The Nature of Risk Assessment

  • Recent criticisms of the conduct and use of risk assessment by regulatory agencies have led to a wide range of proposed remedies, including changes in regulatory statutes and the development of new methods for assessing risk. The mandate to this Committee was more limited. Our objective was to examine whether alterations in institutional arrangements or procedures, particularly the organizational separation of risk assessment from regulatory decision-making and the use of uniform guidelines for inferring risk from available scientific information, can improve federal risk assessment activities. Before undertaking to determine whether organizational and procedural reforms could improve the performance and use of risk assessment in the federal government, the Committee examined the state of risk assessment and the regulatory environment in which it is performed. In this chapter, we define risk assessment and differentiate it from other elements in the regulatory process, analyze the types of judgments made in risk assessment, and examine its current government context. Because one chronic health hazard, cancer, was highlighted in the Committee's congressional mandate and has dominated public concern about public health risks in recent years, most of our report focuses on it. Furthermore, because activities in four agencies--the Environmental Protection Agency (EPA), the Food and Drug Administration (FDA), the Occupational Safety and Health Administration (OSHA), and the Consumer Product Safety Commission (CPSC)--have given rise to many of the proposals for changes in risk assessment practices, our review focuses on these four agencies. The conclusions of this report, although directed primarily at risk assessment of potential carcinogens as performed by these four agencies, may be applicable to other federal programs to reduce health risks.

스마트 폰(Smart-Phone)기반의 재난 감시 및 상황전달시스템 프레임워크(Framework) 구축에 관한 연구 (A Study on the Framework Construction of Disaster Monitoring and Transmitting System based on Smart-Phone)

  • Smart-Phones are utilized in disaster management field because it can deliver disaster information to large population simultaneously and quickly, and provide accurate information through situation-based service using the LBS(Location Based Service). To study on the utilization of smart phone for disaster information collection and dissemination method, this study suggest a framework which connects smart phone by loading application for reporting disaster. The disaster monitoring and situation dissemination system framework using smart phone is composed of 4 parts. First, smart phone application enters image, video, voice and text information and location of the disaster. Second, the disaster report reception and situation dissemination server receives the information, save in the DB, and send through smart phone SMS. Third, store into disaster information database. Fourth, display the disaster report and management information on 2D GIS, support the decision making process in deciding whether to manage as disaster, and disaster management web service which disseminates situation.

도시철도 승강설비의 가용성, 유지보수성 및 안전성 입증 (Validation of Availability, Maintainability and Safety for Subway Elevating Equipment)

  • Purpose: In order to fulfill the RAMS (Reliability Availability Maintainability and Safety) requirements in Korean railway systems, specific target metrics should be measured and met the requirements when ordering a new system. This paper presents a procedure to predict and demonstrate the availability, maintainability and safety of subway elevators, escalators and PSD (platform screen doors) systems. Methods: The system manufacturer predicts availability and maintainability with lab and field tested data. After installation, availability and maintainability are demonstrated based on the actual operational data. The data was collected from the FRACAS (Failure Report and Corrective Action System). Results: Methods and process of assessing and analyzing the availability, maintainability and safety are presented for elevating services and PSD systems. The data obtained through the actual operation of the equipment is analyzed and maintained to predict the RAMS based on the component and system level failure data acquired. Conclusion: This study presented an application using IEC 62278 and operational data which can be used in the design and development stage to achieve the RAMS target value of the subway elevators, escalators and PSD systems.

화학산업에서의 안전성능지수 개발에 관한 연구 (A Study on the Development of Safety Performance Index in Chemical Industry)

  • In order to maintain the continual safety management in a company, it needs to evaluate and monitor its implementation of safety management. Because the number of major-accidents is not an effective method of indicating company's safety performance, various efforts to develop more reasonable indicators have been made in world wide. After Korean government has legally required the PSM report, PSM compliance audit has been developed and made by the authorities concerned since 2005. However, this audit consists of complicate procedures difficult to utilize as companies' own audit program and corresponds to only a conformity check that confirms whether the PSM be operated and maintained properly. So a new index by which to measure easily the level of safety performance and self-monitor the implementation of safety management is needed. We have studied a new method that may quantitatively evaluate the performance of safety management by investigating application cases in foreign countries and doing the domestic survey of lots of companies subject to PSM regulation in Korea. This study proposes three of safety performance indices(SPI) together with the several prerequisite preconditions and the timing for application of each index. Although the first draft of SPI needs further legal support, it might help to evaluate every company's safety level. The second draft of SPI is a voluntarily evaluating method based on web-site online program. The last draft of SPI consists of a series of simple questions about 12 elements of PSM. Also each of 3 indices has differences in evaluation methodology and application area and, therefore, they may be used concurrently.

Forearm Replantation for a Patient Presented with Major Amputation Injury: A Case Report

  • With the development of safety measures for employees who work with dangerous machinery, the frequency of amputation injuries has been decreasing with resultant decrease in replantation procedures. However, in some patients with major amputation injury, replantation is still necessary for the preservation of limb and it's function. The replantation of the upper extremity (UE) is a complex and technically demanding surgical procedure. For the successful replantation of UE, the type of injury, reconstruction sequence, ischemic time, and other combined injury of patient should be considered. We report a case of major amputation of UE by guillotine-type injury and discuss the treatment process of this patient.

