Purpose: The primary objective of this study was to analyze the thickness and height of alveolar bone around the maxillary and mandibular incisors. Additionally, this study aimed to compare bone parameters between Caucasian (CC) and African American (AA) female patients. Materials and Methods: In this retrospective pilot study, 50 female subjects(25 CC and 25 AA) were included. The inclusion criteria were AA or CC women between the ages of 18 and 50 with a normo-divergent facial pattern and Angle's class I, end-on class II, or mild class III malocclusion. The distance from the cementoenamel junction (CEJ) to the buccal and lingual alveolar crest; the alveolar ridge thickness at the mid-root and apex; and the buccal and lingual bone thickness at 3, 6, and 9mm from the CEJ were measured. Results: No significant difference was found (P>0.05) in the cortical bone thickness at 3mm, 6mm, or 9mm from the alveolar crest between CC and AA populations for most measurements. A significant difference in bone thickness was found (P<0.05) for the lingual surface of the central incisor, with maxillary bone thickness found to be higher than mandibular bone thickness. The measurements of lingual thickness were larger than those of buccal thickness for both races. Conclusion: There were no differences in maxillomandibular anterior alveolar bone measurements between normo-divergent adult AA and CC women, except for a few parameters at varying locations. However, future studies can be planned based the current pilot study data, which may provide valuable information.

Cerebral palsy is one of the primary handicapping conditions of childhood. The prevalence of malocclusions in patients with cerebral palsy is approximately twice than in general population. Even though these high rates of malocclusions, most clinicians may feel uncomfortable about treating such problems to reduce inclination of anterior teeth because to reduce of protrusion makes to decrease risk of trauma. This is the case report about mitigation of maxillary anterior teeth protrusion in patient with cerebral palsy. A 14 year old boy who had cerebral palsy visited our dental hospital. He had severe protrusive maxillary anterior teeth and narrow arch form. He was experienced at using Castillo morales appliance in early childhood. He had mild mental retardation and was able to learn simple skills. He and his parents had willing to improve his dental problems. A gentle impression taking on maxilla was done. Removable appliance was made including median screw and labial bow. We provide a period of adaption for 3 weeks. After of anterior teeth through activation of labial bow was done once a month by dentist. The treatment carried out for 10 months and we could observe reduced labial inclination of maxillary right central incisor and more wide arch form. Hawley type retainer was set at maxilla for retention. In conclusion, accompanying careful case selection and treatment, patient with cerebral palsy can be treated and should not be ignored their orthodontic needs.

Objectives: The purpose of this study was to investigate the demographic and clinical characteristics of patients with medication-related osteonecrosis of the jaw (MRONJ) and to elucidate factors affecting recurrence in surgical treatment. Materials and Methods: A total of 51 patients who were diagnosed with MRONJ were analyzed according to demographic and clinical features and treatment results through a retrospective chart review from 2013 to 2017 in the Department of Oral and Maxillofacial Surgery, Korea University Anam Hospital, Seoul in Korea. Results: Alendronate composed the majority of medication doses (55.6%), followed by ibandronate (20.0%), risedronate (15.6%), and zoledronate (6.7%). Forty patients (88.9%) were given oral medication, and five patients (11.1%) were intravenously treated, and the mean duration of medication use was $61.1{\pm}42.9$ months. A total of 10 patients (22.2%) had a drug holiday before MRONJ-induced dental treatment lasting an average of $6.8{\pm}7.0$ months. MRONJ occurred 2.7 times more in the mandible, with 41 cases (73.2%) occurring in the mandible and 15 cases (26.8%) occurring in the maxilla, and the prevalence of affected posterior parts (premolar-molar) was six times greater than that of the anterior parts (incisor-canine) (48 cases vs 8 cases, 85.7% vs 14.3%). The most common dental cause of MRONJ was tooth extraction (69.6%). Regarding recurrence, there was no statistical difference in recurrence rate according to either site or stage. However, recurrence occurred in 4 out of 34 cases (11.8%) in the primary closure group and 9 out of 20 cases (45.0%) in the secondary healing group, and there was a statistical difference with respect to closure technique. Conclusion: The identified risk factors in patients taking bone resorption inhibitors can aid dental clinicians in ensuring prevention and proper treatment of MRONJ.

