A Study on Maternity Aids Utilization in the Maternal and Child Health and Family Planning (농촌(農村)에 있어서 분만개조요원(分娩介助要員)의 봉사(奉仕)에 의(依)한 모자보건(母子保健)rhk 가족계획(家族計劃)에 관(關) 연구(硏究))

  • This study was conducted to assess the effectiveness of service by maternity aids concerning maternal and child health in improving simultaneously infant mortality, contraception and vital registration among expectant mothers in rural Korea, where there is less apportunity for maternal and child health care. It is unrealistic to expect to solve this problem in rural Korea through professional persons considering the situation of medical facilities and the socioeconomic condition of residents. So, we intended to adopt a system of services by maternity aids who were educated formally among indigenous women. After the women were trained in maternal and child health, contraception, and registration for a short period, they were assigned as a maternity aids to each village to help with various activities concerning maternal and child health, for example, registration of pregnant women, home visiting to check for complications, supplying of delivery kits, attendance at delivery, persuasion of contraception, and invitation for registration and so on. Mean-while, four researchers called on the maternity aids to collect materials concerning vital events, maternal child health, contraception and registration, and to give further instruction and supervision as the program proceeded. A. Changes of women's attitude by services of maternity aid. Now, we examined to what extent' such a service system to expectant mothers affected a change in attitude of women residing in the study area as compared to women of the control area. 1) In the birth and death places, there were no changes between last and present infants, in study or control area. 2) In regard to attendants at delivery, there were no changes except for a small percentage of attendance (8%) by maternity aid in study area. But, I expect that more maternity sids could be used as attendants at delivery if they would be trained further and if there was more explanation to the residents about such a service. 3) Considering the rate of utilization of sterilized delivery kit, I am sure that more than 90 percent would be used if the delivery kit were supplied in the proper time. There were significant differences in rates between the study and the control areas. 4) Taking into consideration the utilization rate of the clinic for prenatal care and well baby care, if suck facilities were installed, it would probably be well utilized. 5) In the contraception, the rate of approval was as high as 89 percent in study area as compared to 82 percent in the control area. 6) Considering the rate of pre-and post-partum acceptance on contraception were as much as 70 percent or more, if motivation to use contraception was given to them adequately, the government could reach the goals for family planning as planned. 7) In the vital registration, the rate of birth registration in the study area was some what improved compared to that of the control area, while the rate of death registration was not changed at all. Taking into account the fact that the rate of confirmation of vital events by maternity aids was remarkably high, if the registration system changed to a 'notification' system instead of formal registration ststem, it would be improved significantly compared to present system. B. Effect of the project Thus, with changes in the residents' attitude, was there a reduction in the infant death rate? 1) It is very difficult problem to compare the mortality of infants between last and present infants, because many women don't want to answer accurately about their dead children especially the infants that died within a few days after birth. In this study the data of present death comes from the maternity aides who followed up every pregnancy they had recorded to see what had happened. They seem to have very reliable information on what happened in first few weeks with follow up visitits to check out later changes. From these calculaton, when we compared the rate of infant death between last and present infant, there was remarkable reduction of death rate for present infant compare to that of last children, namely, the former was 30, while the latter 42. The figure is the lowest rate that I have ever heard. As the quality of data we could assess by comparing the causes of death. In the current death rate by communicable disease was much lower compare to the last child especially, tetanus cases and pneumonia. 2) Next, how many respondents used contraception after birth because of frequent contact with the maternity aid. In the registered cases, the respondents showed a tendency to practice contraception at an earlier age and with a small number of children. In a comparison of the rate of contraception between the study and the control area, the rate in the former was significantly higher than that of the latter. What is more, the proportion favoring smaller numbers of children and younger women rose in the study area as compared to the control area. 3) Regarding vital registration, though the rate of registration was gradually improved by efforts of maternity aid, it would be better to change the registration system. 4) In the crude birth rate, the rate in the study area was 22.2 while in the control area was 26.5. Natural increase rate showed 15.4 in the study area, while control area was 19.1. 5) In assessment of the efficiency of the maternity aids judging by the cost-effect viewpoint, the workers in the Medium area seemed to be more efficiency than those of other areas.

