소아 단백뇨 검사에 있어서 요비중의 유용성 (Urine Specific Gravity as a Useful Tool for Screening Proteinuria in Children)

  • 목 적 : 24시간 소변내 단백량을 예측하는데 있어 spot urine의 단백은 소변량에 따른 보정이 필요하여 spot urine의 protein/creatinine(P/Cr)비를 이용하여왔다. 하지만 크레아티닌 측정에 많은 시간과 비용이 소모되는 단점이 있고, 비중(specific gravity; SG)을 이용하여 creatinine(estimated creatinine; Cr-est)을 산출할 수 있다는 보고가 있어, 24시간 소변내 단백량을 예측하는데 P/Cr-est비의 유용성을 알아보고자 본 연구를 시행하였다. 방 법 : 1998년 3월부터 6월까지 고려대학교 의료원 구로병원 소아과에 내원한 환아 147명을 대상으로 spot urine에서 단백, creatinine과 비중을 측정하여 creatinine과 비중의 상관관계 및 그 관계식을 구하고 24시간 소변내 단백량과 비교하였다. 결 과 : 1) spot urine의 creatinine과 비중은 통계적으로 의의 있는 상관관계를 보였다(R=0.407, P<0.001, Cr=SG. x 4485.82-4482.87). 2) 24시간 소변내 단백량은 P/Cr비와 통계적으로 의의 있는 상관관계를 보였다(R=0.771, P<0.001, 24시간뇨중 단백=0.338 x (P/Cr) +667.885). 3) 24시간 소변내 단백 량은 P/Cr-est 비와 통계적으로 의의 있는 상관관계를 보였다. (R=0.723, P<0.001, 24시간뇨중 단백=0.354 x (P/Cr-est) +726.044) 결 론 : 소아단백뇨를 예측하는데 있어 spot urine의 비중으로 산출한 creatinine치를 이용하여 얻어진 P/Cr-est비가 선별검사로서 유용하게 사용될 수 있으리라 생각된다.

  • Protein malnutrition of children is one of the most serious nutritional deficiencies in developing country. Urea nitrogen excretion in ureotelic animals is the function most sensitive to dietary protein. The 24 hours excretion of creatinine in the urine of a given subject is remarkably constant from day to day. The creatinine excretion of different individuals of the same age and sex is also quite constant. Low ratios of urinary urea to creatinine are found children low protein intake. The foregiving world-wide investigations indicate that the urea nitrogen/creatinine ratios seems to be a good biochemical indicator to distinguish among group with different levels of protein intake. The purpose of this study is to evluate an indicator of protein intake on the elementary school children ranged from 6 to 8 years of age living in rural and urban areas. Each child measured for height and weight of body. weight measured by means of a plate from scale and height by a vertical measuring rod. Biochemical test were taken from a finger-tip and urine. Hemoglobin level in the blood was measured by cyanomethemoglobin method. From the urine samples, urea nitrogen and urea creatinie were determined by Folin-Wu method and: calculate the ratio. The following result were obtained: 1) Mean of the body weight and height in urban children(Seoul) was higher and heavier than rural children(Kyunggi, Kangwon). And 12% of boys, 18% of girls in Kyunggi and 25% of boys, 22% of girls in Kangwon area weight less than 80% of Korean Physical Standard weight level. 2) The mean hemoglobin values of boys and girls in Seoul are children were 13. 3g/100ml, 13.1g/100ml and the mean of hemoglobin values in Kyunggi 12.9g/100ml of boys, 12.4g/100ml of girls, and 12.4g/100ml of boys, 12.9g/100ml of girls in Kangwon children. It is found that 22% to 24% children inrural area (Kyunggi, Kangwon) had hemoglobin level less than 12g/100ml which means anemia. 3) The mean of hematocrit level of Seoul, boys and girls children were 33.5%, 34.1% and 33.4%, 33.1%, in Kyunggi area and 33.1%, 32.9% in Kangwon area. 4) Urea nitrogen/creatinine ratios in Seoul children were 9. 0, 10. 0 of boys and girls, the ratio were 8.2, 8.0 in Kyunggi boys and girls children, and 7.5 and 7.4 in Kangwon boys, girls children. Low-income rural and upper-income urban background large differences between two groups in the urea nitrogen/creatinine ratio(Seoul: Kangwon in male, female children. p<0.05, p<0.001). The urea nitrogen/creatinine ratio definetly seems to be a good indicator of the quantity of the protein intake. However, whether or not it is an indicator of the quality of the ingested protein ramains to be seen.

