Association of Thymidylate Synthase 5'-UTR 28bp Tandem Repeat and Serine Hydroxymethyltransfarase C1420T Polymorphisms with Susceptibility to Acute Leukemia

  • Background: The current study was aimed to elucidate the association of thymidylate synthase (TYMS) 5'-UTR 28bp tandem repeat and cytosolic serine hydroxymethyltransferase (cSHMT) C1420T polymorphisms with acute leukemia in South Indian subjects. A total of 812 subjects [523 healthy controls, 148 acute lymphoblastic leukemia (ALL) cases and 141 acute myeloid leukemia (AML) cases] were screened for TYMS 5'-UTR 28bp tandem repeat and cSHMT C1420T using PCR-AFLP and PCR-with confronting two-pair primers (CTPP) approaches. TYMS 5'-UTR 2R allele frequencies of controls, ALL and AML cases were 35.3%, 28.0% and 30.1% respectively. This polymorphism conferred protection against ALL (OR: 0.71, 95%CI: 0.53-0.96) while showing no statistically significant association with AML (OR: 0.79, 95%CI: 0.58, 1.07). The cSHMT variant allele (T-) frequencies of ALL and AML cases (6.42% and 5.68% respectively) were significantly lower compared to controls (58.3%). This polymorphism conferred protection against ALL (OR: 0.049, 95%CI: 0.029-0.081) and AML (OR: 0.043, 95%CI: 0.025-0.074). The TYMS 5'-UTR 2R2R genotype was associated with a lower total leukocyte count, smaller percentage of blasts, and more adequate platelet count compared to 2R3R and 3R3R genotypes in ALL cases. No such genotype-dependent differences were observed in AML cases. ALL cases carrying the cSHMT C1420T polymorphism showed higher disease free survival compared to those with the wild genotype. To conclude, the TYMS 5'-UTR 28bp tandem repeat reduces risk for ALL while cSHMT C1420T reduces risk for both ALL and AML. Both also influence disease progression in ALL.

Evaluating Picture Quality of Image Plates in Digital CR Systems (디지털 CR시스템에서 Image plate의 화질 평가)

  • Lab effectively supplemented the effects of outside radiation on image plates in the process of image acquisition of CR (computed radiography) systems and conducted for effective utilization in the case of clinical application. For this, Lab classified the storage places and time periods of image plates and compared and analyzed the differences between small dark spots. Lab also assessed the concentration distribution within the boundaries of images. Lab compared and measured the number of dark spots in a light room and a dark room depending on the storage places of image plates and found that dark spots slightly increased in an image plate when stored in a light room on the first and second days. Dark spots increased in proportion to the length of time stored. In the case of the image plate stored in a dark room, the number of dark spots remarkably decreased. With regard to picture quality as related to the location of image plates, the damage to picture quality could be reduced by locating regions of interest in the center. With regard to differences in sharpness following changes in the thickness of subjects, fewer scatter rays occurred and sharpness improved by reducing the thickness of subjects as much as possible. To get medical images of excellent quality, image plates should be managed effectively and it is desirable to keep images plates in dark iron plate boxes and not to expose them to outside radiation for a long time.

An Effect of Visual Acuity Protection by Natural Pigment (Anthocyanine) Extracted from Fermented Purple Sweet Potato (발효 자색고구마 추출 천연색소(anthocyanine)에 의한 시력보호 효과)

  • Purpose: The purpose of this study was to research any effect on vision protecting or decreasing VDT syndrome of extracted anthocyanine from fermented purple sweet potato and blueberry. Methods: Subjects were aged 19-20 years old who do not have ophthalmic and systemic diseases and over -N4.00 D of refraction error. 40 mg of extracted anthocyanine from fermented purple sweet potato, from blueberry, and control group, placebo were dosed at separate try. After 2 hours later, subjects were directed perform visual display terminal (VDT) work for 2 hours. Objective refractive error was measured before dosing anthocyanine and after VDT work for 2 hours. Degree of head ache, eye pain and strain and subjective symptoms of neck, shoulder and waist was also examined through interviews by dividing its degree into severe, moderate, slight or none. Results: After 2 hours VDT work, vision protection effect in terms of refractive error for dominant eye was decreased by $0.031{\pm}0.21$ D in the group of extracted anthocyanine from fermented purple sweet potato, $0.006{\pm}0.32$ D in the group of extracted anthocyanine from blueberry. However, there was significantly myopic progression in the placebo group by $0.144{\pm}0.28$ D (t=2.27, p=0.03). Conclusions: It is considered that extracted anthocyanine from fermented purple sweet potato inhibits increase of refraction anomalies of dominant eye rather than non-dominant eye after VDT work.

