인공심장판막치환 환자에 대한 임상적 고찰 (Clinical Evaluation of Prosthetic Valve Replacement in Valvular Heart Disease)

  • From 1982 to 1987, six patients underwent left ventricular aneurysmectomy with concomitant myocardial revascularization. Simultaneous repair of postinfarction ventricular septal defect was performed in 3 patients and left ventricular thrombectomy in two. There was no hospital mortality and late mortality during 220 months* follow up period-.[Mean 36.7 months, range 13 to 72 months] Their condition was improved in all. They live in physical condition of NYHA class I-II. We believe early surgical intervention can be life-saving, and can be done without undue surgical risk, even in cases of postinfarction VSD and LV aneurysm with failing heart.

  • A 42-year-old Marfan female had a chronic dissection of the descending aorta[Type B and the aneurysmal expansion of the descending aorta caused compression of the trachea resulting in respiratory distress which mimicked bronchial asthma. The patient has been successfully managed by resection and replacement of the aneurysm in the descending aorta. The operation could be done without the aid of the partial cardiopumonary bypass. As the patient had been prepared by unilateral axillo-bifemoral by-pass using prosthetic graft 8mm in diameter 10days prior to the main operation.

  • The usual treatment for aortic aneurysms is surgical replacement with a prosthetic graft; however the associated morbidity and mortality rates must be considered. Endovascular placement of self-expandable stent-graft is a safe noninvasive treatment that can be an alternative to the surgical repair, the postoperative course of the 2 cases of thoracoabdominal aortic aneurysms was uneventful and no complication has been associated with the stent-graft during the 17 months and 5 months follow-up studies,.

심장변막치환후 Ticlopidine과 Aspirin의 혈전방지 효과 (Prevention of thromboembolism with ticlopidine and aspirin after cardiac valve replacement)

  • Prevention of thrombombolism after rosthetic cardiac valve replacement is essential for the patients. About 90% of patients are free of major and minor thromboembolic complications 5 year after replacement of cardiac valves with prosthetic devices when they are under control of anticoagulant therapy. Ticlopidine is a drug that alter platelet function to have an antithrombotic effect. It is an antiaggregating agent which inhibits primary platelet function to have an antithrombotic effect. It is an antiaggregating agent which inhibits primary platelet aggregation induced by ADP and increases the production of prostaglandin $D_{2}$. Aspirin in small doses inhibits platelet synthesis of prostaglandins by irreversibly blocking the enzyme cyclo-oxygenase. Platelet secretion and aggregation are impaired with Ticlopidine and Aspirin. the thromboembolic event sof 54 patient s who were treated with Ticlopidine and Aspirin after cardiac valve replacement were evaluated and compared with that of 79 patients who were treated with Wafarin and Aspirin after the same type of operation. The follow-up period ranged from 4 to 110 months (mean of 48 months). there were 11 major thromboembolic episodes including three deaths in the warfarin goup during mean follow-up period of 56 months. two cases of CVA and one hemoarthrosis were noted due to overdose of Warfarin. Inticlopidine group, there was only one fatal thromboembolic epdisode three month after mitral valve replacement during mean follow-up period of 18 months. Two episodes of hypermenorrhea resulting anemia ere noted in the ticlopidine group. We measured the parameters of platelet function in aggreagation curve of platelet with platelet aggregometer (chrono-log Aggregometer, Model No. 430) Aggregation test was performed with three final concentrations of epinephrine in 10 uM/L, ADP in 5uM/L. 28 patients with prosthetic cardiac valves and 35 healthy volunteers were subgrouped as follows to analyze the effect of antithrombotic drugs used. Group I ; 11 patients treated with 250-500 mg of ticlopidine and 0.5gm of Aspirin as a daily single dose after cardiac valve replacement (14 St. Jude Medical and 1 Carpentier-Edwards, 9 patients with atrial fibrillation among them) Group II ; 10 patients treated with 3-5 mg of Warfarin and 0.75 gm of Aspirin daily to prolong prothrombin time around 20 seconds for more than 6 months and single Aspirin dose was maintained afterward as a life-long regimes(3 St. Jude Medical, 1 Hall-Kaster and 7 Carpentier-Edwards valve, 9 patients in atrial fibrilation). Group III ; 7 patients who quit anticoagulant treatment (Warfarin + Aspirin) 6-12 months after the regime as group II (3 St. Jude Medical. 1 bjork-Shiley, 1 Hall-Kaster, 3 Carpentier-Edwards valve, 2 of them are with atrial fibrillation). Group IV ; 35 healthy vounteers (28 males and 7 females). The following results were obtained. 1. The mean maximal platelet aggregability in Group I induced by 10uM/L epinephrine was 15.6%, and 17.5 and 18.7% in BM in proportion to the induction by 5 and 10 uM/L ADP. 2. The mean maximal platelet aggregability in Group II induced by 10uM/L epinephrine was 16.5%, and 27.4 and 44.7% in BM in proportion to the induction by 5 and 10uM/L ADP. 3. The mean maximal platelet aggregability in group III induced by 10uM/L epinephrine was 65%, and 56.5 and 51.8% in BM in proportion to the induction by 5 and 10 uM/L ADP. 4. The mean maximal platelet aggregability in the normal subjects induced by 10 uM/L epinephrine was 64%, and 65 and 69% in Bm inproportion to the induction by 5 and 10 uM/L ADP. 5. Reversible change of platelet aggregation curve induced by 5 and 10uM/L was noted all of the patients in Group I. conclusion : Ticlopidine is an antiaggregating agent which inhibits primary platelet aggregation induced by ADP, and increases the production of prostaglandin $D_{2}$. Ticlopidine and Aspirin produced a significant inhibition of platelet in the presence of ADP and epinephrine in our study. Acccording to our brief experience, 250 mg of ticlopidine and low dose of Aspirin resulted synergistic superior effect to each drug alone in prevention of thromboembolism after prosthetic cardiac valve replacement.

