Background/Aims: Endoscopic submucosal dissection (ESD) for superficial non-ampullary duodenal tumors (SNADETs) is associated with a high rate of en bloc resection. However, the technique for ESD remains challenging. Recent studies have demonstrated the effectiveness of S-O clips in colonic and gastric ESD. We evaluated the efficacy and safety of duodenal ESD using an S-O clip for SNADETs. Methods: Consecutive patients who underwent ESD for SNADETs between January 2011 and December 2021 were retrospectively enrolled. Propensity score matching analysis was used to compare patients who underwent duodenal ESD with the S-O clip (S-O group) and those who underwent conventional ESD (control group). Intraoperative perforation rate was the primary outcome, while procedure time and R0 resection rate were the secondary outcomes. Results: After propensity score matching, 16 pairs were created: 43 and 17 in the S-O and control groups, respectively. The intraoperative perforation rate in the S-O group was significantly lower than that in the control group (p=0.033). A significant difference was observed in the procedure time between the S-O and control groups (39±9 vs. 82±30 minutes, respectively; p=0.003). Conclusions: The S-O clip reduced the intraoperative perforation rate and procedure time, which may be useful and effective in duodenal ESD.
This study aims to verify the practical effectiveness of learning management system (LMS) by introducing a LMS enhancing digital assessment utilizing automatic item generation in order to strengthen college student's mathematics performance. Teaching assisted with digital assessment in the LMS was applied to college mathematics classes, and the research question is whether or not students in the classes utilizing the LMS perform better than the regular classes. In particular, a calculus course, which is the foundation of important artificial intelligence technology in the future, was utilized in this study. The participants of this study were 248 freshmen in science and engineering who were taking calculus courses at a small to mid-size university. A total of 156 freshmen were selected after applying a propensity score matching method (PSMM), 78 from classes utilizing the LMS and 78 from regular classes without the LMS assisted with the digital assessment. As a result, it was found that there was a statistically significant difference in the math academic growth of students who used the LMS and those who did not. In other words, when LMS was used in calculus, students' academic growth was greater. The results of this study are meaningful in that they observed students' academic growth and confirmed that LMS enables a positive role in students' academic growth. In addition, if digital assessment is strengthened and LMS that enables individualized learning analysis is introduced and implemented in educational institutions, it is expected to play a major role in strengthening students' academic performance.
Purpose: Open pelvic bone fractures are relatively rare and are considered more severe than closed fractures. This study aimed to compare the clinical outcomes of open and closed severe pelvic bone fractures. Methods: Patients with severe pelvic bone fractures (pelvic Abbreviated Injury Scale score, ≥4) admitted at a single level I trauma center between 2016 and 2020 were retrospectively analyzed. Patients aged <16 years and those with incomplete medical records were excluded from the study. The patients were divided into open and closed fracture groups, and their demographics, treatment, and clinical outcomes were compared before and after 1:2 propensity score matching. Results: Of the 321 patients, 24 were in the open fracture group and 297 were in the closed fracture group. The open fracture group had more infections (37.5% vs. 5.7%, P<0.001) and longer stays in the intensive care unit (median 11 days, interquartile range [IQR] 6-30 days vs. median 5 days, IQR 2-13 days; P=0.005), but mortality did not show a statistically significant difference (20.8% vs. 15.5%, P=0.559) before matching. After 1:2 propensity score matching, the infection rate was significantly higher in the open fracture group (37.5% vs. 6.3%, P=0.002), whereas the length of intensive care unit stay (median 11 days, IQR 6-30 days vs. median 8 days, IQR 4-19 days; P=0.312) and mortality (20.8% vs. 27.1%, P=0.564) were not significantly different. Conclusions: The open pelvic fracture group had more infections than the closed pelvic fracture group, but mortality was not significantly different. Aggressive treatment of pelvic bone fractures is important regardless of the fracture type, and efforts to reduce infection are important in open pelvic bone fractures.

There are two types of clinical research to figure out risk factor for disease using collected data. One is prospective study to approach the subjects from the present time and the other is retrospective study to find the risk factor using the subject's information in the past. Both approached and study design are different but the purpose of the two studies is to identify a significant difference between two groups and to find out what the variables to influence groups. Especially when comparing the two groups in clinical research, we have to look at the difference between the impact clinical variables by group while controlling the influence of the baseline characteristics variables such as age and sex. However, in the retrospective study, the difference of baseline characteristic variables can occur more frequently because the past records did not randomly assign subjects into two groups. In clinical data analysis use covariates to solve this problem. Typically, the analysis method using the analysis of covariance of variance, adjusted model, and propensity score matching method. This study is introduce the way of equality adjustment between groups data analysis using covariates in retrospective clinical studies and apply it to the recurrence of gastric cancer data.
Background: This study aims to analyze the effects of insurance types on the medical service uses for heart failure inpatients using propensity score matching (PSM). Methods: 2014 National inpatient sample based on health insurance claims data was used in the analysis. PSM was applied to control factors influencing the service uses except insurance types. Negative binomial regression was used after PSM to analyze factors that had influences on the service uses among inpatients. Subjects were divided by health insurance type, national health insurance (NHI) and medical aid (MA). Total charges and length of stay were used to represent the medical service uses. Covariance variables in PSM consist of sociodemographic characteristics (gender, age, Elixhauser comorbidity index) and hospital characteristics (hospital types, number of beds, location, number of doctors per 50 beds). These variables were also used as independent variables in negative binomial regression. Results: After the PSM, length of stay showed statistically significant difference on medical uses between insurance types. Negative binomial regression provided that insurance types, Elixhauser comorbidity index, and number of doctors per 50 beds were significant on the length of stay. Conclusion: This study provided that the service uses, especially length of stay, were differed by insurance types. Health policy makers will be required to prepare interventions to narrow the gap of the service uses between NHI and MA.
