노인의 동통에 관한 조사연구 (A Descriptive Study on Pain of Elderly)

  • The main purpose of this study was getting related to the pain charactristic data of elderly. It contains past and present health status, daily living activity level, pain frequency, causes, pain area, pain intensity, pain worse causes, and pain releave causes, pain management methods. The subject were 79 elderly whose age were over 65 years old. They were lived in their home environment. Half of them were resident of middle range city at province and the others were Seoul city. The data was collected from Dec. 1st. 1995 to Jan. 30th. 1996. Interviews were held with semi-structured questionaire after pilot study by researchers. Pain measurement tool were used graphic rating scale and Abstract of Korean Pain Language Scale. To analize the subject's general characteristics, past and present health status, daily living activity level, characteristics about pain, pain management methods statistical SPSS for win frequency were employed. The findings were as follows ; 1. There were 33(41.8%) male and 46(58.2%) female Below 69 year old were 20(25.3%), 70-79 year old were 42(53.2%), over 80 year old were 17(21.5%), mean age was 74(from 65 to 89). 2. Buddhist were 24(30.4%), Christian were 29(36. 7%), the other religious status or non religians were 26 (32.9%). 3. Past good health status were 63(79.7%), not so good status were 6(7.6%). Present good health status were 19(24.1%), moderated health status were 6(7.6%), not so good status were 14(17.7%). 4. Daily living activity limitation were 39(49.4%), nonlimitation of activity were 5(6.3%). 5. Walking limitation were 3(3.8%), nonlimitation walking were 52(65,8%). 6. Insomnia was 23(29.1%), no difficult were 38(48.1%). 7. Chronic pain complaints were 64(81%), diseases causes of pain were 25(31.6%), bad health behavior causes of pain were 27(34.2%). 8. Most pain area were back 30(29.4%), leg 17(16.7%), knee 16(15.7%), arm 13(12.7%), teeth, chest and head were each 5(4.9%), loin, trunk were each 4(3.9%), the other areas were 3(3%). 9. Pain intensity was 3.49(mean) by Korean Language Scale, 6.59(mean) by graphic rating scale. Sensitive pain was 3.5(47.9%), affective pain was 3(20.8%) It was high pain level and sensitive pain. 10. Most pain worse causes moving was 35(44.3%), pain relieving causes rest was 29(36.7%), 11. Pain management method were medication 40(42.1%), physiotherapy 23(24.2%), hospital 12(12.6%), the others 7(7.4%), none 13(13.7%). The conclusion ; Present health status of eldery was not so good. Almost half of them have some diseases. Most common diseases of eldery were arthristis, respitatory and heart problems. Foully nine percent of elderly had limitation of daily living activities. Eighty one percent of eldely had chronic pain. Most of them was back pain (30%). Pain intensity was high(score over 3.5). The worsening pain causes was moving and releiving causes was rest. Pain management method were pain medication, physiotherapy. Therefore, Nursing care plan for the elderly have to focus on pain because majority of elderly have chronic high level of pain related to the arthritis.

운동부하검사로 유발된 여성의 협심통증에 대한 예비연구 (A preliminary study on women's anginal pain induced by exercise stress test)

  • Purpose: Women have been often underdiagnosed and undertreated when they have as high mortality as men of ischemic heart disease, such as angina. One of the reasons of women's under treatment is associated with either vague, non-traditional symptoms or longer delay in seeking professional help when they experience ischemic heart disease. This study was conducted to investigate the characteristics of women's anginal pain induced by the treadmill test and to explore the potential relationship(s) between anginal pain and other psycho-physiologic factors. Methods: Of 22 female patients referred to treadmill test, 7 with positive finding participated in this study. Anginal pain in the past was analyzed by Rose questionnaire, whereas anginal pain induced by the treadmill test was identified by McGill pain scale, visual analogue scale and present pain index. Results: Women expressed more heaviness than sharp pain, and complained splitting more often than subjects in previous study that included both men and women. Pain intensity by VAS $3.64{\pm}3.94$, Pain rating index was $5.14{\pm}7.29$, present pain index was $1.57{\pm}1.81$, duration of pain was $5.14{\pm}4.8$ min. Exercise intensity was $6.0{\pm}4.63$ METs, exercise duration was $364.29{\pm}141.39$ sec, ST change was -2.0mm and rate of perceived exertion was $16.29{\pm}2.06$. The relationships between pain intensity and pain duration, ST segment changes were significant. Correlation among pain measures was significant. Conclusion: Generalization of these pilot findings may be inappropriate, and therefore, further larger study is needed.