  • Until today, the personal information of subjects has been centralized in many companies or institutions. However, in recent days, the paradigm has gradually changed in the direction that subjects control their personal information and persue their self-sovereignty. Globally, individual data sovereignty is strengthened by the European Union's General Data Protection Regulation(GDPR) and the US California Consumer Privacy Act(CCPA). In Korea, a few alliances consist of various companies are creating technology research and service application cases for decentralized ID service model. In this paper, the current decentralized ID service model and its limitations are studied, and a improved decentralized ID service model that can solve them is proposed. The proposed model has a function of securely storing decentralized ID to the third party and a linkage function that can be interoperated even if different decentralized ID services are generated. In addition, a more secure and convenient model by identifying the security threats of the proposed model and deriving the security requirements, is proposed. It is expected that the decentralized ID technology will be applied not only to the proof of people but also to the device ID authentication management of the IoT in the future.

Comparison of Recognition and Fit Factors according to Education Actual Condition and Employment Type of Small and Medium Enterprises (중소규모 사업장의 교육 환경과 고용형태에 따른 호흡보호구 인식도 및 밀착계수 비교)

  • There was a difference in recognition of respirators according to the educational performance environment. they were showed higher recognition of respirators of group by internal and external mix trainer, less than 6 months, over 1hour, more than 5 times, variety of education. To identify the relationship between types of job classification(typical and atypical)and the levels of recognition of respirators, a total of 153 workers in a business workplace. mainly, typical workers showed higher recognition of respirators than atypical workers. Training of correct wearing showed high demands both typical and atypical workers. Descriptive statistics(SAS ver 9.2)was performed. the results of recognition of respirators were analyzed the mean and standard deviation by t-test, and anova, fit factor is used geometric means(geometric standard deviation), paired t-test, Wilcoxon analysis(P=0.05). Particulate filtering facepiece respirators (PFFR) is one of the most widely used items of personal protective equipments, and a tight fit of the respirators on the wearers is critical for the protection effectiveness. In order to effectively protect the workers through the respirators, it is important to find and evaluate the ways that can be readily applicable at the workplace to improve the fit of the respirators. This study was designed to evaluate effects of mask style (cup or foldable type) and donning training on fit factors (FF) of the respirators, since these are available at various workplace, especially at small business workplace. A total of 40 study subjects, comprised of employment type workers in metalworking industries, were enrolled in this study. The FF were quantitatively measured before and after training related to the proper donning and use of cup or foldable-type respirators. The pass/fail criterion of FF was set at 100. After the donning training for the cup-type mask, fit test were increased by 769%. but foldable-type mask was also increased after the donning training, the GM of FF for the foldable-type mask and it's increase rate were smaller as compared to the cup-type mask. Furthermore, the differences of the increase rates of the GM of FF in employment type of the subjects were not significantly for the foldable-type mask. These results imply that the raining on the donning and use of PFFR can enhance the protection effectiveness of cup or foldable-type mask, and that the training effects for the foldable-type mask is less significant than that for the cup-type mask. Therefore, it is recommended that the donning training and fit tests should be conducted before the use of the PFFR, and listening to workers opinion regularly.

The Overview of the Public Opinion Survey and Emerging Ethical Challenges in the Healthcare Big Data Research (보건의료빅데이터 연구에 대한 대중의 인식도 조사 및 윤리적 고찰)

  • Purpose: The traditional ethical study only suggests a blurred insight on the research using medical big data, especially in this rapid-changing and demanding environment which is called "4th Industry Revolution." Current institutional/ethical issues in big data research need to approach with the thoughtful insight of past ethical study reflecting the understanding of present conditions of this study. This study aims to examine the ethical issues that are emerging in recent health care big data research. So, this study aims to survey the public perceptions on of health care big data as part of the process of public discourse and the acceptance of the utility and provision of big data research as a subject of health care information. In addition, the emerging ethical challenges and how to comply with ethical principles in accordance with principles of the Belmont report will be discussed. Methods: Survey was conducted from June 3th August to 6th September 2020. The online survey was conducted through voluntary participation through Internet users. A total of 319 people who completed the survey (±5.49%P [95% confidence level] were analyzed. Results: In the area of the public's perspective, the survey showed that the medical information is useful for new medical development, but it is also necessary to obtain consents from subjects in order to use that medical information for various research purposes. In addition, many people were more concerned about the possibility of re-identifying personal information in medical big data. Therefore, they mentioned the necessity of transparency and privacy protection in the use of medical information. Conclusion: Big data on medical care is a core resource for the development of medicine directly related to human life, and it is necessary to open up medical data in order to realize the public good. But the ethical principles should not be overlooked. The right to self-determination must be guaranteed by means of clear, diverse consent or withdrawal of subjects, and processed in a lawful, fair and transparent manner in the processing of personal information. In addition, scientific and ethical validity of medical big data research is indispensable. Such ethical healthcare data is the only key that will lead to innovation in the future.

