Intractable Coronary Spasm Requiring Percutaneous Coronary Intervention after Coronary Artery Bypass Grafting in a Patient with Moyamoya Disease

  • Moyamoya disease (MMD) is characterized by progressive steno-occlusive lesions of the distal or proximal branch of the internal carotid arteries, and cerebrovascular symptoms are its major complications. Extracranial vascular involvement including the coronary artery has been reported, and some case reports have described variant angina or myocardial infarction. However, no report has yet described a case of myocardial infarction after coronary artery bypass grafting (CABG). Here, we present a patient with MMD who suffered cardiac arrest caused by myocardial infarction due to a coronary spasm after offpump CABG and who was discharged successfully after treatment with a veno-arterial extracorporeal membrane oxygenator and percutaneous coronary intervention.

장기간의 로봇 보조 스텝훈련이 만성 뇌졸중 환자의 하지 근력과 보행속도에 미치는 영향: 예비 연구 (Long-term Effect of Robot-assisted Step Training on the Strength of the Lower Extremity and Gait Speed in a Chronic Stroke Patient: A Preliminary Study )

  • PURPOSE: The present research examined the effects of progressive robot-assisted step training on the strength of the lower extremity and gait speed of an individual with stroke through changes between the baseline and the intervention stage (1, 3, 6, 9, and 12 months). METHODS: A single-subject (A-B) design was performed for a chronic stroke patient aged 70 years old. The robot-assisted step training was conducted three times a week during 12 months (40 minutes/session), and the assessment was conducted a total of seven times between the baseline and the intervention (No. 1, 3, 6, 9, and 12 months) to determine the effect of the intervention. RESULTS: As a result of the intervention, the muscle strength at the lower extremity of the paralysis side increased by the greatest extent 12 months after the intervention compared to the baseline, and the gait speed via the 10-meter walk test was increased as well. CONCLUSION: Long-term robot-assisted step training might be an effective intervention for improving the strength of the paretic lower extremity muscles and gait speed in stroke patients with difficulty walking independently. Further studies with sufficient sample sizes and a randomized control group will be needed to evaluate the long-term effects of robotic stepping rehabilitation.

무릎 골관절염 노인을 위한 재활운동프로그램의 근력 및 통증에 미치는 효과 (The Effects of Rehabilitation Exercise Program on Muscle Strength and Pain in Elderly with the Knee Osteoarthritis)

  • Purpose: This study was to investigate the effects of rehabilitation exercise program on the muscle strength and pain in elderly with knee osetoarthritis. The progressive rehabilitation program included flexibility, strength, endurance, and active range of motion exercise and functional activities. Methods: Nineteen subjects with osteoarthritis of the knees randomized to exercise group(n=10) or control group(n=9), but fourteen subjects completed the trial with an adherence of 70% to the intervention and 77% to the control. The knee pain with VAS and quadriceps strength were evaluated at pre- and post-intervention. Results: The data were analysed with the two-way repeated ANOVA and paired t-test. The pain and quadriceps muscle strength had no difference among the groups and times. Within experimental group, pain was significantly decrease and quadriceps muscle strength significantly increased between pre and post intervention, while the control group showed no changes. Conclusion: Rehabilitation exercise program for knee osteoarthirits appear to be effective in knee pain and quadriceps muscle strength.

운동프로그램이 퇴행셩 슬관절염 노인의 체력에 미치는 영향 (The Effects of Exercise Program on Physical Fitness in Elderly with the Knee Osteoarthritis)

  • Purpose: This study was to determine the effects of exercise program for knee osteoarthritis on the physical fitness in elderly. The progressive exercise program included flexibility, strength, endurance, active range of motion and function activities. The physical fitness(back muscle strength, grip strength, flexibility, muscle power, agility, equilibrium) were evaluated with Helmas(Health Management System) made by Sewoo system at pre- and post- intervention. Results: The data were analysed with the two-way repeated ANOVA and paired t-test. The physical fitness had no difference among the groups and times except muscle power. Within experimental group, muscle power and agility were significantly increased between pre and post intervention, while the control group showed no change. Conclusion: Exercise program for knee osteoarthirits appear to be effective in muscle power and agility.

