Analysis on the Characteristics of Hazardous Chemical Transport Vehicle Accidents in Korea (국내 유해화학물질 운송차량사고 특성분석)

  • Purpose: In this study, the characteristics of hazardous chemical transport vehicle accidents during the last six years from 2014 to 2019 was analyzed. This study is to prevent chemical transport accidents. Method: Using statistics from the Ministry of Environment, chemical transport vehicle accidents were classified into accident type, accident cause and vehicle type. Result: Of the total 506 cases chemical accidents, 105 cases were caused by transport vehicle. More than 20 percent of total accidents were reported. Most of the accidents were caused by spill·leak. The main causes of the accident were traffic accidents(49 cases) and management negligence(29 cases). This was more than 74% of all transport vehicle accidents. By vehicle type, 57 cases(54%) were most common in tank lorries, followed by trucks (39 cases, 37%) and trailers (9 cases, 9%). Conclusion: In order to prevent accident of transport vehicle, thorough inspection of the transport vehicle is required and safe operation of the driver. In addition, the government needs to expand real-time monitoring of transport vehicles and comprehensively manage different systems by the relevant ministries.

The Study on Common Factors of Typical CFIT Accident with Go-around Failure and Go-around Gate Operation of Foreign Carriers (An Analysis of Korean CFIT Accidents through TEM) (복행실패로 발생한 CFIT사고의 공통요인 및 외항사 복행게이트 운영 실태에 대한 연구 (한국 대표적 CFIT사고의 TEM 분석을 중심으로))

  • There have been CFIT(Controlled Flight Into Terrain) accidents that can be prevented if the crew executed go-around. This study is to analyse the common factors of three typical CFIT accidents of Korea in TEM(threat and error management) frame, and the examples of go-around gate and the countermeasures of eight airlines through the survey facilitating go-around to prevent CFIT. The common factors found in three typical CFIT accidents occurred in Korea or by Korean carriers turned out to be in mountainous terrain, in bad weather while in non-precision approach or circling approach by captain as PF(Pilot Flying) when crew make monitoring errors and communication errors. It also turned out that the crew in all three typical tragic CFIT accidents did not execute go-around in unstabilized approaches. The captains did not respond immediately when first officers advised them to go-around until it is too late. Seven out of eight Airlines answered that they use stabilized approach height as 1,000 feet to be stabilized earlier to have more safety margin by enhancing go-around gate regardless of the weather to prevent CFIT in the survey.

  • Population aging and the usage of small devices in implant prosthetic procedures have led to many incidents of dental aspiration and ingestion. Various preventive measures have been introduced to prevent these accidents. However, accidents can occur at any time. Dental aspiration and ingestion lead to fatal consequences if the issue is not promptly and appropriately dealt with. Preparing a collaborative system for dealing with accidents before they occur can prevent further sequelae. This study involves ingestion and aspiration accidents that occurred during dental treatment: two ingestion cases and one aspiration case. All dental foreign bodies were removed according to the guidelines presented in our review. With the cooperation of other medical departments, the issues were quickly resolved. Simple and accurate protocols should be provided to all dentists and dental staff to respond to such dental emergencies. In addition, collaboration among other medical departments should be established before any accidental ingestion and aspiration events occur.

  • This study analyzed the accidents and researched on risk of the actual condition in roof works. This study revealed that 68% of accidents in roof works occurred on small sized construction sites. And 79% of accidents are caused by fall. It is simply because most laborers do not wear the safety harness and the mounted equipment is not properly installed. According to our research, we realize that most of roof works have some risk of the fall accidents. Most of roof working sites have the danger of falling at ladder and temporary walkway. In oder to prevent accidents in roof works, the basic rule such as wearing the safety harness must be followed.

  • Using transportations like railway, aviations and roads have been increasing continuously. Traffic accidents have been increasing as well. To prevent or lessen these accidents become a big issue to solve. Therefore, more enforced safety levels are being required to meet. In railway field, continuous efforts of railway safety are being executed to prevent traffic accidents. In present, railway accidents have been decreasing since subway accident happened in Daegu Subway in 2003. However, safety levels in railway field have not yet achieved as much as in advanced countries. Basic concept for enforced safety level in railway system is to prevent or minimize accidents by managing systems like related facilities, management method, organization, education. This is the same concept to minimize error rate in six sigma theory. In this paper, we are to apply six sigma techniques used in manufacturing industry to railway safety and go through ways to make more efficient railway safety system.

