Do Knots Matter in Superior Labrum Anterior to Posterior Lesions Repair?

  • Background: In general, the outcomes of arthroscopic repair for superior labrum anterior to posterior lesions (SLAP) are favorable, however, persistent pain and limitation of motion are not rare complications. One of the possible cause is a "knot-ache". This study evaluated the results of reoperation of symptomatic recurrent SLAP lesions and asked whether the knot is associated with postoperative complications. Methods: Between 2005 and 2015, a total of 11 patients who had undergone arthroscopic SLAP repair were reoperated for recurrent symptomatic SLAP lesion. By retrospective chart review, operative findings, the visual analogue scale for pain (pVAS), the range of motion (ROM), and functional scores were analyzed. Results: The mean age of the study participants was 38.3 years, and the mean follow-up period was 42.5 months. In the primary operation, there were nine cases of repairs with conventional knot-tying anchors and three cases with knotless anchors. Impingement of the knots during abduction and external rotation of the shoulder was observed in the all cases with knot-tying anchors. The mean pVAS, ROM, and functional scores significantly improved with reoperation. At the final follow-up, the mean satisfaction VAS was 8.3. Conclusions: The knots of suture anchor maybe a possible etiology of the pain, which we termed a "knot-ache". Considering that reoperation is performed due to pain after primary repair, the use of knotless suture anchor may have benefits of eliminating one of possible cause, "knot-ache". Therefore, authors suggest the use of knotless anchors during reoperation for recurrent or recalcitrant pain after primary SLAP repair.

보수용접에 따른 이종금속 용접부의 잔류응력 해석 (Residual Stress Analysis for Repair Welding in Dissimilar Metal Weld)

  • Alloy 600 and Alloy 82/182 materials have been used widely in PWR plants. But these materials are known to be susceptible to PWSCC(Primary Water Stress Corrosion Cracking). Recently, there have been several PWSCC events in major components due to repair welding, because repair welding in the dissimilar metal welds during the construction increases residual stress significantly on the inner surface of welds. In this paper, various residual stress analyses for repair welding were performed using FEM to check the effect of repair welding on residual stress distributions in PZR safety/relief nozzle. The results indicate that for inside surface repair welding, high tensile residual stress is developed on the inside surface of the nozzles.

Reconstruction Techniques for Tissue Defects Formed after Preauricular Sinus Excision

  • Background Preauricular sinuses are congenital abnormalities caused by a failure of fusion of the primitive tubercles from which the pinna is formed. When persistent or recurring inflammation occurs, surgical excision of the infected tissue should be considered. Preauricular defects inevitably occur as a result of excisions and are often difficult to resolve with a simple suture; a more effective reconstruction technique is required for treating these defects. Methods After total excision of a preauricular sinus, the defect was closed by a plastic surgeon. Based on the depth of the defect and the degree of tension when apposing the wound margins, the surgeon determined whether to use primary closure or a posterior auricular flap. Results A total of 28 cases were examined. In 5 cases, including 2 reoperations for dehiscence after primary repair, reconstruction was performed using posterior auricular transposition flaps. In 16 cases of primary closure, the defects were closed using simple sutures, and in 7 cases, closure was performed after wide undermining. Conclusions If a preauricular defect is limited to the subcutaneous layer and the margins can be easily approximated, primary closure by only simple suturing may be used to perform the repair. If the defect is deep enough to expose the perichondrium or if there is tension when apposing the wound margins, wide undermining should be performed before primary closure. If the extent of the excision exposes cartilage, the procedure follows dehiscence of the primary repair, or the tissue is not sufficiently healthy, the surgeon should use a posterior auricular flap.

Bilateral cleft lip repair with simultaneous premaxillary setback and primary limited rhinoplasty

  • Background: Functional closure of the orbicularis oris muscle and esthetic reconstruction of nasolabial components are impossible in patients with severely deformed premaxilla. Here, we review a surgical strategy for patients with unremedied premaxilla retrospectively. Results: Vomerine ostectomy and premaxillary setback with nasolabial repair were performed in 12 patients with bilateral cleft lip and palate. The mean age of patients was 21.7 months. The extent of ostectomy varied between 3 and 11 mm. There were no serious complications from defective perfusion to the premaxilla or the philtral flap. The follow-up period ranged from 2 to 25 months. Proper positioning of the premaxilla and satisfactory nasolabial esthetics were achieved in all patients. Conclusions: We performed nasolabial repair after premaxillary setback without jeopardizing the premaxillary segment or the philtral flap. Our surgical strategy could be recommended in poor socio-economic circumstances due to the cost effectiveness of limiting the number of surgeries.

