DENTAL CARE AND THE STATE OF EDUCATION AND TRAINING OF DENTISTS FOR THE HANDICAPPED (장애인의 치과진료 현황과 치과의사의 교육 및 훈련 정도에 대한 조사연구)

  • A survey was undertaken among a group of dentists working at private dental clinics to investigate the state of dental care and the level of education and training in the dental care to people with handicaps. Forty four percents of dental clinics take care of one more patients per month. The types of handicaps were physical, mental, sensory handicap and autism. The common problems are difficulty in communication and delay of treatment. Forty nine percents of dentists got education and nineteen percents had clinical experience for dental care of handicapped patients. Most of dentists felt necessity of training program and establishment of specialized dental hospital supported by government for the handicapped. This study indicated the necessity of undergraduate and postgraduate education and clinical training for dental care of handicapped patients.

Oncologist Perspectives on Breast Cancer Screening in India-Results from a Qualitative Study in Andhra Pradesh

  • Background: It is important to understand the perceptions of oncologists to understand the comprehensive picture of clinical presentation of breast cancer. In the absence of clear evidence, clinical practice involving patients of breast cancer in India should provide insights into stages of breast cancer with which women present to their clinics and mode of screening of breast cancer prevalent in Andhra Pradesh. Materials and Methods: A qualitative study was conducted to understand the perceptions of oncologists regarding clinical presentation of breast cancer, stages at which women present to clinics, and mode of screening of breast cancer prevalent in Andhra Pradesh. In-depth interviews (IDI) were conducted with ten practising oncologists from various public and private cancer hospitals in Hyderabad city to understand their perspectives on breast cancer and screening. The data were triangulated to draw inferences suitable for the current public Health scenario. Results: Late presentation was indicated as the most important cause of decreased survival among women. Most women present at Stage 3 and 4 when there is no opportunity for surgical intervention. The results indicate that there is a huge gap in awareness about breast cancer, especially in rural areas and among poor socioeconomic groups. Even despite knowledge, most women delay in reporting due to reasons like fear, embarrassment, cost, ignorance, negligence, and easy going attitude. Conclusions: It is important to improve awareness about breast cancer and screening methods for promoting early screening. The study inferred that it would be beneficial to establish cancer registries in rural areas. Also, the policymakers need to make key decisions which among three methods (breast self examination (BSE), clinical breast examination and mammography) can best be used as a screening tool and how to successfully implement population wide screening program to prevent mortality and morbidity from breast cancer in India.

Mass Mortality of Doctor Fish(Garra rufa obtusa) Caused by Citrobacter freundii Infection (Citrobacter freundii 감염에 의한 Doctor fish(Garra rufa obtusa)의 집단 폐사)

  • In this paper, we described a case of mass mortality of doctor fish from a private fish hatchery farm in Korea with a history of abnormal swimming behavior, diffuse bleeding on the skin and fins and sudden death caused by fish pathogenic bacteria, Citrobacter freundii. Twelve moribund fish fingerling samples were submitted to College of Veterinary Medicine, Seoul National University in October 2008 for diagnostic examination. Diagnostic results showed that the morphological and biochemical properties of the bacteria isolated from the moribund fish were C. freundii. The remaining diseased fish from the hatchery farm were given treatment based on our recommendation and successfully recovered.

Mass Mortality of Adult Koi Carp(Cyprinus carpio) in the Early Winter by Aeromonas hydrophila Infection (초겨울 비단잉어에서 발생한 Aeromonas hydrophila에 의한 대량 폐사)

  • Mass mortality was occurred in adult koi carp(Cyprinus carpio) at a private pond near Samchunpo city in the early November, 2007. Its water temperature was $11^{\circ}C$ and turbidity was very high. The fishes diseased were shown widely distributed hemorrhages, deep ulcerations on the whole body surface and severe erosions in the all fins. In necropsy, excess ascitic fluid was observed and spleen and kidney affected were enlarged. In microscopic observation, it was examined widely distributed severe dermal ulceration, multifocal gill lamella hyperplasia, severe fibrinous pericarditis, multifocal nephritis and hepatitis. The bacteria suspected were isolated using tryptone soya agar and identified as motile Aeromonas hydrophila as results of biochemical tests using API 20E and 20NE. According to previous reports, A. hydrophila infection were mainly occurred in fingerling at high temperature. However, this case exhibited that A. hydrophila infection can develop on adult koi carp even in the winter season.

