Wolf-Hirschhorn syndrome (WHS)은 4번 염색체 단완의 결실로 발생하며, 지능 및 성장 발달의 저하, 경련, 선천성 심기형과 특징적인 두개안면기형 등을 초래하는 질환이다. 사이가 넓고 돌출된 미간과 코의 기저부가 넓은 특징을 보이는 얼굴 모양은 그리스 전사의 투구(Greek warrior helmet appearance) 와 비슷한 모양을 보인다. 구강 내에서는 구순열, 우상치아, 원뿔형 치관, 다수의 결손치와 이로 인한 만기 잔존 유치 등의 소견을 보인다. 본 증례는 충치치료를 주소로 내원한 9세 여환으로 타병원에서 WHS으로 진단받았으며, 성장 지연, 정신 박약 및 WHS의 특징적인 얼굴 형태를 보였다. 구강 검사를 통해 다수의 영구치 결손과 만기 잔존된 유치 및 다수의 충치를 관찰하였으며, 환자의 협조도 미약으로 전신마취 하에 충치치료 하였다. 다수의 영구치 결손으로 현재 맹출한 영구치 및 유치의 관리가 중요하고, 이를 위해서는 구강 위생 관리 및 불소도포를 위해 주기적인 치과 검진이 필요하다.
치아의 맹출은 치아가 구강내 교합평면에 도달하여 기능적인 위치를 차지할 때까지의 치아 이동 및 골조직 내에서의 발달로 이루어지는 생리학적인 양상으로 이해되어진다. 그러나 치아가 정상적으로 맹출해야 하는 시기를 지나서도 골조직 내에서 맹출하지 않을 수 있으며, 이러한 치아를 매복치라 일컫는다. 이러한 치아 매복의 주된 요인은 국소적인 것으로 악궁내 공간 부족, 치아 위치 이상, 과잉치, 맹출 경로의 감염, 낭종 및 안면 외상으로 인한 치배의 영향 등이 있다. 또한 쇄골두개이골증, 골다공증 등의 전신적 그리고 유전적 질환도 맹출 장애와 맹출 지연 등과 관련이 있다. 현재까지 매복치의 대부분은 영구치로 보고되었으며 반면에 유치의 매복에 대한 것은 극히 소수에 불과하고, 매복된 유치의 대다수는 제 2 유구치이다. 유치의 매복은 유착에 의해 이차적으로 생긴 저위교합과는 구별되어야 하며, 유치의 매복에 대한 병인은 유치 치배의 비정상적인 발달로 인해 조기에 유착이 일어나는 것으로 추측되어지고 있으나, 아직까지 정확하게 밝혀진 바는 없다. 유치의 맹출 실패로 나타날 수 있는 문제점으로는 계승 영구치 발달 및 맹출 방해, 낭종 형성이나 감염의 발생 등을 생각해 볼 수 있다. 본 증례들은 연세대학교 치과대학병원 소아치과에 내원한 환아의 구강 및 방사선학적 검사에서 유구치의 매복이 관찰되었기에 이에 보고하는 바이다.
This study was undertaken to evaluate the pulpal responses to the pulp-capping materials such as glutaraldehyde and formocresol in pulpotomy technique, especially in the primary dentition. Mandibular primary canines and molars of 5 dogs (aged about 8-9 weeks)were selected for this study. The intervals of observation for histologic study of pulpotomized primary teeth with 2% glutaraldehyde, formocresol and calcium hydroxide in the usual manner ranged from 2 hours, 1 week, 2 weeks, 3 weeks and 5 weeks after experiments respectively. Each specimens were fixed with 10% formalin and decalcified in 5% nitric acid. All slides were stained with Hematorylin-Eosin and examined histopathologically. The results were as follows; 1. In calcium hydroxide groups, formation of dentin bridge was initiated in 1 week after experiments and completed in 5 weeks after experiments. 2. Formation of dentin bridge was not seen, whereas necrosis of pulp tissue was noted, in formocresol and glutaraldehyde groups. 3. Duration of tissue reactions and tissue changes were similar, in formocresol and glutaraldehyde groups. 4. In formocresol and glutaraldehyde groups, amputation surfaces of the pulp were covered with blood clots, beneath which coagulation necrois was noted, but inflammatory cells were not prominent, in 2 hours and 1 week after experiments. But coagulation necrosis was proceeded to the apical portion, accompanied by infiltration of inflammatory cells, since 2 weeks after experiments. And suppuration or gangrene of the pulp tissue were noted in 3 weeks and 5 weeks groups. 5. Suppuration or gangrene of pulp seemed to provoke the resorption of dentin wall, and inflammatory changes and resorption of roots were noted in the periodontal membrane near the periapical region. 6. As compared with calcium hydroxide groups, resorption of the root was pronounced in form or cresol and glutaraldehyde groups. Effects of medicaments to the succedaneous tooth germ were not seen.
