North Korean Leaders' Personality Reflection on Provocation Patterns: Narcissism and Fear

  • Over the last 70 years, South Korea has gone through countless provocations from North Korea. Provocation is something that must be delicately calculated before the actual action, since it can be extended to a serious conflict. Among the possible factors, this paper focuses on the leader's characteristic, because 1) under North Korean dictatorship, the leader's thoughts are critical influence to policies and 2) personal characteristic, especially if it is the dictator's, hardly changes even when the external circumstances change. The purpose of this paper is in analyzing the correlations between North Korean leader's characteristics and provocation pattern. First, three generations of Kim Il-sung, Kim Jong-il, and Kim Jong-un's personal characteristics will be described. Next, particular provocation features in respective leader's period will follow. Lastly, how each leader's characteristic and concerns were reflected on the provocations will be analyzed.

Relationship of trunk muscle atrophy and provocation position in patients with chronic low back pain

  • Objective: The purpose of this study is to compare chronic low back pain patients' pain provocation position so as to identify the relevance with lumbar stabilizing muscles atrophy and pain provocation position. Design: Cross-sectional study. Methods: Fifty five chronic low back pain patients were participated in this study. Subjects were eligible for study participation if they were 35-55 years old and had experienced low back pain for more than 3 months. Subjects were questioned about pain and pain provocation test were done. And then they were inspected their cross sectional area (CSA) of lumbar muscles (erector spinae, iliopsoas, and multifidus) by using computed tomography. Analyze the relevance through the result data with painful area, aspect of pain and pain provocation position. Results: CSA of erector spinae showed significant decrease on ipsilateral extension position (p<0.05). Iliopsoas muscle showed significant decrease on contralateral position (p<0.05). Multifidus showed significant decrease on the position of contralateral extension and contralateral flexion (p<0.05). Conclusions: Based on the results of our study, it may be possible to evaluate muscle atrophy by assessing causing position.

Social Information Processing according to Sex and Types of Aggression of Children (아동의 성과 공격성 유형에 따른 사회정보처리과정 : 해석단계와 반응결정단계를 중심으로)

  • The purpose of this study was to explore differences in social information processing according to children's sex and types of aggression in response to instrumental and relational provocation factors. Two hundred and fifty-one 4, 5, and 6 graders were selected from an elementary school in Seoul. To evaluate their social information processing, the Intent Attributions and Feelings of Distress(Crick, 1995; Fitzgerald & Asher, 1987) and Response Decision Instrument(Crick & Werner, 1998) were revised and analyzed. A peer-nomination measure(Crick, 1995; Crick & Grotpeter, 1995) was used to select aggressive groups. Data were subjected to descriptive statistical analysis and multivariate [2(sex: M, F)${\times}$3(type of aggression: overt, relational, overt and relational aggression)] analysis of variance. Findings revealed that children's social information processing patterns were different according to sex and type of aggression. Also aggressive children responded differently in their social information processing according to instrumental and relational provocation factors. Implications of these findings for the role of gender, aggression type, and provocation type are discussed in order to better understanding of children's social information processing.

Drug eruption by antihistamine mistaken for chronic urticaria in a child

  • Although rare, antihistamines can cause adverse effects, including drug-induced eruptions or anaphylaxis. A 4-year-old child visited the pediatric department of a hospital for skin eruptions after administration of antihistamines, (e.g., ucerax [hydroxyzine] or leptizine [levocetirizine]), for cholinergic rashes; he did not have pruritus. Skin prick, intradermal, and drug provocation tests were performed to determine the relationship between the antihistamines and eruptions. Levocetirizine induced wheals in the skin prick test and a rash in the oral drug provocation test. In contrast, ketotifen induced no reaction in the skin prick test but showed a positive reaction in the oral provocation test. Our case report highlights that children can experience the same types of adverse reactions as seen in adults, and cross-reactivity between various antihistamines can occur.

  • Allergy is conditions when a hypersensitivity reaction happens with a certain element, called as an allergen, which is commonly not reactive to ordinary individuals. Allergic diseases involve various organs or systems in the body. The purpose of allergy tests is to make a diagnosis of allergic diseases and to identify the affecting allergens. In vivo tests, more relevant in clinical situation, include skin test, patch test and provocation test. In in vitro tests, there are specific IgE test, histamine releasing assay, and lymphocyte activation test, safer and more objective than in vivo tests. In the view point of clinical practice, skin test, provocation test, total IgE test and specific IgE test were reviewed in depth.