Risk Factors for Hypertension of Middle Aged Male Workers using Data from Health Check-ups (중년 남성 근로자의 고혈압 발생에 관련된 요인의 검토)

  • The present study was conducted to identify the factors related to the hypertension, and to offer the evidence of 1st prevention of hypertension. 2,230 male workers aged 30-59 years were observed the relationships between hypertension and age, BMI, glucose intolerance, smoking, alcohol drinking, regular exercise, eating habits using data from health check-ups in 2011. As a results, the incidence rate of hypertension was 18.8% of 30-39 year old group, 23.8% of 40-49 year old group, 33.0% of 40-49 year old group. The incidence rate of hypertension was significantly higher as age and BMI ascend, and it was higher in the group of abnormal glucose intolerance, regular alcohol intake, no regular exercise than their respective counterparts. The multivariable-adjusted odds ratio of hypertension increased significantly as there is an increase in age, and the group of abnormal glucose intolerance, smoking, regular alcohol intake, no regular exercise. In conclusion, obesity, glucose intolerance, alcohol intake, and physical inactivity are risk factors for hypertension, therefore we need the control of these factors for 1st prevention of hypertension.

  • Recently due to the enhancement of education and lifestyle, the trend of healthcare services are changed to a more active and differentiated service in which a continuous self health care is possible. The Smart-Telemedicine system offers medical services by merging Blue-tooth and telecommunication modules to former blood pressure, blood sugar, heartbeat and temperature measuring devices. Moreover, it could analyze one's health pattern which would be helpful for the patient to prevent potential future illness. In addition, the easier accesses to various remote controllable medical check-up programs are offered to public as a number of available smart phone are rapidly escalating. The Smart-Telemedicine system provides the most ideal interactive medical service via accessible smart phones and mobile medical check-up devices at anywhere and anytime. It is very beneficial since it can save patients' time and money because people can reach to the service right at their home and be allowed to take charge of their health care process via longitudinal health data. Therefore, not only social costs that occur in elderly community would be saved, but also business in various forms of medical service field transactions could be possible. This paper will suggest the Smart-Telemedicine System for preventive medicine, its design and analysis of business models and the evaluation of those model.

Practices and Needs of Health Promotion Program among Workers in A Petrochemical Industry Complex (석유화학단지 근로자들의 건강행위 실천 정도와 건강증진 요구도)

  • Objectives: To propose the basic data for the development of the optimum health promotion program (HPP) for the workers in a petrochemical industry complex. Methods: The subjects were 553 workers who worked at the six plants in a petrochemical industry complex. From February to March 2003, questionnaire survey was conducted by the nurses with occupational health specialty about the life styles, interests, self-cognitions, practices of health behavior, subjective symptoms thought to be related the working environments, burden to the own health, needs and intentions to participation of the HPP. Data analysis was done using SPSS program (ver 10.0) with ${\alpha}=0.05$ of statistical significance. Results: The mean age and tenure of the subjects were 39.7 years and 14.2 years, respectively, and proportion of smokers was around 50%. Two of third (66%) satisfied their own job, and 58% recognized themselves healthy. The most frequent health behavior were exercise (60%) and regular health check (87%) in individual and job based, respectively. About half of them (58%) had at least one of the symptoms thought to be related the working environments, and the most frequent one was odor (21%). These symptoms were more frequent in case of the shorter career. The needs of HPP was relatively higher in exercise (66%), cancer prevention (51%), stress control (47%), and intention to participation was also the highest in exercise (64%). Job satisfaction was higher in case of the higher interests and cognitions to their own health, and thought that the ownership's interest to workers' health be high (p<0.01), but no associations with the subjective symptoms. The mean level of burden to the own health were $14.1{\pm}13.9$ of physical, $11.7{\pm}13.6$ of emotional, $9.1{\pm}10.7$ of behavioral as out of one hundred. They felt the more burden in case of office workers, the lower interests and cognitions to their own health, the lower satisfaction to their job, the frequent drinking group, and complained the subjective symptoms (p<0.05). Abnormal results of the regular health check was higher in the frequent drinking group (p=0.083). Conclusions: For the optimum HPP for the workers in the petrochemical complex, the ownership's interest to workers' health would be enhanced, and the priority would be focused on exercise and drinking habits control.

Analysis of the Damaged Range Caused by LPG Leakage and Vapor Clouds Considering the Cold Air Flow (찬공기 흐름을 고려한 LPG 누출 및 증기운에 의한 피해 영향 범위 분석)

  • When LPG leaks from the storage tank, the gas try to sink to the ground because LPG is heavier than air. The gas easily creates vapor clouds causing aggressive accidents in no airflow. Therefore, It is important to prevent in advance by analyzing the damaged range caused from LPG leakage and vapor clouds. So, this study analyzed the range of damaged by LPG leakage and vapor clouds with consideration of the cold air flow which is generated by the topographical characteristics and the land use status at night time in the Jeju Hagari. As a result of the cold air flow using KLAM_21, about 2 m/s of cold air was introduced in from the southeast due to the influence of the terrain. The range of damaged by LPG leakage and vapor cloud was analyzed using ALOHA. When the leak hole size is 10 cm at the wind speed of 2 m/s, the range corresponding to LEL 60 % (12,600 ppm) was 61 m which range is expected to influence in nearby residential areas. These results of this study can be used as basic data to prepare preventive measures of accidents caused by vapor cloud. Forward, it is necessary to apply CFD modeling such as FLACS to check the vapor cloud formation due to LPG leakage in a relatively narrow area and to check the cause analysis.