Purpose: The aim of this study was to evaluate the effects of oral health care programs in 3 school-based oral health care center among primary schoolchildren. Methods: School-based oral health care programs included fluoride mouth rinsing, pit and fissure sealing for permanent premolars and molars, fluoride gel application and chewing of xylitol candy. All of the programs were carried out by one dental hygienist among 'D' primary schoolchildren in Daegu city under the supervision of a dentist. Baseline dental examinations were completed and preventive care was implemented for 544 children during one year. All of the children visited a school-based oral health care center every three months for a regular check-up. The final oral examination was conducted from March 15 to April 1, 2004. The data analysis data was made on the basis of SAS 8.01. Mean differences between 2003 and 2004 data were compared by paired t-test. Corresponding p-values were considered significant at values less than 0.05. Results: The DMF rate and DFT index were reduced to 8.0% and 8.4% during one year respectively, but there were no statistically significant differences. The DMF rate was significantly reduced (16.3%) after a one year program of school-based oral health care practice. The DMFT(Decay Missing Filling Tooth) index was also reduced compared to 2003 throughout the entire grade. Conclusion: School-based oral health care programs can reduce the prevalence of dental caries prevalence among schoolchildren during one year. This program also improved the oral health capacity of schoolchildren. It is recommend that the school-based oral health care program should be extended to every primary school in Korea.
Objectives: The aim of this study was to assess the morphologic characteristics of two types of stainless steel crowns (SSCs) for the first primary molar using a 3D scanner. Study design: Two types of SSCs, KIDS CROWN (KC) and 3M ESPE ND-96 (ND), for the first primary molars were scanned using a 3D scanner. The mesiodistal and buccolingual diameters at the height of the contour and the cervical margin, occlusocervical diameters on the mesial, distal, buccal, and lingual aspects were measured, and the crown shape ratio, the smooth surface crown height ratio, and the cervical convergence were calculated. Results: In the crown shape ratio of the mandibular SSC, KC was larger buccolingually compared with ND. In the smooth surface crown height ratio, ND was larger than KC in all of the maxilla and mesial, distal, and lingual aspects of the mandible. ND was more convergent to the cervical mesiodistally and buccolingually compared with KC. Conclusion: In the superimposed images of the maxillary SSC, the mesiolingual and distolingual line angles of KC were more prominent compared with ND. In the mandible, ND demonstrated higher cusps and more obvious buccal developmental lobes than KC. ND showed a larger cervical undercut than KC.
Newley developed dentin bonding system which contains self-etching primer performed the removing of smear layer and etching procedure at once. So, it make possible more simple and shorter working time, the stronger bonding strength than conventional dentin bonding system. Cavities were prepared in the buccal and lingual surface of 30 extracted primary molars, and randomly assigned into three equal groups of 10 each. All cavities were filled with Z-100 composite resin following manufacturer's specifications after priming of three different dentin bonding system. Specimens stored for 7 days in 37 degree C water, thermocycled for 1000 cycles between 5 degree C and 55 degree C, immersed in 50% silvernitrate solution for 24 hours, and embedded in resin before being sectioned longitudinally. Data were analyzed by t-test and ANOVA. Mac-Bond II and Clearfil Linear-Bond II showed less microleakage at the occlusal and gingival margins compared to Scotchbond Multi Purpose. All dentin bonding systems showed significantly less microleakage at the occlusal margins compared to the gingival margins. But, there were no significantley difference between each group.
