저자등은 하악 과두 골절에 있어 골절의 양상, 환자의 요구, 교합의 상태, 한자의 나이등을 고려하여 기능적 처지가 필요하다고 판단된 환자들에 있어 악간 고정기간 없이 activator를 이용한 기능적 치료를 한 결과, 빠른 악관절 기능의 회복을 관찰하고 정상적인 하악의 전, 측방 기능운동을 유도할 수 있었기에 문헌 고찰과 함께 보고하는 바이다.
PURPOSE. The aim of the present study was to compare the stress distributions on the dental implants, abutments, and bone caused by different overdenture attachment types under functional chewing forces. MATERIALS AND METHODS. The 3D finite element models of the mandible, dental implants, attachment types, and prostheses were prepared. In accordance with a conventional dental implant supported overdenture design, the dental implants were positioned at the bone level in the canine teeth region bilaterally. A total of eight models using eight different attachment systems were used in this study. All the models were loaded to simulate chewing forces generated during the centric relationship (450 N), lateral movement (400 N), protrusive movement (400 N), and also in the presence of a food mass unilaterally (200 N). Stress outputs were obtained as the maximum principal stress and the equivalent von-Mises stress. RESULTS. In all attachment types, higher stress values were observed in the abutments, dental implants, and bone in the magnet attachments in different loading conditions. The highest stress values were observed among the magnet systems in the components of the Titanmagnetics model in all loading conditions (stresses were 15.4, 17.7, and 33.1 MPa on abutment, dental implant, and bone, respectively). The lowest stress value was observed in the models of Zest and O-Ring attachments. CONCLUSION. The results of the present study implied that attachment types permitting rotation and tolerating various angles created lower stresses on the bone, dental implants, and abutments.
This study was accomplished to analyse and compare the occlusal contact patterns during eccentric mandibular movements in adult with normal occlusion. 50 subjects(male 27, female 23), who had natural occlusion and no symptom of temporomandibular disorder, were selected. Teeth contact patterns during mandibular eccentric movements were recorded and the distribution of tooth contacts in maximum intercuspation analysed by T-scan system. And then, tooth contact numbers recored by T-scan and silicone bite registration at centric occlusion were analysed and compared. The results obtained were as follows : 1. Antero-posteriorly, the qualitative center of occlusal contacts in centric occlusion were in the first molar areas, but there was a slight deviation in left-right directions. Thus, distribution of occlusal contacts were not bilaterally symmetric. 2. During the mandibular movements from centric occlusal position to right lateral and left lateral directions, the frequency that maxillary canine joined in lateral guidance was relatively high, but pure canine protected occlusion or pure group function occlusion had small frequency. 3. During mandibular protrusive movement, one or more maxillary central incisors frequently joined in protrusive guidance. 4. During mandibular eccentric movements, working and balancing side premature contact was observered in relatively high frequency. 5. In centric occlusal position, the numbers of occlusal contacts recorded on T-scan were relatively smaller than on silicone bite registration.
This study was performed to measure the changes of the mandibular movement and the masticatory muscular activities - anterior temporal and masseter muscle of both side - reflected by intentional increase of anterior guidance angie. For this study, 5 volunteers (3 males and 2 females with average age of 24.0) were selected. Each volunteer had Angle's classification I and did not have any missing tooth except third molar and any extensive restorations. Metallic guide plate was made at volunteer's working model fabricated by improved dental stone and cemented to the palatal surface of maxillary central incisor using resin cement(Panavia $21^{(R)}$) and then adjusted not to give any occlusal interferences at intercuspal position. The activity of masticatory muscles and the changes of mandibular movement were recorded by EMG and Sirognathograph in Biopak analysing system(Bioresearch Inc., Milwaukee, Wisconsin, USA). Measurement was done at before experiment, immediatley after placement, 1 week after placement, immediately after removal, and 1 week after removal. The results were as follows: 1. Moderate phonetic disturbance and mild headache were occured to 3 volunteers for 2 days after setting and 1 volunteer had positive reaction to percussion and slight midline diastema. But all of these clinical signs were diappeared 1 week after removal and the other volunteer did not have any special clinical sign. 2. In the EMG of the mandibular rest position, the mean value of anterior tempotal muscle was increased immediately after placement(p<0.01) and then decreased 1 week after placement(p<0.05) and increased 1 week after removal(p<0.05) but not recovered as before experiment. The mean value of masseter muscle was decreased during the experiment period. 3. In the EMG during mandibular protrusive movement, all muscular activity was decreased during the experiment period. Reduced activity was not recovered 1 week after removal(p<0.03). 4. During the habitual opening, anteroposterior movement of mandible was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not statistically significant(p>0.1). Vertical movement was not shown significant difference during the experiment period(p>0.1). Lateral movement was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not recovered as before experiment. The opening and closing velocity of mandible was shown minor changes but not statistically significant. 5. During the habitual opening, anteroposterior movement of mandible was decreased 1 week after placement(p<0.05) and then increased immediately after removal and recovered 1 week after removal as before experiment. Vertical movement was not shown significant changes. Lateral displacement of mandible was increased continuously and recovered 1 week after removal. Opening velocity was temporarily increased immediately after removal but recovered and closing velocity was not shown significant changes. 6. During the right side chewing, anteroposterior movement of mandible was increased immediately after removal but recovered and vertical movement was not shown statistically significant results. Lateral displacement and velocity of mandible were not shown significant results. 7. During the left side chewing, the changes of mandibular movement pattern were not shown statistically significant results.
