아동혈압의 시계열 변화 양상 및 평균혈압에 관련된 요인 분석 (The Change of Children's Blood Pressure and Factors Affecting the Level of Blood Pressure In Children)

  • To show the distribution and change of blood pressure according to age and to find factors affecting the level of blood pressure in primary school children, a follow-up study was conducted from 1986 to 1989 on 401 first grade children attending primary school in Kangwha County in 1986 and their parents. The blood pressure of the children was significantly increased according to age. The average annual increase was 1.8mmHg in systolic blood pressure and 2.5mmHg in diastolic blood pressure. The level of blood pressure did not show any significant difference in both sexes. Among children who were at or above the 80th percentile of blood pressure in the first grade, 35 and 30% of them have remained at the same level of systolic and diastolic blood pressure respectively in the fourth grade. But we could not find any significance in the tracking of blood pressure of children who were at or above the 90th percentile of blood pressure in the first grade. Weight and pulse rate were shown to be significant factors affecting systolic blood pressure in children of both sexes and mother's blood pressure and skinfold thickness were also affected systolic blood pressure in girls. The variables significantly affecting diastolic blood pressure were arm circumference and pulse rate for boys and height and pulse rate for girls.

Prognostic factors in children with extracranial germ cell tumors treated with cisplatin-based chemotherapy

  • Purpose: To evaluate the outcomes and prognostic factors in children with extracranial germ cell tumors (GCTs) treated at a single institution. Methods: Sixty-six children diagnosed with extracranial GCTs between 1996 and 2012 were included in the study. Primary treatment was surgical excision, followed by six cycles of cisplatin-based chemotherapy. The survival rates were compared according to the International Germ Cell Cancer Cooperative Group classification used for GCTs in adults to validate the classification guidelines for GCTs in children. Results: The median patient age was 4.4 years. In 34 patients (51.5%), the primary tumor site was the gonad. Extragonadal GCTs were detected in 32 patients. The 5-year overall survival and event-free survival (EFS) were $92.0%{\pm}3.5%$ and $90.4%{\pm}3.7%$, respectively. In univariate analysis, tumor histology, metastasis, and elevated alpha-fetoprotein were not prognostic factors in children with extracranial GCTs. However, EFS was poorer in patients with mediastinal disease (n=12, $66.7%{\pm}13.6%$) than in those with nonmediastinal disease (n=54, $96.0%{\pm}2.8%$) (P=0.001). The 5-year EFS was lower in patients older than 10 years, (n=21, $80.0%{\pm}8.9%$) compared with those younger than 10 years (n=45, $95.2%{\pm}3.3%$) (P=0.04). Multivariate analysis identified the mediastinal tumor site as the only independent prognostic factor. Conclusion: The prognosis of children with extracranial GCTs was favorable. However, nongerminomatous mediastinal tumors were associated with poor survival in children. Further research is needed to improve the prognosis of children with malignant mediastinal GCTs.

국민학교 아동의 온도개념 형성에 관한 조사 ("A survey of Elementary School Children,s Concept of Temperature")

  • This students ideas in science are diverse and unique It is realized that children's preconceptions and misconceptions established before lessoning hgve a crucial in fluence on the following education. so it is meaningful to analyse the children's concept of temperature for the better teaching strategy in this study. This survey of the Elementary school children's concept is designed for the subtopics of temperature as the relation between heat and temperature, He relation between volume(size) and temperature, the temperature of change of state on water, the temperature of substances in the condition of thermal equilibrium. Using Clinical method, this research was executed to 306 children at elementary school. The network method or the analyse of questionnaires were used to analyse the children's response. Findings of this survey are as follow. Students are already familiar with such concept as this increase of temperature by geating, but they think every substance is not the case. Many students appears to believe that the temperature of an object(substance) is related to its size(vloume) Qualitative tasks are difficult than qualitative ones. This trend appear highly in the low grade students. Don't know the temperature of change of state on water and it's stability They think that the temperature is determined by the heating time(period) and the temperature of heating source. Students think, in general that temperature of substance in the condition of thermal equilibrium is different.

