The UGT1A9*22 genotype identifies a high-risk group for irinotecan toxicity among gastric cancer patients

  • Several studies have shown associations between irinotecan toxicity and UGT1A genetic variations in colorectal and lung cancer, but only limited data are available for gastric cancer patients. We evaluated the frequencies of UGT1A polymorphisms and their relationship with clinicopathologic parameters in 382 Korean gastric cancer patients. Polymorphisms of UGT1A1*6, UGT1A1*27, UGT1A1*28, UGT1A1*60, UGT1A7*2, UGT1A7*3, and UGT1A9*22 were genotyped by direct sequencing. In 98 patients treated with irinotecan-containing regimens, toxicity and response were compared according to the genotype. The UGT1A1*6 and UGT1A9*22 genotypes showed a higher prevalence in Korean gastric cancer patients, while the prevalence of the UG1A1*28 polymorphism was lower than in normal Koreans, as has been found in other studies of Asian populations. The incidence of severe diarrhea after irinotecan-containing treatment was more common in patients with the UGT1A1*6, UGT1A7*3 and UGT1A9*22 polymorphisms than in controls. The presence of the UGT1A1*6 allele also showed a significant association with grade III-IV neutropenia. Upon haplotype and diplotype analyses, almost every patient bearing the UGT1A1*6 or UGT1A7*3 variant also had the UGT1A9*22 polymorphism, and all severe manifestations of UGT1A polymorphism-associated toxicity were related to the UGT1A9*22 polymorphism. By genotyping UGT1A9*22 polymorphisms, we could identify high-risk gastric cancer patients receiving irinotecan-containing chemotherapy, who would experience severe toxicity. When treating high-risk patients with the UGT1A9*22 polymorphism, clinicians should closely monitor them for signs of toxicity such as severe diarrhea or neutropenia.

  • Recently, with the development of information and communication infrastructure, the number of Internet access devices is rapidly increasing. Smartphones, laptops, computers, and even IoT devices are receiving information and communication services through Internet access. Since most of the device operating environment consists of web (WEB), it is vulnerable to web cyber attacks using web shells. When the web shell is uploaded to the web server, it is confirmed that the attack frequency is high because the control of the web server can be easily performed. As the damage caused by the web shell occurs a lot, each company is responding to attacks with various security devices such as intrusion prevention systems, firewalls, and web firewalls. In this case, it is difficult to detect, and in order to prevent and cope with web shell attacks due to these characteristics, it is difficult to respond only with the existing system and security software. Therefore, it is an automated defense system through the collection and analysis of web shells based on artificial intelligence machine learning that can cope with new cyber attacks such as detecting unknown web shells in advance by using artificial intelligence machine learning and deep learning techniques in existing security software. We would like to propose about. The machine learning-based web shell defense system model proposed in this paper quickly collects, analyzes, and detects malicious web shells, one of the cyberattacks on the web environment. I think it will be very helpful in designing and building a security system.

Low serum 25-hydroxyvitamin D levels, tooth loss, and the prevalence of severe periodontitis in Koreans aged 50 years and older

  • Purpose: Vitamin D deficiency may cause bone loss and increased inflammation, which are well-known symptoms of periodontal disease. This study investigated whether serum 25-hydroxyvitamin D (25(OH)D) levels are associated with periodontal disease status and tooth loss. Methods: Cross-sectional data from 5,405 individuals aged ≥50 years (2,253 males and 3,152 females) were obtained from the 2008-2010 Dong-gu study, a prospective cohort study of risk factors for chronic diseases. Periodontal examinations were conducted to evaluate the number of remaining teeth, the periodontal probing depth (PPD), the clinical attachment level (CAL), and bleeding on probing. The percentages of sites with PPD ≥4 mm and CAL ≥4 mm were recorded for each participant. The severity of periodontitis was classified using the Centers for Disease Control and Prevention and the American Academy of Periodontology case definitions. Serum 25(OH)D levels were classified as reflecting severe deficiency, deficiency, insufficiency, or sufficiency. Multivariate linear regression analysis was performed to assess the associations of serum 25(OH)D levels with periodontal parameters and the number of remaining teeth after adjusting for confounders including age, smoking status, alcohol consumption status, month of blood collection, and physical activity. Multivariate logistic regression was used to evaluate the association between serum vitamin D levels and severe periodontitis. An overall statistical analysis and a stratified analysis by sex were performed. Results: Overall, the rates of severe deficiency, deficiency, insufficiency, and sufficiency were 6.5%, 67.9%, 22.4%, and 3.2%, respectively. After adjustment for confounders, vitamin D levels were directly associated with the number of remaining teeth, an association that was significant in males, but not in females. Sufficient serum 25(OH)D was associated with a low frequency of severe periodontitis. Conclusions: This population-based cross-sectional study indicates that low serum 25(OH) D is significantly associated with tooth loss and severe periodontitis in Koreans aged 50 years and older.

