Since 1976 the catches of sardine increased rapidly in Korea. However due to the poor facilities of preservation, most sardine landed was used only for fish meal as feeds. The aims of this study are to investigate the processing of sardine as a protein. concentrate and to solve related problems under our particular circumstances. Using the ethyl alcohol and isopropyl alcohol, the storage effect for further processing, the optimum processing conditions of sardine protein concentrate and amino acid composition of the product were determined. The utilization of sardine protein ,concentrate as a supplement of bread and noodles was also studied. Chopped sardine meat could be stored in isopropyl and ethyl alcohol without significant deterioration as a raw material for tile further processing. High qualify sardine Protein concentrate could be produced by the method, that is five times five minutes extraction with isopropyl or ethyl alcohol at $80^{\circ}C$ under adequate mixing. In the first step of the extraction, the solvent was added as much as 10 times tile sample amount and the equal volume of additional solvent was also used for the second to fifth step extraction. In the products extracted using isopropyl alcohol and ethyl alcohol, the yields of sardine protein concentrate were $21.2\%$ and $20.3\%$ respectively, and the dry basis contents of protein in the two products were $80.5\%$ and $75.8\%$, the lipid being $0.22\%$ and $0.27\%$ respectively. Isoproyl alcohol was superior to superior alcohol for the extraction of fresh sardine. In amino acid composition of sardine protein concentrate, no difference was found between the products of isopropyl and ethyl alcohol extraction except a little difference in the amount of amino acid between them. In the supplementation of bread and noodles, taste panel showed that supplemented bread and noodles were well accepted when $3\%$ of wheat flour was replaced by sardine protein concentrate.

Sixty-six patients with squamous cell carcinoma of the oropharynx were treated with radiation therapy and retrospectively analyzed to evaluate the treatment result in the Department of Therapeutic Radiology, Korea Cancer Center Hospital between January 1980 and December 1986. There were 42 patients with carcinoma of the tonsil including the fossa and pillar, 9 patients with carcinoma of the base of tongue,12 patients with carcinoma of the soft palate, and 3 patients with carcinoma of the posterior and lateral pharyngeal walls. Considering all oropharyngeal sites of involvement together, response rates for T1, T2, T3, and T4 were 80%, 77%, and 40%, respectively, with a overall response rate of 70%. The response rate for N1, N2, and N3 were 69%, 63%, and 40%, respectively, with the overall regional response rate of 70%. In lower T status, undifferentiated carcinoma and primary tumor arising from the soft palate, higher response rates were obtained. The S year overall and disease-free survival rate were 56%, 55%, respectively. A better prognosis was obtained in early T stage (T1+T2) (p<0.01) and in patients without tumor extension into adjacent structures in carcinomas arising from tonsillar area (p<0.01). Through this study we suggest that, in terms of anatomical and functional preservation, radiation therapy seems to be an effective method for the primary treatment of patients with oropharyngeal carcinoma.

Background: This study was designed to evaluate the effectiveness of T3 sympathicotomy in treatment of palmar hyperhidrosis. Material and Method: During the period of June to December 1998, 50 patients (24 females and 26 males) suffering from palmar hyperhidrosis either in isolation (n=37) or in combination with axillary hyperhidrosis (n=13) were operated. The mean age of the patients was 20 years. The bilateral sympathetic trunks were severed on the 3rd rib (2nd and 3rd ganglia) for the isolated palmar hyperhidrosis and on the 3rd and 4th ribs for the combined type using electrocoagulation scissors. A linear analogue scale was used to assess the degree of sweating on the palms, face, trunk, and feet (ranged 0 to 10:0 = anhidrosis: 10 = excessive sweating) as well as the patient's satisfaction with the surgery (ranged 0 to 10:0 = regret; 10 = completely satisfied). Result: All of the patients were relieved from palmar hyperhidrosis. A mean palmar sweat production score after T3 sympathicotomy was $1.5\pm$0.8. Some degree of compensatory sweating had occurred in 39 patients (78%) with a mean score of 3.4$\pm$1.6. Gustatory sweating occurred in 2 patients (4%). The mean score of the patient's satisfaction after the surgery was 8.5$\pm$1.2. Conclusion: Palmar hyperhidrosis can be successfully relieved by the T3 sympathicotomy. When considering the advantages of T3 sympathicotomy with respects to a better preservation of facial sympathetic function, less occurrence of severe compensatory sweating, and lower incidence of gustatory sweating. We recommend T3 sympathicotomy as a treatment of choice for palmar hyperhidrosis.