This case report is a treatment of patient with hemophilia A and autism. The patient's chief complaint was treatment of dental caries on entire dentition and he has an impacted mesiodens located on the apex of the upper right primary central incisor. The patient was consulted with pediatrician and anesthetist about the detailed discussion of the complexities of hemorrhagic disorder. Because he had some problems of behavior management and bleeding, the treatment was done under the gerneral anesthesia. The following results were obtained. 1. Consult with the patient's physician and hematologist about the replacement therapy and bleeding tendency. 2. For the severe hemophiliac child who requires extensive or surgical treatment, general anesthesia may be indicated for the comprehensive care. 3. For the hemophiliac child who has a behavior management problem due to autism and other defects, general anesthesia may be considerable. 4. Care must be taken during dental procedures not to causing a bleeding. 5. Local hemostatic methods must be acquired for the emergency state. 6. Neve prescribe aspirin or non-steroidal anti-inflammatory drugs such as ibuprofen, indomethacin, and phenylbutazone. These drugs affect platelet aggregation and exaggerate the bleeding defect. 7. Do not be afraid of hemophiliac patient, and never compromise quality of dental care.

The congenital missing of mandibular second premolars is among the common dental anomaly in children. When a second premolar is diagnosed as congenitally missed, we should consider many factors influencing the treatment plan such as patient's age, states of roots of 2nd primary molar, degree of crowding, skeletal growth pattern, facial profile, procumbency of the incisor and lower facial height, etc. The mineralization of the second premolars begins in the majority of cases at the age of $2\sim2\frac{1}{2}$ years, but this period varies more widely than those for other permanent teeth. Also, mandibular second premolars show the greatest variations in differentiation and calcification. For this reason, aplasia of this group of teeth cannot be diagnosed at early age and with the same degree of certainty. From the clinical studies with 2 cases and some literature review on late development of second premolars, it could be summarized as follows : 1. The 2 cases showed marked delay in the development of mandibular second premolars. 2. After the crypt formation, the speed of calcification seemed nearly normal, suggesting that the delay was due to differentiation rather than calcification. 3. When one is encountered with similar conditions, it would be desirable to consider the possibility of delayed tooth development.

Supernumerary teeth are characterized by an excess number of teeth, which can be responsible for a variety of irregularities in the primary and transitional dentition. Supernumerary teeth, especially in the maxillary anterior region, may prevent the eruption of adjacent permanent teeth and cause their ectopic eruption, diastema, root resorption, or formation of dentigerous cyst. Therefore, early diagnosis of supernumerary teeth is important for prevention of such complications, and adequate treatment should be given according to their location, number, and morphologic features. In this case, four supernumerary teeth in the maxillary anterior region were disturbing the eruption of adjacent permanent incisors. Two of them were located in proximity to the central incisor tooth germs that their immediate removal may injure the permanent tooth germs. In order to minimize such complications, surgical extraction of the four supernumerary teeth was performed in two stages. At first, only two inverted conical supernumerary teeth were extracted. The other two tuberculous supernumerary teeth, close to the permanent tooth germs, were extracted later after their natural dislocation. In that way, we could minimize affects on the neighboring permanent tooth germs and also the amount of alveolar bone removed during surgery.