  • Nowadays, urban townhouses are being developed in various forms according to the characteristics of different regions in consideration of the trends of the housing market. Misperceiving the needs of consumers or their characteristics as a house for living, however, they often end up becoming products that are not suitable for urban life or degraded on account of reckless regional development. It is so unfortunate that such trial and error keeps being repeated. Urban townhouses are advantageous because there is no such problem as either invasion of privacy or noise from neighbors, and it is possible to have one's own garden and enlarged parking space, obtain quality of grounding, and plan unique interior and exterior design. They are also equipped with the strengths of apartment houses as well, for example, the efficiency of joint control in crime and disaster prevention or security, architecture of diaphragm walls with the separation of gates, or the planning of common space like a central square or park. Therefore, there is a great chance that they can be developed as the types of urban housing. Accordingly, the purpose of this study is to establish the basic direction of developing housing products right as space for urban life and maximize the roles of urban townhouses. By understanding their spatial as well as functional elements as a house for living, this author aims to provide a guideline for housing product development to realize urban townhouses that can meet consumer needs.

A Study of Determinants of Video-on-Demand View : Focusing on the Correlation between COVID-19 and Movie Views (영화 VOD 시청 건수 결정요인 : 코로나 19와 영화 시청의 관계를 중심으로)

  • The government's social distancing policy and concerns about COVID-19 are increasing restrictions on outdoor leisure activities. Based on the decrease in outdoor leisure activities and the increase in indoor leisure activities, The purpose of this study is to examine the correlation between the degree of new confirmed cases of COVID-19 and the number of VOD views. This study conducted a time series analysis for 348 days from February 18, 2020 to January 31, 2021. Data were collected from the number of daily VOD views provided by the Korean Film Council and the number of new confirmed cases of COVID-19 provided by the Korea Centers for Disease Control and Prevention. The analysis showed that the number of confirmed COVID-19 cases has a significantly positive effect on the number of daily movie VOD views at the 5% significance level. This results indicate that the more confirmed cases of COVID-10, the more people watch movie VOD as indoor leisure activities. While previous studies examined the relationship between the confirmed cases of COVID-19 and indoor leisure activities in general, this study tried to academically contribute by analyzing the impact on specific indoor leisure activities. The practical implications of this study are as follows. The results of this study show that efficient promotions are possible based on significant social issues, such as infectious diseases. According to the results, promotions that respond quickly to changes are more effective than long-term promotions considering the climate or seasons. Due to the limitations of the data, the current study was conducted based only on PPV, but future research should also consider various billing forms such as PPM and SVOD.

  • The prevalence of obesity is increasing worldwide, and since obesity is associated with dietary factors and sedentary lifestyles, it is a disease that is readily developing in the modern population. Because obesity is accompanied by serious complications such as diabetes and cardiovascular disease, prevention and treatment are important. Currently, drugs such as liraglutide and phentermine are used to treat obesity by suppressing appetite and inducing gastrointestinal motility delay. However, various side effects may occur, including thyroid cancer, cardiovascular problems, and central nervous system disorders. Therefore, to explore an obesity treatment method with relatively few side effects, a method known as "fat browning" was introduced to change white adipose tissue into brown adipose tissue to increase energy consumption. Ongoing studies are attempting to find effective natural substances to safely induce browning. Many natural substances have been identified. The induction of browning by treatment with natural substances generally involves three mechanisms: positive control of browning-inducing factors, inhibition of differentiation into white adipose tissue, and the activation of mechanisms related to browning. In this study, we describe plant extracts with known browning-inducing effects, such as strawberry, black raspberry, cinnamomum cassia, and Ecklonia stolonifera extracts. We also summarize the underlying mechanisms of action identified thus far, including the signaling pathway mediated by these extracts to induce browning. Furthermore, the effects of brown adipose tissue generated through browning on heart disease as an endocrine organ disruptor are discussed.

Effect of Low Pressure Steam Explosion Treatment on Prevention of Resin Exudation from Wood under High Temperature Surroundings (저압증기폭쇄(低壓蒸氣爆碎)처리가 고온영역(高溫領域)하에서의 목재(木材) 수지삼출(樹脂渗出) 예방에 미치는 영향)

  • This study was carried out to find the efficient methods for preventing resin exudation from spruce board during high temperature environment service.In the dry-air oven test, in-use temperature related to resin exudation of 110℃ could be obtained by the appropriate kiln drying without any special treatment, and the in-use temperature of the radio-frequency/vacuum (RF/V)-dried boards was about 20℃ higher than that of the conventional kiln-dried boards.In the autoclave test, resin exudation was not found from any board dried in the conventional kiln and in the RF/V kiln after the low pressure steam explosion (SE) treating, while for the control resin exudations were more severe in the autoclave test above 130℃ than in the dry-air oven test. It, therefore, would be suggested that resin exudation during high temperature service condition can be prevented more effectively by super-heated steaming green boards than by high-temperature drying boards with low moisture.

