정상인의 상지 원위부에 대한 운동학적 분석의 신뢰도 (The Reliability of Kinematic Analysis for Distal Upper Extremity in Normal Person)

  • Purpose: To evaluate statistical differences among three measurements of range of motion (ROM) with Rapael Smart Glove (RSG) group 1, 2 and manual goniometer group. To investigate reference value of the kinematic analysis for range of motion (ROM) of distal upper extremity with Rapael Smart Glove (RSG). Methods: Sixteen normal persons without limitation of motion (LOM) enrolled in the study. The study was performed at two separate times and by two investigators on 16 normal adults. We compared ROM with RSG for measuring joint angles. We compared degrees of forearm supination/pronation, wrist flexion/extension and radial deviation/ulnar deviation during ROM of 16 participants using RSG. After one week, degrees of each motion were measured in the same way by other investigator to evaluate the reliability. Results: Statistical differences among three groups were showed. Most results of paired t-test between two RSG groups were over 0.05 and exceptions are supination, extension, and finger %. Conclusion: Our findings demonstrate that ROM of normal persons obtained by kinematic analysis with RSG are not valid as normal reference value for distal upper extremity motion. But, the reliability of between two RSG groups was showed with paired t-test and Pearson's correlation except supination, extension and finger %.

The Effect of Arm Movements in the during Standing Position on Lower Limb Global Synkinesis and Balance in Stroke Patients

  • Background: Stroke patients require arm movement exercising for various stimulations in standing position for various stimulations rather than in a sitting position because they require integrated skillful movements, such as stretching, holding, and controlling. Objective: This study was conducted to provide foundational clinical data about lower limb global synkinesis in stroke patients using arm movements in a standing position. Design: Randomized controlled trial. Methods: The subjects were divided into a control group (n = 10) and an experimental group (n = 10), and a pre-test was conducted to evaluate leg global synkinesis (GS) and balance. Intervention method is stretching an arm to hold a ball, repeating supination and pronation of the hand only while maintaining the arm extended as much as possible, repeating shoulder abduction and adduction while holding the pegboard. This was followed by a three-week intervention during which re-measurement was conducted in the same way as was done for the pre-test. Results: The control group showed a significant difference in GS and balance during plantar flexion (p<.05), and the experimental group showed a significant difference in GS and balance during all movements (p<.05, p<.01, respectively). There was a significant difference in GS and balance between the two groups during dorsiflexion (p<.05, p<.01, respectively). Conclusion: The findings demonstrate that human arm movements in a standing position can reduce GS in the affected limb, and balance can be improved by stimulating the surrounding tissues of the affected limb and changing them positively.

뇌경색 후 수부 미세운동 장애에 팔사혈(八邪穴)(EX-UE9) 전침을 적용한 호전 증례보고 (A Case Report of a Patient with Motor Disturbance of the Hand After Cerebral Infarction Treated with Electroacupuncture on Pal-sa (EX-UE9))

  • Objective: The purpose of this case report is to evaluate the effect of electroacupuncture on Pal-sa (EX-UE9) for unilateral motor disturbance of the hand after cerebral infarction. Methods: One patient with unilateral motor disturbance of the hand following cerebral infarction (right basal ganglion and corona radiata) was treated with acupuncture, herbal medication, and electroacupuncture on Pal-sa (EX-UE9) once daily from June 30, 2016 to July 4, 2016. We evaluated improvement using the Box and Block Test (BTT) and 10-seconds Test, including the Finger Individual Movement Test (FIMT), the Hand Pronation and Supination Test (HPST), and the Finger Tapping Test (FTT). Results: After five days of treatment, increase of FIMT and FTT scores was observed after electroacupuncture on Pal-sa (EX-UE9). However, no increase was observed in BBT or HPST scores. Conclusions: This study suggests that electroacupuncture on Pal-sa (EX-UE9) can help treat motor disturbance of the hand after cerebral infarction.

노인의 워커 사용에 따른 보행 시 하지 관절 3차원 동작 분석에 관한 연구 (The Study of 3D Motion Analysis on Lower Limb during Walking with Walker on Older People)

  • Purpose : The purpose of this study was to find out the difference motion of hip, knee and ankle joint during walking according to using walker on older people. Method : Korean older people of 34 subjects was participated in this study. Participants was measured joint motion on hip, knee and ankle joint during both conditions (walking with walker and without walker). The measured data were analyzed using independent t-test to investigate the difference of joint motion on the both condition. The statistical analyses were performed using Predictive Analytics Soft Ware (PASW) for windows(Ver. 19) and p-value less than .05 were considered significant for all cases. Result : The study showed that more joint motion on hip flexion and ankle pronation is increased by using walker. And hip extension, knee external rotation and ankle plantar flexion is decreased by using walker. Conclusion : This study suggest that using walker on older people was change the motion of the lower limb joint during walking. Therefore, It is necessary to develop a new walker that can reduce dependency and ensure stability on older people during walking.

