Prevalence and Factors Associated with Opisthorchis viverrini Infection in Khammouane Province, Lao PDR

  • Opisthorchis viverrini (OV) liver flukes are common parasites found in central and southern Laos and constitute a major public health problem in the country. Laos people continue to have the habit of extensively consuming raw or half-cooked fish which are intermediate hosts. This study aimed to determine the prevalence and factors associated with OV infection in the population of Thakek district, Khammouane Province. This cross-sectional analytic study covered 237 subjects who filled out structured questionnaires. Fecal examination for OV infection was performed by Kato's thick smear method. Data analysis was carried out using STATA Version 10.0. Multiple logistic regression was applied. The results showed that the infection rate of OV was 54.8%. Factors associated with OV infections were gender, a habit of defecation in fields and raw fish (goi bplaa dip) consumption. Opisthorchiasis and associated cholangiocarcinoma development thus appear to remain as important concerns in Laos.

High Endemicity with Clonorchis sinensis Metacercariae in Fish from Yongjeon-cheon (Stream) in Cheongsong-gun, Gyeongsangbuk-do, Korea

  • The infection status with Clonorchis sinensis metacercariae (CsMc) was examined in freshwater fishes from Yongjeon-cheon (a branch of Nakdong-gang) located in Cheongsong-gun, Gyeongsangbuk-do, the Republic of Korea (Korea). A total of 750 fishes in 19 species were examined by the artificial digestion method for 2 years (2019 and 2020). CsMc were detected in 378 (51.4%) out of 735 fishes in 14 species (73.7%), and the infection intensity was 666 per fish infected. In 2019, CsMc were found in 172 (68.0%) out of 253 fishes in 10 species, and the infection intensity was 565 per fish infected. In 2020, CsMc were detected in 206 (62.2%) out of 331 fishes in 10 species, and the infection intensity was 751 per fish infected. The other zoonotic trematode, ie. Metagonimus spp., Centrocestus armatus, Echinostoma spp. and Clinostomum complanatum, metacercariae were also detected in fishes from the survey streams, but their endemicities were relatively low. Conclusively, it was first confirmed that CsMc are highly endemic in fishes from Yongjeon-cheon in Cheongsong-gun, Gyeongsangbuk-do, Korea.

Comparison of Infection between Internal-External and External Percutaneous Transhepatic Biliary Drainage in Treating Patients with Malignant Obstructive Jaundice

  • Purpose: Percutaneous transhepatic biliary drainage (PTBD) is a form of palliative care for patients with malignant obstructive jaundice. We here compared the infection incidence between internal-external and external drainage for patients with malignant obstructive jaundice. Methods: Patients with malignant obstructive jaundice without infection before surgery receiving internal-external or external drainage from January 2008 to July 2014 were recruited. According to percutaneous transhepatic cholangiography (PTC), if the guide wire could pass through the occlusion and enter the duodenum, we recommended internal-external drainage, and external drainage biliary drainage was set up if the occlusion was not crossed. All patients with infection after procedure received a cultivation of blood and a bile bacteriological test. Results: Among 110 patients with malignant obstructive jaundice, 22 (52.4%) were diagnosed with infection after the procedure in the internal-external drainage group, whereas 19 (27.9%) patients were so affected in the external drainage group, the difference being significant (p<0.05). In 8 patients (36.3%) in the internal-external group infection was controlled, as compared to 12 (63.1%) in the external group (p< 0.05). The mortality rate for patients with infection not controlled in internal-external group in one month was 42.8%, while this rate in external group was 28.6% (p< 0.05). Conclusion: External drainage is a good choice, which could significantly reduce the chance of biliary infection caused by bacteria, and decrease the mortality rate at one month and improve the long-term prognosis.

