A Comparison of Hydromorphone-Bupivacaine and Fentanyl-Bupivacaine in Patient Controlled Epidural Analgesia after Thoracotomy (개흉술 후 경막외 통증자가조절을 설시한 환자에서 Hydromorphone-Bupivacaine과 Fentanyl-Bupivacaine 투여의 비교)

  • Background: Hydromorphone has an intermediate lipid solubility range that falls between morphine and fentanyl. Lipophilic activity during opioid epidural administration is important in relation to both the side effects and analgesic efficacy. The purpose of this study was to compare epidural hydromorphone and fentanyl when concomitantly infused with bupivacaine in patients undergoing a thoracotomy. Methods: Seventy-seven thoracotomy patients, with patient-controlled epidural analgesia (PCEA), were blindly allocated into two groups [group F (n = 34); 0.1% bupivacaine and fentanyl $5{\mu}g/ml$, group H (n = 34); 0.1% bupivacaine and hydromorphone $16{\mu}g/ml$)]. The basal PCEA rate and demand dose were 4 ml/hr and 3 ml, respectively. The visual analogue scale (VAS) for pain, and pruritus, sedation and nausea were measured at 6, 12 and 24 hours after the operation. Results: There were no significant differences in the VAS pain scores and the incidences of pruritus, nausea and sedation between the two groups. The total infused volume after 24 hours was lower in H compared to that of F group (P < 0.05). Conclusions: We conclude that epidural hydromorphone or fentanyl administration has a similar analgesic efficacy and shows similar incidences of side effects, when concomitantly infused with bupivacaine, in the management of acute pain following a thoracotomy.

A Case of Allergic Contact Dermatitis Aggravated by Steroid Ointment (스테로이드 외용제로 악화된 알레르기성 접촉피부염 치험 1례)

  • Objectives : The purpose of this paper is to report the effect of Korean medicine treatments on allergic contact dermatitis aggravated after applying steroid ointment. Methods : We treated a 63-year-old woman with allergic contact dermatitis on the both forearm, jaw, neck, chest with Korean medicine. Before Korean medicine treatments she has taken oral steroids and applied steroid ointment for several years to treat dermatitis, but long-tern steroid therapy aggravated her symptoms like skin pruritus, erythema, exudates or edema. After Korean medical treatment for 27 days, we found that the size of erythema was reduced, and symptoms like pruritus, exudates and edema were lost. We recorded pictures of changes on symptoms. Results & Conclusions : The symptoms of allergic contact dermatitis were improved. Erythema, edema, exudate were lost. Thus Korean medicine treatments are effective on allergic contact dermatitis and could be a great alternative to steroid treatment.

A Case of Nummular Eczema Treated with Bokryeongkamcho-tang and Yeongkyechulkam-tang (복령감초탕(茯苓甘草湯)과 영계출감탕(苓桂朮甘湯)으로 치료한 화폐상습진 환자 치험 1례)

  • Objectives : The purpose of this study is to report the clinical effectiveness of herbal remedies (Bokryeongkamcho -tang. Yeongkyechulkam-tang.) on nummular eczema. Methods : The 49 year-old male patient has been suffered from nummular eczema for 8 months. At first, he was treated with Bokryeongkamcho-tang for 56 days. And then he was treated with Yeongkyechulkam-tang for last 40 days. During the treatment we checked pruritis degree changes and extent changes by asking. And we checked how prulent exudates, crusts, scratch marks are changes by taking photos. First evaluation was performed before treatment, second evaluation was performed after taking Bokryeongkamcho-tang 56 days, third evaluation was performed after taking Yeongkyechulkam-tang 40 days. Results : After taking Bokryeongkamcho-tang 56 days, pruritus was decreased and prulent exudents were stopped, and crusts were disappeared. Nummular eczema on trunk, arm, thigh was disappeared. But light pruritis was still going on, and small circle-shaped vesicles groups were lagged on forearm and shin, ankle. So we found other symptoms from the patient, and administered Yeongkyechulkam-tang. After taking Yeongkyechulkam-tang 40 days, pruritus was almost decreased and nummular eczema on fore arm, shin was disappeared. Conclusions : This clinical case indicates that herbal remedies(Bokryeongkamcho-tang. Yeongkyechulkam-tang.) are effective on nummular eczema.

The Effects of Low Level Laser Therapy on Decrease of Atopic Dermatitis Symptoms (저출력 레이저 치료(LLLT)가 아토피 피부염의 증상 완화에 미치는 영향)

  • Objectives The purpose of this study is to evaluate the effects of Low Level Laser Therapy on Atopic dermatitis symptoms. Methods 19 patients (7 men, 12 women) were involved in this study. The intensity of SCORAD index (Erythema/darkening, Edema/papulation, Oozing/crust, Excoriation, Lichenification/prurigo, Dryness) and the symptom of SCORAD index (Pruritus) in two regions were measured. One region was treated by LLLT, the other region was not treated. Then a comparative study of index score of two regions was analyzed. Results After treatment, there were statistically significant differences in Edema/papulation, Excoriation (p<0.001), Oozing/crust (p<0.01), and Pruritus(p<0.05) index. Conclusions Based on the results, we concluded that LLLT is an effective in the treatment of atopic dermatitis. Also, further controlled studies with clinical cases of appropriate treatment time, wavelength, and intervals are needed.

