Health Belief Model Approach to Health Beliefs, Attitude, and Health Behaviors Concerning HIV / AIDS. (건강신념모델을 적용한 AIDS 예방의 건강신념, 태도 및 건강행동)

  • This study was conducted in order to identify the extent to which Health Belief Model (HBM) constructs explain the likelihood of taking preventive behaviors for AIDS among the young adolescents in Korea. HBM was applied as the theoretical framework for developing questionnaire items in this study. The survey instrument included all of the constructs of Health Belief Model, namely, perceived susceptibility, perceived severity, perceived benefit, perceived barriers, cue to actions for preventive behaviors concerning AIDS. Additionally, demographic characteristics of the respondents, their sexual experiences, and AIDS Knowledge Test were included in the study. Each of HBM constructs were developed with a 5-point Likert type scale from l(never agree) to 5 (absolutely agree). The survey was conducted with a total of 247 military men in a city on September 18, 1996, using self-reported questionnaire. The results of the study were summarized as follows: 1. Because the subjects for this study were military soldiers, their demographic characteristics were limited to all men, young age, and ummarried. Educational status was evenly distributed between high school graduates and university students. 2. On the average, the respondents started their first sexual relationship at 18 years old and 82.6% of them did not use condom when having their first sexual experience. Thirty-one percent of the subjects had sexual contact with prostitutes and the average number of sexual contact with prostitutes was 5 times during the past 2 years. 3. The results of AIDS Knowledge Test scores demonstrated that the respondents had a high level of knowledge about AIDS. However, some misconceptions about transmission of AIDS through casual contact were still prevailed. Sixty-six percent of the respondents expressed that people infected with HIV should be isolated from the society in order to protect the general public. 4. All the respondents expressed that they had heard about AIDS before. TV was found to be the source which provided information on AIDS most frequently. 5. Among fundamental constructs of Health Belief Model, scores of perceived benefit of taking preventive action against AIDS marked the highest score, while scores of perceived susceptibility were the lowest. As a result of Multiple Stepwise Regression analysis, 13 variable groups were found to predict the preventive action by 25%. Among them, only perceived benefit variables was the most significant factor to explain preventive behaviors by 17%.

Reliability and Validity of the Korean Version of ISAAC Questionnaire (한국판 국제 소아천식 및 알레르기 질환 연구 설문지의 신뢰도 및 타당도 연구)

  • Recent increases of asthma and allergies in childhood made the need for a standardized approach to international and regional comparisons of their prevalence and severity. To address these issues, 'International Study of Asthma and Allergies in Childhood (ISAAC)' is currently underway. In Korea, 'Nationwide Study of Asthma and Allergies in Korean Children' began in 1995 according to ISAAC protocol. ISAAC written and video questionnaires were used in this survey, but their reliability and validity were not evaluated properly yet. In this study, our aim was to evaluate the reliability and validity of two kinds of questionnaires and their usefulness in international and regional comparisons. The test and retest of two questionniares were completed by male(n=110) and female(n=111) middle school students with two and three weeks interval each. Kappa(or weighted kappa) were calculated from each questions and validity coefficients were estimated from those statistics. In Korean version of written questionnaire, the questions for allergic rhinitis, atopic dermatitis, allergic conjunctivitis, and food allergy proved to have high kappa values (or weighted kappa values) and validity coefficients and they can be used in further studies without any correction. But some questions about asthma(especially nocturnal cough, wheezing in exercise, and severe asthma) and drug allergy need to be revised for better under-standing to study subjects. Video questionnaire has the same degree of reliability and validity when compared to written questionnaire and this is the unexpected result. Accordingly, it also need to be revised to overcome the racial and cultural differences of the study subjects. In conclusion, the Korean version of written and video questionnaires may be considered to be useful methods in international and regional comparisons of asthma and allergic diseases in childhood after correction of some questions.