Infantile Colic: A Survey of Physicians in Pakistan

  • Purpose: Infantile colic diagnostic criteria were established by Rome IV. A universally accepted management remains to be established. We aimed to evaluate diagnostic criteria, management strategies, and perceived regional prevalence of infantile colic in Pakistan, as well as its effect on physicians and parents. Methods: A questionnaire was distributed amongst 1,256 physicians. Results: We received 800 replies. Wessel and Rome IV criteria were used by most physicians for diagnosis; however, the response "any infant who cries a lot" was selected by older physicians (48% of those over 60 years), physicians in rural areas (32%), physicians practicing in private clinics (27%), and general physicians (30%). Estimated prevalence of infantile colic ranges from 21-40%. Reassurance was the most widely recommended management strategy followed by herbal teas (51%), switching to a different formula (49%), probiotics (28%) and antibiotics (26%), discontinuation of breastfeeding (14%), elimination of dairy products from the breastfeeding mothers' diet (6%), and the administration of colic drops (1%). Most physicians considered the negative impact of colic on their personal lives and the parents as mild-to-moderate. Notably, 38% of percent of physicians routinely screened for maternal depression, and 45% of physicians were aware of the association between infantile colic and shaken baby syndrome. Conclusion: Most physicians in Pakistan diagnose and manage infantile colic according to the established guidelines. However, the guidelines pertaining to treatment planning are not followed. Educational efforts directed toward general physicians and doctors practicing in rural areas and clinics must be implemented to avoid unnecessary testing and treatment burden.

The Effects of Health Education on Treatment Compliance of Patients with High Blood Pressure and Diabetes in Private Clinics (개인 의료기관에 내원한 고혈압 및 당뇨병환자에게 실시한 건강교육이 치료 순응도에 미치는 영향)

  • To provide fundamental educational materials for the effective management of patients with high blood pressure and diabetes in korea, and to expand the scope of such educational businesses the survey was administered between September 2007 and February 2008 to 1,213 consenting patients suffering from high blood pressure and diabetes in private medical centers within Daejeon in Korea. The patients were provided with standardized education materials and were monitored whether they were conducting a controlled management of their blood pressure and diabetes level as well as monitoring changes in their daily routine. Out of the 1,213 patients, 51.3% of the patients suffered only from high blood pressure, 26.3% suffered only from diabetes and 22.4% suffered from both high blood pressure and diabetes (Condor difference was not considered in this research). Physical education was the most commonly used education upon visitation to these medical centers, which was then followed by eating habits and weight management, respectively. The education demanded and preferred from the patients was similar to the education provided during the visitations. 56.7% of the medical doctors responded that the majority of the patients were passive in controlling and managing their illnesses. 63.7% of the participants believed that the education provided through this research was effective.

Population-related factors affecting the regional distribution of medical institutions in Korea (지역별 요양기관의 분포에 영향을 미치는 인구관련 요인)

