  • We treated the persistent ruminative vomiting of a 13-year-old autistic boy by using a behavioral and psychopharmacological approach in a closed ward. Before the interventions, the patient had lost about 10kg of body weight due to very high-frequency ruminative vomiting. Together with psychopharmacological trials, the psychiatric treatment included a combination of a behavioral approach and food regulation that emphasized food restrictions, and we provided regular light meals. There was a considerable reduction of the ruminative vomiting and 2kg weight gain was achieved during the admission. These therapeutic gains were maintained at the 4-month follow-up assessment and the vomiting symptom reached a near-zero level and the weight loss were recovered by about 8kg. The issues related to the treatment approaches for ruminative vomiting with autism are discussed.

  • Purpose: This study was conducted to investigate the effects of forest walking program on stress and recovery among schizophrenic patients in a closed ward. Methods: A non-equivalent control group pre-posttest design was used. The participants were composed of 36 patients with schizophrenia admitted in two psychiatric hospitals. The experimental group (n = 18) received the forest walking program twice a week with a total of 6 weeks. Results: The study results revealed that the forest walking program was effective on stress(${\chi}^2=34.11$, p<.001), subjective recovery(${\chi}^2=22.40$, p<.001) and clinical recovery(F=28.34, p<.001) of hospitalized patients with schizophrenia. Conclusion: The findings indicate that the forest walking program for schizophrenic patients in the closed ward can be an effective nursing intervention to reduce stress and improve recovery ability. We suggest that mental health nurses may adapt this program to help patients with schizophrenia in a closed ward.

  • The purpose of this study is to identify the inpatient service quality at mental hospitals and to examine the effect of service quality on patient's overall satisfaction and customer loyalty. Data collection was done through conducting a survey of inpatients in three mental hospitals located in Chungnam, Korea using self administered questionnaire. The questionnaire included the modified version of 'service satisfaction scale for psychiatric patients' invented by Chul Kwon Kim and other members, which consists of five dimension of service quality: staff attitude, treatment quality, ward environment, access/cost and ward rule. Total 236 questionnaires were gathered and 219 were used for analysis. The data was analyzed by using version SAS9.2 and path analysis model was applied to test our hypothesis. As a result, the four factors of service (staff attitude, treatment quality, ward environment and ward rule) were extracted, which counted for 62.89% of the common variance. Moreover, Cronbach Alpha showed relatively high internal consistency of answer, all exceeding 0.6. According to the study, ward environment and treatment quality turned out to have direct influence on the patient's satisfaction. In addition, staff attitude and overall satisfaction directly influenced the customer loyalty while ward environment and ward rule indirectly had influence using overall satisfaction as a medium. In conclusion, in order to enhance the satisfaction and customer loyalty in mental hospitals, efforts in improving environmental service quality in particular is highly demanded.

  • Purpose: This study was intended to measure the knowledge and attitudes of nurses working in a psychiatric ward regarding the subject of pain. Methods: In this descriptive study, data were collected in psychiatric wards(or hospitals) located in the Kyungnam and Kyungpook area from December 2013 to January 2014, using a modified version of the McCaffrey and Pasero Clinical Manual Survey Regarding Pain. The content of the survey follows a logical progression from common pain management problems through mechanisms of pain, assessment and pharmacology. Results: The average score of knowledge regarding pain was roughly $21.59{\pm}2.93$ among the 181 participants. The nurses selected Tylenol as the preferred choice (81.2%). Seventy-one percent of respondents didn't use Pain Intensity Instruments during the pain assessment. The subjects preferred the Numerical Rating Scale rather than Visual Analogue Scales. Pharmacology questions were the least likely to be answered correctly. No specific correlations were found between score and demographic variables of age and experience. No specific correlation between score and educational level was noted. Conclusion: Nurses require ongoing education on pain management, particularly regarding pharmaceutical agents.

  • The main purpose of this study was to investigate attitudes toward mental illness and the mentally ill of psychiatric nurses in Korea and to identify the relationships between the nurses' attitudes and demographic variables. Subjects in this study were 122 psychiatricnurses and 111non-psychiatric nurses in active service at 12 hospitals in Seoul. The insrtument used for measuring attitudes was a questionaire developed by Cohen and Struening (1962) referred to as the“Opinions about Mental illness(OMI) Scale”made up to 51 Likert-type items. The findings of this study indicated hatnurses showed negative attitudetoward mental illness and the mentally ill: Very high on social restrictiveness (factor D), low on Mental Health Ideology (factor C), and Interpersonal Etiology (factor E). • More and Tore emphasis is put on moral treatment in the care of psychiatric patients. Methods employed for moral treatment rake use of therapeutic activity for the mentally ill patient. In some psychiatric hospitals all of the therapeutic activities are categorized as occupational therapy, while in others these activities are departmentally categorized -recreational, musical, occupational and bibliotherapy. All of these constitute separate departmental therapies. By research, it is known at this time, that the above activities have been of great help in personality integration, reeducation and early recovery of patients, as well as in their social adjustment after discharge. The goal of these therapeutic activities incorporate the goals set by healthier interpersonal relationship and psychotherapy. Put the therapeutic activities require a prolonged time of therapy for effectiveness, economic support, special trained personnel, careful planning, guidance and proper evaluation. Therefore this treatment is a very complicated one. But it may be the most profitable therapy for certain patients. One true fact about this treatment is, that simple recreation, occupation, music, reading, etc., make the patients life in the ward more interesting and have a good effect on the interpersonal relationship between patients. Thus it has been proved that this kind of treatment demands the active and full participation of the personnel and the full support of the hospital administration.