Purpose Though many information systems researchers have made various attempts to investigate the relationship between information technology capability and firm performance from diverse perspectives, we have not come to a conclusion yet with some mixed results. In this research, focusing on the adoption of Enterprise Resource Planning (ERP) systems by firms as a proxy measure for information technology capability, we reexamine whether the association is significantly positive. Design/methodology/approach Previous research on this topic had some limitations to the samples and analysis method. Some research focused only on the 1990s or early 2000s, and other studies failed to adequately compare the impact of ERP adoption on firm performance between the treatment group and the control group. In this research, extending previous analysis approaches with the matched sample comparison of IT leaders and the control group, we attempt to apply propensity score matching in combination with difference-in-difference analysis with a sample of Korean firms that adopted ERP systems in the late 2000s. We match ERP adopters and non-adopters with propensity score matching and compare their financial performance with difference-in-difference estimation between the pre- and post-adoption periods. Findings According to our analysis, we find no positive and significant relationship between ERP adoption and firm performance in profit ratios. This research shows that, contrary to the era of proprietary information systems, standardized information systems today have no additional competitive advantages over competitors.
Purpose: The purpose of this study was to demonstrate the effects of visiting nursing services in long-term care (LTC) insurance on changes in health status and physical functions among the elderly. Methods: We analyzed survey data on the living status and welfare needs of the elderly from 2008 and 2011 using the difference-in-difference method with propensity score matching. Results: The subjects were 76 elders including 38 visiting nursing service users(intervention group) and 38 home-based LTC service users (control group). Results from DID analysis confirmed that the subjective health status level of the intervention group was 2.2 points higher (p=.044) and the depression level was 0.3 point lower (p=.039) than the control group. In addition, the intervention group's ADL score was 1.9 (p=.027) and IADL 3.9 (p=.030), showing that their health was deteriorated less. Conclusion: Visiting nursing service in LTC insurance was associated with delayed deterioration of subjective health status, depression, ADL and IADL. These findings suggest the need of rebuilding visiting nursing service programs focusing on prevention services, which will be more contributive to elderly health care and the reduction of social costs.
Background: This study was conducted to investigate preoperative carcinoembryonic antigen (CEA) as a prognostic factor in colorectal cancer. Methods: Between January 2000 and July 2011, 1298 patients with primary adenocarcinoma colorectal cancer without metastasis, who underwent curative resection were retrospectively identified. The patients were divided into two groups according to serum CEA level at primary diagnosis: a high CEA (HCEA) group (serum CEA ${\geq}6ng/mL$) and a normal CEA (NCEA) group (serum CEA <6 ng/mL). A 1:1 propensity score matching analysis was applied to reduce bias. Finally, 364 patients were enrolled in this study. Matched variables were age, gender, preoperative chemoradiotherapy, tumor site, cell differentiation and pathologic stage. Results: The clinicopathological characteristics of the two groups did not differ significantly difference. The systemic metastasis rate was 16.5% (30/182) and 25.3% (46/182) in the NCEA and HCEA groups, respectively (p=0.039). There were no significant differences in local recurrence or metastatic sites between groups. The 5-year disease-free survival (DFS) rate of the HCEA group was worse than that of the NCEA group; however, there was no significant difference in overall survival between the two groups. Conclusion: Elevated preoperative CEA was related to frequent systemic recurrence and low DFS. Therefore, elevated preoperative CEA could be considered a prognostic factor for worse clinical outcomes in patients with colorectal cancer.
The Innovation Procurement Policy was introduced as part of the strategic public procurement policy to improve firms' innovation capabilities and enhance the public sector's ability to solve social problems by designating and purchasing so-called 'innovative products.' The pilot procurement project for innovative products was first introduced in 2019, and the policy system for designating and discovering innovative products by government departments, as well as the priority purchase system, was established in 2020. Hence, this study conducted a quantitative analysis focusing on the effectiveness of the innovation procurement system in supporting firms after it was fully implemented. For this purpose, corporate financial and employment data from 2017 to 2021 were used, and propensity score matching(PSM) and difference-in-difference(DID) methods were utilized as analytical tools. The study found that the innovation procurement system contributed to corporate growth and employment and created additional public and private sales channels. Moreover, it is necessary to enhance the innovation procurement system, such as matching innovative product-producing companies with existing SME support policies, for companies to become self-sustaining after the innovative product designation ends.
The purpose of this study is to analyze the effectiveness of an agriculture and rural development ODA project in southern Nepal's Terai region to improve food security, and to suggest appropriate project approaches to improve food security. The subject of the analysis is the KOICA-funded 'Inclusive Rural Development project', and a framework for analyzing food security was established based on the FAO's four pillars of food security, and indicators were proposed to measure the food security of project farmers. The data for this study were obtained from a survey of project beneficiary farmers and their control group, and the effects of the project on food security were measured using a a t-test analysis, difference-in-difference analysis, propensity score matching-linear regression combined model (PSM-OLS) and a propensity score matching-difference-in-difference combined model(PSM-DID). The results showed that, in general, the ODA project treatment group had significant improvements in each of the four pillars of food security (food availability, access, utilization, and stability). In addition, cooperative membership had a positive impact on food security improvements, and there were also differences in outcomes among different ethnic groups in the Terai region. The implications of this study are that the Korean government's ODA policy should set aside the food security sector and incorporate food security indicators to contribute to the improvement of food security in ODA partner countries, and that agricultural and rural development ODA projects should include food security indicators as performance indicators in their pre-planning to contribute to securing food security for vulnerable groups as food insecurity is higher among vulnerable groups.
