Form 36-Question Health Survey version 2 (SF-36v2). We enrolled 40 transsexual participants. Results: Analysis of quality of life health concepts demonstrated statistically significant (p<0.01) diminished quality of life among the transsexual participants as compared to the Korea male and female population. FTM transgender participants reported more higher hostile, phobic than MTF transgenders. Overall, in all psychologic status examination, Transgender individuals are within normal population boundary. On all category, result is improved post-operatively. Conclusion: Transgender participants reported mental status within normal boundary. SRS improved their quality of life and mental stability.

  • The causes for airway obstruction include foreign body aspiration, congenital structural abnormalities of the airway, infection, etc. And the potential causes of acute respiratory distress contain many situations, like hyperventilation, vasodepressor syncope, asthma, etc. A major factor that leads to the exacerbation of respiratory disorders is undue stress, either physiologic or psychologic. Psychologic stress in dentistry is the primary factor in the exacerbation of preexisting medical problems. Adequate pretreatment medical and dental evaluation of the prospective patient can often prevent respiratory problems from developing. The dentist can modify patient management to minimize the risk of exacerbating these conditions. When dental anxiety is a major factor, the use of psychosedative procedures and other stress-reduction techniques should also be considered. This is the report of a children case of airway obstruction and respiratory distress owing to sedation complication by use of Chloral hydrate and Ketamine before extraction of the mesiodens in a patient with bronchial asthma and tonsillar hyperplasia. After these situations, the patient was consulted & referred to the department of Pediatrics and Otorhinolaryngology.

Psychometric Charateristics of Occupational Low Back Pain Patients (일부 재해성 요부손상 환자의 심리적 특성)

  • This study was done for identifying the factors which affect psychologic symptoms of low back(LBP) patients. The study subjects were 43 work-related low back pain patients, 28 work-related non-low back pain patients and 47 general low back injury patients. The study materialis SCL 90-R for checking psychologic symptoms and questionnaire for obtaining general information about the subjects. The data were analyzed by model of analysis of covariance adjusted by several variables such as gender, age, education and marital status and then compared the least square means of symptom score between groups. To identify the factors that affect psychologic symptom, duration of suspension, return to work and interaction factor of these two variables were analyzed by multivariate model and we calcuated partial correlation coefficient of these variables. As a result, work-related LBP patients showed higher score of symptoms in somatization, depression and psychosis than work-related non-LBP and non-work-related LBP. Duration of suspension and return to work were significant explanatory variables for psychologic symptom score of work-related LBP. Then, we may conclude that the treatment and rehabilitation programe for work-related LBP should cover the strategy of early return to work.

  • There are many work-related injuries and diseases of fisheries in Korea. Fishermen have exposed to harmful environment, such as natural disasters, ultraviolet, noise, vibration, organic solvents, ergonomic risk factors, loss of fresh water and food, and psychologic stress. They have suffered from numerous work-related health problems, including musculoskeletal disorders, cardiovascular diseases, infectious diseases, pyschological diseases, decompression sickness, and so on. Sea accidents can cause deaths of fishermen and if sea pollution is occurred, fishermen have a great economic damage. However, the concern of these injuries and diseases has been little because they are small in number and have decreased. Fisheries are important core industries, so the problems of fisheries have to be solved by a government. But work-related injuries and diseases are not investigated properly and accurate diagnoses are not made due to loss of concern. We hope that this article will be a cornerstone to solving the work-related problems of fisheries.

Management Strategy by Evaluation on Comprehensive Health Problem in the Community-dwelling Elderly of Korea (재가노인의 포괄적 건강문제 평가와 관리방안)

  • Purpose: The aim of this study was to define the health problem in the community-dwelling elderly of Korea and to compare differences of CAPs(Client Assessment Protocols) by characteristics. Method: Data was collected by visiting nurse from 556 elderly over 65 years in selected metropolitan areas. To evaluate the functional state of elderly in the community, I used "RAI-MDS HC; Residental Assessment Instrument Minimum Data Set-Home Care(2.0 version)" and established information exchange system among resources, by developing the data into a computer program. Results: The health problem of 'preventive health measures' was the largest(99.6%), and then 'health promotion(85.3%)', 'visual function(75.5%)', 'psychologic drug(68.9%)', 'pain(68.5%)', ‘social function(59.2%)', 'communication disorders(56.2%)', 'environmental assessment(53.2%)', 'depression & anxiety(46.9%)', 'oral health(43.4%)' followed. The number of health problems was average 10.16 in the community-dwelling elderly. Conclusion: The results suggest the need to emphasize the importance of assessment of the health problem of the elderly. We can apply it in the distribution of community resources and the development of service providing programs by figure out the health problem and resource in need for the elderly in the community.

The Prevalence and Factors of Irritable Bowel Syndrome in University Students (일대학교 대학생의 과민성 장 증후군 유병률 및 영향요인 연구)

  • Purpose: The purpose of this study was to examine the prevalence and factors on Irritable Bowel Syndrome (IBS) symptoms in university students. Methods: This study was descriptive survey research. The subjects were 187 university students from one university. The exclusion criteria were 1) had GI surgery 2) had severe physiologic or psychologic problems 3) had liver, bile duct, pancreatic, and intestinal disease that required treatment. The ROME III was used to diagnose IBS. Sociodemographic characteristics, general health state by SF-36, psychological health by SCL-90-R, stress by perceived stress scale, sleep disturbance by Korean sleep scale, and diet habits were measured. Data were analyzed by using SPSS 17.0. Results: Seventeen students (9.1%) had IBS by ROME III criteria, but 46% had some GI problems at that time. Abdominal pain or discomfort continued for last six months or more in 14.3%. Fiber intake, obsessive-compulsiveness, hostility, psychoticism, global severity index, positive symptom distress, role physical, bodily pain, social functioning, and mental health were significantly different between IBS group and Non IBS group. Conclusion: Over nine percent of the students were diagnosed by Rome III criteria, and almost half of them had some GI problems. Diet, personality, general health state, and psychological health were factors of IBS.