  • North Korea had recently conducted the fourth nuclear test and ICBM tests, dared nuclear provocation targeting the Korea and the international community. It is determined based on experiments of nuclear and missile in progress in North Korea that the technical level of nuclear force reached the completion stage of standardization, lightweight, and variation. It is expected to become reality that North Korea executes the nuclear provocation targeting the Korea and the international community in the near future. Nuclear bomb is an absolute weapon that the logic of counterattack after allowing the first strike of the other party cannot be applied due to its tremendous destructive power. Therefore, as the opponent to North Korea that it decided to hold the nuclear, the exercise of anticipatory self-defense in order to guarantee a minimum of right to life is not a choice, but the only essential correspondence concept. At the moment that the North Korean nuclear provocation is expected in the near future, it shall be provided with competence to strike the origin region of provocation by forming a national consensus of preemptive strike enforcement. Also, in preparation for the fifth nuclear test of North Korea, which is anticipated, the national competence must be mobilize to be able to ensure the 'Nuclear Option' from the international community.

The Change of Peripheral Eosinophil Count after Bronchial Provocation with Inhaled Histamine in Bronchial Asthmatics (기관지 천식 환자에서 히스타민 기관지유발검사후 말초혈액 호산구수의 변화)

  • Background: Recently, bronchial provocation of the airway of atopic asthmatic subjects with inhaled allergen has been shown to produce an initial peripheral blood eosinopenia followed by an eosinophilia occurring approximately 12 to 18 hrs after the challenge. However there are few studies about the change of peripheral eosinophil count (PEC) after bronchial provocation with nonspecific stimuli such as histamine or methacholine. Interestingly our preliminary study demonstrated a notable change of PEC during bronhial provocation with inhaled histamine in some asthmatic subjects. This study was designed to reevaluate our preliminary data and to further investigate the change of PEC during as well as after bronchial provocation with inhaled histamine in bronchial asthma tics. Methods: Sixteen asthmatic subjects participated in this study. Bronchial provocation with inhaled histamine was done between 9 AM and 12 MD. Blood samplings for PEC were done with 5 minutes intervals during the procedure, and repeated at 1 hour, 2 hours, 4 hours, 8 hours, 24 hours, and 48 hours after the procedure. Results: The results were as follows; 1) The patients were divided into two groups characterized by each pattern in the change of PEC during the procedure. A group (11 of sixten, group I) showed an increasing pattern of PEC and another group (5 of sixteen, group II) showed a decreasing pattern of PEC during the procedure. 2) Group I demonstrated a tendency to maintain continuously higher level of PEC than the baseline value until 48 hours after the procedure. 3) Group II demonstrated a tendency to maintain continuously lower level of PEC than the baseline value until 48 hours after the procedure. 4) There were no significant differences in their clinical parameters including baseline eosinophil count, baseline $FEV_1$, $PC_{20}$ of histamine, and serum IgE level between group I and group II. Conclusion: Our results suggest that the change of PEC produced by inhaled histamine in asthmatic subjects is much different from that produced by inhaled allergen, and that each patient may have their individual characteristics in the change of PEC in response to bronchial provocation with inhaled histamine. Alternatively these findings suggest that eosinophils may be partially involved in the early asthmatic reaction.

Usefulness of drug provocation tests in children with a history of adverse drug reaction

  • Purpose: There are very few reports of adverse drug reactions (ADR) and almost no study of drug provocation test (DPT) in Korean children. We aimed to assess the role of DPT in children with unpredictable ADRs, and compare the causative drugs and clinical characteristics between detailed history of ADRs and result of DPTs. Methods: We included 16 children who were experienced ADRs referred to pediatric allergy clinic at Ajou University Hospital (January 2006 to December 2009). With various suspected drugs, 71 DPTs were done in 16 patients using our own protocol, and skin tests to antibiotics were combined in ADRs to antibiotics in medical history. Results: There were 17 (23.9%) positive DPTs results out of 71 individual DPTs, and 11 patients (68.8%) from 16 patients were positive to at least one drug. Drugs causing positive reactions were acetaminophen in 5 (31%), Non-steroidal anti-inflammatory drugs in 4 (25%), penicillin in 3 (19%), cephalosporin in 2 (13%), and cotrimoxazole, macrolide and lactose in 1 each. Conclusion: DPT seems a safe and useful procedure to confirm causative drug and identify safely administering alternative drugs in children with ADR.

Biomechanics of Sacroiliac Joint Dysfunction and Clinical Disease (엉치엉덩관절 통증과 임상 질환에 대한 생체역학)

  • Pain originating from the sacroiliac joint(SIJ) has been associated with poor performance, yet specific diagnosis of sacroiliac dysfunction(SID) has been difficult to achieve. Clinical presentation of SID appears that pain and poor performance is responsive to local analgesia of periarticular structures with poorly defined pathology, and poor performance with bony pathological changes present as a result of chronic instability. Previous research indicates that physical examination cannot diagnose SIJ pathology. Earlier studies have not reported sensitivities and specificities of composites of provocation tests known to have acceptable inter-examiner reliability. Tests based on mechanics as manual provocation for SIJ pain have formed the basis of tests used to diagnose SIJ dysfunction. In this review summary, the purpose of this study was to describe the sacroiliac tests with a model of examination, diagnosis, and management of SID. Further research is warranted to determine whether SIJ tests is reliable means of evaluating innominate impairments.