Background: Various studies and experimental trials have been done to develop bioprosthetic devices to treat complex congenital heart disease due to the limited usage of homograft tissue. The purpose of the present study was to evaluate the effect of diamine bridges with using L-lysine, as compared with using ethanol. Material and Method: Porcine pericardium was fixed at 0.625% GA (commercial fixation). An interim step of ethanol (80%; 1 day at room temperature) or L-lysine (0.1 M; 2 days at $37^{\circ}C$) was followed by completion of the GA fixation (2 days at $4^{\circ}C$ and 7 days at room temperature). The tensile strength and thickness of the porcine percardium were measured, respectively. The treated pericardiums were implanted subcutaneously into three-week old Long-Evans rats for 8 weeks. The calcium content of the implants was assessed by atomic absorption spectroscopy and the histology. Result: Ethanol pretreatment ($13.6{\pm}10.0ug/mg$, p=0.008), L-lysine pretreatment ($15.3{\pm}1.0 ug/mg$, p=0.002), and both treatment ($16.1{\pm}11.1ug/mg$, p=0.012) significantly inhibited calcification, as compared with the controls $(51.2{\pm}8.5ug/mg)$. L-lysine pretreatment ($0.18{\pm}0.02mm,\;1.20{\pm}0.30kg$ f/5 mm) significantly increased the thickness and tensile strength, as compared with ethanol pretreatment ($0.13{\pm}0.03mm,\;0.85{\pm}0.36$ 1.0 kg f/5 mm) (p<0.01, p=0.035). Conclusion: The diamine bridges using L-lysine seemed to decrease the calcification of porcine pericardium fixed with glutaraldehyde, and this was comparable with Ethanol. Additionally, it seemed to enhance the thickness and tensile strength.
Purpose : Many believe that the Sasang mentioned in the I Ching and the Sasang of Sasang Medicine (as expounded by Dr. Lee Je-ma in the book Longevity and Life Preservation in Oriental Medicine) refer to different concepts. This is untrue. In order to understand the thought patterns of Dr. Lee Je-ma and his book, it is necessary to first understand the concepts of the I Ching. The I Ching was the most respected text in Dr. Lee Je-ma time, and served as the foundation upon which his medicine stood. The purpose of this research is to understand the concept of Sasang in the I Ching and how it applies to the Sasang medicine. Method : The authors first defined the term Sasang according to the theory of I Ching. It was then discussed in relation to theories of modern science. Inferences were made as to how Sasang corresponds to the terminologies and concepts of modern science. The characteristics of Sasang interpreted through modern science were then applied to the physiology, pathology and pharmacology of Sasang Medicine. Results and Conclusion : 1. The Sasang theory of the I Ching organizes seemingly random and isolated natural phenomena into four distinct groups according to various attributes. The particular characteristics representing each of these four categories are known as Sasang. 2. The Sasang theory of I Ching has a strong correlation to the Theory of Relativity and the Theory of Complementarity, as well as the Digital and Fractal Theories. 3. By applying the Sasang Theory to various fields, the seemingly unrelated principles of physics, chemistry, biology and medicine can be seen as parts of a whole. 4. Sasang Medicine categorizes human morphology, physiology and pharmacology into four categories according to the characteristics defined by the Sasang Theory of the I Ching. 5. Grouping new discoveries of modern physics, chemistry, biology and medicine according to the Sasang Theory will bring to light the intricacies of the Sasang Theory while facilitating the incorporation of modern science into Sasang Medicine.
