Finite element analysis of stress distribution on supporting bone of posterior implant partial dentures by loading location (유한요소 분석을 이용한 하중 위치에 따른 구치부 임플란트 국소의치 지지골의 응력 분포 연구)

  • The purpose of this study is to evaluate the effect of three different oblique mechanical loading to occlusal surfaces of posterior implant partial dentures on the stress distributions in surrounding bone, using 3-dimensional finite element method. A 3-dimensional finite element model of a posterior implant partial dentures composed of three unit implants, simplified 3 gold alloy crown and supporting bone was developed according to the design of AVANA self tapping implant for this study. Three kinds of surface distributed oblique loads(300 N) are applied to following occlusal surfaces in the three crowns; 1) All occlusal surfaces in the three crown(load of 300 N was shared to three crown), 2) Occlusal surface of centered crown (load of 300 N was applied to a centered crown), 3) Occlusal surface of proximal crown(load of 300 N was applied to a distal proximal crown). In the results, 141 MPa of maximum von Mises stress was calculated at third loading condition and 98 MPa of minimum von Mises stress was calculated at first loading condition. From the results, location and type of occlusive loading conditions are important for the safety of supporting bone.

Anatomy of the diaphyseal nutrient foramen in the long bones of the pectoral limb of German Shepherds

  • This study investigated the anatomy of the nutrient foramen (NF) in German Shepherds by recording the number, site, position, and direction of penetration of the nutrient canal (NC) in the humerus, radius, and ulna of 50 individuals. The site index of the nutrient foramen (SI) was calculated as the ratio of the length to the NF site from the proximal end to the greatest length of the bone. The NF diameter was measured using different sized needles. Most humeri had only one NF on the caudal surface, particularly on the lateral supracondylar crest, or distal cranial surface. All radii had one NF, usually on the caudal surface, while most ulnae had one NF located on either the cranial or lateral surfaces. The SI and NF diameters were 58.0~59.5% and 0.73~0.78 mm in the humerus, 30.4~30.9% and 0.74~0.76 mm in the radius, and 29.3~29.8% and 0.67~0.68 mm in the ulna, respectively. With the exception of the relatively proximal NF of the radius, the direction of penetration followed Berard's rule. This study provides novel information on the location and diameter of the NF and direction of the NC in the long bones of the pectoral limb of German Shepherds.

Hemiarthroplasty for Osteosarcoma of Proximal Tibia (근위 경골의 골육종에 시행한 반관절성형술)

  • Purpose: The proximal tibial sarcoma patients, especially in their growing ages have problems of reconstruction. This study is to devise a methodology which can circumvent this limitations. Materials and Methods: Four cases of proximal tibial osteosarcoma underwent hemiarthroplasty. The mean age was 13 years (11~15) with a mean follow-up of 64 months (47~89). The procedure consists of ultrahigh molecular weight polyethylene (UHMWPE) liner as an substitute for the joint surface and this piece was fixed to the remaining tibial bone stock with Ender nail and bone cement. Results: Final functional score was 23.5 (78.3% of control) by MSTS criteria. All the cases showed stable joint without pain. Hemiarthroplasty related complications were absent. By saving the femoral physis, expected leg length discrepancy could be minimized by this procedure. Conclusions: Hemiarthroplasty of proximal tibia can be an option in pediatric sarcoma patients.

The Fine Structure of Human Fetal Nail Matrix (한국인 태아 조기질의 미세구조에 관한 연구)