소아 단백뇨 검사에 있어서 단회뇨 단백 크레아티닌 비에 영향을 미치는 요인 (The Factors Affecting Accurate Quantitaion of Proteinuria Using Sp ot Urine Protein/Creatinine Ratio in Children)

  • 목 적 : 단회뇨를 이용한 단백/크레아티닌 농도 비(P/C ratio)가 24시간 요단백량과 밀접한 상관관계가 있다는 연구결과가 보고되고 있다. 이에 저자들은 소아를 대상으로 하여 24시간 요단백량과 단회뇨의 P/C ratio 사이의 상관관계를 요단백량과 요크레아티닌 배설량에 따라 분석하고 이런 상관관계에 영향을 미치는 요인들에 대하여 알아보고자 하였다.방 법 : 2003년 9월부터 2007년 12월까지 부산 백병원 소아청소년과 신장클리닉에 내원한 환아 210명을 대상으로 24시간 채뇨를 실시하여 단백량 과 크레아티닌 양, 사구체 여과율을 측정하였고, 24시간 채뇨 직후의 단회뇨를 이용하여 P/C ratio 를 측정하였다. 결 과 : 24시간 요단백량과 단회뇨의 P/C ratio는 전체 환자를 대상으로 하였을 때 0.840의 상관계수를 가지는 유의한 양의 상관관계를 보였으며, 24시간 요단백량에 따라 분류된 군과 크레아티닌 배설량에 따라 분류한 각 군에서 모두 유의한 상관관계를 보였다. 24시간 요단백량과 단회뇨의 P/C ratio 사이의 오차에 관여할 수 있는 일일 요단백량, 사구체 여과율, 크레아티닌 배설량, 연령, 성별 등에 대해 다중회귀분석을 실시하였고 요 크레아티닌 배설량만이 통계적으로 유의한 인자로 분석 되었고, 나머지 일일 단백량, 사구체 여과율, 연령, 성별들은 유의하지 않았다. 결 론 : 소아에서 광범위한 조사군을 대상으로 하여 연령이나 성별에 따른 크레아티닌 배설량을 고려하여 P/C ratio의 cutoff치를 설정한다면 단회뇨의 P/C ratio는 24시간 요단백량을 대치할 수 있는 방법으로 사용될 수 있을 것으로 생각한다.

EFFECT OF LEVEL OF FEED INTAKE ON THE EXCRETION OF PURINE DERIVATIVES AND PURINE DERIVATIVES TO CREATININE RATIO IN THE URINE OF SHEEP

  • Urinary purine derivatives and creatinine excretion was measured in a total of 4 white Alpine sheep. They were given diets 718 to 1060 g/kg dry matter (DM) of roughage. The crude protein content of this diets was on average $93.87{\pm}5.57g$ in kg DM. Purine derivatives-N excretion increased linearly with incremental DM intake and was significantly correlated (n = 16) with amounts of digestible organic matter (DOM) intake: allantoin-N (mg) = 1.205 (${\pm}0.070$) $\times$ DOM (g) - 136.709 (${\pm}37.399$), r = 0.9770, RSD = 22.97; uricacid-N (mg) = 0.131 (${\pm}0.041$) $\times$ DOM (g) + 11.380 (${\pm}21.881$), r = 0.6306, RSD = 13.44; Hypoxanthine-N (mg) = 0.049 (${\pm}0.014$) $\times$ DOM (g) - 28.640 (${\pm}7.708$), r = 0.6544, RSD = 4.73; total purine derivatives-N (mg) = 1.385 (${\pm}0.083$) $\times$ DOM (g) - 90.261 (${\pm}44.552$), r = 0.9706, RSD = 27.47. Microbial protein synthesis per kg DOM was estimated of 113 g. The urinary creatinine-N excretion was on average 9.10 mg/kg live weight (LW) with a standard error of 0.12 mg creatinine-N per kg LW. The excretion of creatinine excreton was not related to feed intake. Daily creatinine excretion (mg/d) was calculated from individual LW measurements and the average creatinine excretion (mg/kg LW). It was possible to predict the daily urinary purine derivatives excretion (r = 0.9720 for allantoin, r = 0.9886 for total purine derivatives) from the ratio of purine derivatives (mg/100 ml) and creatinine (mg/100 ml) in the urine and the daily creatinine excretion.