천식 환자에 있어서 최대호기 유속의 자가 모니터링을 이용한 복약지도의 유용성 (Clinical Outcome of Educational Program Using Self-monitoring of Peak Expiratory Flow Rate for Asthma Patients)

  • The effects of pharmacist's intervention for asthma patients using self-monitoring of peak expiratory flow rate in medication teaching model was evaluated for 3 months in improving clinical outcomes including emergency visits, hospitalizations, antibiotics use, symptoms and sleep disturbance. Twenty seven patients were enrolled in study and twenty three patients completed the follow-up schedules. The selected patients were given the pre-designed instruction for medication including appropriate use of medication, metered-dose inhaler(MDI) technique, identifying and controling asthma triggers and recognizing early signs of deterioration. There were significant improvements in clinical outcomes, in terms of emergency visits, hopitalizations, antibiotics use, symptoms and sleep disturbance. There were also significant improvements in the MDI use, environmental control, and medication knowledges. There was a progressive increase in peak expiratory flow rate during the three-month intervention. In conclusion, pharmacist's intervention using self-monitoring of peak expiratory flow rate has a significant impact on improving clinical outcomes in asthma patients.

Effects of Progressive Core and Ankle Muscle Strengthening Exercises Using Thera-Band on Body Balance

  • Purpose: This study aims to compare the effect on balance during core and ankle muscle strengthening exercises using the Thera-Band. Methods: 21 healthy college students were recruited. The participants were divided into a core strengthening group (CSG), an ankle strengthening group (ASG), and a non-exercise group (NEG). CSG and ASG were performed twice a week for a total of 4 weeks, and static and dynamic balance were measured before and after the intervention. The static balance were measured as stability index and weight distribution index using Tetrax®. The dynamic balance was measured in each direction by the Y balance test. The Thera-Band intensity was increased after 2 weeks of exercise, and the exercise was subsequently performed. Results: Participants showed static balance with Tetrax®, a significant difference was noted between normal eye closes and pillow with eye closes in ASG (p<0.05). In the case of dynamic balance with the Y balance test, a significant difference was observed in posterolateral direction (PL) and composite score (CS) between each group for the pre- and post-intervention differences (p<0.05). A significant difference was observed between PL and CS in CSG (p<0.05). Conclusion: These findings show that the progressive Thera-Band exercise had a positive effect on balance abilities. It was confirmed that core strengthening was effective for dynamic balance, and ankle strengthening was effective for static balance.

  • Bronchioloalveolar cell carcinoma accounts for less than 6% of all primary lung cancer but has distinct clinical and radiological features and unusual pathologic appearance. The characteristic features are its peripheral location and tendency of rapid progression to diffuse type via aeroginous and lymphatic route without surgical intervention. Among them, mucin secretory type bronchioloalveolar cell carcinoma is the rarest and most distinctive. We experienced a case of mucin secretory type bronchioloalveolar cell carcinoma in a 47 year old female with roentgenographic findings of chronic progressive pulmonary consolidation with muliple cavities.

Magnetic Resonance Imaging as a Biomarker for Duchenne Muscular Dystrophy

  • Muscular dystrophy is a hereditary musculoskeletal disorder caused by a mutation in the dystrophin gene. Duchenne muscular dystrophy (DMD) is one of the most common, and progresses relatively faster than other muscular dystrophies. It is characterized by progressive myofiber degeneration, muscle weakness and ultimately ambulatory loss. Since it is an X-linked recessive inheritance, DMD is mostly expressed in males and rarely expressed or less severe in females. The most effective measurement tool for DMD is magnetic resonance imaging (MRI), which allows non-invasive examination of longitudinal measurement. It can detect progressive decline of skeletal muscle size by measuring a maximal cross-sectional area of skeletal muscle. Additionally, other techniques in MRI, like $T_2$-weighted imaging, assess muscle damage, including inflammation, by detecting changes in $T_2$ relaxation time. Current MRI techniques even allow quantification of metabolic differences between affected and non-affected muscles in DMD. There is no current cure, but physical therapist can improve their quality of life by maintaining muscle strength and function, especially if treatment (and other forms of medical intervention) begins in the early stages of the disease.