THE EFFICACY OF PROPHYLACTIC ANTIBIOTICS FOR MANDIBULAR THIRD MOLAR EXTRACTION (하악 제3대구치 발치를 위한 예방적 항생제의 효용성)

  • This study analyzed the incidence of wound infection after the operation of mandibular third molar extraction in relation with antibiotic prophylaxis with the object of young and healthy patients. The study object was 1,177 mandibular third molars of 850 men of 20 to 25 years old without any specific systemic disease. Three methods of preventive antibiotic medication were selected according to the preventive antibiotic medication previously reported; three experimental groups were selected based on them, and the antibiotic used was amoxicillin($^{(R)}Kymoxin$, Yuhanyanghaeng, Seoul). The group 1 includes the patients that took the antibiotic orally before the operation(one hour earlier, 500mg) and for three days after the operation(250mg per time, three times/day), the group 2 is the ones that took the same antibiotic orally only once about one hour before the operation(500mg), and the group 3 did not take any antibiotics before and after the operation. And to compare the difficulties and the degrees of extraction during operations which can be possibly related to the wound infection after the operations, the mandibular third molars' impacted depths and extraction methods were investigated as well. To check if the wound was infected, observations with an internal of one week were performed twice after the operation, and the meaningfulness of the infection incidence was verified through Chi-square test using SPSS program(SPSS Inc., IL, USA). There was no statistically significant difference between the antibiotic medication methods and the wound infection incidence after the operation among the experimental groups. As examining the relations between the mandibular third molar operation methods and the wound infection incidence after the operation, there existed a statistically meaningful difference in the infection incidence according to the operation methods(p=0.020). And there was no statistically significant difference in the wound infection incidence according to the impacted depth of the mandibular third molar. Therefore, it is thought that there exists little necessity of prophylatic antibiotics medication when extracting the mandibular third molar of young and healthy men without any systemic disease in general; however, in case when it is expected that the possibility of infection will be high or the wound on the tissue will be severe, it is sure that the prophylactic antibiotics medication will be necessary.

Prevalence of Metabolic Syndrome and Its Associated Factors among Health Checkup Examinees in a University Hospital (종합건강검진 수검자들의 대사증후군 유병률 및 관련요인)

  • The purpose of this study was to estimate the prevalence of metabolic syndrome and determine the distribution of the clustering of the metabolic risk factors, and we wanted to evaluated the related factors in urban areas. 1,388 adults of 30 years and over, not recognized as taking medicines for or having cardiovascular diseases, who underwent health package check-up at the health promotion center of a university hospital. All subjects were measured by height, weight, waist circumference, blood pressure and blood chemistry(lipid profile). As a results, the prevalence rates of metabolic syndrome of study subjects were 21.7%, and the rates of metabolic risk factors were HDL-C, blood pressure, TG, abdominal obesity and FBS in that order. And the factors such influencing on metabolic syndrome as age, BMI, smoking habits, vegetable consumption and family history of the diabetes. Consequently, it is suggested that the evaluation and intervention for lifestyle factors may be needed in order to the risk management of metabolic syndrome.

Study on Compliance of Personal Health Record Application in Patients with Atopic Dermatitis (아토피피부염 환자의 개인별 증상 기록에 대한 순응도 연구)

