원발성 월경통의 추나 치료에 대한 체계적 문헌 고찰 (Chuna Manual Therapy for Primary Dysmenorrhea: A Systematic Review)

  • Objectives The purpose of this study is to evaluate the effectiveness of Chuna therapy for primary dysmenorrhea. Methods Researchers searched on 7 electronic databases (Pubmed, National Digital Science Library, China National Knowledge Infrastructure, Wangfang med online, Korean Studies Information Service System, Research Information Sharing Service and Oriental medicine Advanced Searching Integrated System). The search included Korean, English, Chinese reports and there was no limit on the search period. All of randomized controlled clinical trials (RCTs) that used Chuna manual therapy for primary dysmenorrhea were selected. Results 27 RCTs met required condition. Meta-analysis showed positive results for Chuna manual therapy for primary dysmenorrhea in terms of therapeutic effects and reduction of symptom scores compared to west medicine, herbal medicine, acupucture and other treatments. Conclusions Above results showed that performing chuna is effective in treating dysmenorrhea. However, in some studies, there was no statistic significance between the experimental group and the control group. Also, according to Cochrane Risk of Bias (RoB) evaluation method, quality of the studies were not high enough. Since most of the materials were in Chinese, more high-quality clinical trials about Chuna therapy for primary dysmenorrhea are needed in Korea.

  • Objectives : The aim of this study was to evaluate the effect of $SP_6$ moxibustion on the primary dysmenorrhea. Methods : The data was collected from subjects who was suffering for dysmenorrhea. The subjects were employed using Measure of Menstrual Pain(MMP) and Menstrual Symptom Severity List(MSSL) questionnaire. We treated the subjects with $SP_6$ moxibustion and measured Skin Resistance Variability(SRV) before and after treatment. The method of analysing data was paired t-test. Results : 1. $SP_6$ moxibustion produced a significant decrease in symptoms of primary dysmenorrhea. Results : 2. The SRV of 2, 3 area was significanltly changed after $SP_6$ moxibustion. Conclusions : $SP_6$ moxibustion was effective in decreasing the symptoms of primary dysmenorrhea. The SRV results suggest that women with primary dysmenorrhea may have organic energy deficiency and functional abnormality of the hypothalamus-hypophysis-ovarian axis.

원발성 생리통에 대한 계지복령환(桂枝茯笭丸) 단독치료와 침(鍼) 치료 병행 차이 연구 (A Pilot Study of the Difference between Gyejibongnyeong-hwan and Gyejibongnyeong-hwan combined Acupuncture Therapy on the Primary Dysmenorrhea)

  • Purpose : The purpose of this study is to identify the difference of clinical effects of gyejibongnyeong-hwan and acupuncture therapy on primary dysmenorrhea. Methods : We studied with ten patients recruited from April, 2003 to Feburary, 2004. Women with organic disease as uterine myoma, ovarian cysts and pelvic inflammatory disease were excluded from this study. We treated them with gyejibongnyeong-hwan or gyejibongnyeong-hwan combined acupuncture therapy for 8weeks. The severity of dysmenorrhea was measured by VAS(Visual Analog Scale). Results : gyejibongnyeong-hwan significantly decreased the severity of dysmenorrhea. And Gyejibongnyeong-hwan combined Acupuncture therapy significantly decreased the severity of dysmenorrhea. The change of VAS of gyejibongnyeong-hwan group is significantly higher than gyejibongnyeong-hwan combined acupuncture. Both of Gyejibongnyeong-hwan and acupuncture did not show hepatic and renal virulence. Conclusion : This study shows that gyejibongnyeong-hwan has remarkable effects on dysmenorrhea patients. Obviously further researches concerning all these area still necessary.

  • Objective : The purpose of this paper is to report the effect of Danggwihweyeokgaohsuyu-saenggang-tang(DHGOST) on primary dysmenorrhea from the disease pattern identification diagnostic system based on Shanghanlun provisions. Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, the patient was diagnosed with 'Reverting Yin Disease'. She took DHGOST for 75 days. This paper evaluated the results of treatment by Measure of Menstrual Pain(MMP) and Menstrual Symptom Severity List(MMSL) Results : The MMP score was declined from 8.3 to 3.5. And the MMSL score was declined from 46 to 38. Conclusions : This case study suggests that DHGOST has effectiveness on primary dysmenorrhea who is diagnosed with 'Reverting Yin Disease'.