초등학교 아동의 치아우식성 식품섭취 및 영양교육 효과에 관한 연구 (A study on the intake of foods causing dental caries and the effect of nutrition education for primary students)

  • 본 연구는 초등학교 아동의 식습관, 간식습관 및 섭식실태를 급식교와 비급식교에 따라 비교 분석함으로서 치아우식증 발생과의 관련성을 규명하고, 조사대상 아동들에게 영양교육 프로그램을 실시한 후 교육효과를 평가하고자 실시되었다. 1. 급식여부에 따른 식습관 조사 결과, 급식교 아동이 비급식교의 아동보다 아침식사의 규칙성, 식사속도, 식사량의 적당성 및 식사시간의 규칙성에 있어 바람직한 편이었고, 고기나 생선, 해조류, 우유나 유제품의 섭취정도도 약간 높게 나타나 급식을 통하여 올바른 식생활 태도가 형성되는 것으로 보여졌으며 전반적으로 영양교육 후 식습관에 있어 긍정적인 변화를 나타내 영양교육의 효과가 있었음을 알 수 있었다. 2. 카라멜, 초콜렛 및 사탕류, 비스켓류 등의 치아우식유발 지수가 높은 우식성 식품을 간식으로 자주 섭취하고 있었고 충치유발 요인 인지 시 간식을 덜 사먹겠다는 아동이 급식교에서 다소 많았으며, 물리적 청정작용을 하는 껌을 간식으로 씹는 경우는 1주일에 2∼3회가 가장 높게 나타났다. 또한 영양교육 후 치아우식에 영향을 줄 수 있는 시간인 저녁 식사 후에 간식을 먹는 아동과 TV 광고로 인해 간식을 선택하는 아동의 비율이 낮아졌으며, 세정식품인 채소와 과일, 단백질, 해조류, 우유 또는 유제품의 섭취는 교육 후 약간 증가하였다. 3. 치아우식과 관련이 있는 당질군을 제외한 모든 식품군에서 비급식교 아동보다 급식교 아동의 섭취횟수가 높게 나타났으며 1인 1일 우식성 식품의 평균 섭취 횟수는 평균 5.04회였고, 고체식품보다는 액체식품으로 섭취하는 횟수가 다소 높았다. 치아표면에서 청청작용을 하여 치면세균막의 형성을 감소시켜 우식증을 예방하는 세정식품의 평균 섭취 횟수는 1인 1일 기준식품 표준섭취 횟수보다 높은 9.33회로 나타났다. 1인 1일 우식발생 가능 시간은 100.9분이었는데, 액체 식품의 우식발생 가능 시간은 56.2분으로 44.6분인 고체 식품에서 보다 높게 나타났다. 이상의 결과에서 식습관이 치아우식발생과 밀접한 관계를 가지고 있으므로 초등학교 아동에 있어 치아우식을 유발할 수 있는 간식의 섭취를 줄이되 우식성 식품을 간식으로 섭취한 후에는 잇솔질을 반드시 실시하도록 하며 정규식사에서 치아를 보호하는 세정식품의 섭취에 대한 영양교육이 지속적이고 단계적으로 꾸준히 이루어져야 한다고 본다.

일부 초등학생의 치아우식활성검사를 이용한 계속구강건강관리 효과 (The Result of Oral Health Care on Primary School Children Using Dental Caries Activity Test)

  • 보건의료분야의 제반환경의 개선에도 불구하고 국민들의 구강건강상태는 아직도 OECD국가 중에서 매우 열악한 상태이다. 국민들의 구강건강을 효율적으로 증진유지하기 위해서는 중대구강병인 치아우식증을 예방하고 조기에 치료할 수 있는 구강보건진료가 이루어져야 한다. 특히 평생구강건강유지에 중요한 연령층인 초등학교학생들을 대상으로 정기적인 계속구강건강관리제도가 시급히 필요하나 초등학생 대상 계속구강건강관리방식의 치아우식예방관리법의 효과에 관한 사례가 부족한 실정이다. 따라서 광주광역시 소재의 S 치과에 2005년 12월부터 2006년 2월에 내원한 만6세에서 13세 환자 중 환자보호자의 동의를 받은 총 39명을 대상으로 실험군과 대조군으로 분류하여 치아우식활성도와 치아우식경험도를 조사하여 지역사회 치과의원에서 기초자료로 활용할 목적으로 시행한바 다음과 같은 결과를 얻었다. 1. 계속구강건강관리군의 기간별 Alban's test 및 Lactobacillius test 점수는 6개월경과 후 초진시의 1.4배에서 1.5배 감소되었고, 12개월경과 후 초진시의 1.5배에서 1.6배 감소되었다(p<0.001). 2. 계속구강건강관리군의 기간별 간이구강위생지수는 6개월과 12개월경과 후 초진시의 1.6배 감소되었다(p<0.05). 3. 12개월경과 후 우식경험영구치지수는 계속관리군(1.88개)에서 대조군(2.72개)보다 1.4배 감소되었으며, 우식영구치지수는 계속관리군(0.07개)에서 대조군(0.85개)보다 12배 감소되었다(p<0.05). 4. 12개월경과 후 제1대구치건강도는 계속관리군(98.04%)에서 대조군(95.97%)보다 1.02배 높게 나타났다(p<0.05). 이상과 같은 결과에 의하여 지역사회 치과의원에서 지속적인 계속구강건강관리제도를 시행하여 아동의 평생구강건강상태를 유지시켜야 한다고 사료되었다.