몰입형 가상현실 프로그램 Rapael Smart Glove가 뇌졸중 환자의 상지기능에 미치는 영향 (Effects of Immersive Virtual Reality Intervention on Upper Extremity Function in Post-Stroke Patients)

  • Purpose: The purpose of this study was to investigate the effects of an immersive, virtual reality-based exercise program on range of motion and dexterity in the upper extremities of stroke patients. Methods: Fifteen patients with hemiparesis after stroke participated in this study. The participants participated in Rapael Semart GloveTM, an immersive, virtual reality-based exercise program, performed for 30 minutes-, 3 times per week for 4 weeks. The Rapael Smart GloveTM program and a Box and Block Test (BBT) were used to measure range of motion and to assess dexterity, respectively, pre-and post-intervention. Results: Range of motion in pronation and supination of the forearm and flexion, extension, and ulnar deviation of the wrist improved after the intervention. Dexterity measured by BBT also improved. However, range of motion in flexion and extension of the fingers and radial deviation of the wrist did not improve. Conclusion: This study presents the effects of an immersive, virtual reality-based exercise program on hand function. In the future, a study comparing an immersive, virtual reality- based exercise program to other upper-extremity interventions for stroke patients should be conducted. A study about the effects of an immersive virtual reality program on activities of daily living is also needed.

발의 형태에 따른 거골하 관절 가동술과 능동운동이 하퇴 근활성도에 미치는 영향 (The Effect of Lower Muscle Activities on the Sub talar Joint Mobilization and Active Exercise of Ankle Shape)

  • Purpose : The purpose of this study is to know the muscle activation after sub-talar joint mobilition and active exercise, and the low leg muscle activity through the well-balanced interaction of ankle joint around muscle. Methods : For this study 61 experimental subjects are divided into 24 people of supination foot group, pronation foot group 17 people of, 20 people of control group through navicular drop test. Surface EMG was used in order to measure the muscle activities. following is the result of the data analysis about each experiment that has been carried on a week, 2 weeks, 3 weeks, 4 weeks before, and even comparing with pre-experimental state. Results : In electromyogram study, the higher muscle activation there was before the experiment, the more muscle activation increase there was after the experiment in Tibialis Anterior, Peroneus Longus, Peroneus Brevis.(p<.05). Conclusion : This study shows the balanced activation of foot and ankle-around muscle. It shows that foot shape affects the balanced activation recovery of lower leg muscles.

각속도 측정시스템을 이용한 특발성 파킨슨병 환자와 정상인의 빠른 손놀림 동작의 비교 (Comparison of Movement of Rapid Alternating Movements of Hands in Idiopathic Parkinson's Disease Patients and Normal Subjects using Angular Velocity Measurement System)

  • The purpose of this study was to analyze bradykinesia of forearm movement in patients with Parkinson's disease (PD) as compared to those of normal subjects. A gyrosensor was selected for the measurement of forearm movement, because it can provide angular velocity signal which is free from the gravitational artifact and also because it can be conveniently used during clinical test of bradykinesia. Forty PD patients (age: $65.7\pm11.1$ yrs, H&Y stage:$2.3\pm0.5$), 14 age-matched elderly subjects ($65\pm3.9$ yrs) and 17 healthy young subjects ($24\pm2.1$ yrs) participated in this study. Angular velocity during forearm movement of pronation/supination was measured in both arms. Suggested quantitative measures of bradykinesia were root-mean-squared (RMS) angular velocity, RMS angle, peak power and total power which were derived from the angular velocity. ANOVA showed that all measures were significantly different among three groups (p<0.001). Subsequent post-hoc test revealed that all measures in PD patients were significantly smaller than in healthy elderly and healthy young subjects (p<0.05). This results suggest that PD patients can be differentiated from normal subjects using suggested measures.

기능적 전기자극이 경직성 뇌성마비 아동의 상지 기능에 미치는 영향 (The Effects of Functional Electrical Stimulation on Hand Function of Children With Spastic Cerebral Palsy)