Case Report of an Ataxia Patient Treated by Oriental Medicine (조화운동불능 환자 한방 치험 1례)

  • We presented an ataxia patient who was difficult to diagnose and treated her by oriental medical treatment. We applied various methods of oriental medicine including herb medicine, acupunture, moxibustion and so on. After that, we checked the extent of dysarthria, pruritus, finger to nose and rapid alternative movement and measured the progress of general condition with Modified Barthel Index(MBI). After the oriental medical treatment was applied to her, ataxia, dysarthria, pruritus, finger to nose and rapid alternative movement were improved. The score of MBI was improved from 45 to 71, too. Even though there is no exact pattern identification about ataxia in oriental medicine, the oriental medicine can have an effect on the ataxia patient. In addition, more clinical study and observation should be needed.

The Case Report of Tinea Pedis with Id Reaction (이드반응이 동반된 족부백선 환자 치험 1례)

  • Objective : This study was carried out to find out the effect of Korean medical treatments on tinea pedis with id reaction. Method : We treated a 33-year-old man was afflicted with tinea pedis complicating id reaction with herbal medicine, acupuncture, ointment, wet-dressing and steam treatment. After treatments, we measured the status of scales, fissures and the degree of pruritus. We followed up symptom changes by taking pictures and compared before and after. Result & Conclusion : Three days after admission, erythema occurred at upper and lower limbs and the degree of pruritus increased by id reaction. As tinea pedis improved, id reaction decreased, too. Consequently, both sites of primary lesions of tinea pedis and secondary id reactions were significantly improved, and accompanied symptoms were resolved simultaneously.

  • Objectives : The purpose of this study is to report the oriental medical treatment on erythema multiforme caused by acute tonsillitis. Methods : Patient who has been ill Acute tonsillitis suffers from erythema, edema, pruritus, burning sensation of systemic skin. The patient was admitted to Dunsan oriental hospital for 7 days and was treated with oriental medicine(acupuncture, herbal-medicine, and vapor treatment). To evaluate the results of this treatment, the improvement of symptoms(erythema, pruritus, burning sensation) was identified by patient's subjective expression and in lesion-symptom improvement. Results : The symptoms caused by acute tonsillitis on total skin were improved. Conclusions : This study shows that the oriental medical treatment effects on the erythema multiforme caused by acute tonsillitis. Also, the public suggests that the correlation between the skin and tonsil.

Beneficial Effects of Natural Environment Therapy for Atopic Dermatitis

  • Objectives: Natural environment therapy is considered as an option for atopic dermatitis. This study was aimed to examine the beneficial effects of natural environment therapy among patients with atopic dermatitis who have moved in an atopy-free village. Methods: The questionnaire-based survey was conducted from parents of 20 children with atopic dermatitis (12 male and 8 female, median age 8 years, ranging from 6 to 13 years). The questionnaire consisted of 10 questions for the changes of two typical symptoms of pruritus and sleeping difficulty after moving into an atopy-free village. Results: 75.0% and 88.9% of patients have a family history (at least one parent and one of siblings respectively) of allergic disorders. The median staying period of patients was 27.6 months (ranging from 2 to 55 months). The score indicating pruritus was significantly improved from $6.2{\pm}2.8$ into $4.8{\pm}1.8$ (p<0.05), and sleeping difficulty was also improved from $5.7{\pm}2.9$ into $4.3{\pm}2.1$ (p<0.05) too. Conclusions: This study suggests that the natural environmental therapies could be a choice for patients with atopic dermatitis, which should be studied further for their scientific evidence and mechanisms.

A Case Report of Hypersensitivity Vasculitis Improved by Korean Medicine Treatment (과민성 혈관염의 한방치료 증례보고 1례)

  • Introduction: The aim of this study is to report the effect of Yukmijiwhang-tang-gamibang on the treatment of hypersensitivity vasculitis. Case presentation: A patient with hypersensitivity vasculitis was treated with herbal medicine (3 times a day) and dressing (once per day) for 16 days. The severity of hypersensitivity vasculitis was assessed using the numeral rating scale (NRS) and pruritus score. After treatment, pain and itching symptoms were reduced, the NRS score improved from 5 to 1, and the pruritus score improved from 2 to 0. No adverse reactions were noted and no relapse of hypersensitivity vasculitis occurred within 38 days after the end of treatment. This result shows that herbal medicine could be effective and safe for treating hypersensitivity vasculitis. Conclusion: According to the results, Korean medicine such as Yukmijiwhang-tang-gamibang can be effective and safe for the treatment of hypersensitivity vasculitis.

A Case of Nummular Eczema Treated by Gagamtongchung-san and Korean Medicine Therapy (가감통청산 및 한방 치료를 통한 화폐상 습진 치험 1례)

  • Objectives : The aim of this study is to report the effect of Gagamtongchung-san and Korean medicine on the treatment of nummular eczema. Methods : A 52-year-old male had suffered from nummular eczema in forearm, dorsum manus and fossa cubitalis. We treated him with herbal medicine, acupuncture and herbal external medicine. In order to evaluate the results of these treatments, we observed pruritus and heat sensation of the patients and took photographs of the lesions. Results : After treatment, the nummular eczema lesions of patient was remarkably disappeared, NRS of pruritus and heat sensation decreased from 10 to under 2 and DLQI score changed from 28 to 9. Conclusions : According to the result, Gagamtongchung-san and Korean medicine can be effective for the treatment of nummular eczema and the improvement of quality-of-life.