통증 어휘에 따른 통증 종류와 강도 간의 관련성 (Relationships between Pain Types, Pain Intensity and Terms Used to Describe Pain)

  • Purpose : The purpose of present study was to improve communication of pain expressing terms and pain intensity between patient and physical therapist, and initiated to objectify a measurement of subjective pain. Methods : Data were delivered to 249 people by a self-completion questionnaire, and analyzed 160 copies except for 87 mark error of the collected 247 questionnaires. The questionnaire included a question on 55 terms used to describe pain, the type of the pain, and pain intensity using VAS (visual analogue scale). Results : The results were as follows; 'Tight' was the most frequently being expressed term of muscular pain, subsequently to 'knot' and 'dull'. 'Tear' was the term representing the most strong pain of muscular pain, subsequently to 'rupture' and 'squeeze'. 'Stinging' was the most frequently being expressed term of neurologic pain, subsequently to 'get shocked' and 'wriggle'. 'Burn' was the term representing the most strong pain of neurologic pain, subsequently to 'sear' and 'get shocked'. 'Creak' was the most frequently being expressed term of joint pain, subsequently to 'peel' and 'out of joint'. 'Break' was the term representing the most strong pain of joint pain, subsequently to 'peel' and 'crack'. Conclusion : The objectification of pain terms will be used to help physical therapist to check the patient's pain.

요통환자의 일반적 특성에 따른 한국어판 오스웨스트리 장애 설문지의 라쉬분석 (A Rasch Analysis of the Korean Version of Oswestry Disability Questionnaire According to General Characteristics of Patients With Low Back Pain)

  • Three hundred and fourteen low back pain patients completed the Korean version of the Oswestry Disability Questionnaire (KODQ) and the Rasch analysis was then applied to identify the inappropriate items and to determine ODQ item difficulties according to a subject's characteristics. For women and youths, the 'sex life' item showed misfit statistics, whereas for older subjects it was the 'pain intensity' and 'standing' items. Also, in the acute low back group the 'pain intensity', 'standing' and 'sex life' items showed misfit statistics. For all subjects, the most difficult item was 'pain intensity', whereas the easiest item was 'walking'. But for the older and acute groups 'lifting' was the most difficult item and for those who have a visual analogue scale score of 3 or less 'sitting' was the most difficult item. These results show that differential item functioning is present in several items according to sex, age, acute and chronic pain, and VAS score. This study may be useful for adjusting the KODQ item difficulties for low back pain patients with different characteristics.

요추부 불안정성을 가진 요통환자의 요추부 안정화 운동과 둔근 강화 운동이 통증, 기능장애 및 심리사회수준에 미치는 효과 비교연구 (Comparison of Lumbar Stabilization Exercises and Gluteal Strengthening Exercises on Pain, Disability and Psychosocial Factors in Low Back Pain Patients with Lumbar Instability)