  • The differentiation of nail matrix and fine structure of matrix cells were studied with light and electron microscope using specimens from nails of thumb finger in Korean fetuses 14 to 24 weeks old. Fetal nail matrix consisted of two horizontal layers, thicker ventral and thinner dorsal matrices, originating from invagination of epidermis in proximal nail field. Matrix being generally thicker in its distal region than the apex became gradually thickened with increase of the fetal age. Each matrix consisted of single layer of basal cells and multiple layers of squamous cells which are arranged close to and parallel to the central axis of the nail mairix. The process of keratinization of fetal nail matrix was noted to be occured concurrently in the ventral and dorsal matrices along the central axis of matrix toward distal and dorsal direction. Squamous cells became matured with accumulation of tonofilaments, increase of keratohyalin granules, discharge of membrane coating granules, and narrowing of intercellular spaces, thickening of plasma membrane and finally being transformed into horny cells of nail plate. Horny cells of nail plate filled with fibrous elements in the electron dense amorphous substance. These findings of keratinization process of fetal nail matrix appeared to be similar to those of keratinization in epidermis and inner root sheath of the hair. In the nail matrix, however, corresponding region to the keratogenous zone of growing hair follicle was not observed. Vacuolated squamous cells of nail matrix seen on light microscopy was considered to be artefactual product, but squamous cells with condensed small nuclei rarely found adjacent nail plate was considered to be one of the squamous cells with unknown function. Proximal end of nail plate was observed on dorsal surface of nail field distal to the proximal nail fold at 14 and 16 weeks old human embryos. Proximal prolongation of the proximal end of nail plate was occured with advancing fetal age and afterward 21 weeks nail plate invaded into nail matrix. Melanin granule containing cells and Merkel cells were present only on the basal layer of dorsal nail matirx.

  • Children and teenagers have a higher frequency of proximal surface caries in the posterior teeth than adults. For proximal restoration, class II amalgam or stainless steel crown has been widely used in the past, however composite resin restoration is getting ore popular due to it's superior cosmetic appearance. When applying composite resin on proximal area, various types of matrix bands can be utilized according to the operator's reference or skill. Such bands have several clinical effects including suitability for proximal margin, reduction of micro-leakage, moisture-control against saliva and ease finishing and polishing. In this case report, orthodontic bands were utilized instead of matrix bands as a remedy for proximal restorations in both primary and permanent teeth and their clinical advantages are as follows. 1. Orthodontic bands showed superior marginal adaptation compared to conventional matrix bands and moisture-control against saliva was excellent. 2. While applying composite resin, deformation of restoration material was estimated to be insignificant due to he rigidity of the orthodontic bands. 3. Natural tooth contour of the orthodontic bands facilitates to reproduce proximal tooth contour of the restoration. 4. In general, pediatric dentists are accustomed to applying orthodontic bands and this may allow pediatric dentists to make proximal composite restorations more efficiently than other dental specialists.

Fixation with Absorbable Suture Material in Akin Osteotomy (흡수성 봉합사를 이용한 무지외반증 Akin 절골술)

  • Purpose: The Akin osteotomy which is a closing wedge osteotomy of the proximal phalanx widely used for the correction of hallux valgus has several methods of fixation. we tried to report the effects of the fixation using an absorbable suture material during the Akin osteotomy for the hallux valgus. Materials and Methods: This study was based on 448 cases of 346 patients who were able for follow-up more than 12 months among the patients who had an Akin osteotomy together with the surgery of hallux valgus between March of 2006 and May of 2010. Absorbable suture material had been used in all cases. Radiologically displacement and union of osteotomy site were observed after the surgery, and clinically postoperative complication such as skin irritation, pain and satisfaction were investigated. Results: Radiologically all cases had showed complete union and no case had the loss of an correction due to loss of fixation. Also, any case had no skin irritation due to a knot. Three cases had a medial cortical breakage due to a strong knot, and the initial one case among them had additionally fixed the osteotomy site for four weeks using K-wire, and the remaining two cases had fixed a suture on an articular surface without any fixation of an additional wire. If a medial cortical bone was lost by carrying out an ostectomy due to proximal protrusion of proximal phalanx, three cases could show union through the fixation of suture on an articular surface. Conclusion: This study considers that the fixation of the osteotomy site using an absorbable suture material in an Akin osteotomy was effective method and the advantage of this procedure was unnecessity of the material removal and no skin irritation.

Influence of the amount of tooth surface preparation on the shear bond strength of zirconia cantilever single-retainer resin-bonded fixed partial denture