For patients with bimaxillary protrusion, significant retraction and intrusion of the anterior teeth are sometimes essential to improve the facial profile. However, severe root resorption of the maxillary incisors occasionally occurs after treatment because of various factors. For instance, it has been reported that approximation or invasion of the incisive canal by the anterior tooth roots during retraction may cause apical root damage. Thus, determination of the position of the maxillary incisors is key for orthodontic diagnosis and treatment planning in such cases. Cone-beam computed tomography (CBCT) may be useful for simulating the post-treatment position of the maxillary incisors and surrounding structures in order to ensure safe teeth movement. Here, we present a case of Class II malocclusion with bimaxillary protrusion, wherein apical root damage due to treatment was minimized by pretreatment evaluation of the anatomical structures and simulation of the maxillary central incisor movement using CBCT. Considerable retraction and intrusion of the maxillary incisors, which resulted in a significant improvement in the facial profile and smile, were achieved without severe root resorption. Our findings suggest that CBCT-based diagnosis and treatment simulation may facilitate safe and dynamic orthodontic tooth movement, particularly in patients requiring maximum anterior tooth retraction.
Although pantograph has been used to investigate whether the determinents of the mandibular movement were possible contributing factors of TMJ click, there was the problems to understand the role of tooth morphology upon the occurrence of click because of using appliance without tooth contacts. There Were advantages to evaluate the effects of tooth morphology upon the mandibular movements, because intraoral tracing device(Functiograph$^{(R)}$) had been obtained maintaining occlusal contact between the upper and lower natural teeth during mandibular movement. The purpose of this study was to record the mandibular eccentric movement quantitatively performed in 20 adult control subjects and 20 adult subjects with TMJ click and to investigate the effects of occlusion upon the occurrence of TMJ click. The obtained results were as follows : 1. The average ICP-P distance was $3.07{\pm}0.73mm$ in subjects with TMJ click, $2.14{\pm}0.85mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.001). 2. The average ICP-P distance was $3.07{\pm}1.14mm$ in subjects with TMJ click, $2.61{\pm}0.96mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.05). 3. The average distance of right and left lateral movement was not statistically significant between subjects with TMJ click and control subjects. 4. The average lateral displacement from midline during RCP was $0.75{\pm}0.54mm$ subjects with TMJ click, $0.16{\pm}0.17mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.001). 5. The average lateral displacement from midline during protrusive movement was $0.88{\pm}0.54mm$ in subjects with TMJ click, $0.20{\pm}0.23mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.001). 6. The average angle of right and left lateral movement was $144.2{\pm}20.20^{\circ}$ in subjects with TMJ click, $138.15{\pm}20.09^{\circ}$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.05).
