Breast cancer is the most commonly diagnosed invasive cancer among women. Many factors, both genetic and non-genetic, determine a woman's risk of developing breast cancer and several breast cancer risk prediction models have been proposed. It is vitally important to risk stratify patients as there are now effective preventive strategies available. All women need to be counseled regarding healthy lifestyle recommendations to decrease breast cancer risk. As such, management of these women requires healthcare professionals to be familiar with additional risk factors so that timely recommendations can be made on surveillance/risk-reducing strategies. Breast cancer risk reduction strategies can be better understood by encouraging the women at risk to participate in clinical trials to test new strategies for decreasing the risk. This article reviews the advances in the identification of women at high risk of developing breast cancer and also reviews the strategies available for breast cancer prevention.
Objectives : The purpose of this study was to survey Korean solders' oral healthcare recognition and perceived oral health concern. Methods : The data was collected from a questionnaire given 157 soldiers in Chungcheongnamdo. The data was analyzed into t-test, one-way ANOVA and Pearson's correlation analysis. Results : 25.5% replied that they were concerned about oral health. Soldiers who have received oral healthcare education exhibited higher recognition on the prevention of dental caries and periodontal disease. Moreover, those who have used oral hygiene devices also showed higher recognition on the prevention of dental caries and periodontal disease. The recognition of preventive effects on dental caries, in particular, showed a statistically significant difference (p<0.05). In regard to the relationship between oral health concern and recognition of oral healthcare, those who were more concerned about oral health were higher recognition of periodontal disease prevention(r=0.254, p<0.01). Conclusions : To improve concern and recognition of oral health for the members, the soldiers needs to develop oral health education and policy.
The development of digital technology is changing the paradigm of the healthcare industry to preventive and consumer-oriented. The combination of the ICT industry and the bio-healthcare industry is emerging as a core industry in the era of the Fourth Industrial Revolution. The Korean government has also selected the bio-healthcare industry as one of the three key future development industries. In May, the government announced its bio-health industry innovation strategy and set a goal of 300,000 employees. Therefore, analyzing the effects of employment on the related industries of the digital bio-healthcare industry is very important for the establishment of future industrial and technology development policies. The research method restructures the integrated classification of 32 industries into 34, including the digital bio-healthcare industry, using the classification criteria of the government and professional institutions, and then reorganizes the digital bio-healthcare industry into eight industries classified as one industry group. The analysis data was taken from the Bank of Korea's 2019 data. Various trigger coefficients and ripple effects coefficients were rewritten using the analysis method of the Input-output Statistics. The analysis of the results compares the employment-induced effects of the digital bio-healthcare industry and the ripple effects of related industries in production, investment and value-added. In addition, in terms of investment effect, the effects of in-house and related industries were compared. It is hoped that the results of this study will be used to establish employment and industrial policies.
Unmet healthcare is an important indicator for measuring accessibility of healthcare services. We analyzed the following four data from a nationally representative sample of South Korean population: Korea Health and Nutrition Examination Survey (KNHANES, 2007-2021), Community Health Survey (CHS, 2008-2021), Korea Health Panel Survey (KHP, 2011-2019), and Korean Welfare Panel Study (KOWEPS, 2006-2021). The proportion of individuals reporting unmet healthcare needs were 6.0% (KNHANES), 5.1% (CHS), and 13.1% (KHP). Annual percentage change (APC) which identifies trend for the follow-up period was -9.4%, -9.4%, and -5.3%, respectively. The proportion of individuals reporting unmet healthcare needs due to cost were 1.2% (KNAHANES), 0.5% (CHS), 2.7% (KHP), and 0.4% (KOWEPS). The APC was -10.4%, -16.1%, -11.5%, and -19.1%, respectively. Compared to the previous year, the rate of unmet healthcare needs decreased slightly, but the rate of unmet health care needs due to cost tended to increase. Overall, higher rates of unmet healthcare needs were reported in the low-income and the elderly population. Although it was confirmed through the APC that the rate of unmet healthcare experience has decreased over the past decade, it can be seen that there is still a disparity by income level and age. These results suggest the need for an appropriate health benefit coverage policy for the low-income and the elderly.