  • Few public health researchers have paid research attention to the location of medical institutions in Korea. Previous studies were published in geography journals, and relied on limited data in terms of geographic regions and the type of medical institutions. This study utilized nationwide data covering 8 types of medical institutions. We obtained data from Health Insurance Review and Assessment Service and National Population and Housing Census. The correlation coefficients of resident, daytime, university-graduate population, and the population of different age groups (fewer than 15, 15~64, 65 or more) were compared to understand their relative association with the location of medical institutions. Medical clinic, dental clinic, oriental medical clinic, and pharmacy, all of which are almost completely operated by private sector, showed strong positive correlation with population. Hospital-level medical institutions, which are operated by both public and private sector, had moderate positive correlation. Daytime population and university-graduate population, rather than resident population, were more correlated with the location of medical clinics. The correlation coefficients of the population of 15~64 age group and the location of medical institutions were greater than that of other age groups. The results showed that daytime and university-graduate population are more important than resident population to explain the location of medicalrelated facilities. The results also suggests that the population of age groups (especially, 15~64) might be one of important influence factors in the location of medical institutions.

A Study on the Turnover of Dental Technicians (치과기공사의 이직에 관한 연구)

  • Having taken as cause of dental technicians separation from their service a variety of individual idiosyncrasies, function-related factors, external environment factors, functional satisfaction, organizational commitment, and intention to remain, the present study conducted a mail questionnaire poll and interviews on those dental technicians who are mow in the service with private dental technical workshops and dental technical workshops in general hospitals and private dental clinics. The resultant findings from analysis of the repondent's general characteristics of cause of separation form service were revealed, as follows : 1) It was shown that the higher the academic career, the considerably higher cause of position commitment among causes of individual idio-syncrasies, reflecting a statistically significant result(P<0.05) 2) The less the number of working hours as viewed from the average number of working hours per day, the higher the positive emotion, reflecting a significant relation(P<0.01) 3) Causes from performing duties among function-related causes diclosed a statistically significant relation among marital status, career, monthly income, and positions(P<0.01) 4) Causes from wages showed a statistically significant relation in the average number of working hours per day(P<0.01), monthly income(P,0.05), and positions(P<0.05) 5) Causes from supervision exposed a significant relation in workplaces(P<0.01) 6) Causes from promotion represented a significant relation in positions(P<0.001) 7) Causes from opportunnities for taking a job as seen from external environment causes also disclosed a statistically significant relation in workplaces(P<0.05) 8) Reponsibility for family sustenace reflected a statistically significant relation in sex, age, marital status, career, monthly income, work fields, term of employment, positions, and experience of separation(P<0.001) 9) Functional satisfaction showed a statistically significant relation in marital status, career, monthly income, and the average number of hours a day(P<0.05) 10) Oraganizational commitment disclosed a statistically significant relation in work experience, monthly income, term of employment, positions(P<0.05) and the average number of working hours a day(P<0.001) 11) The intention to remain indicated a statistically significant relation in age, marital status, work experience, monthly income, positions(P<0.001) and term of employment. the average number of working hours a day(P<0.01), and in the performing work field(P<0.05)

Comparative Analysis of Delivery Management in Various Medical Facilities (의료기관별 분만관리 양상의 비교 분석)