The Effect of Ego-resilience, Stress Coping Styles, Teaching, Effectiveness, and Family Support on Satisfaction of Clinical Practice in Nursing Students by AMOS Structural Equation Model (간호대학생의 자아탄력성, 스트레스 대처방식, 교수효율성, 가족지지가 임상실습만족도에 미치는 효과: 구조방정식 모형구축)

  • Purpose: This study aimedto develop a structural equation model on the relationship among ego-resilience, and teaching effectiveness on clinical education, stress coping style, perceived family support, and satisfaction of clinical practice in nursing students in order to increase the satisfaction of clinical practice. Methods: The subjects were 399 undergraduate nursing students enrolled in 7 universities and participating in 10 clinical practice setting areas in 5 cities in South Korea. The research was conducted from December 5th, 2009 to February 20th, 2010. The structural equation model was used to perform the analysis with the statistics program of SPSS win 17.0 and AMOS 5.0. Results: Teaching effectiveness and family support showed a direct effect on satisfaction of clinical practice, while ego-resilience and stress coping styles showed an indirect effect. The biggest total effect on satisfaction of clinical practice was teaching effectiveness followed by family support, ego-resilience, and stress coping styles respectively, which accounted for 50.9%. Conclusion: Based on outcomes of this study, the proposed model allows better understanding of the satisfaction of clinical practice. This result implies that strategies or intervention programs enhancing extrinsic protection factors, motivation factors, and intrinsic protection factors should be considered. In addition, a structural support system for increasing the satisfaction level of clinical practice in nursing students should also be considered.

  • Purpose. The purpose of this study was conducted to provide the hospital infection control and personal protection, protection of the technician based on data by examining the relationship of hepatitis B knowledge of AIDS knowledge and attitudes of dental hygiene students and the purpose of the present study. Methods. The research subjects were 390 dental hygiene students in limited and data were collected through structured questionnaires. The collected data is analyzed by spss version 14.0 a statistical program for the frequency and one-way ANOVA, multiple linear regression analysis. Results. The received training students of AIDS knowledge and attitudes were higher, hepatitis B knowledge scores and attitudes about AIDS was a statistically significant difference (p <0.001), The experience clinical practice students was AIDS knowledge and attitudes were hepatitis B knowledge high score and difference was statistically significant(p <0.05), The knowledge AIDS and hepatitis B, higher score showed the attitudes significant increase was found to be a static relationship between two variables by both statistically (p <0.001). Conclusion. In order to provided with a thorough prevention program for infectious disease, strengthen school health education and clinical training to help achieve a mutually constructive relationship with the patient through proper education about infectious diseases.

A study on personal protection equipment for Infection control at dental offices (치과에서의 감염방지를 위한 개인보호용구착용에 관한 연구)

  • Objectives : The purpose of this study was to minimize cross infection that can take place within dental office, to make dental workers recognize importance of infection management, and to improve dental-care environment that is exposed to infection. Methods : It conducted the self-reported questionnaire survey on the practice of wearing PPE(personal protective equipment) from April 1, 2008 to May 30 targeting 180 dental workers who are working for dental offices where are located in Jeonnam region. Results : 1. As for research subjects' general characteristics, age was the largest in under 29 years old with 75.0%. It was surveyed to be large in over 5 years(56.1%) for working career, in a city(83.3%) for working region, and in having licence of dental hygienist(75.0%). 2. As a result of surveying the practice of wearing PPE, wearing mask given dental care was surveyed to have the highest practice level with 75.6%. Wearing protective glasses showed the lowest practice ratio with 23.3%. 3. As a result of surveying the difference in the practice of wearing PPE according to general characteristics, the difference according to the working region was surveyed to be the greatest. Conclusions : As the above result, the education of infection management for dental workers needs to be performed continuously even after curriculum of school. Even the in-service education on infection management needs to be vitalized. Also, the necessity was examined for always recognizing and practicing importance of infection by using a method of publishing newsletter via post to dental clinics where are located in rural area, with considering regional deviation.

  • This study is performed to confirm the influencing factors of family health protection behaviors using the variables included in Pender's Family Promotion Model. 1. The subjects are 110 families in preschooler family developmental stage, respondents are children's mother or father. These families are almost all nuclear types(95%), function of families is healthy as much as 8.0 the mean FAPGAR score. The prevalence rate of family members' illness was 14.7% these last 3 months, and 21.1 % of families responded suffered from injury for last 2 years. 2. The practice rate of injury prevention behavior is below a half in supervision and modifying of their home and residential environments, especially controlling through collaborative community power. The more familiar function score is the better practicing rates of injury prevention behaviors. 3. The injury prevention behaviors correlate to family size, health status of family member, and children's congenital defects with statistical significance. Families' economic condition correlates also significantly to family health status, cognition of benefits of injury prevention, cognition of the importance of community collecting power. And the recognition of the benefits of injury prevention correlates the adaptive health concept, family norms about injury prevention, economic status. 4. Considering family health promotion model. the general influencing factor is only affected to family protective behavior, and other paths don't affect to family's behaviors. In simple regression, the family protective behavior model explains 27.8%(P=0.05), significant factors are family function status, family size, chronic illness of family members', mother's education level. father's age. 5. To define of familiar preventive behavior as a unit is very important, but it has the limitation to solve the difficulties of family studies going with the operationalized difficulties of health promotion concept.