Background: Thoracoplasty has become a rarity in current clinical practice, although it has been widely employed for well over a century as a procedure for reducing the capacity of the thoracic cavity. Yet we have perform tailoring thoracoplasty following or concomitant with pulmonary resection in 20 patients. The aim of this study is to evaluate the early and late clinical results and also the significance of tailoring thoracoplasty. Material and Method: From March 1995 to June 2005, modified thoracoplasty following or concomitant with pulmonary resection was performed in 20 patients out of a total of 298 pulmonary resections for closing air leaks and for treating persistent pleural space following pulmonary resections, and to tailor the thoracic cavity to accept a diminished lung volume. Of the 20 patients, 14 patients had tailoring thoracoplasty performed concomitant with pulmonary resection, and the remaining 6 patients also had tailoring thoracoplasty performed following pulmonary resection. The subjects ages ranged from 24 to 77 (mean $59.1{\pm}6.4$) and a male preponderance was noted (17 : 3); the number of left and right surgeries was equal. The preoperative primary underlying diseases were lung cancer in 7 patients, pneumothorax with giant bullous change in 6 patients, bronchiectasis in 2 patients, previous pulmonary tuberculosis associated with aspergilloma in 2 patients, empyema with fibrothorax in 2 patients and multiple lung abscesses & destruction due to previous trauma in 1 patient. The operative methods were apicolysis and subperiosteal removal of the 2nd, 3rd and 4th ribs (the costochondral junction to the posterior portions of the ribs) with preservation of the first rib and compression of the anterior chest via cotton bags and elastic bandages. Result: The mean duration of the air leaks after thoracoplasty was $1.6{\pm}0.2$ days (range: $0{\sim}7$ days) and the mean duration of an indwelling chest tube was 7 days (range: $5{\sim}11$ days); the mean duration of hospitalization was $19.2{\pm}2.8$ days (range: $8{\sim}47$ days). The postoperative complications were wound infection (2) and pneumonia (2); reoperation was done due to bleeding (1) in one patient who underwent concomitant thoracoplasty and there was 1 case of wound infection (1) after postresection thoracoplasty. The mortality was 1 patient in the early phase and 4 patients in the late phase. Conclusion: We conclude that tailoring thoracoplasty may be performed to close anticipated persistent pleural spaces and to accommodate the diminished lung volume with acceptable cosmetic results when this procedure is combined with pulmonary resection in selected patients.
The purpose of this paper is to weigh the possibility of endangered Jang-hang wetland at the estuary of Han River to be included on the Ramsar List of Wetlands of International Importance and come up with ways to manage the wetland depending on its biotope patterns. The target area is located between Gimpo bridge and Isanpo I.C. with about $2.7km^2$ area. Through the analysis of RIS(Information Sheet for Ramsar Wetlands), it was known that the wetland is located on the sedimentary topography and formed as a result of sediment at the estuary of the river owing to the concentration of rainfall during summer. The vegetation environment in the area is divided into brackish water and fresh water areas depending on salinity. Rhizosphere soil(RS) of the area was analyzed to be Silt loam while bottom RS to be Sand loam. The plant ecology was composed of 52 families 135 species and 11 varieties and 146 types. Among indigenous species found are Salix koreensis, Phragmites communis and Miscanthus sacchariflorus. The analyzed results of the actual vegetation showed that willow community accounts for 37% of the area and rice field is 13.5%. As for animal ecology, total of 62 species and 25,977 individual wild birds were observed. After comparing and analyzing the RIS we compiled with the Ramsar Site designation standards, it turns out that the Jang-hang wetland meets criteria 1(biographic region), criteria 2,3 and 4(species and ecological communities) and criteria 5 and 6(water birds). Thus, Jang-hang wetland is eligible for the Ramsar site. As a result of establishing and evaluating the biotope types for setting management areas, Jang-hang wetland has a total of 13 different types, and the grade I represents 75.4% of the area while the grade III 0.8% of the land status. We categorized four management zones for the wetland depending on the biotope patterns - preservation, restoration, use and buffer zones and suggested management methods for each zone.