Crown-root fractures occur throughout both crown and root, and are defined as fractures involving enamel, dentin and cementum. The fractures may be grouped according to pulpal involvement into complicated and uncomplicated one. Crown-root fractures often occur on maxillary anterior teeth and comprise 5% of injuries affecting the permanent dentition and 2% in the primary dentition. To restore crown-root fractured tooth, biologic width must be maintained. For maintaining biologic width, such methods as gingivectomy following osteoplasty or orthodontic extrusion or surgical extrusion are available. Surgical extrusion is a method that extracts the tooth and replants the fractured tooth supragingivally. It is indicated when the length of the crown fragment is less than half the length of the clinical root. In these cases, root canal treatment and crown restoration using light-cured composite resin were performed after surgical extrusion. In following periodic examinations, favorable outcome was observed.
The automated dental cavity detection program for a new concept intra-oral dental x-ray imaging device, an auxiliary diagnosis system, which is able to assist a dentist to identify dental caries in an early stage and to make an accurate diagnosis, was to be developed. The primary theory of the automatic dental cavity detection program is divided into two algorithms; one is an image segmentation skill to discriminate between a dental cavity and a normal tooth and the other is a computational method to analyze feature of an tooth image and take an advantage of it for detection of dental cavities. In the present study, it is, first, evaluated how accurately the DRLSE (Direct Regularized Level Set Evolution) method extracts demarcation surrounding the dental cavity. In order to evaluate the ability of the developed algorithm to automatically detect dental cavities, 7 tooth phantoms from incisor to molar were fabricated which contained a various form of cavities. Then, dental cavities in the tooth phantom images were analyzed with the developed algorithm. Except for two cavities whose contours were identified partially, the contours of 12 cavities were correctly discriminated by the automated dental caries detection program, which, consequently, proved the practical feasibility of the automatic dental lesion detection algorithm. However, an efficient and enhanced algorithm is required for its application to the actual dental diagnosis since shapes or conditions of the dental caries are different between individuals and complicated. In the future, the automatic dental cavity detection system will be improved adding pattern recognition or machine learning based algorithm which can deal with information of tooth status.

In order to investigate shape of synaptic vesicles of the tooth pulp afferent boutons and their presynaptic endings (p-endings), and the neuroactive substance of the p-endings in the trigeminal nucleus principalis, rat incisor tooth pulp afferents were labeled by the horseradish peroxidase (HRP) and quantitative ultrastructural analysis and postembedding immunogold labeling were performed. Labeled tooth pulp afferent boutons contained clear, spherical synaptic vesicles (diameter: $45{\sim}55\;nm$) and occasionally dense core vesicles(diameter: $80{\sim}120\;nm$). They formed symmetrical synapses with unlabeled axon terminals (p-endings) containing pleomorphic synaptic vesicles. The ratio of short to long diameter (form factor) of synaptic vesicles of pulp afferent boutons was 0.6 to 0.99, whereas that of p-endings was 0.25 to 0.99. In addition, most of the p-endings showed GABA-like immunoreactivity. These results indicate that the shape of synaptic vesicles is quite different between the tooth pulp afferent boutons and p-endings, and the p-endings may contain GABA as a neuroactive substance in the trigeminal nucleus principalis.
The extraction lot orthodontic treatment can be adopted for aligning crowded dentition, improving facial esthetics and solving a skeletal discrepancy as alternative for a surgical option. Mandibular second premolar extraction was often selected as treatment plan when there we very little or no space shortage in lower arch or limited retraction of the lower incisors was required. The primary object of this study was evaluate a pretreatment condition and examine the amount of tooth movement ior a mandibular second premolar extraction in growing patients. Pretreatment and posttreatment lateral cephalograms of 14 cases that had their four first premolar extracted (4/4 group), 15 cases with upper first and lower second premolar extraction (4/5 group) were selected. Structural method superimposition was conducted to evaluate a difference of dental change between 4/4 and 4/5 group. The results were as follows, 1. Pretreatment factor for 4/4 extraction or 4/5 extraction choice included maxillary incisor axis to occlusal plane, Class II molar relationship, IMPA and interincisal angle. 2. The amount of molar anterior movement in 4/5 group was greater than that of 4/4 group(p<0.05). 3. There was no significant difference between 4/4 group and 4/5 group in aspects of maxillary tooth movement(p<0.05).