  • Background: Lumbar stabilization exercise and gluteal muscle strengthening exercises are widely used to treat for lower back pain patient. The present study aimed to compare the effects of lumbar stabilization exercise and gluteal muscle strengthening exercises on chronic lower back pain with lumbar spine instability, with regard to pain intensity, disability, and psychosocial factors. Methods: Among 53 patients with chronic lower back pain, those with spine instability were selected using 5 examination tests. The selected 28 patients were randomly assigned to lumbar stabilization exercise group (LSE, n=15) and gluteal strengthening exercise group (GSE, n=13). Each group performed the corresponding exercise for 40 minutes, twice a week for 4 weeks. To analyze and compare the effects, pain intensity, the level of low back disability, and psychosocial factors were assessed before and after intervention. Results: There was significant difference in lower back pain intensity between the two groups before and after intervention. The change in low back disability was significant in the GSE group alone following intervention (p<.05), but no other significant difference was found between the groups. Among psychosocial factors, the changes in the fear-avoidance beliefs questionnaire (FABQ)-physical activity and FABQ-total were significant in the LSE group alone following intervention (p<.05). However, no significant difference were found in these factors between the two groups before and after intervention. Conclusions: LSE and GSE for lower back pain with lumbar spine instability showed no significant difference for pain intensity, physical disability, or psychosocial functioning.

The Strength of the Lower Trapezius in Violinists With Unilateral Neck Pain

  • Violinists tend to position the neck asymmetrically to hold the violin between the chin and the left shoulder. Asymmetrical neck posture may induce unilateral neck pain. Previous studies have suggested that individuals with unilateral neck pain exhibit reduced muscle strength of the lower trapezius, but no study has investigated violinists with unilateral neck pain. To this end, we recruited 18 violinists with unilateral neck pain for the present study in which the side on which neck pain was experienced, pain duration, and intensity were recorded. Lower trapezius strength was measured bilaterally in each subject using a handheld dynamometer. Significant differences in lower trapezius strength were evident between the ipsilateral and contralateral sides of neck pain (p<.05). No significant association between neck pain intensity or duration, and the extent of a deficit in lower trapezius strength, was evident (both p>.05). The association between the sides of weakened lower trapezius strength and neck pain was significant (p<.05). In conclusion, violinists with unilateral neck pain exhibited significantly less lower trapezius strength on the ipsilateral compared to the contralateral side of the pain. Unilateral neck pain more frequently involved the left side of the neck, which is used to stabilize the violin during playing. Thus, our study suggests that a possible relationship exists between muscle weakness in the lower trapezius and neck pain.

Determinants of depression in non-cardiac chest pain patients: a cross sectional study

  • Background: Non-cardiac chest pain (NCCP) is a common patient complaint imposing great costs on the healthcare system. It is associated with psychological factors such as depression. The aim of the present study is determining depression predictors in NCCP patients. Methods: The participants of this cross-sectional study were 361 NCCP patients. Patients filled out questionnaires concerning their sociodemographic, lifestyle, and clinical factors (severity of pain, type D personality, somatization, cardiac anxiety, fear of body sensations, and depression). Results: Based on multiple ordinal logistic regression, lack of physical activity (odds ratio [OR], 1.78; 95% confidence interval [CI], 1.09-2.87), sleep quality (OR, 2.98; 95% CI, 1.15-7.69), being a smoker (OR, 1.33; 95% CI, 2.41-4.03), present pain intensity (OR, 1.08; 95% CI, 1.05-1.11), type D personality (OR, 2.43; 95% CI, 1.47-4.03), and somatization (OR, 1.22; 95% CI, 1.15-1.3) were significant predictors of depression in NCCP patients. Additionally, multiple linear regression showed that being unmarried (β = 1.51, P = 0.008), lack of physical activity (β = 1.22, P = 0.015), sleep quality (β = 2.26, P = 0.022), present pain intensity (β = 0.07, P = 0.045), type D personality (β = 1.87, P < 0.001), somatization (β = 0.45, P < 0.001), and fear of bodily sensation (β = 0.04, P = 0.032) increased significantly depression scores in NCCP patients. Conclusions: Physicians should consider the predictors of depression in NCCP patients which can lead to receiving effective psychological consultations and reducing the costs and ineffectual referrals to medical centers.