In this study, extracts of gamma-irradiated Aralia continentalis for health and beauty food resources were examined on antioxidant activities and increasing extraction yield. In photostimulated luminescence analysis, non-irradiated (0 kGy) sample showed an intermediate result of 891.00 photon counts (PCs), whereas irradiated (5 and 10 kGy) samples showed positive results of 79,956.67 and 282,498.67 PCs, respectively. The thermoluminescence ratio of non-irradiated samples was 0.018, whereas the values of irradiated samples (5 and 10 kGy) were 0.395 and 0.464, respectively. Electron spin resonance (ESR) analysis as a physical technique for irradiation identification of Aralia continentalis showed a pair of peaks on a space of 6.0 mT in the symmetric ESR spectrums. For phenolics of gamma-irradiated Aralia continentalis, water extracts had the highest amounts of 7.61 mg/g at non-irradiated condition (0 kGy) while 50% ethanol extracts had the highest amounts of 6.86 mg/g at 10 kGy irradiation. 2,2-Diphenyl-1-picrylhydrazyl scavenging activity of water and 50% ethanol extracts from non-irradiated and irradiated Aralia continentalis were very high (94.99~100.00%). 2,2'-Azinobis-(3-ethylbenzothiazoline-6-sulfonic acid) radical cation inhibitory activity was over 95.00% at $200{\mu}g/mL$ phenolics. Antioxidant protection factor (PF) showed a very high level of activity (1.38~2.51 PF). Thiobarbituric acid reactive substance inhibitory activity was high (70% and 95%, respectively). Thus, treatment of gamma irradiation can be used as a method of storage for long-term preservation of Oriental medicine.
Purpose : Dialysis in children with chronic renal failure presents with many difficulies. The purpose of this study is to find an improved method in chronic dialysis in infants and children less than 2 years of age by analyzing the experience with 10 cases. Methods : A retrospective review of the medical records of 10 patients(6 boys and 4 girls) was conducted. The patients had chronic renal failure and underwent chronic dialysis at Samsung medical center from March 1999 to February 2007. Results : At Initiation of dialysis, the median age was 3 months old(22 days-20 months), the median body weight was 3.75 kg(2.2-10.3 kg), and the median serum creatinine level was 4.3 mg/dL(2.0-11.4 mg/dL). The median duration of dialysis was 29.5 months(3-62 months). Dysplastic kidney disease was the most common underlying renal disease. Two patients were treated with hemodialysis, 4 patients with peritoneal dialysis, and 4 patients eventually switched dialysis modality. Nine of the 10 patients took erythropoietin and anti-hypertensive drugs. At the end of the follow up period, 1 patient received kidney transplantation, 2 patients died due to sepsis, and 5 patients were treated with peritoneal dialysis. Two patients were lost to follow up. The most common complication of dialysis was infection. Achieving vascular access and maintaining proper catheter function were the most important factors in treating patients with hemodialysis. The growth status of patients was aggravated after 6 month of dialysis but improved after 1 year of dialysis. Patients showed better growth on peritoneal dialysis than hemodialysis. Conclusion : Chronic dialysis can be performed successfully in infants and children under 2 years of age. Vascular access was the main limitation of hemodialysis, and infection was the common problem in both hemodialysis and peritoneal dialysis. To improve the patients survival rate and quality of life, major efforts should be directed toward the prevention of infection and preservation of catheter function. Background: Current vascular prostheses are considered still inadequate for reconstruction of small-diameter vessels. To evaluate the potential use of xenograft vessels as small diameter arterial grafts, we implanted porcine vessels in goats. The grafts were treated with two different processes, freezing and acellularization, before implantation, and gross inspection as well as microscopic examination followed after a predetermined period. Material and Method: Bilateral porcine carotid arteries were harvested and immediately stored at $-70^{\circ}C$ within tissue preservation solution. One of them was designated as frozen xenograft vessel. The other one was put on acellularization process using NaCl-SDS solution and stored frozen until further use. Grafts were implanted in the place of carotid arteries of the same goat. The grafts have remained implanted for 1, 3, and 6 months in three animals, respectively. Periodic ultrasonographic examinations were performed during the observation period. After explantation, the grafts were analyzed grossly and histologically under light microscope. Result: All animals survived the experimental procedure without problems. Ultrasonographic examinations showed excellent patency of all the grafts during the observation period. Gross examination revealed nonthrombotic, patent lumens with smooth surfaces. Microscopic examinations of the explanted grafts showed cellular reconstruction at the 6-month stage in both grafts. Although more inflammatory responses were observed in the early phase of frozen xenografts, there was no evidence of significant rejection. Conclusion: These findings suggest that porcine xenograft vessels, regardless of pre-implantation processes of acelluarization or freezing, can be acceptably implanted in goats, although short duration of observation in a small number of animals may limit this study.