Effect of Multifaceted Intervention Program on Multi-Dimensional Psychologic Condition, Empowerment, Work Readiness, and Functional Capacity in Industrially Injured Workers (다면재활프로그램이 산업재해근로자의 다차원 심리상태, 역량강화, 직업복귀준비 및 작업능력에 미치는 효과)

  • This study examined the effects of a multifaceted intervention program on the psychological condition, empowerment, work readiness, and functional capacity for job performance of industrially injured workers. Twelve injured workers in C hospital were included in this study. The subjects participated in a social rehabilitation program 2 hours a day, twice a week, for 8 weeks in total, as well as a work hardening program 3 to 4 hours a day, 5 times a week, for 8 weeks in total. The study was conducted from March to September, 2017. A multi-dimensional psychological examination, empowerment scale, work readiness interview and functional capacity evaluation were conducted and the test scores compared before and after the program with the Wilcoxon signed rank test. In the multi-dimensional psychological examination, there were significant differences in the levels of anxiety, depression, lack of social support, and somatization symptoms (p<.05) except anger. The participants also showed significant differences in the empowerment scale, work readiness scale, and functional capacity evaluation. This study suggests that a multifaceted intervention program can be effective in improving the psychologic condition, empowerment, work readiness, and functional capacity of industrially injured workers and, in turn, may improve their rate of returning to work.

Factors Influencing Activities-specific Balance Confidence in Community-dwelling Old Adults (지역사회 거주 노인의 활동 특이적 균형자신감에 영향을 미치는 요인)

  • Purpose: The purpose of this study is to identify factors influencing activities-specific balance confidence in community-dwelling older adults. Methods: This is secondary analysis of data from an intervention study for improving cognitive function. The data were collected from March 2 to September 30, 2017 at a senior center. Data of 131 older adults were included for this secondary analysis, and were analyzed by using t-test, ANOVA, and multiple regression. Results: The mean score of activities-specific balance confidence is 65.08 out of a possible range of 0-100. The significant factors affecting activities-specific balance confidence among old adults include 'more than 85 years old', 'waist circumference', 'depressive symptoms', 'activity restriction due to fear of falling', and 'self-rated health' which explained 52.8% of the variance. Conclusion: The study results indicate that psychologic factors as well as physical condition should be considered for interventions to increase activities-specific balance confidence.

  • This is a reprospective study on the care of odontogenic infections in admission patients with geriatric diseases. The study was based on a series of 480 patients at Dong San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, From Jan. 1, 2000, to Dec. 31, 2002. The Obtained results were as follows: 1. The systemic malignant tumor was the most frequent cause of the geriatric diseases with odontogenic infectious diseases, and refractory lung disease, systemic heart disease, type II diabetes mellitus, cerebrovascular disease, bone & joint disease, senile psychologic disease were next in order of frequency. 2. Male prediction(57.5%) was existed in the odontogenic infectious patients with geriatric diseases. But, there were female prediction in senile psychologic disease, systemic heart disease and cerebrovascular disease. 3. The most common age group of the odontogenic infectious patient with geriatric disease was the sixty decade(47.9%), followed by the seventy & eighty decade in order. 4. In the contents of chief complaints on the odontogenic infectious patients with geriatric disease, peak incidence was occurred as toothache(52.7%), followed by extraction wish, tooth mobility, oral bleeding, oral ulcer, fracture of restoration, gingival swelling in order. 5. In the diagnosis group of odontogenic infectious diseases, periodontitis, pulpitis & periapical abscess were more common. 6. In the treatment group of odontogenic infectious diseases, the most frequent incidence(34.2%) was showed in primary endodontic treatment (pulp extirpation, occlusal reduction and canal opening drainage) and followed by scaling, incision & drainage, only drugs, pulp capping, restoration in order.

Analysis of Researches on Nursing Intervention for Elderly Health Promotion from 1994 to 2004 (국내 노인 건강증진 간호중재연구 분석 - 1994년에서 2004년까지 -)

  • Purpose: The purpose of this study was to analyse researches on nursing intervention for elderly health promotion. Method: A literature review of 38 researches was carried out using guidelines developed by the present researchers. Result: The research papers studied in this research were theses written for doctoral or master's degree and published between 1994 to 2004. These researches applied nursing intervention for health promotion of elders and used experimental study design. The average period of nursing intervention was about 9 weeks and the average frequency was three times per week. As for major characteristics of subject groups, most of them were over 65 years old (76.3%) and resided in community (65.8%), and each group consisted of 10 to 29 elders. The dependent variables used in these studies included physiological variables (blood pressure, blood cholesterol level, pulmonary function, fasting blood sugar, blood cortisol level, body mass etc.), psychologic variables (depression, quality of life, life satisfaction, loneliness, anxiety etc.), cognitive variables (perceived benefits, perceived barriers, self-efficacy, self-concept, subjective health status etc.), activities of daily livings, health promotion behavior, pain, risks of fall, and variables related to Trans Theoretical Model. The majority of these studies (78.9%) applied exercise programs for health promotion including exercise motivating programs (15.8%). Conclusion: We suggest that more various nursing interventions must be applied to promote elders' health and to take care of their chronic diseases.