  • 목적 : 본 연구는 목적은 기능적 전기자극이 경직성 뇌성마비 아동의 상지 기능에 미치는 영향을 알아보는 것이었다. 연구방법 : 11세에서 16세 사이의 경직성 뇌성마비 아동 20명을 대상으로 실험을 실시하였다. 기능적 전기자극은 1회 20분, 주당 3회, 총 8주를 실시하였고, 기능적 전기자극이 상지 기능에 미치는 영향을 알아보기 위해 Jebsen Hand Function Test, 수정된 Ashworth 척도, 3차원 동작분석을 실시하였다. Jebsen Hand Function Test, 수정된 Ashworth 척도는 실험 전부터 8주간 각 2주마다 측정을 실시하였고 3차원 동작분석은 실험 전과 8주 후에 측정을 실시하였다. 결과 : 기능적 전기자극 후, Jebsen Hand Function Test에서 6개의 하위 검사 모두에서 각 과제의 수행을 위한 경과 시간의 유의한 감소가 나타났고(p<.05), 수정된 Ashworth 척도에서도 통계학적으로 유의한 상지의 경직도의 감소가 나타났다(p<.05). 3차원 동작 분석 결과 기능적 전기자극 적용 전에 비해 적용 8주의 경과 후 손바닥 두드리기와 검지 두드리기에서는 통계학적으로 유의한 감소를 나타내었고(p<.05), 아래팔의 회내-회외 운동에서는 기능적 전기자극 적용 전에 비해 적용 8주의 경과 후에는 통계학적으로도 유의한 증가를 나타내었다. 결론 : 본 연구의 결과를 통해, 기능적 전기자극은 경직성 뇌성마비 아동의 상지 기능 향상을 위한 효과적인 치료 방법임을 알 수 있었다. 그리고 향후 기능적 전기자극을 위한 다양한 방법과 장기간의 적용에 대한 지속적인 연구가 필요할 것으로 생각된다.

수정된 강제유도 운동치료가 편마비를 가진 뇌성마비 아동의 상지 기능에 미치는 영향 (The Effects of Modified Constraint-Induced Movement Therapy on Hand Functions of Children With Hemiplegic Cerebral Palsy)

  • 목적 : 본 연구의 목적은 수정된 강제유도 운동치료가 편마비 뇌성마비 아동의 상지 기능과 관련된 요소들에 미치는 영향을 알아보는 것이다. 연구방법 : 8세에서 11세 사이의 편마비 뇌성마비 아동 4명을 대상으로 실험을 실시하였다. 수정된 강제유도 운동치료는 하루 5시간 동안의 건측 제한과 이 시간동안 2시간의 환측의 집중적인 과제훈련, 주당 5회, 총 8주를 실시하였고, 수정된 강제유도 운동치료가 상지 기능과 관련된 요소에 미치는 영향을 확인하기 위하여 Jebsen Hand Function Test와 3차원 동작분석을 실시하였다. Jebsen Hand Function Test는 실험 2주마다 반복 측정을 실시하였고, 3차원 동작분석의 경우에는 실험 전과 실험 후에 측정을 실시하였다. 결과 : 수정된 강제유도 운동치료 후, Jebsen Hand Function Test에서는 하위 검사 모두에서 경과 시간의 유의한 감소가 나타났고(p<.05), 3차원 동작 분석 결과 수정된 강제유도 운동치료 적용 전에 비해 8주의 경과 후 손바닥 두드리기 검사와 검지 두드리기 검사에서 통계학적으로 유의한 감소가 나타났고(p<.05), 아래팔의 회내와 회외 운동에서는 수정된 강제유도 운동치료 적용 전에 비해 8주의 경과 후 통계학적으로 유의한 증가가 나타났다. 결론 : 본 연구의 결과를 통하여, 수정된 강제유도 운동치료는 편마비 뇌성마비 아동의 상지 기능과 관련된 요소를 효과적인 치료하는 방법임을 알 수 있었다. 그리고 향후 수정된 강제유도 운동치료를 위한 개별화된 적용 방법과 개인의 상태에 따른 다양한 적용 기간의 연구가 필요할 것으로 생각된다.

종골 골절 후 거골하 관절면의 전위 정도와 운동 범위의 상관 관계 (Correlationship Between Degree of Displacement and Range of Motion of the Subtalar joint after Calcaneal fracture)

  • The characteristics of the patients after the calcaneal fracture that were associated with an unsatisfactory outcome were subtalar incongruity, decreased Bohler angle ratio of the fractured to the normal side, an age of more than fifty years, work involving strenuous labor, and increased time missed from work due to the injury. The purpose of this study was to examine the reliability of measurements of the range of motion of the subtalar joint. To determine reliability, evaluates of the correlatioinship between the degree of the displacement of the subtalar joint and Circle draw test after the calcaneal fracture. Fifty patients who had had fifty five calcaneal fractures were managed with open reduction and internal fixation. The results were reviewed retrospectively, between 4months and three years after the operation, with use of an evaluation system for the subtalar joint and with plain radiographs. At follow up evaluation, the result was assessed on the basis of restoration of anatomy and function of the subtalar joint. We evaluated the subtalar joint with plain films that consist of anteroposterior projection, lateral projection, calcaneal axial view, and Broden's view, and the measurements of the displacement of the subtalar joint surface after the calcaneal fracture. And we evaluated the range of motion of the subtalar joint with Circle draw test for physical evaluation. Circle draw test was evaluated and demonstrated the motion of flexion-supination-adduction and extension-pronation-abduction of the subtalar joint. And there are correlationship between the degree of the displacement and range of motion of the subtalar joint after the calcaneal fracture. The report critically reviews methords used to measure Circle draw test for physical examination of the follow up after the calcaneal fracture.