Background : Tuberculosis (TB) remains a significant public health issue worldwide, particularly among healthcare workers (HCWs) at high risk of exposure. Latent tuberculosis infection (LTBI) is a state where individuals are infected with Mycobacterium tuberculosis but do not show clinical symptoms. Early detection and treatment of LTBI are crucial to prevent progression to active TB. This study aimed to investigate the prevalence and risk factors of LTBI among Korean Medicine (KM) workers in Seoul, South Korea. Methods : This study analyzed 368 adults aged 19 and over working in Korean medicine institutions in Seoul by September 2023. Participants underwent a tuberculin skin test (TST) and completed a survey collecting demographic information, occupation, work duration, smoking status, BCG vaccination, TB history, and comorbidities. Data were analyzed using descriptive statistics and chi-square tests, with significance set at p < 0.05. Results : The average age of participants was 43.1 years, with an LTBI prevalence rate of 3.5%. Significant risk factors included age and history of TB, Older age and a history of TB were associated with higher LTBI positivity. Conclusion : The study identified the prevalence and risk factors of LTBI among Korean medicine workers in Seoul. The findings highlight the need for targeted LTBI screening and preventive measures, especially for older workers and those with a history of TB. While the prevalence was lower than in other healthcare settings, the results emphasize the importance of regular LTBI testing and prevention education for KM workers. Future large-scale studies are needed to confirm these findings and further understand the relationship between various risk factors and LTBI in KM settings.
Objectives: One of the main objectives of the Targeted Subsidies Law (TSL) in Iran was to improve equity in healthcare financing. This study aimed at measuring the effects of the TSL, which was implemented in Iran in 2010, on equity in healthcare financing. Methods: Segmented regression analysis was applied to assess the effects of TSL implementation on the Gini and Kakwani indices of outcome variables in Iranian households. Data for the years 1977-2014 were retrieved from formal databases. Changes in the levels and trends of the outcome variables before and after TSL implementation were assessed using Stata version 13. Results: In the 33 years before the implementation of the TSL, the Gini index decreased from 0.401 to 0.381. The Gini index and its intercept significantly decreased to 0.362 (p<0.001) 5 years after the implementation of the TSL. There was no statistically significant change in the gross domestic product or inflation rate after TSL implementation. The Kakwani index significantly increased from -0.020 to 0.007 (p<0.001) before the implementation of the TSL, while we observed no statistically significant change (p=0.81) in the Kakwani index after TSL implementation. Conclusions: The TSL reform, which was introduced as part of an economic development plan in Iran in 2010, led to a significant reduction in households' income inequality. However, the TSL did not significantly affect equity in healthcare financing. Hence, while measuring the long-term impact of TSL is paramount, healthcare decision-makers need to consider the efficacy of the TSL in order to develop plans for achieving the desired equity in healthcare financing.
Background: Coronary angioplasty has been replacing coronary artery bypass grafting (CABG) because of the relative advantage in terms of recovery time and noninvasiveness of the procedure. Compared to other Organization for Economic Cooperation and Development (OECD) countries, Korea has experienced a rapid increase in coronary angioplasty volumes. Methods: We analyzed changes in procedure volumes of CABG and of percutaneous coronary intervention (PCI) from three sources: the OECD Health Data, the National Health Insurance Service (NHIS) surgery statistics, and the National Health Insurance claims data. Results: We found the ratio of procedure volume of PCI to that of CABG per 100,000 population was 19.12 in 2014, which was more than triple the OECD average of 5.92 for the same year. According to data from NHIS statistics, this ratio was an increase from 11.4 to 19.3 between 2006 and 2013. Conclusion: We found that Korea has a higher ratio of total procedure volumes of PCI with respect to CABG and also a more rapid increase of volumes of PCI than other countries. Prospective studies are required to determine whether this increase in absolute volumes of PCI is a natural response to a real medical need or representative of medical overuse.