Characteristics of Fluoride-based Anti-stain Chemicals Made from Industrial By-product (II) -Fungicidal Effectiveness against Isolated Fungi Through Laboratory Evaluation and Field Evaluation- (산업 부산물을 이용하여 제조한 플루오르화합물계 목재 방미제의 특성 (II) - 분리 균주에 대한 목재 방미효력 및 야외 효력 평가 -)

  • Two kinds of chemicals (RNF-3 and RNF-4) with high anti-mold effectiveness against stored fungi causing fungal discoloration was selected from among the six kinds of fluoride-based chemicals in the preceding study. In this study, the anti-mold test using 16 species fungi isolated from the softwood lumbers which were fungal discolored and field test in the sawmill was carried out to prove the feasibility for practical using of selected chemicals.For the isolated fungi, the RNF-3 consist of F and Cu showed high mycelial growth control in the PDA medium and fungicidal effectiveness in the japanese red pine (Pinus densiflora), korean pine (Pinus koraiensis) and radiata pine (Pinus radiata) when the concentration was 2% or more. However the RNF-4 consist of F only was not effective compared with RNF-3 because of mycelial growth in the PDA medium and wood treated with 2% or more chemical solution. The RNF-3 also showed a strong anti-mold effectiveness because there was no fungal discoloration for the radiata pine boards treated by 2% and 10 min. soaking in the field test. These results mean that RNF-3 can be used as domestic anti-stain chemicals for prevention of fungal discoloration of the softwood lumber

A Study on the Particle Size of Sand to Prevent Penetration of Subterranean Termite (Reticulitermes speratus kyushuensis) in Wooden Buildings (국내 지중흰개미의 목조건축물 유입 차단을 위한 모래의 적정 입도 연구)

  • Termites cause massive damage to wooden architectural heritage structures. Chemical treatments have been commonly used to control them. In foreign countries, physical barriers made of sheet and particles impenetrable to termite are being used as an alternative to the chemical method. To study the efficacy of physical barriers, we investigated the appropriate sand particle size that can prevent the penetration of R. speratus kyushuensis. Upon evaluating the barrier properties of sand with particle sizes ranging from 0.85 to 4.00 mm, the penetration of termites was found to be effectively blocked at a particle size range of 1.00 to 2.80 mm. At smaller particle sizes, termites managed to move the sand particles and build an almost linear mud tube to penetrate the sand layer. At larger particle sizes, the termites could penetrate the sand layer by passing through the sand gaps.

Effects of an Infection Control Protocol for Coronavirus Disease in Emergency Mechanical Thrombectomy

  • Objective : Since the outbreak of the coronavirus disease 2019 (COVID-19) pandemic, neurointerventionists have been increasingly concerned regarding the prevention of infection and time delay in performing emergency thrombectomy procedures in patients with acute stroke. This study aimed to analyze the effects of changes in mechanical thrombectomy protocol before and after the COVID-19 pandemic on procedure time and patient outcomes and to identify factors that significantly impact procedure time. Methods : The last-normal-to-door, first-abnormal-to-door, door-to-imaging, door-to-puncture, and puncture-to-recanalization times of 88 patients (45 treated with conventional pre-COVID-19 protocol and 43 with COVID-19 protection protocol) were retrospectively analyzed. The recanalization time, success rate of mechanical thrombectomy, and modified Rankin score of patients at discharge were assessed. A multivariate analysis was conducted to identify variables that significantly influenced the time delay in the door-to-puncture time and total procedure time. Results : The door-to-imaging time significantly increased under the COVID-19 protection protocol (p=0.0257) compared to that with the conventional pre-COVID-19 protocol. This increase was even more pronounced in patients who were suspected to be COVID-19-positive than in those who were negative. The door-to-puncture time showed no statistical difference between the conventional and COVID-19 protocol groups (p=0.5042). However, in the multivariate analysis, the last-normal-to-door time and door-to-imaging time were shown to affect the door-to-puncture time (p=0.0068 and 0.0097). The total procedure time was affected by the occlusion site, last-normal-to-door time, door-to-imaging time, and type of anesthesia (p=0.0001, 0.0231, 0.0103, and 0.0207, respectively). Conclusion : The COVID-19 protection protocol significantly impacted the door-to-imaging time. Shortening the door-to-imaging time and performing the procedure under local anesthesia, if possible, may be required to reduce the door-to-puncture and door-to-recanalization times. The effect of various aspects of the protection protocol on emergency thrombectomy should be further studied.