  • PURPOSE. Conventional resin-bonded fixed partial dentures (RBFPDs) are usually made with a two-retainer design. Unlike conventional RBFPDs, cantilever resin-bonded fixed partial dentures (Cantilever RBFPDs) are, for their part, made with a single-retainer design. The aim of this study was to compare the effect of tooth surface preparation on the bond strength of zirconia cantilever single-retainer RBFPDs. The objective is to evaluate the shear bond strength of these single-retainer RBFPDs bonded on 3 different amount of tooth surface preparation. MATERIALS AND METHODS. Thirty extracted bovine incisors were categorized to 3 groups (n=10), with different amounts of tooth surface preparations. Teeth were restored with single-retainer RBFPDs with different retainer surfaces: large retainer of $32mm^2$; medium retainer of $22mm^2$; no retainer and only a proximal connecting box of $12mm^2$. All RBFPDs were made of zirconia and were bonded using an adhesive system without adhesive capacity. Shear forces were applied to these restorations until debonding. RESULTS. Mean shear bond strength values for the groups I, II, and II were $2.39{\pm}0.53MPa$, $3.13{\pm}0.69MPa$, and $5.40{\pm}0.96MPa$, respectively. Statistical analyses were performed using a one-way ANOVA test with Bonferroni post-hoc test, at a significance level of 0.001. Failure modes were observed and showed a 100% adhesive fracture. CONCLUSION. It can be concluded that the preparation of large tooth surface preparation might be irrelevant. For zirconia single-retainer RBFPD, only the preparation of a proximal connecting box seems to be a reliable and minimally invasive approach. The differences are statistically significant.

Blood Flow Rate Estimation using Proximal Isovelocity Surface Area Technique Based on Region-Based Contour Scheme and Surface Subdivision Flow Model (영역기반 윤곽선 기법과 표면 분할 유동모델에 기반한 근위 등속 표면적 기법을 이용한 혈류량 추정)

  • The proximal isovelocity surface area (PISA) method is an effective way of measuring the regurgitant blood flow rate in the mitral valve. This method defines the modelling required to describe the geometry of the isotach of the PISA. In the normal PISA flow model, the flow rate is calculated assuming that the surface of the isotach is either hemispherical or non-hemispherical numerically. However, this paper evaluated the estimate flow rate using a direct surface subdivision flow model based on the height field after isotach extraction using a region-based scheme. To validate the proposed method, the various PISA flow models were compared using pusatile color Doppler images with flow rates ranging from $30\;cm^3/sec\;to\;60\;cm^3/sec$ flow rate. Whereas the hemispherical flow model had a mean value of $29\;cm^3/sec$ and underestimated the measured flow rate by 35%, the proposed model and non-hemispherical model produced a c;ame mean value of $45\;cm^3/sec$, moreover, both flow models produced a similar pulsatile flow rate.

  • The experiment was carried out to investigate the influence of light condition and polarity of explant on adventitious shoot formation from cotyledon of apple in vitro The treatment of light culture after 10days dark treatment showed effective result than other treatments on the rate of adventitious shoot formation on light intensity treatments and also the treatment of blue light treatment after 4days dark treatment showed more effective result than other treatments in light quality treatments. The polarity of explant influence on adventitious shoot formation. Adventitious multiple shoot formation occured at the proximal end of an excised cotyledon. In other words. shoot organogenesis occured at the proximal cut surfaces of both proximal and distal explants rather than at the distal cut surface of proximal explants.

Comparison of Diagnostic Validity between Laser Fluorescence Devices in Proximal Caries (레이저 형광 원리를 이용한 우식 진단 기기의 구치부 인접면 우식 진단 능력 비교)

  • The aim of this study was to evaluate the proximal caries detecting ability and identify the optimal cut-off values of two types of laser fluorescence (LF) devices; classic type (DD) and pen type (DDpen). The number of proximal surfaces participated in this study were 164 surfaces in primary dentition and 438 surfaces in permanent dentition. Each tooth surface was sequentially assessed by two types of LF devices, and bitewing radiograph. The radiographs were classified into 3 groups in primary dentition ($pR_0$, $pR_1$, $pR_2$), and 4 groups in permanent dentition ($PR_0$, $PR_1$, $PR_2$, $PR_3$) according to the depth of caries, and used as gold standard. In primary dentition, the area under the curve (AUC) values of DD were 0.851 and 0.890, and those of DDpen were 0.883 and 0.917, respectively in enamel caries and dentin caries. In permanent dentition, the AUC values of DD were 0.762 and 0.886, and those of DDpen were 0.828 and 0.958, respectively in enamel caries and dentin caries. When detecting proximal caries in posterior teeth with LF devices, DDpen is more useful than DD in both primary and permanent dentition. However, in primary dentition, DD can also be useful to detect proximal caries.