실험적으로 유발한 개 신증후군의 진행 및 치유과정에서의 임상병리학적 변화 (Description of Clinicopathologic Changes during the Development and Clinical Resolution of Experimentally induced Canine Nephrotic Syndrome)

  • 이 연구의 목적은 실험적으로 유발한 면역매개성 사구체 질환이 신증후군으로 진행하는 과정과 치유과정에서 단백뇨 배설에 근거한 조기진단 및 치유과정의 감시에 대한 평가를 하는 것이다. 두당 endotoxin 1 $\mu$g과 native bovine serum albumin 5mg 을 9두의 건강한 개에 정맥주사하여 감작시키고, 그 후 1주일부터 양이온화한 bovine serum albumin 120mg을 주 5회 반복 정맥주사한 결과 5두에서 면역매개성 사구체신염과 신증후군이 발생하였다. 사구체신염이 발생한 실험동물에서는 요단백질/ 크레아티닌 비의 증가 (>1.0), 저알부민혈증 (< g/이), BUN 증가 (>40 mg/dl), 부종 등이 나타났다. 이것은 임상병리학적으로 평가 가능한 면역매개성 사구체시염 및 신증후군 모델견이 제작되었음을 제시하는 것이었다. 사구체신염의 유발과 신증후군의 진행과정에 요단백질/크레아티닌 비의 증가가 저알부민혈증, BUN 증가, 고콜레스테롤혈증보다 1-4주 먼저 출현하였다. 사구체신염에 의해 신증후군이 유발된 실험동물 모두에서 prednisolone 을 2.2 mg/kg씩 1일 2회 경구투여한 결과 혈중 알부민 농도의 증가와 콜레스테롤 농도의 감소가 먼저 일어나고, 그 후 1-4 주에 요단백질/크레아티닌 비가 감소하기 시작하였다. 종합적으로 볼때, 면역매개성 사구체신염성 신증후군의 조기진단에는 요 단백질/크레아티닌 비가 유리한 지표로, 그리고 초기 치유과정의 감시에는 혈청 알부민, 콜레스테롤이 더 유리한 임상 지표로 이용될 수 있음을 확인하였다.

  • Proteinuria is an early hallmark of kidney disease and a major risk factor for systemic cardiovascular diseases. There are several methods to measure proteinuria, such as the urine dipstick test, 24-hour urinary protein excretion method, and spot urine for the protein-to-creatinine ratio. The urine dipstick test is simple but inaccurate. The 24-hour urinary protein excretion method is the gold standard; however, it is cumbersome, especially in children. Spot urine for the protein-to-creatinine ratio is simple and accurate, but has limitations. Specific urinary protein such as albumin can be measured instead of the total protein content. Tests should be avoided in situations that cause transient proteinuria or false-positive results. It should be performed correctly, and its limitations should be recognized and interpreted accurately.

조기 이유한 흰쥐에서 유단백질의 섭취수준과 조성비가 기관성장과 단백질대사에 미치는 영향 (Effects of Milk Protein levels and Casein/Whey Ratios on Organ Growth and Protein Metabolism in Early Weaned Rats)

  • This study was conducted to investigate the effects of protein levels and casein/whey ratios on organ growth and protein metabolism in early weaned rats. Premature rats weaned by the 17th day were fed six semipurified synthetic, isocaloric and gel diets that contained three levels (low, medium and high) and two different combinations(casein/whey ; 80 : 20 or 20 : 80) of milk protein for 8 days. On the 25th day postpartum, frest weigth and DNA, RNA and milk protein contents in brain, liver, kidney and muscle were determined to ascertain organ and cellular growth. Futher, with a view to ascertain protein metabolism and renal functions, serum total protein, $\alpha$-amino N, urea N, and creatinine and creatinine and urinary urea N, creatinine and hydroxproline were determined. Total DNA contents of brain, liver and kidney, which may represent as an index of cell numbers in those organs were significantly decreased in the rats fed diets containing low level protein regardless of casein/whey ratio. However, as fat as the rats fed high protein diets were concerned, their fresh weight, protein contents and GFR of kidney were significantly increased. Furthermore, nitrogen components, $\alpha$-amino N, urea N and creatinie in serum and urine were also increassed. Another observation was that high casein/whey ratio significantly facilitated accumulation of porteins in muscle and kidney and urinary hydorxyproline excretion, not affecting the DNA content of those organs. This study showed that low(8%) or high(32%) contents of protein had less desirable effects either on protein metabolism or on organ cellular growth in prematurely weaned rats, whereas there were no effects on general growth and bone strength.

