The Private Physicians' Opinions of Being Attending Physicians in Teaching Hospitals (개원의의 개방병원 참여에 대한 의견)

  • A mailed survey with structured questionnaire was conducted to study the demand of private physicians who were operating their own clinics in the community to be a attending physician at the general hospital. The responding proportion was 21.6 percent of the 960 private physicians. A total of 207 responders; 65.2 percent wanted to be a attending physician. In particular, the physicians who were male, young, surgeon and teaching hospital careered after specialist were more highly motivated. The major activities what they wanted as a attending physician were medical care for the admission patients. They responded that the hospital charges for the medical services and the responsibility of malpractice issues should be fairly shared by attending physician and hospital according to their contributions. There is growing consensus that the need of attending physician at the general hospital will become wide spread, but little organizational preparation to assure the quality of medical care of attending physicians including training of resident physicians and students. In addition, the effective reimbursement system should be develop to compensate appropriately according to the medical achievement of the attending physicians.

Ubiquitous Environment with Private Health Insurance and the Relevance of Length of Stay (유비쿼터스 환경에서 민영의료보험 가입과 입원기간과의 관련성)

  • The purpose of this study was to analysis of influence between of private health insurance and length of stay with cervical and lumbar simple sprain patients at Ubiquitous medical environment. They were diagnosed as having sprain of cervical and lumbar spine without neurological symptoms in hospitals and clinics from July 1, 2010 to February 29, 2012. This study was t-test, ANOVA and multiple regression analysis. The results were when the number of private health insurance was input as an independent variable, the subjects had one or two accidental insurances(${\beta}$=2.731, P=0.013), length of stay in beds was longer than they had no accidental insurance and when they had more than three ones(${\beta}$=4.991, P=0.000), length of stay was significantly longer. This study has a meaning in that it is domestic practical study which identified relationships between entry of private health insurance and length of stay.

The Impacts of Private Health Insurance on Medical Institution Selection: Evidence from Outpatient Service Utilization among Arthritis Patients (민간의료보험이 의료기관 종별 선택에 미치는 영향: 관절염 환자의 외래 이용을 중심으로)

  • Recently, with the increase in the number of private health insurance subscribers, interest in overuse of the medical service is increasing. This study analyzed the impacts of private health insurance (PHI) on medical institution selection in outpatient service utilization among persons with arthritis. In order to control patients' health status, we extracted outpatient episodes with the same disease (KCD6, M13) from Korea Health Panel. The unit of analysis was an outpatient visit with arthritis in 2014 (n=23,363). In the light of insurance coverage, we redefined three type of private health insurance (ex, indemnity, fixed benefit, and non-insured) as a test variable and two type of medical institution (ex, hospital and physician visit) as a dependent variable. We conducted a probit regression analysis to identify the impacts of PHI on medical institution selection controlling for heteroscedasticity. The results of this study showed that the insured with indemnity were more likely to choose hospital departments than clinics (marginal effect=0.0475, p=0.000). The impact of participation of fixed benefit PHI was not as clear as that of indemnity type (marginal effect=0.0162, p=0.047). In conclusion, this study confirmed that PHI, particularly indemnity type has a significant impact on the selection of medical institutions. Healthcare policy makers should consider that PHI not only affects the overall quantitative increase in healthcare utilization, but also influences the selection of medical institutions.

A Study on the Life Expectancy of the Dental Prosthetic Restorations (치과보철물의 평균수명에 관한 연구)

  • This study was performed to investigate the mean life expectancy of dental prosthetic restorations. The author has examined 352 dental prosthesis clinically and radiologically, and decided the success(survival) and failure(mortality) of the dental prosthesis. The dental prosthesis which had been treated in the Seoul National University Dental Hospital, two private clinics in Seoul, one university dental hospital, and two private clinics in local province were included in this study. The survival analysis using product limit estimator was used and the mean life expectancy of each type of dental prosthesis was calculated. The results were as follows : 1. The life expectancies were 10.5 years in gold crown and bridge, 8.5 years in porcelain fused to metal crown and bridge, 8.3 years in nonprecious metal crown and bridge, 8.1 years in removal partial denture, and 7.7 years in full denture. 2. The causes of mortality were in the order of dental caries(24.6%), fracture of dental prosthesis(19.2%), periodontal problems(18.6%), chronic chewing difficulty and dysfunction due to dental prosthesis(15.0%), excessive exposure of abutments due to the marginal defect of dental prosthesis(14.4%), abnormal occlusion due to severe attrition of artificial teeth in dentures(3.0%), periapical problems(2.4%), perforation of dental prosthesis(1.8%), and loose contacts with neighboring tooth(1.2%). 3. Among survival cases, 66.5% showed normal chewing ability and 31.9% showed partial chewing ability. However, 1.6% of them complained loss of chewing ability. 4. Among failure cases, 6.6% showed normal chewing ability and 38.9% showed partial chewing ability. However, 54.5% of them complained loss of chewing ability.