Infection with Opisthorchis viverrini and Use of Praziquantel among a Working-age Population in Northeast Thailand

  • Infection with Opisthorchis viverrini (OV) due to eating certain traditional freshwater fish dishes is the principal risk factor for cholangiocarcinoma in Northeast Thailand where the infection is endemic and the incidence of this form of primary liver cancer has been the highest in the world. This paper is the second report of a prospective research project to monitor the impacts of a national liver fluke control programme in a rural community of Northeast Thailand. A sample of 684 villagers aged 20-65 years completed an interview questionnaire and were tested for infection using the Kato thick smear technique. The questionnaire was designed for the exploration of associations between OV infection, previous treatment with praziquantel, and knowledge and beliefs about the drug. The data were analysed using descriptive statistics and multiple logistic regression. The overall prevalence of OV infection was 37.2% and was highest in the 20-35 year age group, in those with a university degree and in those employed in the government sector. As many as 91.8% reported eating fish dishes known to place them at risk of infection. In the multiple regression analysis, previous use of praziquantel and lack of knowledge about whether or not the drug has a protective effect against re-infection were the only factors related to OV infection ($OR_{adj}$= 2.31, 95%CI =1.40-3.79 and $OR_{adj}$= 1.95, 95%CI= 1.24-3.05). The findings were discussed in terms of the possibly unwise dependency on praziquantel as a primary element in a control programme.

임상실습 중 치위생학과 학생들의 감염관리에 대한 인식과 수행정도에 관한 연구 (The study of awareness and performance of Dental hygiene students for infection control during clinical training)

  • 본 연구는 임상실습을 경험한 치위생학과 학생들을 대상으로 임상실습지에서 치과감염관리에 대한 인식과 수행정도를 조사하여 병원내 감염을 최소화 시키고 교육과정개발과 교육프로그램을 마련할 때 감염관리 지침과 행동지침에 필요한 기초자료를 제공하기 위해 시행하였다. 경북에 소재하는 4년제 대학에서 임상실습을 실제 경험한 치위생학과 학생을 대상으로 설문조사하였으며 치과의원, 치과병원, 대학병원이나 종합병원에 실습한 학생을 구분하여 각 실습지에 따른 치과감염관리에 대한 인식과 수행정도를 비교하였다. 연구결과 학생들은 감염방지에 대한 교육 경험자가 많았으나 실습지에 따라 감염방지관리의 수행정도는 대학병원이나 종합병원에서 높았으며 실습지에 따라 차이가 나타났다. 따라서 학생들이 임상실습시 감염관리에 대한 인식과 수행정도를 높이기 위해서는 감염관리 지침서를 제작하여 임상실습기관에 배부하고 담당자를 통해 관리하며 학교 측에서는 임상실습과 연계성 있는 감염관리 교육프로그램을 운영하여 체계적인 제도를 마련해야 할 것으로 생각된다.

보건소 방문간호요원의 감염관리에 대한 중요성 인지도 및 실천도 (Perception of Importance and Performance in the Infection Control of Visiting Nursing Personnel in Public Health Centers)

  • The work of visiting nursing personnel consists mostly of direct nursing and it is absolutely necessary that training of the nursing personnel for infection control be emphasized because most of patients are composed of elderly people with chronic diseases and malnutrition who are highly susceptible to infection. Accordingly. this study was designed to provide basic data for the development of an infection control program in public health centers as well as the promotion of perception and performance of infection control by analysing the perception of importance and performance in the infection control of visiting nursing personnel who carry out visiting nursing services. The subjects of this study consisted of a number of visiting nursing personnel working for 15 public health centers. 15 health-center branches. and 16 health clinics. The collection of data was grounded on a questionnaire method. The questionnaire breaks down to 8 questions about general characteristics and 108 questions in connection with the perception and performance of infection control. The results of this study are as follows: 1. 82 subjects (78.1%) responded that it is necessary to control infection. 79 subjects(75.3%) responded that education is necessary for infection control. 50 respondents (47.6%) answered in the affirmative in regard to the role of infection carriers. 2. The mean score for the perception of importance in infection control was 4.55 and that performance accounts for 4.03. this turns out to be very high. 3. In terms of the perception of importance per each field of infection control. medication. the management of contamination and sterilization, the control of environments and instruments. washing hands and individual hygiene as well as the prevention of cross-infection and the supervision of infection stand at 4.72. 4.64. 4.60. 4.58. 4.52 and 4.21 respectively. In terms of performance. the supervision of infection. the control of environments and instruments, individual hygiene and the prevention of cross infection. medication. washing hands, and the management of contamination and sterilization was 4.21. 4.12. 4.10. 4.03. 3.91 and 3.81 respectively. 4. In terms of the correlation between the perception of importance and the performance of infection control. the score stands high in all fields: this means the high perception of importance leads to high performance statistically (P< .05). 5. In terms of the perception of importance in general characteristics. there is no significant difference while the form of working places. working career and the career as visiting nursing personnel show a significant difference in terms of performance(P< .05). From the results. it is suggested that the visiting nursing personnel should be constantly educated with the aid of the development of infection control programs and that instructions and instruments for the management of infection should be worked out so as to enhance the performance of infection control on the part of nurses in applicable communities.