  • The Lion Air JT610 accident in Indonesia in October 2018, along with the Ethiopian Airline ET302 accident in March 2019, is an significant aircraft accident that detects defects of the B737MAX aircraft. Shortly after the accident, the FAA prohibited operation of the aircraft. This action has affected the market environment of airlines and aircraft manufacturers around the world. In October 2019, Indonesian Traffic Safety Committee released an accident investigation report for Lion Air JT610, which concluded that the causes of the accident were MCAS design defects, lack of education and training, and errors in the repair process. This paper summarizes the flight control system of the B737MAX aircraft, the causes of the accident based on the final accident investigation report, and provides considerations for aircraft design and retrofit.

S 전자 근로자 집단 백혈병 사건 (Workers' Compensation of Semiconductor Leukemia Victims)

  • Objectives: To review the occurrence of cancer cluster in S Electronics semiconductor factories, process of epidemiological researches and lawsuit for industrial accident compensation. Methods: Occupational Safety and Heatlh Research Institute (OSHRI) epidemiological research report, Seoul National University (SNU) field survey report and Seoul Administrative Court ruling were investigated. Result:: OSHRI denied the association between working environment and outbreak of cancers, but SNU survey team reported that carcinogens do exist in workplace and the workers are exposed to them. Seoul Administrative Court ruled that 2 victims got cancer due to hazardous chemicals in working environment and be compensated but 3 victims were not acknowledged. Conclusions: Procedure of current OSHRI epidemiological research should be reassessed by the purpose of Occupational Accident Compensation Insurance Law.

화학공정산업에서 안전문화 이행과 영향 변수 (Safety Oimate Practice and its Affecting Variables in the Chemical Process Industry)

  • 이 논문의 주요 목적은 안전 분위기를 확인하여 공장과 근로자의 안전문화 수준에 영향을 주는 변수들을 찾아내고 더 나아가 화학공장의 안전문화를 향상시킬 수 있는 영향요소를 찾는데 있다. 이 논문에 사용된 질문표는 영국의 HSE(Health and Safety Executive) 의 질문표를 근간으로 문헌 조사를 통하여 경영자와 근로자로 분류하여 개발하였다. 안전문화 수준을 확인하기 위해 빈도수 분석을 하였으며 공장규모, 사고 발생, 사고 및 상해 경험과 심각성, 그리고 고용기간 등과 같은 영향변수들은 ANOVA(Analysis of Variance) 분석을 적용하여 테스트하였다. 빈도수 분석의 결과 일반적으로 경영자와 근로자 모두가 안전문화의 수준이 높게 나타났으며 주된 문제점들로는 부적당한 안전 보건 절차와 규칙, 생산에 대한 압력, 그리고 규칙위반으로 나타났다. 또한 ANOVA 분석결과에 의하면, '고용기간'은 다른 안전 문화의 수준을 만드는 유일한 변수인 것으로 나타났다. 그리고 안전문화의 조사로부터 공장들의 안전 문화요소에 대한 주요 3가지 문제점과 안전문화수준을 발견하였으며 안전 이행 능력을 향상시킬 수 있는 2가지 영향요소를 제안하였다.

A Study on Estimation of Radiation Exposure Dose During Dismantling of RCS Piping in Decommissioning Nuclear Power Plant

  • In the dismantling process of a reactor coolant system (RCS) piping, a radiation protection plan should be established to minimize the radiation exposure doses of dismantling workers. Hence, it is necessary to estimate the individual effective dose in the RCS piping dismantling process when decommissioning a nuclear power plant. In this study, the radiation exposure doses of the dismantling workers at different positions was estimated using the MicroShield dose assessment program based on the NUREG/CR-1595 report. The individual effective dose, which is the sum of the effective dose to each tissue considering the working time, was used to estimate the radiation exposure dose. The estimations of the simulation results for all RCS piping dismantling tasks satisfied the dose limits prescribed by the ICRP-60 report. In dismantling the RCS piping of the Kori-1 or Wolsong-1 units in South Korea, the estimation and reduction method for the radiation exposure dose, and the simulated results of this study can be used to implement the radiation safety for optimal dismantling by providing information on the radiation exposure doses of the dismantling workers.

경구용 서방성/지연성 성형제품의 허가 후 변경사항 관리를 위한 SUPAC-MR 응용 (Application of SUPAC-MR in Processing Postapproval Changes to Modified Release Sold Oral Dosage Forms)

  • The objective of this study was to scrutinize the rationale of SUPAC-MR and its application in processing postapproval changes to modified release solid oral dosage forms. The types of postapproval changes that were primarily covered with SUPAC-MR included variations in the components and composition, the site of manufacturing, batch size, manufacturing equipment, and manufacturing process. SUPAC-MR defined levels of postapproval changes that the industry might make. Classification of such categories was based on the likelihood of risk occurrence and potential impact of changes upon the safety and efficacy of approved drug products. In most cases, the changes could be classified into 3 levels. It described what chemistry, manufacturing, and control tests should be conducted for each change level. The important tests specified in SUPAC-MR were batch release, stability, in vitro dissolution, and in vivo bioequivalence tests. It then suggested what type of a filing report should be submitted to the FDA for each change level. In general, level 1 changes could be reported in an annual report, whereas level 2 and/or 3 changes could be submitted in changes-being-effected or prior approval supplements. It could be understood that the purpose of SUPAC-MR was to maintain the safety and quality of approved modified release solid oral dosage forms undergoing certain changes. At the same time, it contributed to providing a less burdensome regulatory process with the manufacturers when they wanted to make postapproval changes. European regulatory agencies also implemented SUPAC-like regulations in handling such changes to drug products. Therefore, in this study a recommendation was made for KFDA and the Korean industry to evaluate thoroughly the usefulness of these guidances and regulations in dealing with postapproval changes to modified release solid oral dosage forms.