Hospital Admission Rates for Ambulatory Care Sensitive Conditions in South Korea: Could It Be Used as an Indicator for Measuring Efficiency of Healthcare Utilization? (한국의 의료기관 외래진료 민감질환 입원율: 의료이용 효율성 지표로의 활용 가능성?)

  • Background: Hospital admissions for ambulatory care sensitive conditions (ACSCs), which are widely used as an indicator of poor access to primary care, can be used as an efficiency indicator of healthcare use in countries providing good access to health care. Korea, which has a national health insurance (NHI) system and a good supply of health care resources, is one such country. To quantify admission rates of ACSC and identify characteristics influencing variation in Korean health care institutions. Methods: By using NHI claims data, we computed the mean ACSC admission rate for all institutions with ACSC admissions. Results: The average ACSC admission rate for 4,461 institutions was 1.45%. Hospitals and clinics with inpatient beds showed larger variations in the ACSC admission rate (0%-87.9% and 0%-99.6%, respectively) and a higher coefficient of variation (7.96 and 2.29) than general/tertiary care hospitals (0%-19.1%, 0.85). The regression analysis results indicate that the ACSC admission rate was significantly higher for hospitals than for clinics (${\beta}=0.986$, p<0.05), and for private corporate institutions than public institutions (${\beta}=0.271$, p<0.05). Conclusion: Substantial variations in ACSC admission rates could suggest the potential problem of inefficient use of healthcare resources. Since hospitals and private corporate institutions tend to increase ACSC admission rates, future health policy should focus on these types of institutions.

A Study on the Fear in Private Dental Clinics (치과내원환자의 치과 치료시 공포감에 관한 연구)

  • From April 9th to May 4th 2007, making a random selection of three private dental clinics in Busan, this study investigated the fear for dental treatment for 178 adults over twenty. Using four items of General Characteristics and DFS of twenty items, the degree of fear for dental treatment was evaluated by self administrated questionnaire. The obtained results were as follows. 1. Reliability analysis of each factor in details was 0.662~0.921 in Cronbach' alpha Coefficients. For Cronbach' alpha Coefficients was over 0.6. Dental Fear Survey Scale was reliable. 2. Dental stimulus response scale of three factors appeared more fear than two factors of the rest. The fear for dental treatment was higher in wemen, older ages, high education level and high income. 3. Factor of dental stimulus response scale, avoidance of dentistry and physiologic response scale accounted for 66.1%($R^2=66.1$)of the variation in dental fear factor as a result from stepwise regression analysis.

Feline Demographics and Disease Distribution in the Republic of Korea

  • The population of pet cats has increased significantly, from 0.3% in 2002 to 5.6% in 2017. Large-scale feline demographic and disease data from Korea are lacking. The aim of this study was to investigate the demographic data (breed, sex, and age) and disease distribution of cats who visited private veterinary practices in Korea. Data including breed, sex, age, and disease, were compiled from 32,728 electronic medical records from 30 selected private veterinary practices, between January 1, 2016, and December 31, 2017. Diseases were classified based on the International Classification of Diseases 11 by the World Health Organization, and then compared and cross-analyzed according to breed, sex, and age. Korean shorthair was the most common breed. There was a high distribution of young cats, with 77.6% of the cats under 4 years of age, and an average age of 2.5 years. Diagnoses related to preventative medicine were the most frequent and diagnoses common to young cats had higher incidence. This demographic data and information about disease distribution can be used as a basis for future research and may be helpful for determining priorities in the diagnosis of diseases and establishing strategies for health management in cats.