치과위생사의 진료실 감염방지에 대한 행태 분석 (The Analysis of the Prevention against Virus Infection in Dental Hygienist at Medical Treatment)

  • 1. 조사 대상자는 총 128명으로 연령은 24세~28세가 71.1%(91명)으로 가장 많았으며, 임상경력은 2년~5년 사이가 36.7%(47명) 우세하게 나타났으며 일 진료환자수는 56.3%(72명)가 25명 이하이고, 대부분 치과위생사수가 3명 이하인 치과의원에 근무하는 것으로 조사되었다. 2. 감염노출 행태 및 감염방지처치실태는 응답자의 76.6%(98명)가 날카로운 기구나 주사바늘에 찔린 경험이 있으며 대부분이 즉시 소독하는 방법으로 감염방지 처치를 하며 진료시 감염은 경험하지 않은 것으로 조사되었다. 3. 감염방지교육 실태에 대한 조사결과 응답자의 75.8%(97명)가 감염관리에 대한 이론과 시술법에 대한 교육경험이있는 것으로 응답하였으며 89.1%(114명)가 병원의 감염 관리에 대한 교육이 필요하다고 생각하며 학생시절(49.2%, 63명)에 학교교육을 통해서(45.3%,58명) 경험하였으며 응답자의 81.3%(104명)가 감염방지교육의 확대를 주장하고 있는 것으로 나타나 학교교육을 통해서 뿐만 아니라 지속적인 보수교육이 필요하다고 판단된다. 4. 치과위생사의 감염방지 행태분석결과 의료용 장갑을 사용하는 비율은 24세~28세의 임상경력이 5년~10년이면서 치과병원에 근무하는 치과위생사일수록 높았으며 진료 후 대부분이 폐기하며 진료시 마스크를 착용하는 비율은 29세, 임상경력이 10년 이상의 경우에서 높게 나타났으며 근무하는 병원규모에 상관없이 높게 나타났으며 진료시 마스크 습기가 찰 경우 대부분이 교체하는 것으로 나타났다. 5. 환자진료시 보안경사용 및 기구세척 과 재처리시 고무장갑을 사용하는가에 대한 조사 결과 보안경을 사용하는 비율은 나이, 임상경력, 근무하는 병원규모에 상관없이 낮은 것으로 나타났으며 대부분이 기구세척과 재처리시 고무장갑을 사용하는 것으로 나타났다. 6. 진료감염방지 교육 경험자와 비경험자에 대한 진료시 감염방지행위의 차이를 분석하기 위하여 각종 감염방지행태 항목을 변수로 하여 독립표본 T검증(Independent -sample T Test)을 실시한 결과 대부분의 항목에서 진료감염방지 교육을 경험한 치과위생사가 진료시 감염방지 행위를 잘하고 있는 것으로 나타났다. 7. 진료 시 의료용 장갑을 사용 및, 진료 후 의료용 장갑폐기, 기구세척 및 재처리시 두꺼운 가사용 고무장갑 사용의 항목에서 두집단 간에 유의수준 0.05, 0.01에서 각각 의미 있는 차이가 있었으며 나머지 3개 항목은 유의수준 0.05에서 의미 있는 차이를 보이지 않았다. "진료시 마스크 사용"에 대한 항목에서는 두집단 간에 큰 차이를 보이지 않았는데 이는 감염방지교육과 상관없이 진료시 기본필수사항으로 인식하기 때문인 것으로 판단된다.