통증 평가도구 개발을 위한 기초조사 (A Preliminary Study for Development of a Pain Questionnaire)

  • The present study was designed to investigate the general characteristics of pain patients and to analyze the properties of Korean pain expression terms as a preliminary step in the development of a pain questionnaire. Questionnaires were administered to 73 adult patients (53 males, 20 females) with knee, ankle, neck, low back, and shoulder pain. The mean duration of pain was 16.2 months (SE=3.3). The results were as fellows : 1. The data show that there are over 30 words in the Korean language to describe the varieties of pain experience even within this small sample. 2, There was low significant relationship between present pain intensity using visual analogue scale and the selected numbers of pain words from the pain questionnaire (p<.01). 3. In order to separate basic factors, a principal component analysis with varimax rotation was performed. The principal component analysis produced 8 factors. The proportion of variance explained by these factors was $71.0\%$. The first factor accounting $26.8\%$ of the variance was labeled 'cruelty and fear related pain' ; second 'pain produced from deep tissue' : third 'skin-punctuating related pain' ; and fourth 'miscellaneous and complicated pain'. Results of this study might be utilitzed in developing a pain questionnaire for pain patients.

A hybrid cardiac rehabilitation is as effective as a hospital-based program in reducing chest pain intensity and discomfort

  • Background: Health care services effort to provide alternative cardiac rehabilitation (CR) models to serve patients according to their preferences and needs. So, the present study aimed to assess and compare the effects of hospital-based and hybrid CR programs on chest pain intensity and discomfort in cardiac surgery patients. Methods: In this prospective study, 110 cardiac surgery patients were invited to the CR department of a hospital in the western part of Iran between March and July 2016. Patients were divided into two groups: hospital-based and hybrid CR. The hospital-based program included 26 sessions, and the hybrid program included 10 training sessions and exercise. The Brief Pain Inventory and Pain Discomfort Scale were used as research instrument, and data were analyzed using the paired t-test and ANCOVA. Results: The results indicated that both hospital-based and hybrid CR are effective in reducing the chest pain intensity and discomfort of cardiac surgery patients (P < 0.05). In addition, the comparison of scores before and after treatment using ANCOVA shows that no significant differences were observed between the two programs (P > 0.05). Conclusions: Traditional hospital-based CR delivery is still the first choice for treatment in developing countries. However, hybrid CR is as effective as a hospital-based program in reducing pain components and it includes only 38% of the total cost in comparison to hospital-based delivery. So, we recommend using hybrid CR according with the recommendations of American Heart Association about using CR for the management of angina symptoms.

Musculoskeletal Pain Status of Local Farmers in Tigray, Ethiopia: A Cross-Sectional Survey

  • Background: Agricultural work is physically demanding and is associated with a high frequency of musculoskeletal disorders. It is challenging to comprehensively understand the present status of work-related diseases and injuries among farmers in underdeveloped countries. Objects: This study aimed to elucidate the current status of work-related musculoskeletal disorders in local farmers in Tigray, Ethiopia, and identify the agricultural factors associated work-related musculoskeletal pain (AFWMP) and healthy living and healthy behavior factors associated work-related musculoskeletal pain (HFWMP). Methods: The Institute for Poverty Alleviation and International Development at Yonsei University conducted a survey of 126 households in Tigray, Ethiopia in 2014. A total of 116 individuals (73 men, 43 women) representing each household answered the questionnaires. Results: 1) Work-related musculoskeletal pain (WMSP) most commonly occurred when performing heavy lifting and most frequently occurred in the lower back. 2) Age, self-perceived labor intensity, and months of farming work were significantly higher in the pain group than those in the non-pain group. 3) Overall work-related musculoskeletal pain intensity (WPI) showed positive and negative correlations with years of farming experience and self-perceived health status, respectively. 4) In binary logistic regression, the occurrence of WMSP showed significant associations with self-perceived labor intensity. 5) On multiple linear regression analysis, age, months of farming work, and self-perceived health status had a significant impact on overall WPI. Conclusion: The WMSP of farmers in Tigray, Ethiopia was related to the characteristics of farm working and health status. Furthermore, HFWMP and AFWMP were the chief factors affecting the occurrence of WMSP in farmers in Tigray. Therefore, both HFWMP and AFWMP should be considered for clinical health assessments of farmers with WMSP in underdeveloped African countries.