시설보호 아동의 스트레스 대처행동과 행동문제간의 관계 (The relationships between institutionalized children's stress coping behaviors and behavior problems)

  • The purpose of current study was to investigate the relationships between institutionalized children's stress coping behaviors and behavior problems. The subjects were composed of 80 primary school 4th, 5th, 6th graders(45 boys, 35 girls) and 55 middle school 1th, 2th graders(42 boys, 13 girls) who were institutionalized in jeonju. The major findings were as follows: 1) The institutionalized children's stress coping behaviors were significantly different between the groups based on gender, grade, and period in institution. 2) The institutionalized children's problem behaviors were statistically different between the groups based on gender and period in institution. 3) In regression analysis models, active or passive coping strategies were negatively or positively related to internalized behavior problems respectively. Furthermore period in institution was positively related to externalized behavior problems.

  • Recently, due to the child-friendly city project being conducted by UNICEF, the movement to create a child-friendly cities has been actively promoted by the South Korean government. However, due to the lack of specific physical plans to improve the basic rights of children in the city, the project has been mainly implemented for educational and institutional projects. In this study, we investigated the activities of children and parents living in elementary school neighborhoods. Based on our investigation, we attempted to generate the desired results that can be referred to by physical planners looking to make changes to school neighborhoods by comparing and analyzing children and parents' activity range and life patterns in the neighborhood. The GPS and traffic log surveys were conducted as the primary research methods. The results of this study are as follows: First, both children and parents are most likely to walk in their neighborhoods. In addition, the use of private cars and school vehicles was high, but the use of public transportation was low. Second, the longer parents and children stayed together, the wider the range of their activities. Third, children who spent a lot of time with parents were more likely to have diverse life patterns than children who did not spend a lot of time with parents. Fourth, parents who live in elementary school neighborhoods frequently used commercial facilities and cultural centers around the school. Therefore, it is important to construct facilities around the school for parent-use.

  • What characteristics of computer games encourage persistence, engagement, and learning for both girls and boys\ulcorner The purpose in conducting this research was to identify characteristics of computer games that engage all children-girls and boys- in significant mathematical learning. A set of criteria for evaluating gender fair computer games was developed. Much of this research had involved observing children playing computer games in pairs and small groups. Two game softwares were analyzed by way of dialogues that occurred with the games.

학령기 아동의 행복감에 영향을 미치는 요인에 대한 종단 연구 (A Longitudinal Study of Factors Associated with Happiness in Primary School Children)

  • 본 연구의 목적은 학령기 아동의 생태체계요인이 행복감에 미치는 영향을 종단적으로 확인하고자 하는 것이다. 특히 애착이론과 생태체계이론에 근거하여 그 영향력을 살펴보고자 하였다. 이를 위해 한국청소년패널조사(KYPS)의 초4패널자료를 활용하여 잠재성장모형으로 분석하였다. 또한 성별에 따른 차이를 살펴보기 위해 다집단분석을 실시하였다. 분석 결과, 첫째 학업성취, 자아존중감, 부모애착, 또래애착, 교사애착, 지역사회애착, 행복감은 5년 동안 감소추세로 나타났으며 초기값과 변화율의 분산값이 유의하여 개인 간의 차이가 있는 것으로 나타났다. 둘째, 생태체계요인 중 학업 성취를 제외하고 모든 변인이 행복감에 영향을 미치는 것으로 나타났다. 초등학교 4학년 시점에 자아존중감, 부모애착, 또래애착이 높을수록 당해년도 행복감이 높은 것으로 나타났다. 지역사회애착 초기값이 높을수록 행복감은 느리게 감소하는 반면에 5년 동안의 자아존중감, 부모애착, 교사애착, 지역사회애착이 빠르게 감소할수록 행복감이 빠르게 감소하는 것으로 나타났다. 셋째, 다집단분석을 한 결과 성별에 따른 차이가 없는 것으로 나타났다. 이러한 연구결과들을 바탕으로 학령기 아동의 행복감에 대한 이론적 및 실천적 함의를 제시하였다.