개심수술후의 사망예에 대한 임상적 고찰 (Mortality Analysis of Open Heart Surgery (75 Cases))

  • From 1976 through June 1980, 75 patients underwent Open heart operation at Korea University Hospital.Of the 75 patients, 39 were congenital heart cases and 36 were acquired heart disease cases. 39 cases of congenital heart disease were consisting of 16 T.O.F.,4 A.S.D., 10 V.S.D., 3 P.S., 1 P.D.A., 1 V.S.D. + Mi, 1 Truncus arteriosus, 1 Ebstein, 1 D.C.R.V., 1 Single ventricle. Among 36 valvular replacement cases, 18 cases of MVR, 3 cases of AVR, 6 cases of Double valve replacement, and 10 cases of Open Mitral commissurotomy, were performed. Postoperative mortality rate of congenital heart disease was 25.6% and that of acquired heart disease was 8.3%. Overall mortality rate of open heart surgery was 17.3%. Among 16 cases of postoperative death cases, 5 cases of autopsy were performed. Postoperative cause of death of our series were intracranial bleeding, pacemaker failure, low output syndrome, protamine anaphylaxis, bleeding, prosthetic valve embolism, C V A, miliary tuberculosis, hypothermia due to pump failure.

소아 및 청소년기에 있어서의 판막대치이식술에 관한 연구 (Cardiac valve replacement in children and adolescents)

  • Valve replacement in children and adolescents, aging below 20 years [Mean 15 years 4 months], has been done at Seoul National University Hospital from March 1977 to September 1982 . Seventy-Seven patients have received 91 artificial valves 4 prosthetic valves and 87 bioprosthetic valves. 63 patients had acquired valve lesions and 14 patients had congenital valve lesions. Among the patients with acquired valve lesion, 32 patients had the history of rheumatic fever. Seventy-five patients survived operation: 12 patients [ 15.6%] died within one month postoperatively and 3 patients [3.9%] during the follow-up period with the overall mortality rate of 19.5% Thromboembolic complication occurred in 3 patients with 2 deaths: 3.9% embolic rate or 3.74% emboli per patient-year. One patient who had been on coumadin anticoagulation died from cerebral hemorrhage. Actuarial survival rate was 77.6% at 1 years after surgery, after then there were no death.

Graft Perforation by a Spinal Bony Spur: An Unusual Cause of Late Bleeding after Thoracoabdominal Aorta Replacement

  • We report an unusual case of delayed bleeding after open surgical repair of a thoracoabdominal aortic aneurysm. A 79-year-old man developed a massive retroperitoneal hematoma 49 days after Crawford type III thoracoabdominal aorta replacement. During emergency surgery, a tear was found in the prosthetic vascular graft caused by a sharp bony spur arising from the second lumbar vertebral body. This rare, but potentially lethal, complication indicates that attention should be paid to sharp bony structures during open repair of the descending aorta.