Hands-on Tools to Prevent Human Errors in Highway Construction (고속도로 건설현장의 인적오류 예방을 위한 실무자용 도구 개발)

  • Objective: The aim of this study is to reclassify human errors and to develop hands-on tools to apply the new classification for preventing human error accidents in highway construction site. Background: The main cause of accidents in highway construction was reported as the carelessness of workers. However, such diagnosis could not help us operationally prevent accidents in real workplace. Method: The accidents in highway construction were reanalyzed and the causes of human error were reclassified in order to educate and improve the awareness of human error in highway construction. Field survey and interview with safety managers and workers were conducted to find the causal relationship between the actual accidents and the human errors. Results: The most frequently observed human errors in highway construction were classified into six categories such as mis-perception, distraction, memory fail, slip, cognition error and mis-judgment. In order to provide hands-on tools to increase the awareness of human error in construction field, the human error checklist and card sorting diary were developed. Especially, the card sorting diary was designed to increase the ability in human error inspection of safety manager at construction site. Moreover, posters were developed based on actual accident cases. Conclusion: We suggested that the improved awareness and analytical report on checklist, card sorting diary and posters for construction field could collectively prevent the accident. Application: The classification of human error, hands-on tools and posters can be directly applicable on highway construction site. This analytical and collective approach preventing human error-related accident could be extended to other construction workplaces.

  • With the introduction of the law regarding severe penalties for major accidents, employers, management executives, and corporations have significantly increased the number of safety managers and invested extensively in acquiring ISO certifications to prevent accidents in industrial sites. Moreover, the implementation of the Smart Safety Management System (SSMS) has facilitated the management of personnel and safety equipment. While IoT-based management systems have been applied to safety gear such as helmets, safety harnesses, and protective clothing, the responsibility for safety shoes still primarily lies with on-site managers and individuals, leaving a vulnerability to accidents. In this study, we aim to implement a Raspberry Pi-based sensor device to proactively detect workers' safety shoe usage upon entering the site. The goal is to confirm the usage of safety shoes and prevent accidents that may occur due to non-compliance with safety shoes regulations.

Development of Drowsiness Checking System for Drivers using Eyes Image Histogram (눈 영상의 히스토그램을 이용한 운전자의 졸음 상태 체크 시스템 개발)

  • Approximately 23% of traffic accidents appear to be caused by drowsiness while driving. This fact shows that drowsy driving is a big factor in many traffic accidents. Therefore, the development of a drowsiness checking system is necessary to prevent drowsy driving. In this paper, we analyse the changes of the histogram of eye region images which are acquired using a CCD camera. We develop a drowsiness checking system using this histogram change information. The experimental results show that our proposed method enhances the accuracy of checking drowsiness by nearly 98%, and can be used to prevent vehicle accidents due to the drowsiness of a driver.

  • The number of deaths in the korean construction industry is more than three times the OECD average. Although safety management system should be improved to prevent the safety accidents, it is difficult to improve due to domestic safety conditions. Especially, in order to prevent accidents at construction sites, there is an increasing tendency to monitor the movement of workers and equipment in real time by introducing a location positioning system. Therefore, this study proposes a system that can monitor the position of workers and heavy equipments in real - time, detect danger and transmit alarms so that workers can pay attention to safety and keep safety. The system is expected to reduce safety accidents by transmitting alarms to workers so that they can pay attention.

  • As the construction industry has been increasingly complex, aging workers, hard-working avoiding young generation, foreign workers having language problem for the past decades, delivering of construction qualified workers has been a rising problem. This misalignment between the complex jobs and the number of skilled workers will continue to be in issue that the construction industry will face for following decades. Construction field workers who are working outside in uncontrollable condition unlike manufacturing industry are exposed to easily construction safety accident. There are so many efforts to prevent and control the construction accident. However, the relationships between the construction works and construction safety accident has not been well dealt and investigated based on objective accident data except for the qualitative study through interview, delphi and so on. Therefore this study analyzes job risk matrix exposed to safety accidents based on statistic data for 20 years from Korean Occupational Safety & Health Agency in order to prevent the construction field accidents.