The term "Oligodontia" or "Hypodontia" have been used to describe variable degrees of reduction in number of teeth. Oligodontia may occur alone or as a result of some syndrome. Although the teeth are derived in part from ectoderm, the current opinion of reason of oligodontia should be reserved for those disorders in which there is abnormal development of one or more ectodermal tissues. 7 year 5 months old female was refered to the department of pedodontics, college of dentistry Yonsei university for evaluation and replacement of absent teeth. She had no special inf.ectious disease in her childhood, and her parents were healthy. She had no special syndrome of ectodermal disorders except the saddle nose, yellow and fine hair, and notched upper anterior central incisor. Panex radiogram was showing 6 anterior primary teeth, 2 permanent first molars and 2 unerupted first bicuspid in mandible. Another permanent teeth were absent. and normal number of primary and permanent teeth in maxilla. Lateral cephalogram showed no special abnormality in growth pattern. We had evaluated lower anterior decayed teeth with jacket resin and chrome steel crown and removable partial denture at missing area. We had got good results for rehabilitation of function and aesthetic.
Posterior crossbites are abnormal buccal, or lingual relationship of a tooth or teeth of the maxilla, the mandible, or both when the teeth of the two arches are in occlusion and involve the molars and premolars. Posterior crossbites are classified as dental, muscular(functional), or skeletal. In an effort to avoid occlusal interferences caused by the inadequate arch width, the patient deviates the mandible laterally upon closure to achieve maximum intercuspation. This is described as functional posterior crossbite. Correction of functional posterior crossbites in the primary & early mixed dentition as early as possible after diagnosis has been recommended, because crossbites do not automatically improve with the eruption of the permanent teeth. Functional posterior crossbites, if left untreated, may have deleterious effects on the development and function of the TMJ. The diagnosis and management of three cases is presented. Each patient with functional posterior crossbites is treated using the bilateral maxillary expansion appliance.
Space maintainers needs following extraction of primary molars, due to severe dental caries under general anesthesia. Delivery of conventional space maintainers has three of the disadvantages of the method, there being a impression taking and time consuming due to laboratory procedure, and delivery problem due to relaxed tongue. For this problem improved, modified space maintainer is prefabricated before procedure, and adjust, delivery in operation room, favorable results were obtained. The following results were obtained. 1. Delivery time is very reduced than conventional space maintainers. 2. After space maintainer delivery, follow check-up, without impingement of soft tissue, distortion of wire. After space maintainer delivery, continuous research is required so that space maintainer delivery is applied to usual dental treatment, and it is necessary of long term research.
유구치의 깊은 우식은 치수병소을 유발하고 이를 조기에 치료하지 않을 경우에는 치수강저의 수많은 부근관을 통하여 치근분지부 병소로 이어진다. 지금까지 대부분의 교과서나 문헌에서는 이와 같은 경우를 발치의 적응증으로 기술하여 왔고 임상에서도 상례적으로 발치를 흔히 시행하였다. 그러나 유구치의 조기 상실은 수많은 부작용을 낳는 것이 필연적이다. 어린이의 골재생 능력 이 우수하며, 치수강저와 치근분지부를 잇는 많은 부근관이 있음을 생각해 볼 때 유구치 치수 내의 감염원을 치수절제술로 제거하고, 이미 발생한 치근분지부 병소에 대해서는 소파술을 시행한다면 치근분지부 골조직의 신속한 재생이 가능하지 않을까 하는 의문이 제기되었다. 본 증례는 $3{\sim}6$세 어린이에서 유구치 치근분지부 방사선 투과상이 $2{\sim}4mm$ 정도의 깊이를 보이는 비교적 경미한 수준이면서, 연조직 누공을 보이는 10개 증례를 선별하여 치수절제술과 치근분지부 소파술을 시행한 결과, 모두에서 방사선적으로 골재생이 관찰되었다. 이를 통하여 유구치 치근분지부 병소나 누공을 보이는 모든 경우가 발치의 적응증은 아니며, 이 방법이 치근분지부 병소를 가진 유구치를 잔존시킬 수 있는 하나의 대안이 될 수 있음을 알 수 있었다. 그러나 이 방법이 객관적 인 타당성을 얻기 위해서는, 병소의 크기와 진행정도, 계승 영구치의 성숙도, 환아의 연령 등 적응증에 대한 보다 심층적인 검토와 연구가 필요할 것으로 사료되었다.