A New Perspective in Health Approach and the Effect of Health Life on Health Status (건강에 대한 새로운 접근인식과 건강생활 효과)

  • This paper discusses a new perspective in health approach and the effect of health life on health status, and suggests some points to be improved in the establishment of health policies hereafter. Today's health problems changed from the acute epidemic diseases caused by simple factor to the multifactorial chronic diseases. Therefore, the health approach tries the holistic health approach, that is, the integrative approach by the interdisciplinary cooperation. With the advent of holistic health and holistic medicine in 1970's, the behavioral medicine was born in 1977, which is the interdisciplinary field concerned with the development and integration of behavioral and biomedical science knowledge and techniques relevent to prevention, diagnosis and treatment. As a means to the reducement of the unnecessary medical utilization and medical expense, the medical self care and self-help movement is well under way in the countries of America, England, and Canada, in which lay persons are encouraged to actively function for themselves to prevent, detect and treat common illnesses and to promote positive health habits through the health activation programs such as The Course for Activated Patients and The Health Activated Person. This study shows that the individual health life gives great positive effect to his health status with the reducement of medical expense. These above facts suggest that the preventive health programs such as health education should be developed and strengthened instead of medical care-oriented health policy.

A Comprehensive Approach to Posttraumatic Lymphedema Surgical Treatment

  • Background Posttraumatic lymphedema (PTL) is sparsely described in the literature. The aim of this study is to propose a comprehensive approach for prevention and treatment of PTL using lymphovenous anastomosis (LVA) and lymphatic vessels free flap, reporting our experience in the management of early-stage lymphedema. Methods A retrospective observational study was performed between October 2017 and July 2022. Functional assessment with magnetic resonance lymphangiography and indocyanine green lymphography was performed. Patients with lymphedema and functional lymphatic channels were included. Cases with limited soft tissue damage were proposed for LVA, and those with acute or prior soft tissue damage needing skin reconstruction were proposed for superficial circumflex iliac artery perforator lymphatic vessels free flap (SCIP-LV) to treat or prevent lymphedema. Primary and secondary outcomes were limb volume reduction and quality of life (QoL) improvement, respectively. Follow-up was at least 1 year. Results Twenty-eight patients were operated using this approach during the study period. LVA were performed in 12 patients; mean reduction of excess volume (REV) was 58.82% and the improvement in QoL was 49.25%. SCIP-LV was performed in seven patients with no flap failure; mean REV was 58.77% and the improvement QoL was 50.9%. Nine patients with acute injury in lymphatic critical areas were reconstructed with SCIP-LV as a preventive approach and no lymphedema was detected. Conclusion Our comprehensive approach provides an organized way to treat patients with PTL, or at risk of developing it, to have satisfactory results and improve their QoL.

Application of Systems Biology to Traditional Korean Medicine (시스템생물학의 한의학적 응용)

  • In Korea and China, traditional medicine's holistic approaches, based on the views of whole-body and whole-person, have been applied to make the solution of health problem. However, these holistic approaches of traditional korea or chinese medicine have been limited in interpreting their theories in a view of modern scientific aspects of medicine. This limitation seems to be mainly due to the reductionism approaches of modern scientific medicine. Traditionally, science has taken a reductionism approach; dissecting biological systems into their constituent parts and studying them in isolation. However, systems biology based on omics technologies is providing a new thought and method for traditional medicine's research and interpretation. Systems biology uses integrity study as the characteristic and bioinformatic technology as the key method for connecting reductionism and holism. Therefore, it has much in common with the theory of traditional medicine. It was reviewed that how systems biology is applied to traditional medicine in Korea and China. Also it was suggested that more future researches on interpretation between traditional medicine and systems biology must be focused on personalized medicine since systems biology will have a major impact on future personalized therapeutic approaches.

Factors Associated With Success or Failure of Quit Attempts: A Clinical Approach for Lung Cancer Prevention

  • The objective of the study is to investigate the success rate of quit attempts and identify factors associated with success or failure of quit attempts in a quit smoking clinic. A cohort study was conducted with 495 smokers who enrolled in a quit smoking clinic from 2005 to 2008. The factors leading to quit smoking successfully were "being Malay", "having high blood pressure" "type of Nicotine Replacement Therapy" and "duration of follow up". In contrast, clerical staff had negative association to quit smoking. People who started smoking in their teenage years had a high risk of relapse. Integration of active follow up and tailor-made support programmes for quitters appear necessary in order to maintain their non-smoking status and encourage them to be permanent quitters. Integration of quit smoking clinics and primary care clinics could be another potential step for the success of quit smoking programmes.