Protein Status of Indigenous Nguni and Crossbred Cattle in the Semi-arid Communal Rangelands in South Africa

  • The objective of the current study was to determine factors influencing concentrations of protein-related blood metabolites in indigenous Nguni and crossbred cattle in the semi-arid communal rangelands in South Africa. The body condition scores (BCS), packed cell volume (PCV) and serum concentrations of protein-related metabolites were determined seasonally in 100 cattle raised on communal rangelands from August 2007 to May 2008. Nguni cattle had lower (p<0.05) albumin-globulin ratio, albumin, urea and creatinine, and higher (p<0.05) globulin concentrations than the local crossbreds. Local crossbreds had higher (p<0.05) alanine aminotransferase and alkaline phosphatase concentrations and lower (p<0.05) aspartate aminotransferase concentrations in the postrainy season than Nguni cattle. The creatinine concentrations of Nguni and crossbred cattle were lowest in the sour rangeland during the hot-wet season. The albumin concentrations of Nguni and crossbred cattle were higher (p<0.05) whilst PCV, albumin-globulin ratio and creatine kinase concentration were lower (p<0.05) in the sour rangeland than in the sweet rangeland. Total protein, albumin, globulin, aspartate aminotransferase and creatine kinase concentrations of Nguni and crossbred cattle were lower (p<0.05) in the hot-wet and late cool-dry seasons than in other seasons across rangeland types. Urea concentrations in both breeds were highest in the sweet rangeland in the hot-dry season compared to other seasons. It was concluded that Nguni cattle had lower concentrations of protein metabolites than local crossbreds and protein deficiencies were most prominent in the sweet rangeland during the cool-dry seasons.

Grazing system and floor type effects on blood biochemistry, growth and carcass characteristics of Nguni goats

  • Objective: Purpose was to determine the effects of grazing system and floor type on concentrations of blood metabolites, activity of creatine kinase, body weight and carcass characteristics of castrated Nguni goats. Methods: Forty eight, 7 month old goats were randomly allocated to herding and tethering treatments from 0800 to 1300 hours and accommodated on slatted and earth floors daily. Blood samples were collected by jugular venipuncture every fifteenth day for metabolite analysis. Slaughter was done at a commercial abattoir following 5 months of monitoring. Results: Tethered goats had significantly higher concentrations of urea (5.19 mmol/L) (p<0.001), creatinine ($55.87{\mu}mol/L$) (p<0.05), total protein (64.60 g/L) (p<0.01), and globulin (49.79 g/L) (p<0.001), whereas herded goats had higher glucose (3.38 mmol/L) (p<0.001), albumin (15.33 g/L) (p<0.05), albumin/globulin ratio (0.34) (p<0.01), and body weight (24.87 kg) (p<0.001). Slatted floors caused higher (p<0.01) albumin at 15.37 g/L. The interaction of grazing system and floor type affected creatinine, total protein, globulin at (p<0.01) and albumen/globulin ratio at (p<0.01). The least creatinine concentration and albumin/globulin ratio was in herded and tethered goats that were accommodated on earth floors, respectively. The highest total protein and globulin concentrations were in serum of tethered goats that were accommodated on earth floors. The highest (p<0.05) dressing percentage (45.26%) was in herded goats accommodated on slatted floors. Conclusion: Herding of goats lowered globulin concentration, improved estimated feed intake, blood glucose and albumin concentrations, albumin globulin ratio, increased body weights and weight related carcass characteristics. Floor type had very little effects on metabolites where earth floors only reduced albumin concentration. Tethering and housing goats on earth floors resulted in double stress that increased chronic infections.

꽃사슴의 녹용 성장기간 중 혈액성분의 변화에 관한 연구 (A Study on the Change of Blood Constituents During Growth Period of Velvet Antler in Korean Spotted Deer)

  • This study was carried out to evaluate the change of blood constituents during the growth of antlers in Korean spotted deer (Cervus nippon). Samples of blood, obtained from the jugular vein of twenty five deer (3 to 6 year-old males) were taken in 10 days interval from casting to cutting of antler and were analyzed the blood parameters. Total-protein concentration in blood serum showed significantly differences during growth period of velvet antler (p<0.01), and it had similar tendency for total protein and albumin concentration, but albumin concentration was about 40-45% of total protein concentration. Urea and creatinine concentration was tended to inverse proportion, and it was almost 30:1 for ratio of urea and creatinine concentration. The concentration of total protein, creatinine, and uric acid concentration in blood during growth period of velvet antler was similarly increased and decreased but urea concentration was opposed. The concentration of total-bilirubin and direct-bilirubin at casting was higher than those at other period(P<0.05).