FCST의 음양균형장치를 활용한 원발성 월경곤란증의 증례보고 (Primary Dysmenorrhea Cases Managed with Yinyang Balancing Appliance of FCST, a TMJ Therapy for the Balance of Meridian and Neurological System)

  • Therapeutic effect of Yinyang Balancing Appliance of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance was observed in primary dysmenorrhea cases. Two primary dysmenorrhea cases were managed with the Yinyang Balancing Appliance on temporomandibular joint (TMJ), combined with acupuncture. Clinical outcome measurement was based on subjective measures and clinical observations. The patients showed positive changes even after the initial treatment and this effect maintained over the follow-up period. Although it is not clear the effect is a sustaining or temporary in its nature, a positive effect was observed and further clinical and biological research on FCST is expected.

칠제향부환(七製香附丸)이 원발성(原發性) 월경통(月經痛) 환자(患者)의 복부(腹部) 및 수장부(手掌部) 온도(手掌部 溫度)에 미치는 영향(影響) (The effect of Chiljehyangbuhwan on the abdomial & palmar temperature in the primary dysmenorrhea patients)

  • Purpose: We intended to observe the relations between Chiljehyangbuhwan and abdominal & palmar temperature according to primary dysmenorrhea severity. Methods: We selected the 100 primary dysmenorrhea patients by the screening tests (first screening test-inquiry, second screening test-clinical test. additionally WHR (Waist-to-Hip ratio) by Inbody 2.0). By the fixed blocked randomization and double blind method, Chiljehyangbuhwan or placebo was administered for 1 menstration period. Finally, 69 patients remainded. Before and after administration, we measured 4 points abdominal temperature (Chon-jung (CV17), Chung-wan(CV12), Kwan-won(CV4). Chung-guk(CV3)) by DITI(DOREX Inc., USA). And then we checked the difference of temperature $({\Delta}T)$ between CV17 and CV12/CV17 and CV4/CV17 and CV3/CV12 and CV4/CV12 and CV3. Also, we measured 2 points (palmar region, upper front of forearm) for the difference of palmar temperature $({\Delta}T)$. Then, we checked palmar temperature minus upper front of forearm temperature and took an average of right and left ${\Delta}T.$ After that. we compared ${\Delta}T$ with primary dysmenorrhea severity evaluated by VRS (verbal rating scale) and MVRS (multidimensional verbal rating scale). In dysmenorrhea severity. we standardized scale score and 3-group-severity by score (mild, moderate. severe). Besides, we compared palmar ${\Delta}T$ with abdominal ${\Delta}T$. For statistics, we used ANOVA and Spearman's rho correlations. SPSS 13.0 for windows. Results: In case of MVRS, though Chiljehyangbuhwan was correlated to abdominal ${\Delta}T$(CV12 and CV3/CV12 and CV4). it was not correlated to palmar ${\Delta}T$. In case of VRS, though Chiljehyangbuhwan was not correlated to abdominal ${\Delta}T$. it was correlated to palmar ${\Delta}T$. However. palmar ${\Delta}T$ was not correlated to abdominal ${\Delta}T$. Statistically they showed significant result (p<0.05). Conclusion: The primary dysmenorrhea patients showed that severity by MVRS was connected with abdominal ${\Delta}T$ (CV12 and CV3/CV12 and CV4) and severity by VRS was connected with palmar ${\Delta}T$ after Chiljehyangbuhwan administration. So we can consider Chiljehyangbuhwan partially effects the abdominal & palmar temperature according primary dysmenorrhea severity. However, palmar temperature was not correlated to abdominal temperature. Therefore, we need further study.

Efficacy of Commiphora myrrha and Honey in Primary Dysmenorrhea: A Randomized Controlled Study

  • Background: Dysmenorrhea is the most common menstrual complaint in young women with a prevalence as high as 90% and is responsible for substantial repeated short-term absenteeism from school and work in young women. The objective of this study was to compare the efficacy of Commiphora myrrha and honey with mefenamic acid in primary dysmenorrhea. Materials and Methods: This prospective standard controlled trial was conducted at Luqman Unani Medical College Hospital and Research Center Vijayapura, India where 40 diagnosed patients of primary dysmenorrhea were randomly assigned to receive test drug (powdered Commiphora myrrha gum resin10g with 30g honey in two divided doses) or active control drug (mefenamic acid 250mg TID) for first 3days of menstruationfor two consecutive cycles. The primary outcome measure was reduction in severity of pain assessed by numerical pain rating scale (NPRS), and secondary outcome measures were improvement in quality of life (QOL) assessed by SF-36 and reduction in perceived stress score (PSS). Results: During first cycle treatment no significant difference was found in NPRS score (p=0.085) between the groups however significant difference in NPRS score (p<0.001) was seen during 2nd treatment cycle. Significant reduction (p=0.022) in the perceived stress score was noted and overall quality of life was markedly improved after treatment in both the groups. Conclusion: These data suggest that Commiphora myrrha gum resin with honey is an effective herb in reducing symptoms of primary dysmenorrhea. These results need to be confirmed by a properly designed trial with a larger sample size. Trial registration: Clinical Trial Registry India CTRI/2017/09/009596.

