느릅나무 근피드레싱이 쥐에 유발된 욕창의 섬유아세포성장인자와 혈관내피성장인자에 미치는 효과 (The Effect of Ulmus Root-bark Dressing in Fibroblast Growth Factor and Vascular Endothelial Growth Factor of Induced Pressure Ulcer in Rats)

  • Purpose: The purpose of this study was to investigate the effect of Ulmus root-bark dressing in fibroblast growth factor (FGF) and vascular endothelial growth factor (VEGF) of induced pressure ulcers in rats. Methods: 54 male Sprague-Dawley rats were used and randomly divided into 2 groups. The rats were anesthetized and pressure ulcers were induced at 140 mmHg for three hours, using a personally-designed pressing apparatus. Ulmus dressing was applied in the experimental group (n=27) and saline gauze dressing in the control group (n=27). Each of the dressings was changed every other day, and after a month, the wounds were examined by microscopy biweekly for 20 weeks. Results: After 4 weeks, the epidermis of the wounds was recovered, but inflammatory infiltration of the dermis was remained. After 6 weeks, inflammatory cells were diminished and the number of capillaries was decreased. Examined by immunofluorescence staining, the FGF positive cells in the experimental group changed negatively after 18 weeks, but the control group still existed even after 20 weeks. VEGF positive cells in the experimental group also changed negatively after 20 weeks, but the control group still existed. Conclusion: The findings of this study demonstrate that Ulmus dressing is effective in minimizing scar formation and inflammatory reaction by decreasing FGF and VEGF in the terminal phase of wound healing.

욕창 방지를 위한 체압 모니터링 시스템 개발 (Development of The Physical Pressure Monitoring System to Prevent Pressure Ulcers)

  • This study suggests a Healthcare System for elderly and disabled who have mobility impairment and use a wheelchair for long time. Seating long time in a wheelchair without reducing pressure causes high risk of developing pressure sores. Pressure sores come with great deal of pain and often lead to develop complication. Not only it takes time and effort to treat pressure sores but also increases medical expenses. Therefore, we will develop a device to help to prevent pressure sores by measuring pressure distribution while seating in a wheelchair and wirelessly send information to user device to check pressure distribution in real time. The equipment to measure body pressure is composed of FSR sitting mat which is a sensor measuring part and an user terminal which is a monitoring part. The designed mat is matrix formed FSR sensor to measure pressure. The sensor send measured data to the controller which is connected to the end of the mat, and then the collected data are sent to an user terminal through a bluetooth. Developing a pressure monitoring system will help to prevent those who have mobility impairment to manage pressure sores and furthermore relieve their burden of medical expenses.

A Novel Approach to Prevent Pressure Ulcer for a Medical Bed using Body Pressure Sensors

  • Despite numerous air mattresses marketed to prevent Pressure Ulcers (PU), none have fully succeeded due to residual pressure surpassing critical levels. We introduces an innovative medical bed system aiming at complete PU prevention. This system employs a unique 4-bar link mechanism, moving keys up and down to manage body pressure. Each of the 17 keys integrates a sensor controller, reading pressure from 10 sensors. By regulating motor input, we maintain body pressure below critical levels. Keys are equipped with a servo drive and sensor controller, linked to the main controller via two CAN series. Using fuzzy or PI/IP controllers, we adjust keys to minimize total error, dispersing body pressure and ensuring comfort. In case of controller failure, keys alternate swiftly, preventing ulcer development. Through experimental tests under varied conditions, the fuzzy controller with tailored membership functions demonstrated swift performance. PI control showed rapid convergence, while IP control exhibited slower convergence and oscillations near zero error. Our specialized medical robot bed, incorporating 4-bar links and pressure sensors, underwent testing with three controllers-fuzzy, PI, and IP-showcasing their effectiveness in keeping body pressure below critical ulcer levels. Experimental results validate the proposed approach's efficacy, indicating potential for complete PU prevention.

