A Study on Analysis and DMAIC Preventive Operations for Cargo Handling Accidents In Inchon Port (6시그마 기법을 적용한 인천항 항만하역 재해예방 대책에 관한 연구)

  • Many countries of the world is driving forward development of large-scale hub port. They are now pushing ahead with setting up of effective marketing strategies to survive in keen competitions of the 21st century port industry. The port is of ever increasing importance for the bridge connecting sea and road transportation in handling international cargoes. The port, differently from general working places, is a closed area required for security, customs, and quarantine procedures. The loading and unloading is being done differently by ports, cargoes, and ships. To do loading and unloading, a lot of equipment and different types of labor are required, which flow is complicated and safe management is essential. As above mentioned the port is very unique and very deteriorated working place in its working environment. The purpose of this study is to propose ways to reduce and prevent from port accidents. As first step to do this, we have collected 923 accidents happened at Incheon Port during the period of 1994 to 2003. We have thoroughly analyzed characteristics, harmfulness, and risk of the loading/unloading they have done, as well as the accident frequency and relationship between the accidents. As second step to further analyze, We have employed DMAIC technology, an advanced process of 6 sigma presently in spotlight as the best program for management innovation. This analysis results in recognition of important accident characteristics, causes and effects analysis, critical causes of accident, and suggestions to decrease accidents.

Theoretical Model for Accident Prevention Based on Root Cause Analysis With Graph Theory

  • Introduction: Despite huge investments in new technology and transportation infrastructure, terrible accidents still remain a reality of traffic. Methods: Severe traffic accidents were analyzed from four prevailing modes of today's transportations: sea, air, railway, and road. Main root causes of all four accidents were defined with implementation of the approach, based on Flanagan's critical incident technique. In accordance with Molan's Availability Humanization model (AH model), possible preventive or humanization interventions were defined with the focus on technology, environment, organization, and human factors. Results: According to our analyses, there are significant similarities between accidents. Root causes of accidents, human behavioral patterns, and possible humanization measures were presented with rooted graphs. It is possible to create a generalized model graph, which is similar to rooted graphs, for identification of possible humanization measures, intended to prevent similar accidents in the future. Majority of proposed humanization interventions are focused on organization. Organizational interventions are effective in assurance of adequate and safe behavior. Conclusions: Formalization of root cause analysis with rooted graphs in a model offers possibility for implementation of presented methods in analysis of particular events. Implementation of proposed humanization measures in a particular analyzed situation is the basis for creation of safety culture.

Revision of Safety Standards through Analysis of Fatal Accidents Related to Mobile Scaffold in Construction Sites (건설현장 이동식 비계의 사망사고 분석을 통한 안전기준 개선 방안)

  • Mobile scaffold is used as a work platform in construction sites. According to industrial accident data, 10 fatalities occur annually on average due to falling from mobile scaffold in construction sites. To prevent such fatal accidents, safety standards must be improved by analyzing the causes of fatal accidents related to mobile scaffold. Accordingly, we analyzed domestic and international safety standards for mobile scaffold and analyzed the fatalities (117 persons) caused by mobile scaffold accidents in domestic construction sites during 2011-2020 and found that fall, overturn, and collapse accounted for 88% (103 persons), 10.3% (12 persons), and 1.7% (2 persons) of fatalities, respectively. The main causes of fatal accidents were found to be non-installation or insufficient installation of safety rails and outriggers and incorrect sequence of work. Based on the analysis results, we proposed strategies for improving the safety standards, such as the use of advanced guard rails and wheel-attached outriggers, to enhance the safety and field applicability of the mobile scaffold. Technology development and improvements in the safety standards for the use of mobile scaffold will aid in the prevention of fatal accidents caused by mobile scaffold.

Safety device design for preventing pedestrian accidents (보행자 교통사고 예방용 거리측정 및 무선 전송장치 설계)

  • In spite of the recent enforcement of policies to reduce traffic accidents, pedestrian safety accidents on the back road continue to occur. In this paper, we propose a device design to prevent traffic accidents capable of measuring distance and wireless communication to quickly notify pedestrians of the vehicle's approach in a safety blind spot. The accuracy of vehicle and pedestrian distance measurement was tested using ultrasonic and radar sensors with a wide sensing range, and the possibility of responding to accidents that could provide safety for pedestrians in emergency situations was verified through a wireless communication connectivity test between target boards.

Improvement Implication of Research Lab Safety based on Multiple Correspondence Analysis of Accident-related Factors (사고 특성요인들의 다중대응분석에 기반한 연구실안전 개선 방안)

  • Unlike in general manufacturing process, safety management in laboratory-based research area is complicated because the latter generally involves trying untested methods or handling unusual substances in small amounts. Laboratory accidents in South Korea have recently shown an increasing trend. Unfortunately, statistics on such accidents are not officially published by any domestic public agencies. In this study, multivariate analysis was performed on the relationships between variables to develop effective strategies for preventing laboratory accidents. A Cross-Tabulation Analysis of accident-related factors in 179 accident cases revealed that the laboratory type, accident type, and unsafe-act type are all statistically significant, whereas the unsafe condition and management factors differ with the statistical criteria. Furthermore, the results of a Multiple-Correspondence Analysis showed that accidents can be divided largely into three groups having different accident causes and injury types; this confirms the necessity of different strategies to prevent accidents of each type. The findings also reveal differences between the distribution of accident types mentioned in the accident case collection books and actual reported cases. This suggests that an official statistical system administered by a public institution would be necessary for effective prevention of laboratory accidents.