  • The purpose of this study is to identify the clinical effects of Gyebongnyeong capsule in the treatment of primary dysmenorrhea patients. We studied ten patients visit Se-Myung University Oriental Medical Center from February, 2005 to March, 2005. Women with organic disease such as ut. myoma, ov. cyst and pelvic inflammatory disease were excluded from this study. We treated them with Gyebongnyeong capsule for two menstrual cycles. The severity of dysmenorrhea were measured by VAS (Visual Analog Scale) and Patient's Global Assessment. Gyebongnyeong capsule significantly decreased the severity of dysmenorrhea. Gyebongnyeong capsule did not show hepatic and renal virulence. This study shows that Gyebongnyeong capsule has remarkable effects on dysmenorrhea patients.

The Effect of Microcurrent Stimulation on Pain and Quality of Life in Women with Primary Dysmenorrhea

  • Objective: This study aimed to investigate the potential benefits of microcurrent stimulation as a non-invasive therapeutic approach for managing pain and improving the quality of life in women suffering from primary dysmenorrhea. Design: A case study. Methods: This study was conducted, involving a cohort of 6 women diagnosed with primary dysmenorrhea, aged between 20 to 30 years. Participants were received microcurrent stimulation using low-intensity microcurrents for 30 minutes every day for 4 weeks. The intensity of microcurrent stimulation was 25 μA and the frequency was 8 Hz. The intervention was administered between the menstruations, with pain intensity and quality of life being assessed at baseline, and then at the end of menstrual cycle. Pain intensity was evaluated using a visual analog scale (VAS) and menstrual symptom questionnaire (MSQ), while the quality of life was assessed through the stress response inventory (SRI), state trait anxiety inventory (STAI), center for epidemiologic studies depression (CES-D), and menstrual distress questionnaire (MEDI-Q). Results: After the intervention, participants demonstrated a statistically significant reduction in pain intensity, as evidenced by improved VAS scores (p<0.05). However, the changes in MSQ scores did not reach statistical significance. Regarding quality of life measures, no statistically significant differences were found in the SRI, STAI, CES-D, and MEDI-Q scores after the intervention (p>0.05). Conclusions: This study suggest that microcurrent stimulation holds promise as a potential treatment option for alleviating pain associated with primary dysmenorrhea.

원발성 월경통 환자와 속발성 월경통 환자의 심박변이도, 체성분, 적외선 체열 촬영, 맥전도 검사 상 특징 비교 고찰 (Comparative Analysis of the Heart Rate Variability, Body Composition, Digital Infrared Thermal Imaging and Electro Pulse Graph Between Primary Dysmenorrhea Patients and Secondary Dysmenorrhea Patients)

  • Objectives: The purpose of this study is to analysis differences of heart rate variability (HRV), body composition, digital infrared thermal imaging (DITI), and electro pulse graph by whether there are some causes of dysmenorrhea or not. Methods: We studied 89 patients who took at least one test among HRV, Body composition, DITI, and electro pulse graph visiting Kyung Hee University Hospital at Gangdong from March 1, 2014 to May 7, 2019. Results: There were statistically significant differences in Low frequency (LF), LF norm and High frequency norm (HF norm) of HRV between primary dysmenorrhea patients and secondary dysmenorrhea patients. There were statistically significant differences in Body Mass Index (BMI) and body fat percentage of Body composition between two groups. But both BMI were included in normal range. There were no statistically significant difference in DITI results. And there was significant difference of Estimated circulation resistance (ECR) between two groups. Conclusions: The results suggest that hormone imbalance of secondary dysmenorrhea patients can affect HRV results. And because of high ECR, phlegm pathology can be more considered in secondary dysmenorrhea patients. Further study should be conducted to confirm the results of body composition and DITI differences.