당뇨발을 위한 생체역학적 치료방법들에 관한 고찰 (A Review of Biomechanical Treatments for the Diabetic Foot)

  • Diabetic foot ulcers result from abnormal mechanical loading of the foot, such as repetitive pressure applied to the plantar aspect of the foot while walking. Diabetic peripheral neuropathy causes changes in foot structure, affecting foot function and subsequently leading to increased plantar foot pressure, which is a predictive risk factor for the development of diabetic foot ulceration. To early identify the insensitive foot makes it possible to prevent diabetic foot ulceration and to protect the foot at risk from abnormal biomechanical loading. Abnormal foot pressures can be reduced using several different approaches, including callus debridement, prescription of special footwear, foot orthosis. injection of liquid silicone, Achilles tendon lengthening, and so forth. Off-loading of the diabetic wound is a key factor to successful wound healing as it is associated with reduced inflammatory and accelerated repair processes. Pressure relief can be achieved using various off-loading modalities including accommodative dressing, walking splints, ankle-foot orthosis, total contact cast, and removable and irremovable cast walkers.

폼매트리스 사용 중환자의 욕창발생위험군별 적정 체위변경시간 (Optimal Time Interval for Position Change for ICU Patients using Foam Mattress Against Pressure Ulcer Risk)

  • Purpose: This study was done to identify the time interval to pressure ulcer and to determine the optimal time interval for position change depending on pressure ulcer risk in patients using foam mattress in intensive care units. Methods: The Braden scale score, occurrence of pressure ulcers and position change intervals were assessed with 56 patients admitted to an intensive care unit from April to November, 2011. The time to pressure ulcer occurrence by Braden scale risk group was analyzed with Kaplan-Meier survival analysis and log rank test. Then, the optimal time interval for position change was calculated with ROC curve. Results: The median time to pressure ulcer occurrence was 5 hours at mild or moderate risk, 3.5 hours at high risk and 3 hours at very high risk on the Braden scale. The optimal time interval for position change was 3 hours at mild and moderate risk, 2 hours at high and very high risk of Braden scale. Conclusion: When foam mattresses are used a slight extension of the time interval for position change can be considered for the patients with mild or moderate pressure ulcer risk but not for patients with high or very high pressure ulcer risk by Braden scale.

고관절 골절로 입원한 노인 환자의 욕창 발생 위험요인 (Factors Influencing on Pressure Ulcer Incidence among Older Patients with Hip Fracture in a Hospital)

  • Purpose: This study aimed to identify the incidence and risks for pressure ulcer among older patients with hip fracture. Methods: The subject were 215 older patients suffering from hip fracture who were admitted for surgical operation from January 1, 2012 to April 30, 2016 in a university-affiliated hospital. The incidence of pressure ulcer was collected retrospectively through medical record review and the risk factors were analyzed using Cox's proportional hazard model. Results: Out of the total, 32 patients (14.9%) developed pressure ulcer with the average occurrence period being 4.72 (${\pm}3.81$) days. Stage II pressure ulcer was the most common at 72.0%. Risk factors included ambulation status before injury (p= .039), spinal anesthesia (p= .029), and stay at intensive care unit after operation (p= .009). Conclusion: Despite pressure ulcer prevention efforts, the incidence remained relatively high. Considering the identified risk factors, more efforts is needed for early detection and prevention of pressure ulcers in such patients.

A Peculiar Case of Ischemic Fasciitis Appeared on a Pressure Ulcer after 10 Days of Negative Pressure Wound Therapy

  • In this article, we reported a single case of ischemic fasciitis in a young woman with a progressive immobilization due to a multifocal demyelinating disease of central nervous system, which appeared on an extensive pressure ulcer of the sacral region treated with 10 days of negative-pressure wound therapy (NPWT). Wound examination revealed a significant nontender brown neoformation (9 cm in length × 10 cm in width × 7 cm in height), fixed to the sacrum, presenting hard consistency, and grown in the central portion of the sacral pressure sore. The histologic examination showed central fibrinoid necrosis, and vascular and atypical fibroblastic proliferations, and a diagnosis of ischemic fasciitis was made. Ischemic fasciitis is a rare benign proliferation of atypical fibroblasts that occurs in physically weak patients with reduced mobility. In the literature, the relationship between the use of NPWT on pressure ulcers and the development of ischemic fasciitis is, to the best of our knowledge, not described yet.