치과위생사의 감염관리실태와 관련요인 (Infection control realities and relevant factors in dental hygienists)

  • Objectives : The purpose of this study was to improve the practice level on dental hygienists'infection prevention by examining the actual condition of dental hygienists'infection control and analyzing factors of having influence upon this. Methods : A questionnaire survey was carried out from July 18, 2011 to August 26 targeting dental hygienists who work at medical institutions where are located in Daegu, Busan and Ulsan Metropolitan Cities and Gyeongsangnam buk-do. After then, the following conclusions were obtained. Results : 1. As a result of surveying practice by item in the practice level of infection prevention, the items with high practice level in the management of infectious diseases were surveyed to be 12 months(89.6%) for health-checkup cycle and to be having experience of vaccination(78.0%) for hepatitis type B. The items with high practice level in the management and practice of washing hands were indicated to be in order of regularly paper towel(87.7%) and hands cleaning after regular medical examination(80.5%). In the item of the practice on wearing and managing individual protection equipment, regularly wearing(93.1%) rubber globes given washing implements was indicated to be high. 2. As a result of analyzing working career, working institution, working region, and practice level of infectious-disease management, the appearance of fulfilling infection control guidelines at medical institution and the experience of education for infection control were indicated to have difference depending on working institution. Regularly health checkup was indicated to have difference depending on respondents' working career and working institution. 3. The whole average in the practice level of infection prevention according to working career, working institution, and working region was indicated to be 2.55 out of 3-point perfection. 4. Wearing latex gloves was indicated to have statistically significant difference depending on working institution(p<0.001) and working region(p<0.001). The exchange of latex gloves every patient and the use of paper apron had statistically significant difference depending on working region(p<0.001). 5. As a result of comparing the frequency of using protection equipment for preventing infection according to the management of infectious diseases, the statistically significant difference was shown depending on the appearance of infection control guidelines at medical institution(p<0.001), the appearance of having experience of health checkup(p<0.01), and the appearance of having experience of vaccination for hepatitis type B(p<0.05). Conclusions : The above-mentioned findings showed that the denture satisfaction of the denture-wearing senior citizens was linked to their subjective oral health awareness. Therefore it will be possible to improve denture-wearing elderly people's quality of life when oral health plans geared toward boosting their denture satisfaction are carried out.

Knowledge, Attitude and Practice of School Nurses in the United Arab Emirates about HPV Infection and Vaccine

  • Background: In 2008, the Health Authority in Abu Dhabi (the capital of the United Arab Emirates) introduced HPV vaccine free of charge for high school girls entering grade 11, becoming the first state in the Middle East to do so. The objectives of this study were to assess the knowledge, attitude and practice of school nurses in the Emirate of Abu Dhabi about HPV infection and the vaccine. Materials and Methods: A quantitative study was designed and conducted from June to August 2012 in Emirate of Abu Dhabi. Data were collected through direct face to face interviews. from one hundred and twenty five nurses. Results: Knowledge of HPV infection and HPV vaccine was almost universal among the school nurses (97%). The majority of the participants (71%) thought that the HPV vaccine was good. Cultural unacceptability (45%) and lack of women's concern about their own health (21%) were rated as the top barriers for the successful introduction of the vaccine in the UAE. More than half of the sampled nurses (58%) have either given this vaccine to school girls or taken it themselves. The majority (95%) did not come across any side effects from the vaccine. The level of qualification and the place of work did not significantly affect the correct knowledge of HPV infection or cervical cancer prevention methods. Conclusions: The knowledge and attitude of the sampled school nurses in Abu Dhabi State about HPV infection and vaccine is very good in both the public and private sectors. However, a knowledge gap in cervical cancer screening methods was identified.