  • Today, toxicology is used for many purpose, in many fields. Classification of special toxic effect is related next 4 important principles. 1. The chemical substance must move to target organ or tissue that can induce Biological effect. For this movement, we have to understand the physical-chemical characteristic of substance, and the rout of absorption, metabolism, diffusion and excretion of toxic substance. 2. Every biological effect that induced by chemical substance is not harmful. For example, some specific chemical substance is not harmful in liver enzyme system. 3. The strength of biological effect induced by chemical substance is deep related with dose. Nearly all substance is not effective below the specific dose, and it may toxic to death over the specific dose. It is the 'Dose - response relationship' But carcinogen may toxic whether it is law dose or not. 4. The information that was obtained by experimental animal test, could have to adapt in human biology. Because biological effect of chemical substance could be different in every biological species. In past, drugs was obtained by animal or plants. But in the future, it could be obtained by biochemistry, and genome project. Therefore, in Oriental medicine, research and approach is needed at this time, and have to develop new method of experience in toxic method.

Structural Factors of the Middle East Respiratory Syndrome Coronavirus Outbreak as a Public Health Crisis in Korea and Future Response Strategies

  • The recent Middle East respiratory syndrome coronavirus (MERS-CoV) outbreak has originated from a failure in the national quarantine system in the Republic of Korea as most basic role of protecting the safety and lives of its citizens. Furthermore, a number of the Korean healthcare system's weaknesses seem to have been completely exposed. The MERS-CoV outbreak can be considered a typical public health crisis in that the public was not only greatly terrorized by the actual fear of the disease, but also experienced a great impact to their daily lives, all in a short period of time. Preparedness for and an appropriate response to a public health crisis require comprehensive systematic public healthcare measures to address risks comprehensively with an all-hazards approach. Consequently, discussion regarding establishment of post-MERS-CoV improvement measures must focus on the total reform of the national quarantine system and strengthening of the public health infrastructure. In addition, the Korea Centers for Disease Control and Prevention must implement specific strategies of action including taking on the role of "control tower" in a public health emergency, training of Field Epidemic Intelligence Service officers, establishment of collaborative governance between central and local governments for infection prevention and control, strengthening the roles and capabilities of community-based public hospitals, and development of nationwide crisis communication methods.

Analysis of Relating Factors with Customer Loyalties in Medical Services (의료서비스에 대한 고객충성도 관련요인 분석)

  • This study was performed to explore multidimensional customer royalties and relating factors. 900 households, $1\%$ sample were randomly selected from K city located in Kangwon province. Interview survey was performed with structured Questionnaire for the entire people, 923 persons who experienced medical service utilization during one year before survey on time, september, 2004. In comparison of customer royalties by sociodemographic characteristics, the older group showed the higher level of cognitive and attitudinal royalties significantly. Education and income level also, showed negative relationship with cognitive, attitudinal and behavioral loyalties significantly. The more being a female and a visitor at oriental medicine clinic, the higher level of attitudinal loyalties. Customer satisfaction was a critical explaining variable for cognitive, attitudinal and behavioral loyalties. In multiple regression analysis, some sociodemographic characteristics showed significant relation with customer loyalties and customer satisfaction was the strongest relating variable to customer loyalties. In conclusion, multidimensional approach for customer loyalty is useful for understanding customer behaviors comprehensively. Future studies should focus on developing the more specific and valid measurement tools for customer loyalty in medical service.

On the Development of Preventive Electrical Safety Plan Using Operation Concept Document (시스템 운용 개념서 활용을 통한 예방적 전기안전 계획서 개발에 관한 연구)

  • The current electrical safety plan developed for the national electricity system has been playing an important role in handling a variety of electrical accidents. However, it can be more effective to prevent or reduce those accidents if a preventive electrical safety plan is available. In this paper, an approach to developing the safety plan is described. We first discuss the contents that should be included in the plan. Noting that an effective plan requires the system level consideration of all the factors affecting the safety issues, an operation concept document (OCD) is considered. The OCD can allow us to understand the behavior of the safety system based on the operation environment, the system functions, the performance requirements, all at the system level of the safety system. Thus, the OCD can be useful in developing the safety plan. A detailed description then follows on how the OCD is developed for the electrical safety system under study. Finally, we discuss how the developed OCD can be used in deriving the preventive electrical safety plan.