욕창 환자들의 질병 체험에 관한 현상학적 연구 (A Phenomenological Study on Illness Experience of Patients with Pressure Ulcer)

  • Purpose: The purpose of this study was to describe the illness experience of patients with pressure ulcer. Methods: A phenomenological methodology was used for the study. The data were collected by individual in-depth interview with seven participants with pressure ulcer during 2013~2014. All interviews were audio-taped and verbatim transcripts were made for the analysis. The data were analyzed using Colaizzi's phenomenological method. Results: All participants had underlying disease, such as spinal paralysis and diabetes. Average period of having pressure ulcer was 18 months, ranged from 3 to 36 months. A total of seven theme clusters were derived from the analysis; unexpected wound, inherent vulnerability to infection, reversal of the treatment policy, unpleasant and strange feeling of wound, sweeping fear and helplessness, socioeconomic burden, and healing through specific actions and reflection. The participants faced various contradictory and paradoxical situations in managing their pressure ulcers as well as underlying diseases in their everyday life. However, they slowly overcome these situations by strictly practicing concrete action-oriented strategies that they have learned through suffering and appreciating miraculous wound healing. Conclusion: The results of this study can help developing a patient-specific intervention program with sufficient emotional support by providing insights of the paradoxical illness experience of patients with pressure ulcer.

맥문동 혼합 하이드로콜로이드막의 제조 및 화상치료 효능평가 (Therapeutic Effect of Hydrocolloid Membrane Containing Liriope platyphylla Extracts on the Burn Wounds of SD Rats)

  • 맥문동(Liriope platyphylla)은 염증(inflammation), 당뇨(diabetes), 신경퇴행성질환(neurodegenerative disorder), 비만(obesity), 변비(constipation), 아토피질환(atopic dermatitis) 등의 다양한 임상질환에 매우 우수한 치료효과를 나타내는 것으로 알려져왔다. 또한, 하이드로콜로이드막(hydrocolloid membranes, HCM)은 피부경화증 피부궤양(scleroderma skin ulcers), 피부궤양(cutaneous ulcers), 영구적 고막천공(permanent tympanic membrane perforations), 욕창(pressure sores), 욕창궤양(decubitus ulcers)과 같은 피부질환 치료에 많이 사용되고 있다. 따라서 본 연구에서는 기능성이 우수한 맥문동 추출물을 HCM에 혼합하여 맥문동 혼합 하이드로콜로이드막(HCM-LP)을 제조하고, 물리화학적 특성을 분석한 뒤 2도 화상을 유발한 SD 랫드에 14일 동안 처리하여 치료효과를 분석하였다. 그 결과, 일반 하이드로콜로이드막(HCM)에 비하여 HCM-LP에서 인장강도와 흡수성은 각각 38.4%, 46.3% 감소하였으나 표면거칠기는 38.1% 증가하였다. 화상을 유발한 SD 랫드에서 HCM-LP를 처리한 결과, 화상 유발 14일 후에 HCM-LP처리그룹은 GZ처리그룹에 비하여 유의적으로 화상크기 감소를 나타내었을 뿐만 아니라 흉터감소, 재상피화, 신생혈관형성 그리고 세포외기질의 침적을 유도하였다. 따라서 이러한 결과는 HCM-LP가 신생혈관형성과 연결조직형성 조절을 통해 SD 랫드에서의 화상 치료를 향상시킴을 의미한다. 또한, 본 연구는 HCM-LP가 피부상처의 치료에 적용할 수 있는 다른 기능성 물질을 포함하는 HCM의 개발에 대한 가능성을 제시하고 있다.