인조판막 이식후의 난소혈종 1예 (Ovarian Hematoma After Double Valve Replacement -A Report of Case-)

  • Anticoagulation therapy with Warfarin and Dipyridamole is useful after prosthetic heart valve replacement for the prevention of thromboembolic accidents. Here presented a case of right ovarian hematoma, 41 years old, female who has been already treated double valve replacement due to mitral insufficiency with 27 mm $Bj{\ddot{o}}rk-Shiley$ mitral, and 29 mm Hancock tricuspid valve successfully on 27th, April, 1976. Just after the operation, patient was treated the anticoagulation therapy with Dipyridamole 300 mg, and Heparin, and later switched to Warfarin 3.75 mg or 5 mg po, as the maintenance dose. Three and half months after the anticoagulation therapy, patient complained the lower abdominal pain and vaginal spotting and which revealed right ovarian hematoma due to ovulation, manifested due to anticoagulation therapy. Patient was discharged postoperative 15 th day with the maintenance dose 5 mg Warfarin and Dipyridamole 300mg po to maintain the prothrombin time 30%, after the uterus and both. ovaries and both adnexae are resected out for the prevention of the further hemorrhage of ovary.

Outcomes of Nonpledgeted Horizontal Mattress Suture Technique for Mitral Valve Replacement

  • Background: Most surgeons favor the pledgeted suture technique for heart valve replacements because they believe it decreases the risk of paravalvular leak (PVL). We hypothesized that the use of nonpledgeted rather than pledgeted sutures during mitral valve replacement (MVR) may decrease the incidence of prosthetic valve endocarditis (PVE) and risk of a major PVL. Methods: We analyzed 263 patients, divided into 175 patients who underwent MVR with nonpledgeted sutures from January 2003 to December 2013 and 88 patients who underwent MVR with pledgeted sutures from January 1995 to December 2001. We compared the occurrence of PVL and PVE between these groups. Results: In patients who underwent MVR with or without tricuspid valve surgery and/or a Maze operation, PVL occurred in 1.1% of the pledgeted group and 2.9% of the nonpledgeted group. The incidence of PVE was 2.9% in the nonpledgeted group and 1.1% in the pledgeted group. No differences were statistically significant. Conclusion: We suggest that a nonpledgeted suture technique can be an alternative to the traditional use of pledgeted sutures in most patients who undergo MVR, with no significant difference in the incidence of PVL.

CarboMedics 기계판막의 단기 임상 성적 (Short-term Clinical Experience with CarboMedics Valve)

  • From March 1988 to May 1991, 140 CarboMedics cardiac valve prostheses[75 mitral, 9 aortic and 28 double aortic-mitral] were implanted in 112 consecutive patients[mean age 36.7$\pm$11.6 years, male/female 48/76] by one surgical team operating on adult cardiac patients at Kyoungpook University Hospital Associated Surgical procedures were performed in 19 patients[16.9%]. Total follow up represented 2,345 patient-months[mean 22.4 months] and was 100% complete. Eighty-two patients[73%] were in NYHA functional class IIIor IV preoperatively and 102 patients [95%] were in class I or II postoperatively. Hospital[30 day] mortality was 4.4%, [3/75 mitral, 1/9 aortic, 1/28 double valve replacement] and late death was 1.7%. [1 /74 mitral, 1 /28 double valve replacement] The actuarial survival at 36 months was 94.0% after mitral, 80% after aortic, 92% after double valve replacement, and 93.2% for the total group. The linearized incidence of valve relater death, prosthetic valve thrombosis, anticoagulant related hemorrhage, and reoperation was 1.00%/pt-yr, 0.51%/pt/yr, 0.51%/pt-yr, and 0.51%/pt-yr respectively. The 36 month rates of freedom from valve replated death, thromboembolism, endocarditis, anti-coagulant related hemorrages, and reoperation were 98.75%, 99.08%, 100%, 99.04%, and 99.08% respectively. The 36 month rate of freedom from all valve related complications and deaths including hospital mortality was 90.2%. These fact suggest that the CarboMedics heart valve has excellent short-term result, low incidence of valve-related complications and valve dysfunction, and additional long term follow up study is necessary.