영아기의 심실중격결손에 대한 이라완전교정술과 단계교정술의 비교 (Surgical Management of Ventricular Septal Defect in Infancy)

  • Eighteen infants with a large ventricular septal defect[VSD] underwent primary surgical repair from January 1986 to December 1992. Operation was done because of failure to thrive, medically intractable heart failure, recurrent pneumonia, increased pulmonary vascular resistance[PVR]. Four patients[22.2%] died in the early postoperative period. Relief of heart failure and normalization of growth and weight gain was evident in all survivor. There was no late postoperative death. The results of primary surgical repair of VSD in infancy are compared with those of palliative pulmonary artery banding[PAB] and of VSD closure after PAB. Twenty-seven patients with isolated VSD or with VSD associated with atrial septal defect, patent ductus arteriosus, or coarctation of the aorta underwent initial palliative PAB. There were 3 early postoperative deaths[11.1%]. Severe elevation of PVR persisted in two patients. Closure of VSD and pulmonary artery debanding was done in twenty patients, with 2 early postoperative deaths[10.0%]. Placement of the PAB too close to the pulmonary annulus necessitated trasannular patching in one patient, but any problem caused by migration of the band was not developed. It is concluded that primary surgical repair of VSD in infancy is reasonable and that PAB is indicated only for those patients less than 6 months old with a complicated defect or in an emergency situation.

초.중등 교육시설의 유지관리비 분석 - 인천광역시 초.중등학교를 중심으로 - (The Analysis of Maintenance & Repair Cost in Educational Facilities of Primary and Secondary Schools - Focusing on Primary and Secondary Schools in Incheon Metropolitan City -)

  • The purpose of this study is to provide a basis for calculation of school operation fees for normal functioning of the school facilities, and facility maintenance and repair cost through their life spans, considering remodeling of the facilities and changes of their functions. This study analyzes maintenance and repair records during the three years from 2003 to 2005 in primary, middle and high schools in Incheon Metropolitan city and also the facility management and operation expenditure of the same schools in the city during the year of 2005, and presents quantified unit cost of each item according to the area, the age of the facilities, and the number of classroom, student, and lessons given. The calculation can be a basis for both BTL school facility managers and the general school facility mangers to budget school facility maintenance cost appropriately and the quantity of energy, water, electricity, and telephone fees calculated here can be a criteria for the managers to check the normal maintenance of their facility.

  • Repair of concrete structures has been a focus of attention in recent years not only in Japan but also worldwide. Concrete structures have fallen short of expectations at the time of construction-they should have been perpetual and maintenance-free. However, investigation into their premature deterioration reveals the primary causes: Concrete was made using inadequate materials and/or inadequate mixture proportions due to insufficient consideration and was placed inadequately under insufficient execution control. The secondary causes include insufficient consideration at the time of construction for the environmental conditions to which the structures were to be exposed. In any event, in the current economic climate, structures cannot be demolished and rebuilt as soon as they are damaged, but instead are expected to continue to be in service as long as possible with appropriate repair or retrofitting. This paper analyzes the causes of deterioration requiring repair and introduces relevant repair techniques. At the end, the repair project of the Sanyo Shinkansen Line in which the author was involved through committee activities is reported.