The Effects of Individual Accidents and Neighborhood Environmental Characteristics on the Severity of Pedestrian Traffic Accidents in Seoul (개별 사고특성 및 근린환경 특성이 서울시 보행자 교통사고 심각도에 미치는 영향)

  • Korea's transportation paradigm is shifting from a vehicle-oriented transportation plan to a pedestrian-friendly environment that emphasizes walking safety. However, the level of pedestrian traffic accidents in Korea is still high and serious. The purpose of this study is to investigate factors affecting the severity of pedestrians traffic accidents using the multilevel logistic regression model based on 2015-2017 pedestrian accidents data provided by the Traffic Accident Analysis System(TAAS). The main results of the multilevel logistic regression model showed that 89% of pedestrian traffic accidents in Seoul were explained by individual characteristics such as drivers and pedestrians, and 11% were explained by neighborhood environmental characteristics. The results are as follows : In the individual characteristics such as pedestrians and drivers, the older the pedestrians and the drivers, the higher the traffic accident severity. The severity of traffic accidents was high when the pedestrians were female and the drivers were male. In the case of accident types, traffic accidents were more serious in the cases of heavy vehicles, inclement weather, and occurring at intersections and crosswalks. The results of the neighborhood environmental characteristics are as follows. The intersection density and the crosswalk density tended to reduce the severity of traffic accidents. On the other hand, the traffic light density and the school zones were founded to related to the higher level of traffic accident severity. This study suggests that both individual and neighborhood environmental characteristics should be considered together to prevent and reduce the severity of pedestrian traffic accidents.

Proposed Revision of Standard on Articles for Forklift Trucks in Manufacturing Industries (제조업에서 지게차에 대한 안전보건 기준에 관한 규칙 개정 방안)

  • A forklift truck, commonly used for transporting heavy materials in manufacturing industries, etc., is a useful machine. However accidents occur frequently during the operation of the machine. In order to prevent accidents it is necessary to examine the current safety regulations on a forklift truck. Because the regulations have not revised for a long time, they do not reflect the rapidly changing environment of industries. Proposed revision of standard on articles for forklift trucks was presented after analysing the causes of occurred injuries, comparing domestic standards with international standards and examining the survey results. We analyzed the safety regulation articles and the injuries according to the type of work conducted with forklift. The major differences in safety regulations between Korea and the major foreign countries were investigated. The opinions of field workers on the safety regulations were surveyed using e-mail or interview. As a result, it was found that preventing workers from contacting with the forklift is very important. To prevent accident due to movement of a stopped forklift, actions are necessary to prevent slip of a forklift. Also supervision is required to make workers fasten safety belt and to prohibit workers from using a forklift for undesignated purpose.

  • Safety management paradigm which against human errors in aviation industry is now changing from the follow-up measures after accident in the past to systematic approach that a forecast the hazards and improve the working system of the group to prevent accidents. As human factors are based on the man's specific psychological traits, it takes much time and efforts to prepare the preventive measures. That's why aviation industry is interested in the accident-prevent measurements against human errors. In this thesis, therefore, we are going to introduce the efforts that aviation organizations have tried and recommend management systems and discuss the suggestive facts. At first, we discussed introduction of HFACS which is the systematic accidents-classification system related to human errors in the aviation organization and countermeasure in the aspects of management, technology/engineering, education training. We described about FOQA, LOSA, CRM/TEM, aviation safety information DB in the aspect of management, and explained safety technologies that prevent human errors or avoid technologically when emergency occurs in the aspect of technology/engineering. In the aspect of education training, we explained the application plan about safety programs(LOFT/Simulator use, CRM/TEM application etc).

The causes of gas accidents in the past five years are closely related to the behavior of the operator, such as careless handling of the user, careless handling of the suppliers, and intentional, as well as equipment failure and accident of thought. In order to prevent such accidents, real-time monitoring of hazardous areas in the plant is required. However, when installing a camera in a work space for real-time monitoring, problems such as human rights abuse occur. In order to prevent this, an infrared camera with low resolution with low exposure of the operator is used. In real-time monitoring, image analysis is performed using CNN algorithm, not human, to prevent human rights violation.

  • Commercial gas burning appliances are classified by KGS AB338. Even though there are many types of gas appliances with different purposes and uses, common standards are applied to the appliances. Manifold of commercial gas range could often be corroded by salt water. Gas leaks and accidents could be occurred by the corrosion. According to suggestion of detailed material standards for manifold of commercial gas range, it could help to use safely and prevent gas accidents.