  • This study was conducted to compare the delivery management including laboratory tests, medication and surgical procedures for the delivery in various medical facilities. Two university hospitals, two general hospitals, three hospitals, two private obstetric clinics, and two midwifery clinics in a large city were selected as they permitted the investigators to abstract the required data from the medical and accounting records. The total number of deliveries occurred at these 11 facilities between 15 January and 15 February, 1989 was 789 among which 606(76.8%) were vaginal deliveries and 183 (23.3%) were C-sections. For the normal vaginal deliveries, CBC, Hb/Hct level, blood typing, VDRL, hepatitis B antigen and antibody, and urinalysis were routinely done except the private clinics and midwifery clinics which did not test for hepatitis B and Hb/Hct level at all. In one university hospital ultrasonography was performed in 71.4% of the mothers and in one general hospital liver function test was done in 76.7% of the mothers. For the C-section, chest X-ray, bleeding/clotting time and liver function test were routinely done in addition to the routine tests for the normal vaginal deliveries. Episiotomy was performed in 97.2% of the vaginal deliveries. The type and duration of fluid infused and antibiotics administered showed a wide variation among the medical facilities. In one university hospital antibiotics was not administered after C-section at all while in the general hospitals and hospitals one or two antibiotics were administered for one week on the average. In one private clinic one pint of whole blood was transfused routinely. A wide variation was observed among the medical facilities in the use of vitamin, hemostatics, oxytocics, antipyreptics, analgesics, anti-inflammatory agents. sedatives. digestives. stool softeners. antihistamines. and diuretics. Mean hospital day for the normal vaginal deliveries of primipara was 2.6 days with little variation except one hospital with 3.5 days. Mean hospital day for the C-section of primipara was 7.5 days and that of multipara was 7.6 days and it ranged between 6.5 days and 9.4 days. Average hospital fee for a normal vaginal delivery without the medical insurance coverage was 182,100 Won for the primipara and 167,300 Won for the multipara. In case of the primipara covered by the medical insurance a mother paid 82,400 Won and a multiparous mother paid 75,600 Won. Average hospital fee for a C-section without the medical insurance was 946,500 Won for the primipara and 753,800 Won for the multipara. In case of the primipara covered by the medical insurance a mother paid 256,200 Won and a multiparous mother paid 253,700 Won. Average hospital fee for a normal vaginal delivery in the university hospitals showed a remarkable difference, 268,000 Won vs 350,000 Won, as well as for the C-section. A wide variation in the laboratory tests performed for a normal vaginal delivery and a C-section as well as in the medication and hospital days brought about a big difference in the hospital fee and some hospitals were practicing the case payment system. Thus, standardization of the medical care to a certain level is warranted for the provision of adequate medical care for delivery.

Studies on Veterinary Acupuncture in Korea (한국 수의침술에 관한 조사연구)

  • Since successive operations under the acupuncture analgesia have been reported In China 1971, veterinary acupuncture was brought to the attention over the world and then its research and clinical use have markedly developed. Veterinary acupuncture and moxibustion was originated in China and Korea, but the research activities on them were not very significantly advanced. In this study, status of veterinarians on veterinary acupuncture and moxibustion, acupunctures which has been practiced by civil acupuncturist, therapeutic effects of acupuncture for the animal diseases in the field, and effects of electric acupuncture on the lumen motility in ruminants, etc. were Investigated. One-fifty of veterinarians of private clinics in Korea was interested in acupuncture and moxibustion in veterinary medicine. A lot of veterinarians required the scientific, systemic technique and knowledge or acupuncture. Civil acupuncturists who have been practiced acupuncture and moxibustion were 20 persons, and most of them were very old. They used chiefly three edged needles which were made by themselves and utilized old traditional technique in which the acupoints were stimulated and bled. Treatment with acupuncture for the digestive, muscular locomotive and febrile diseases was very effective. Until nowadays, the research on veterinary acupuncture in Korea has not much progressed. However, the Korean Societh of Veterinary Acupuncture and Moxibustion was organized in 1990. Valuable development for acupuncture through the society are expected. Electroacupuncture regional analgesia on the combinations of Yao-Pang 1, 2 and 4 acupoints was very effective for abdominal operations in cattle. But a lot of experiences and studies on the acupuncture anesthesia were required for clinical use in the future. Therapeutic effects of about 80% were obtained from electroacupuncture on the combinations of Tian Ping-Bai Hui-Shi Zhang, and Guan Yuan Yu acupoints in cattle with luminal atony and tympany In the field. Electroacupuncture on the combination of Tian Ping-Bai Hui-Qi Men, and Tian Ping-Bai Bui-Wei Gen for the treatment of downer cow was not so effective. The effects of stimulation and bleeding from acupoint by three edged needle was excellent for febrile diseases. The luminal contractile movement was accentuated by electroacupuncture on Tian Ping-Bai Hui-Shi Zhang combination and Guan Yuan Yu acupoint. It was concluded that veterinary acupuncture and moxibustion should be utilized for veterinary practice, and the advanced scientific research and clinical experiences must be followed.