An Investigation to the General Characteristics of Doctors in Busan Area (부산지역 의사들의 특성에 관한 조사)

  • To find a basic study for manpower of physicians and medical care systems in Busan, author has studied towards 1,069 doctors who had taken the regular report in 1973, residing in Busan City. The survey was conducted from July 1 to August 31, 1973 and the findings & results obtained through the study for distributions, characteristics, employment of doctors and some medical system were summarized as follows; 1. The ratio between doctor and populations in Busan City was 1:1,887. 2. The doctors who graduated from Susan Medical College were 438 as the most proportion (40.9%). 3. Sex distribution of doctors revealed male 970, female 99 and those belonging to the 30-39 age group were as the most proportion (41.0%). 4. The doctors who had faith in Christianity were the highest (22.3%), 5. By the opening year of clinics, there was increasing tendency after 1950, especially during 1970-1973. 6. At that time of investigation, the doctors who had private clinics were 673 (67.3%) and nonemployees were 27 (2.5%) 7. The total Medical Specialists in Busan were 519, and among them the Internal Medicine Specialists were the highest proportion (16.2%). 8. The clinics employing the disqualified nurse-aids were 237 (22.2%). 9. Most of doctors (81.8%) had opposed attitude to the establishment of new medical colleges. 10. More than half of the doctors (59.8%) agreed to the functional division between Physicians and Pharmacists. 11. The ratio of agreement to the practical application of public medical insurances was 68.7%. 12. The opinion by opening hospital-clinics at Myeon for administrative measures for doctorless rural area was the highest proportion. 13. The doctors who replied as low state of confidence of the citizens to the doctors-themselves were 691 (64.6%).

Medical Care Utilization of Tuberculosis Patients in Private Sector (민간의료기관을 이용하는 결핵환자의 의료이용 분석)

  • In this study we analyzed the insurance claims data to investigate the medical care utilization pattern of tuberculosis patients in private sector. We selected the claims of principal or secondary diagnosis with tuberculosis from claims database of National federation of Medical Insurance, from December 1995 to November 1996. Both spell-based analysis and person-based analysis were carried out. In spell-based analysis, type and location of treatment facilities, distribution of diagnoses, number of outpatient/inpatient treatments were analyzed. Additionally in person-based analysis, number of tuberculosis patients, demographic characteristics, number of treatments per person, frequency and pattern of change in source of care were analyzed. The results were as follows 1. The number of treatments with tuberculosis was 863,641 from 1 December 1995 to 30 November 1996. The number of patients was 313.964. 2. Most of tuberculosis patients in private sector were treated in general hospital (45.8%) and clinics(42.2%) 3. About 77.7% of tuberculosis patients who were treated more than two times did not change the source of care. 18,9% of tuberculosis patients changed source of care only once. Even when we limited tuberculosis patient to those who were treated more than five times and whose treatment period were longer than six months, 94.7% of patients did not change source of care at all, or changed treatment facility only once. 4. The probability of change in source of rare was higher in pulmonary tuberculosis, in twenties, and in rural area respectively than other tuberculosis. In conclusion, healer shopping of tuberculosis patients was not serious as expected. However special attention is needed to pulmonary tuberculosis in twenties and rural area.

Seroprevalence of Babesia gibsoni in Companion Dogs in Korea by Enzyme Linked Immunosorbent Assay using Recombinant BgTRAP Antigen

  • This study assessed the seroprevalence of Babesia gibsoni in companion dogs in Korea by enzyme linked immunosorbent assay using recombinant BgTRAP antigen. Dogs were randomly selected from those admitted for various reasons to local private veterinary hospitals and the Animal Medical Center of Chonbuk National University. With the owners' permission, extra blood was drawn from each dog for serological assays. Of the 188 selected dogs, seven (3.72%) were positive for B. gibsoni, including six of 167 (3.59%) indoor and one of 12 (8.33%) outdoor dogs. Of the seven dogs positive for B. gibsoni, four were aged > 10 years, two were < 1 year, and one was between 1 and 10 years; and two were Yorkshires and one each was Shih-tzu, Maltese, Pekinese, beagle and mixed. Concurrent diseases or chief complaints were anemia in two dogs, both of which had a history of confirmed babesiosis by polymerase chain reaction, and non-anemic diseases in five. Geographically, four dogs were from Jeonbuk/Jeonju, and one each from Seoul, Gyounggi-do, and Jeonnam/Gwangju. To our knowledge, this is the first report of companion dogs in Korea being seropositive for B. gibsoni. Serologic screening of subclinical or carrier dogs can detect this potentially dangerous disease and assess its epidemiology.