  • Objectives : The purpose of this study is to evaluate clinical utilization by measuring compliance with the use of mobile health applications (AtopyPHR developed in a previous study) for patients with atopic dermatitis. Methods : Based on the AtopyPHR and the input period and frequency survey results for each symptom item, a scenario for measuring compliance was derived. The study period was 4 weeks. Participants installed AtopyPHR app and Telegram app on their smartphones, conducted user training on the app, and recorded symptoms using the app for 4 weeks. At the 2nd and 4th week visits, the AtopyPHR data recorded by the user can be viewed on the web page and used for medical decision. Compliance was analyzed by the date the symptoms were recorded. Results : There were 28 participants, all (100%) were compliant, and the compliance was 96.8. The patients were 1 to 18 years old, and the average age was 8.2±5.7 years, 10 males and 18 females. The actual date of participation in recording symptoms was 28.6±0.56 on average. Compared to Week 1, compliance decreased at Week 2, and Week 4 had the highest compliance. Daily check, daily emotion, stool/urine/sleep, and meal management showed high compliance, SCORAD and quality of life were higher than required to record. Conclusions : AtopyPHR was effective in compliance. The results of this study could be used to collect personal health data in daily life through the AtopyPHR, improving participant compliance. It is considered to be meaningful because it measured the compliance with the symptom record actually recorded using the mobile app rather than a questionnaire. This study may be useful not only for personal health care but also for medical decisions, as opinions are given by experts who treat atopic dermatitis.

The Incidence and Risk Factors of Hypertension that Developed in a Male-workers' Cohort for 3 Years (일부 남성근로자의 3년간 고혈압 발생률과 위험요인)

  • Objectives: Cardiovascular disease is one of the main causes of death and morbidity in Korea. In this study, the prevalence and incidence of developing hypertension in a male-workers' cohort were investigated during 3-years follow-up with a view to find the risk factors that affected the development of hypertension. Methods: Among the 5,374 people who participated in a routine health check up, 3,852 people with normal blood pressure and who had no history of hypertension were prospectively followed up for 3 years. The classification of hypertension was based on the JNC7 report (the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure). Life style factors and underlying diseases that were related to the risk factors of hypertension were collected by using a self-report questionnaires via the internet. Results: The prevalence of hypertension was 28.3% (1,520/5,374) at the first screening (2001). It was found that the incidence in 2004 of hypertension for the follow-up subjects (3,711) who had normal blood pressure in 2001 was 7.6 per 100 person-year. Multiple logistic regression analysis of the variables related to the risk factors of hypertension was carried out. The relative risks were 1.037 (95% CI=1.022-1.053) as the age increased 1 year and 1.039 (95% CI=1.023-1.055) as the body mass index increased $1kg/m^2$. The relative risk for the prehypertensive group was 2.501 (95% CI=1.986-3.149) compared to the normotensive group. These results showed that age, body mass index and the baseline blood pressure were significantly related to the incidence of hypertension. Conclusions: The incidence of hypertension was 7.6 per 100 person-year during follow-up. It was concluded that the risk factors for developing hypertension in the short-term were age, BMI, and prehypertension; Especially, this showed that it is necessary for prehypertensives to manage their body weight and blood pressure to prevent hypertension in middle-age by modifying their life style.

Effects of Music on Dental Anxiety and Pain during Ultrasonic Teeth Scaling (치석제거술시 치통 및 불안에 대한 음악의 효과)

  • This study was designed to evaluate the effects of music on dental pain and anxiety during ultrasonic scaling. One hundred and twenty five patients visited dental unit of Soonchunhyang University Chunan hospital for scaling were enrolled during August 1999 and February 2000. The patients were allocated randomly into two groups. music (n = 61) and control groups (n = 65). The music group patients listened to self-selected music using an earphone during scaling. Standardized questionnaire were used to assess the subjective denial pain and anxiety before and after scaling in both groups. In music group, more information on music effect was collected. To evaluate changes of physical signs by scaling, blood pressure and pulse were checked by a wrist check oscillometric. The results were as follows: 1. No significant difference in dental pain and anxiety between before and after scaling was observed in music group. However, the music group patients were satisfied with music because of reduction of pain(93.5%) and anxiety(93.4%), 96.7% of music subjects wanted to listen to music in next scaling. 2. While no significant difference in systolic blood pressure before scaling between music and control groups, systolic blood pressure of control group during scaling was significantly higher than that of music group(P<0.05). 3. In multiple logistic regression. the odds ratio(OR) of pain was decreased with age and female patients had higher OR (1.7, p>0.05). Patients with previous scaling experience complained of more dental pain during scaling than patients without experience. Music was not a significant predictor of denial pain during scaling, controlling for possible confounders. 4. While age. music. and scaling experience were not related to dental anxiety during scaling, sex and dental hygiene index were significant predictors of dental anxiety in multiple logistic regression. 5. Both during and after scaling. mean blood pressure of music patients were significantly lower than control patients after controlling for age, sex, and BMI. In conclusion, our data demonstrated that although listening to music did not disappear the dental pain and anxiety. but reduced the intensity of pain and anxiety during scaling. Furthermore, our data imply thai music has a effect of reduction of increasing blood pressure by scaling.