치매환자의 삼킴장애 중재에 관한 체계적 고찰 (Intervention of Swallowing Disorder in Dementia Patient: A Systematic Review)

  • 목적 : 본 연구는 치매환자의 삼킴장애 중재에 대한 체계적 고찰을 통해 치매환자의 특성에 따른 중재접근법과 효과에 대한 과학적 근거를 마련하고자 하였다. 연구방법 : 주요 검색 데이터베이스로 CINAHL, Cochrane CENTRAL, PsycINFO, PubMed, Science Direct를 사용하였으며 2010년 1월부터 2020년 5월까지 발표된 문헌을 수집하였다. 검색어는'Dementia', 'Alzheimer disease', 'Deglutition disorder', 'Swallowing disorder', 'Dysphagia'를 사용하였으며 최종 5편의 문헌을 선정하였다. 결과 : 중등도-중증 환자를 대상으로 한 연구가 1편, 중증 환자를 대상으로 한 연구가 4편 포함되었다. 중재 유형은 교정적 중재가 1편, 보상적 중재가 3편, 포괄적 중재가 1편이었다. 중등도-중증 환자를 대상으로 행동 및 심리관리를 포함한 보상적 중재가 사용되었다. 중증 환자를 대상으로는 경관식이에 대한 교정적, 보상적 중재 및 다학제적 중재를 포함한 포괄적 중재가 사용되었다. 각각의 연구에서 사용한 모든 중재 방법은 삼킴능력과 관련된 결과변수에 긍정적인 영향을 끼쳤다. 결론 : 본 연구에서는 치매환자의 진행단계에 따라 중재의 목적과 중재 요소가 달라지는 것을 확인하였다. 따라서 치매환자의 삼킴장애 중재는 치매환자의 진행단계를 고려하여 개인화된 중재를 시행했을 때 더욱 효과적임을 확인하였다. 이러한 결과는 치매환자의 삼킴장애를 위한 효과적 중재 설계에 방향을 제시해 줄 수 있을 것으로 사료된다.

Impact of Applied Progressive Deep Muscle Relaxation Training on the Level of Depression, Anxiety and Stress among Prostate Cancer Patients: A Quasi-Experimental Study

  • Background: The aim of this study was to determine the impact of applied progressive muscle relaxation training on the levels of depression, anxiety and stress among prostate cancer patients. Materials and Methods: A quasi-experimental study was conducted at the University Malaya Medical Centre (UMMC) and Universiti Kebangsaan Malaysia Medical Centre (UKMMC) over six months. Prostate cancer patients from UMMC received the intervention and patients from UKMMC were taken as controls. The level of depression, anxiety and stress were measured using Depression, Anxiety Stress Scales - 21 (DASS-21). Results: A total of 77 patients from the UMMC and 78 patients from the UKMMC participated. At the end of the study, 90.9% and 87.2% of patients from the UMMC and UKMMC groups completed the study respectively. There were significant improvements in anxiety (p<0.001, partial ${\eta}^2$=0.198) and stress (p<0.001, partial ${\eta}^2$=0.103) at the end of the study in those receiving muscle training. However, there was no improvement in depression (p=0.956). Conclusions: The improvement in anxiety and stress showed the potential of APMRT in the management of prostate cancer patients. Future studies should be carried out over a longer duration to provide stronger evidence for the introduction of relaxation therapy among prostate cancer patients as a coping strategy to improve their anxiety and stress.