Although TMJ sound is common, its relationship to subjective and objective evaluation and the magnitude and the position of the sound during the mandibular movement is not fully understood. So, the purpose of this investigation was to provide further insight into the characteristics of the TMJ sounds. Twelve subjects (9 men and 3 women) with TMJ sounds were selected from students at Chosun University, School of Dentistry. Condylar movements and TMJ sounds of each subject were recorded and analyzed using a simplified condylar path recorder and a sound checker. Although specific conclusions were difficult to make from this study, the finding suggested the followings. 1. The position of TMJ sounds were visually observed and marked on the condylar tracings during maximum opening and closing, protrusive and retrusive and right and left lateral movements. 2. Information about the size, location and number of condylar deviations and the translation freedom of the condyle was recorded and analyzed objectively. 3. The most obvious characteristic of TMJ sounds was their variability. 4. TMJ sounds were categorized into one of four groups (soft click, hard click, soft crepitus, hard crepitus) by the quality and quantity of the duration and amplitude components.
This work was undertaken to evaluate the integumental response in lower face to hard tissue changes, and to grope the prediction equation for expected integumental profile changes. Cephalometric headplates of 25 persons consisted of 8 Angle's class 1 maxillary protrusive and 17 Angle's class II division 1 patients whose mean age was 15.2 years were traced, diagramatized, and statistically analyzed. The results were as follows; 1. Upper incisor and lips were retracted and convexity of integumental profile decreased concurrently with decrease of hard tissue procumbency, however soft tissue point A', B', and Pog' did not undergo significant changes after orthodontic treatment. 2. Remarkable increment of upper lip thickness and upper lip height was shown and this was related to upper incisor retraction. The ratio between the amount of upper incisor retraction and the increment o f upper lip thickness was approximately 1.16:1. 3. Moderate correlation of upper lip retraction to upper incisor retraction, and of lower lip retraction to lower incisor movement were arranged, and yet comparatively wide variability from subject to subject was shown. 4. It was possible to predict statistically for horizontal alteration of lip position and change of upper lip angulation ground in orthodontic treatment.
Purpose: This study was performed to investigate the prevalence, morphology, and calcification pattern of the elongated styloid process in the Mathura population and its relation to gender, age, and mandibular movements. Materials and Methods: The study analyzed digital panoramic radiographs of 2,706 adults. The elongated styloid process was classified with the radiographic appearance based on the morphology and calcification pattern. The limits of mandibular protrusion were evaluated for each subject. The data were analyzed by using a Student's t-test and chi-squared test with significance set at p=0.05. Results: Bilateral elongation having an "elongated" type styloid process with a "partially mineralized" pattern was the most frequent type of styloid process. No correlation was found between styloid process type and calcification pattern on the one hand and gender on the other, although elongated styloid was more prevalent in older and male populations (p<0.05). Further styloid process elongation showed no effect on mandibular protrusive movement (p>0.05). Conclusion: Dentists should recognize the existence of morphological variation in elongated styloid process or Eagle syndrome apparent on panoramic radiographs. We found higher prevalence of elongated styloid process in the population of the Mathura region when compared with other Indian populations. The calcification of the styloid process was more common in the older age group with no correlation to gender, mandibular movement and site. "Type I" with a "partially calcified" styloid process was observed more frequently in the population studied.
The objective of this study was to compare the condylar path and the anterior angle of articular fossa and to analyze the pattern of condylar path in edentulus patients. Nineteen male and female edentulous patients with normal masticatory system ranging in age 42 to 78, without present symptoms and any history of TMJ disturbance were selected for this study. On the computer analysis on the transcranial radiographs of the TMJ, the angle of slope of articular eminance and condylar path to the Frankfort Horizontal Plane and the height of glenoid fossa was measured respectively, and stuied their interrelationship comparatively. Obtained results were asfollows. 1. The angle of the slope of articular eminence averaged 37.28 degree. and there was no significant difference between the right and left side. 2. The condylar path angle averaged 29.05 degree and there was no significant difference between the right and left side. 3. The height of the glenoid fossa averaged 8.11 mm and there was no significant difference between the right and left side. 4. The sequence of the frequence of condylar movement patterns were concavex curve(39.5% ), 'S' shape curve(34.2%), reverse 'S' shape(15.8%) and convex curve(10.5%). 5. The horizontal distance of the point of the changed curve of the condylar path averaged 2.91 mm. 6. The height of glenoid fossa was highly correlated to the slope of articular eminence and relatively highly correlated to tile condylar path and the condylar path was closely correlated to the slop of articular eminence.