변형 Martius 구해면체 지방피판술을 이용한 신생방광-질루의 치험례 (Neobladder-vaginal Fistula Repair with Modified Martius Bulbocavernosus Fat Pad Flap)

  • Purpose: In developed countries, vesicovaginal fistula occur from various pelvic operations including total hysterectomy, leading to urinary leakage and incontinence. Although various methods have been proposed for adequate tissue coverage in fistula repair, the surgical treatment of is not simple and still controversial. We report a case of neobladder-vaginal fistula repair using modified Martius fat pad flap. Methods: A 62-year-old female patient underwent radical cystectomy with total abdominal hysterectomy and neobladder formation due to invasive bladder tumor 5 years ago. For 3 years following the operation, urine leakage was observed. Exploration demonstrated neobladder-vaginal fistula and primary repair including fistulectomy and direct closure was performed. Urinary incontinence relapsed 2 years after primary repair, and after demonstrating the recurrence of fistula on urography, repair of recurrent fistula was performed. After dissection of vagina and neobladder and closure of fistula by urologic surgeon, fibroadipose flap was elevated, rotated and advanced through the tunnel at vaginal sidewall, and interpositioned to the fistula site between neobladder and vagina. Results: There was no acute complication after the surgery and urethral catheter was extracted on the 8th day after the operation. During six month follow-up period after the operation, there is no clinical evidence of fistula recurrence. Conclusion: From our clinical experience and literature review, we think Martius fat pad flap is a useful technique in management of neobladder-vaginal fistula, for it provides enough vascularity, major epithelization surface and better lymphatic drainage, and also prevents overlapping of vesical, vaginal suture lines at the same time.

식도천공 후 만기 일차 봉합술의 성적 (Delayed Primary Repair of Esophageal Rupture)

  • 식도 천공후 수술시까지 24시간이상이 경과된 환자의 치료법은 아직도 논란이 많다. 1990년이후 연세 대학교 흉부외과학 교실에서는 식도 천공후 24시간이상 경과된 환자 10예중 전예를 일차 봉합술로 치료 하였다. 그중 4명은 인위적인 천공 이었고, 3명은 자연성 천공, 2명은 이물의 연하, 나머지 한명은 외상이 원인 이었다. 식도 천공후 수술시까지 소요된 시간은 평균 116시간 이었고 중앙값은 48시간 이었다. 봉합방법은 천공된 점막부위의 위 아래로 정상 점막이 나올때까지 식도근 절개술을 시행한 후 염증이 있는 점막부위를 변연 절제한다. 식도의 원위부 폐쇄가 있을 경우 수술대에서 식도 확장술을 시행하고 점막과 근육층을 단층 혹은 층층 봉합한다. 봉합부위 위에 늑막이나 심낭 지방을 이용하여 봉합을 강화하였다. 수술 사망은 1예에서 수술후 33일째 발생 하였는데 사망원인은 위괘양의 합병증으로 위출혈이었다. 술후 5명의 환자에서 봉합부위의 유출이 있었지만 사망한 1예를 제외하고 모두 고식적 치료로 완치할 수 있었다. 식도 천공 환자에서 치료법은 여러 가지 방법이 있겠으나 비록 수술시까지 시간이 많이 지났다하더라도 천공의 원인이 양성이고 원위부 폐쇄가 교정될 수 있다면 일차 봉합과 적절한 배농이 우선 되어야 할 것이다.

Surgical Treatment of Degenerative Mitral Valve Regurgitation in the Elderly: Comparison of Early and Long-Term Outcomes Using Propensity Score Matching Analysis

  • Background: It is unclear whether mitral valve (MV) repair for degenerative mitral regurgitation (MR) provides the same advantages in the elderly that it does in the general population. Methods: From 1994 to 2016, 188 elderly patients (mean age, $68.3{\pm}5.50years$) underwent MV repair (n=153) or MV replacement (n=35) for primary degenerative MR. Early and long-term outcomes were compared before and after propensity score matching (PSM). Results: Before PSM, there was a significant difference in operative mortality (p=0.011). Overall survival and freedom from cardiac-related death (CRD) at 5, 10, and 15 years were significantly higher in patients who underwent MV repair (p=0.039 and p=0.007, respectively). In the multivariable analysis, MV replacement was an independent risk factor of CRD. After PSM, operative mortality was not significantly lower in patients who underwent MV repair (p=0.125). Overall survival and freedom from CRD at 5, 10, and 15 years showed no significant difference between the 2 groups in the PSM cohort (p=0.207, p=0.47, respectively). There was no significant difference in freedom from reoperation before or after PSM (p=0.963 and p=0.575, respectively). Conclusion: MV repair for primary degenerative MR might be a valid option in the elderly population if successful repair is possible.