Purpose: We conducted this study with the aim of characterizing safety attitudes (SA) among medical staff in a disadvantaged area of Vietnam and examining associated factors with SA. Methods: A cross-sectional survey was conducted on 442 health staff members at four hospitals in Son La Province from June until August 2021. We used the Vietnamese shortened edition of the Safety Attitudes Questionnaire to measure the SA of study participations. We chose latent class analysis (LCA) to identifying the number of latent classes of SA among the study subjects. Multinomial logistic regression was used to examine factors associated with the identified SA classes. Results: The results of our LCA showed that there were three latent classes, namely high SA group (n=150, 33.9%), moderate SA group (n=236, 53.4%), and low SA group (n=56, 12.7%). The multinomial logistic regression analysis found that medical staff who had university education and above, who were nurses, and who served in non-clinical areas were more likely to be in the moderate SA group and in the high SA group than in the low SA group. Conclusion: Based on these results, several recommendations could be made to improve the SA of healthcare workers in disadvantaged areas. Further research with larger sample sizes and more diverse populations is needed to confirm these findings and to develop effective interventions to improve the SA of healthcare workers in disadvantaged areas.

Background: A series of breast cancer cases were recently reported in a tertiary university hospital in South Korea. Nurses are generally exposed to risk factors for breast cancer such as night shift work, antineoplastic agents, and job strain. However, the epidemiological evidence of excess incidence among nurses remains lacking. This study aims to investigate the excess incidence of breast cancer among nurses in a tertiary university hospital and provide epidemiological evidence of occupational risk factors. Methods: A retrospective cohort was developed using personnel records of female workers in the nursing department who worked from January 2011 to June 2021 in a tertiary university hospital in South Korea. Sick leave records were used to identify cases of breast cancer. The standardized incidence ratio of breast cancer among nurses was compared to the general population. Results: A total of 5,509 nurses were followed up for 30,404 person-years, and 26 breast cancer cases were identified. This study revealed a significantly increased breast cancer incidence among all included nurses, with a standardized incidence ratio of 1.65 (95% confidence interval [CI]: 1.08-2.41), compared to the general population. Workers, who handle antineoplastic agents in their representative department and current and/or former department, had significantly elevated breast cancer standardized incidence ratios of 2.73 (95% CI: 1.008-5.94) and 3.39 (95% CI: 1.46-6.68), respectively. Conclusions: This study provides significant evidence of increased breast cancer risk among nursing staff in a hospital setting, particularly those who handle antineoplastic drugs. Measures that reduce exposure to risk factors should be implemented, especially anticancer drugs, to protect healthcare professionals. Further research at a national level that focuses on healthcare workers is necessary to validate breast cancer incidence and its contributing factors.

Purpose: The study analyzed continuing educational needs with the aim of developing continuing education program for home healthcare nurse specialists. Methods: A convenience sample of 101 home healthcare nurse specialists affiliated with 89 home health care agencies in Seoul, Busan, Dae-gu, Inchon, Jeonnam, Keongnam was used. Data was collected with a self-report questionnaire from May 10-31, 2010, and analyzed with descriptive statistics using SAS 9.13 program. Results: Of the participants, 88.1% reported that continuing education was necessary and 58.2% reported that the education was needed for peak job performance. The participants also reported that it would be proper if continuing education is given on a Saturday(56.4%) by home healthcare institute(49.5%) or homecare nurses association (38.6%) for 1-3 months(56.4%). The participants scored more than 3.0 in the current performance at six of the seven home healthcare nurse specialist roles, and scored more than 3.5 in the necessity of continuing education for seven roles in 47 jobs. Conclusion: Various programs for continuing education have to be developed and need to focused on the jobs needed for home healthcare nurse specialist and the most frequent disease. For this purpose, every home healthcare organization has to select their unique area and develop their own educational program. Furthemore, home healthcare nurses association has to build an education operating system that incorporates all program aspects.
